Abstract

The cement industry faces significant challenges in maintaining operational continuity due to auxiliary equipment failures that impact overall productivity. Weight feeder 532-WF1 at Finish Mill 2 PT Solusi Bangun Indonesia Tbk Tuban experienced 9 unplanned shutdown incidents during June-October 2024 with a total duration of 31.57 hours, resulting in production losses of 8,208 tons of cement. This research aims to identify dominant failure modes and analyze root causes through integrated implementation of Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA) methodologies. FMEA implementation identified 15 potential failure modes with highest Risk Priority Numbers (RPN) for belt drifting (504), alarm failure (448), and belt damage/rupture (360). FTA analysis revealed 14 basic events as root problems, where mechanical misalignment became the dominant contributor. Implemented improvement strategies included standardization of alignment procedures, component material upgrading, and real-time temperature monitoring implementation. Effectiveness evaluation demonstrated zero breakdown achievement during April-May 2025 period, validating the success of risk analysis-based preventive maintenance approach in enhancing weight feeder system reliability. Abstract

The demand for achieving zero accident targets, coupled with various urgent tasks and frequent overtime work, has the potential to increase workload and stress among employees in the K3L (Occupational Health, Safety, and Environment) division at PT PLN (Persero) West Java Distribution Main Unit. Although the company has measured workload using work analysis forms and psychosocial assessments, this approach is considered to not fully represent the actual conditions as it primarily focuses on time aspects and stress levels. This research aims to complement the company's existing workload measurement methods with the Subjective Workload Assessment Technique (SWAT), which considers three main dimensions that is time load, mental effort load, and psychological stress load. The measurement results indicate that the mental effort load dimension has the highest percentage at 34.85%, followed by psychological stress load at 33.20%, and time load at 31.95%. These findings suggest that mental effort load is the dominant factor influencing the mental workload of K3L employees. Based on these results, this research provides recommendations for improvements focused on evaluating and adjusting administrative workload and strengthening managerial support. It is hoped that implementing these proposals can reduce mental workload and foster a more balanced, healthy, and productive work environment. Abstract

House Tapis Citra (HTC) is a small and medium enterprise (SME) in Lampung engaged in traditional tapis fabric craftsmanship, which holds high cultural and economic value. However, it faces challenges in marketing strategy, unclear market segmentation, and weak business management. This study aims to formulate a comprehensive business development strategy using the Business Model Canvas (BMC) and SWOT analysis. The results show that HTC is positioned in Quadrant I (aggressive strategy) with an IFAS score of +2.131 and an EFAS score of +0,669. Recommended strategies include product innovation, digital marketing through social media and marketplaces, diversification of distribution channels, and strategic partnerships. These strategies are expected to enhance competitiveness, expand market reach, and support cultural preservation as well as the creative economy in Lampung. Abstract

Suboptimal warehouse management can lead to dead stock, which refers to spare parts inventory that is not used for a certain period and may accumulate, resulting in financial losses for the company. This issue can arise due to weak inventory control, inaccurate stock records, and purchasing plans that are not based on actual usage data. This study aims to identify the causes of dead stock using Root Cause Analysis (RCA) to determine the root causes and Failure Mode and Effect Analysis (FMEA) to analyze potential failures and determine risk priorities based on the Risk Priority Number (RPN). The results show that the main factors causing dead stock include inaccurate inventory data, purchasing planning not based on actual usage, the absence of standard operating procedures (SOP) for stock management, and the lack of an integrated inventory management system. Based on the RPN calculation, inaccurate inventory data is the highest priority with an RPN value of 392, followed by unused spare parts (336), and full warehouse capacity and unutilized spare parts (252). These findings are expected to support improvements in inventory management to minimize the occurrence of dead stock. Keywords: Dead Stock, FMEA, Inventory Management, RCA, RPN

Abstract

The advancement of Industry 4.0 has increased the demand for real-time, accurate, and efficient production monitoring systems. This study aims to develop a real-time digital system for extracting production output data from machining processes through the integration of a Patlite signal system within an automotive manufacturing environment. The proposed system utilizes three-color signal indicators (red, yellow, green) as machine status inputs, which are processed through a microcontroller and transmitted via an IoT gateway to a centralized database. A simulation-based experimental approach was employed to evaluate system performance. The results indicate that the system achieved an accuracy level of 96.8% in detecting machine status and reduced manual recording time by approximately 80%. Furthermore, the system enabled real-time monitoring through a web-based dashboard, improving production transparency and decision-making efficiency.This study demonstrates that Patlite signals can be effectively transformed into a reliable digital data source, supporting smart manufacturing implementation. Abstract

