Abstract

Self-Medication practices are considered to be a quick solution in treatment when a person can not access the formal health services, but on the other hand, this behavior can cast up the economic burden and health losses if not properly monitored. Meanwhile, smoking is seen as a negative behavior, which can also cause a double burden, both in terms of increased expenditure also has a negative effect on health. This study aims to describe self-medication behavior among smokers in Indonesia and the determinants that influence it. This study is a quantitative study using secondary data from the Indonesia National Socio-Economic Survey (SUSENAS) in 2017. The observation unit is individuals, and the sample criteria are smokers with the probability of doing self-medication due to health problems. The number of analyzed samples was 53.317 respondents. Descriptive analysis is used to see the prevalence of self-medication among smokers. Logistic regression is used to see the determinants that influence a smoker's decision to make self-medication. The results of the follow-up analysis showed that most smokers did Swamedikasi (75.99%). The level of education (p=0,005) and ownership of health (p=0,000) insurance has a significant effect on self-medicated behavior. Through this research, it is expected to be material for learning and advocating about behavior in seeking treatment for smokers.

Abstract

Glaucoma is the second foremost cause of impaired vision. People whosuffered from Glaucoma face the independent expenditure for thetreatment as blind people with Glaucoma could not be cured perfectly. This study intends to analyze the effect of age, types of patient care, typesof Glaucoma, and types of payment on the total cost of care of glaucomapatients at Cicendo Eye Hospital, Bandung, Indonesia. This study uses3,358 patient medical records of Cicendo Eye Hospital, Bandung,Indonesia, in 2018. The 3,358 samples were selected from the patient’smedical record based on patients’ categories indicated or convicted ofhaving Glaucoma. Robust Linear Regression is applied in this study tomeasure the additional cost for Glaucoma treatment. The results showedthat the total cost of patient care was positively and significantly affectedby hospitalization status (p=0.000), age (p=0.000), times of treatment(p=0.000), having primary glaucoma (p=0.000), having congenitalglaucoma (p=0.000), and presence of intraocular (p=0.000). Conversely, patient care’s total cost was negatively and significantly affected usinginsurance (p=0.082). This result would be a precautionary measure forthe medical institution to consider better financial planning, servicedelivery improvement, and the patient’s payment scheme effectiveness.

Abstract

Commuting has become a global phenomenon in various metropolitan cities in the world, including Denpasar. Denpasar is the center of tourist destinations for Indonesian and foreign tourists. This condition makes Denpasar a business and economic center for residents in the surrounding area. Conditions have encouraged an increasing number of commuter workers in Denpasar from sub-urban areas. The people are facing several problems such as traffic, air, noise, thermal pollution, and long duration of commuting. This study aims to analyze the relationship between commuting patterns and health problems among workers in Denpasar metropolitan areas in Indonesia. This study analyzed by using secondary data of the Sarbagita (Denpasar, Badung, Gianyar, and Tabanan) Commuter Survey 2015 from CBS Indonesia. Sample study: commuter workers aged 15-65 years with inclusion criteria are commuters passive commuting with all types of transportation except bicycles/walking.  The sampling method used two stages of cluster sampling. The final sample was 652 persons. Statistical analysis used binary logistic regression. The study shows that around one-third of commuters have commuting-related health problems. Health problems are more perceived by formal workers, low incomes, and low-educated. This study shows that distance and transportation mode had correlated with health problems (OR=1.71). Motorcycle and private car users have more health problems than public transportation (OR=2.49; OR=3.13). The impact of commuting for com-muter health can be minimized by providing public transportation that is fast, safe, comfortable, and inexpensive. Local government must encourage the use of public transportation for commuters through campaigns from a public health perspective.

Abstract

Smoking is a risk factor for infectious diseases such as asthma, chronic obstructive pulmonary disease, lung cancer, hypertension, coronary heart disease, heart failure, and stroke. Nowadays, smoking behavior is not only found among adults but has become a lifestyle for some teenagers in Indonesia. This study aims to study how the influence of smoking behavior on parents on the smoking behavior of adolescents aged 15-24 years in Indonesia. This study used a cross-sectional study design of 4,959 adolescents aged 15-24 years as a result of the 2014 IFLS who were single. The analysis used contains a descriptive analysis and inference. The majority of teen smokers are light smokers who smoke a maximum of 10 cigarettes per day and most start smoking at the age of 12 to 17 years. The results of the analysis using binary logistic regression showed that smoking behavior in parents significantly affected adolescent smoking behavior (p=0.000; OR=1.397; 95% CI=1.173 <OR<1.663). Gender and age groups also influence smoking behavior in adolescents. Meanwhile, the classification of residence was not significant for adolescent smoking behavior (p=0.337; OR=0.920; 95% CI=0.777<OR <1.090). Efforts to prevent teenage smoking habits should be more focused on children who live with smokers' parents.

