Abstract

The cement industry faces significant challenges in maintaining operational continuity due to auxiliary equipment failures that impact overall productivity. Weight feeder 532-WF1 at Finish Mill 2 PT Solusi Bangun Indonesia Tbk Tuban experienced 9 unplanned shutdown incidents during June-October 2024 with a total duration of 31.57 hours, resulting in production losses of 8,208 tons of cement. This research aims to identify dominant failure modes and analyze root causes through integrated implementation of Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA) methodologies. FMEA implementation identified 15 potential failure modes with highest Risk Priority Numbers (RPN) for belt drifting (504), alarm failure (448), and belt damage/rupture (360). FTA analysis revealed 14 basic events as root problems, where mechanical misalignment became the dominant contributor. Implemented improvement strategies included standardization of alignment procedures, component material upgrading, and real-time temperature monitoring implementation. Effectiveness evaluation demonstrated zero breakdown achievement during April-May 2025 period, validating the success of risk analysis-based preventive maintenance approach in enhancing weight feeder system reliability. Abstract

The demand for achieving zero accident targets, coupled with various urgent tasks and frequent overtime work, has the potential to increase workload and stress among employees in the K3L (Occupational Health, Safety, and Environment) division at PT PLN (Persero) West Java Distribution Main Unit. Although the company has measured workload using work analysis forms and psychosocial assessments, this approach is considered to not fully represent the actual conditions as it primarily focuses on time aspects and stress levels. This research aims to complement the company's existing workload measurement methods with the Subjective Workload Assessment Technique (SWAT), which considers three main dimensions that is time load, mental effort load, and psychological stress load. The measurement results indicate that the mental effort load dimension has the highest percentage at 34.85%, followed by psychological stress load at 33.20%, and time load at 31.95%. These findings suggest that mental effort load is the dominant factor influencing the mental workload of K3L employees. Based on these results, this research provides recommendations for improvements focused on evaluating and adjusting administrative workload and strengthening managerial support. It is hoped that implementing these proposals can reduce mental workload and foster a more balanced, healthy, and productive work environment. Abstract

House Tapis Citra (HTC) is a small and medium enterprise (SME) in Lampung engaged in traditional tapis fabric craftsmanship, which holds high cultural and economic value. However, it faces challenges in marketing strategy, unclear market segmentation, and weak business management. This study aims to formulate a comprehensive business development strategy using the Business Model Canvas (BMC) and SWOT analysis. The results show that HTC is positioned in Quadrant I (aggressive strategy) with an IFAS score of +2.131 and an EFAS score of +0,669. Recommended strategies include product innovation, digital marketing through social media and marketplaces, diversification of distribution channels, and strategic partnerships. These strategies are expected to enhance competitiveness, expand market reach, and support cultural preservation as well as the creative economy in Lampung. Abstract

Suboptimal warehouse management can lead to dead stock, which refers to spare parts inventory that is not used for a certain period and may accumulate, resulting in financial losses for the company. This issue can arise due to weak inventory control, inaccurate stock records, and purchasing plans that are not based on actual usage data. This study aims to identify the causes of dead stock using Root Cause Analysis (RCA) to determine the root causes and Failure Mode and Effect Analysis (FMEA) to analyze potential failures and determine risk priorities based on the Risk Priority Number (RPN). The results show that the main factors causing dead stock include inaccurate inventory data, purchasing planning not based on actual usage, the absence of standard operating procedures (SOP) for stock management, and the lack of an integrated inventory management system. Based on the RPN calculation, inaccurate inventory data is the highest priority with an RPN value of 392, followed by unused spare parts (336), and full warehouse capacity and unutilized spare parts (252). These findings are expected to support improvements in inventory management to minimize the occurrence of dead stock. Keywords: Dead Stock, FMEA, Inventory Management, RCA, RPN

Abstract

The advancement of Industry 4.0 has increased the demand for real-time, accurate, and efficient production monitoring systems. This study aims to develop a real-time digital system for extracting production output data from machining processes through the integration of a Patlite signal system within an automotive manufacturing environment. The proposed system utilizes three-color signal indicators (red, yellow, green) as machine status inputs, which are processed through a microcontroller and transmitted via an IoT gateway to a centralized database. A simulation-based experimental approach was employed to evaluate system performance. The results indicate that the system achieved an accuracy level of 96.8% in detecting machine status and reduced manual recording time by approximately 80%. Furthermore, the system enabled real-time monitoring through a web-based dashboard, improving production transparency and decision-making efficiency.This study demonstrates that Patlite signals can be effectively transformed into a reliable digital data source, supporting smart manufacturing implementation.