Drawing activities on a drawing board are an essential part of the learning process; however, manual erasing often hinders the smooth flow of teaching and learning activities. This study aims to design an automatic drawing board eraser system based on the Arduino R3 microcontroller to improve user efficiency and convenience. The system utilizes a DC motor with bidirectional movement on horizontal and vertical rails, controlled by a TB6600 driver and equipped with a microfiber eraser. The research method employs an engineering experimental approach using a prototyping model, which includes stages of literature review, design, prototype development, testing, and evaluation. The trial was conducted with 20 respondents at SD Negeri Cisero through observation, interviews, and questionnaires. Results show that the system operates stably, responds well to both manual and automatic commands, and is effective in cleaning the board. Out of 200 questionnaire responses, 68.5% of respondents gave positive feedback. In addition, force testing was carried out using a 4 kg load. At a speed of 10 m/s with a 22 cm track length, the resulting force was 2.2 N, whereas at a reduced speed of 8.33 m/s with the same distance, the force increased to 2.6 N. These findings indicate that speed affects the frictional force. The system has proven effective in improving time efficiency and user convenience in educational settings. Abstract

CV ALUNA is a company engaged in the production of flip-flops. In its production process, the company faces various defects that may affect product quality and corporate reputation. This research aims to analyze and improve the quality of flip-flop products using the Failure Mode and Effect Analysis (FMEA) method. FMEA is used to identify failure modes, determine the values of Severity (S), Occurrence (O), and Detection (D), and calculate the Risk Priority Number (RPN) for each type of failure. The results indicate that the three main processes with the highest RPN values are cutting, printing, and sanding. The highest RPN value is found in the cutting process at 512, followed by printing and sanding processes, each with a score of 448. Based on these findings, improvement suggestions were proposed focusing on worker accuracy, equipment maintenance, and process supervision. The implementation of the FMEA method is expected to reduce production defects and significantly improve product quality at CV ALUNA. Keywords: CV.ALUNA;  FMEA; Flip-Flops; Product Quality; RPN

Abstract

PT Fukuryo Indonesia is a garment manufacturing company. The garment industry is one of the labor-intensive manufacturing sectors and is highly dependent on human performance, especially sewing operators. The purpose of this study is to determine the level of risk of injury to sewing operators and can provide countermeasure ideas to improve performance quality. For this reason, it is necessary to analyze the work process with the aim of knowing the work posture of sewing operators with the Rapid Upper LimbAssessment (RULA) method and the Nordic Body Map (NBM) questionnaire. Based on the data processing that has been done from the calculation of the Nordic Body Map (NBM) questionnaire, there are sewing operators in the large merging section who have the highest score of 52. And the average score of the Rapid Upper LimbAssessment (RULA) method score is 4. Although a low score is obtained, changes are still needed for work postures. Changes must be made immediately to prevent posture fatigue when working for a long time, for this reason it is necessary to propose improvements to ergonomic seating chairs to minimize and prevent injuries. Abstract

Project Management played an important role in determining to succussed a project. The Tank construction project carried out by PT. ABC encountered obstacles namely the project was completed later than scheduling planned therefore had an impact higher cost. This study aims to optimize the scheduling of tank construction by PT ABC. The sample in this study is an XYZ Project in construction tank. This study uses the critical path method (CPM) and Program Evaluation and Review Technique (PERT) approaches. The results show that there are 18 activities (form A-R as symbolic) in the XYZ Project for construction tank over 87 days as planned. Implementation CPM with forward and backward method project completed around 66 days. Otherwise, PERT analysis used forward and backward method project completed around 67,5 days or 21,5 faster than planned. Therefore, it can be concluded that the both of methods had a gap 1 working days that distributed in 12 work activities namely A, B, C, E, F, G, M, N, O, P, Q, R. Abstract

Crude palm oil (CPO) is the main product of PT SBP. To produce crude palm oil (CPO), the company tests the moisture content, free fatty acid concentration, and impurity content to ensure its quality. PT SBP's CPO production process is inseparable from the risks that can disrupt CPO production. Several factors such as too high free fatty acid (FFA), moisture content (MOIST), and dirt content (DIRT) have the potential to disrupt the CPO production process, which will have an impact on the goals and objectives of PT SBP.  The purpose of this research is to estimate the risk of failure of the production process using fuzzy failure mode and effect analysis (FMEA) technique. Applying fuzzy logic with Matlab 2013b software can help overcome the challenges or limitations of FMEA. In this study, the largest FRPN number as determined using fuzzy calculations is 883 for purchasing raw materials that are not dependent on quality. Thus, the suggestion for improvement is specifically to organize fruit delivery and pay attention to the proper maturity level of palm fruits. Keywords: CPO (Crude Palm Oil), Fuzzy FMEA, Matlab