Abstract

The prevalence of depression among the general population in CentralSulawesi was the highest in Indonesia (12.3%). Scholars revealed thatdisability was one of the main contributing factors of depression and in Central Sulawesi almost half of population was reported having disability (42.1%). This study was intended to identify the association of disability and depression in Central Sulawesi. Cross sectional design study was conducted with utilization of secondary data collection from Riskesdas 2018. Depression, as the main outcome, was assessed by the Mini International Neuropsychiatric Interview (MINI), meanwhile, disability was assessed by the WHODAS instrument. The relationship between these two variables was adjusted by several confounders, namely age, gender, marital status, educational status, and employment status. A 11,9% of 11,926 respondent aged 18-59 years old, was depressed and almost half of them reporting disability (42.1%) and among those who were disabled, one-fifth weredepressed. The risk of depression among respondents with disabilities was 3.25 times higher (p=0.000; 95% CI 2.9-3.6) compared to respondentswithout disability after controlled by confounders (gender, marital status, educational status, and employment status). It is recommended that routine mental health screening needs to be done at Posyandu, Posbindu and among patients at Primary Health Care. Besides, health promotion focused on physical activity in a community such as community gymnastics during theweekend.

Abstract

Community-Based Rehabilitation (CBR) units are growing in developing countries, including Indonesia. However, patient safety measures in CBR units have not yet been conducted, especially regarding flexible, unscheduled appointment system. This study aims to evaluate the unscheduled appointment system as a cause of errors in the care delivery of a CBR unit for children with disabilities in Makassar city. This case study conducted 278-hours observations and three structured interviews. The observation findings, encoded interview findings, field notes, and self-reflective notes were triangulated and link thematically. Four key findings were illustrated in an explorative model. The CBR unit implements a day-based scheduling system without time slots. Working in this system, the therapists experienced random visits, long waiting times, and excessive workload as a result of this flexible scheduling system. Significant risks of human errors and patient safety were revealed due to these consequences. As flexible scheduling systems produce more negative effects to the therapists and children, it is suggested that a CBR unit or a health care unit implement a scheduled appointment system with timeslots to achieve prudent healthcare.

Abstract

The incidence of the COVID-19 pandemic is very important to know because it has a psychological impact on the coronavirus pandemic incident on health workers. The outbreak of the COVID-19 epidemic has caused high levels of stress from health workers and increased illness in the community. The research objectives were to determine midwife anxiety, the role of midwives in midwifery services, and to provide information about the coronavirus (COVID-19) to mothers and children in the Madura region. This type of research is descriptive, the population of this study was midwives in Madura with a total sample of 123 midwives. Data collection uses the Google Form system which is distributed to all midwives in the Madura region. The collected data were analyzed using frequency distribution tables. The results showed that midwives experienced severe anxiety by 16.3%, 17.9% very severe anxiety, and 22.8% moderate anxiety, midwives also provided good midwifery services at 87.0% and provided information about the prevention of coronavirus transmission properly by midwives by 77.2%. This pandemic has had a huge impact on the provision of midwifery services, especially programs that have been implemented routinely so that the program cannot be implemented and midwives also experience anxiety because they are the spearhead in midwifery services. It is hoped that there will be good cooperation between patients and midwives, especially patients, to continue to follow the principles of preventing transmission of the coronavirus and to provide good and correct information to patients, especially when coming from out of town.

Abstract

The Healthy Indonesia Program with a Family Approach (PISPK) is conducted bypuskesmas by integrating existing resources, with the family’s target. All familieswill get access to comprehensive health services. The implementation of PISPKsince 2016 has not been optimal because it has many obstacles. The analysis aimsto identify the obstacles, and to find solutions to implemented PISPK optimally.Implementation research was carried out using Participatory Action Research(PAR). The team (researcher and subject) implemented PISPK together based onstages that integrated into puskesmas management, at 4 puskesmas in SouthLampung. Researchers assisted and recorded data collected qualitatively (self-assessment, in-depth interviews, Focus Group Discussion), and quantitatively. There are any obstacles occurred in the implementation of PISPK such as theabsence of regulations and cross-sectoral supports; lack of knowledge andsupport from village officials, community leaders, and the public; limitedresources; lack of understanding of the substantive; application; lack of dataanalysis capabilities. These obstac-les can be minimized by making somebreakthroughs, such as advocacy and issuance of local government regulations onPISPK involving cross-sectors; increase socialization; periodic coordination, monitoring, and evaluation; making innovations (On Job Training, collaborationwith universities and health volunteer, Healthy Family Coverage Pocket Book, developing data analysis methods). The implementation of PISPK has manyobstacles that can be minimized by optimizing existing potentials and supportfrom stakeholders. Puskesmas need to increase socialization; team organizing;data analysis; coordination, and routine monitoring evaluation. Pusdatin needs toimprove KS applications to be more user-friendly.
