Abstract

An ovarian cyst is a sac filled with liquid or semiliquid material that arises in an ovary. Those cysts were mostly found in women aged 20-50 years. The incidence is 10: 100.000 women. This report reports a case of a child patient aged two years who came to Arifin Achmad Hospital with a chief complaint there is a mass in the lower abdomen area. From the physical examination, the lump in the lower abdomen area was obtained. We performed, laparotomy exploration with cystectomy and found a left ovarian cyst size 12 x 12 x 15 cm.

Abstract

Low magnesium levels are known to be associated with increased risk of diabetes mellitus and its complications. In addition, HbA1c levels are also known to have a correlation with further diabetes complications such as diabetic neuropathy. Diabetic neuropathy is the most common microvascular complication in diabetes mellitus, whose incidence is associated with disability and mortality rates in diabetic patients. This study aimed to assess the correlation between serum magnesium levels and HbA1c with the severity of diabetic neuropathy. This was a cross-sectional study. Severity of diabetic neuropathy is classified according to the Baba's Diabetic Neuropathy Classification (BDC). All subjects were examined for serum magnesium and HbA1C levels. This study consisted of 46 diabetes mellitus subjects, with 69.6% of the subjects suffering from diabetic neuropathy. The average magnesium level in this study was 1.87 mg/dl ± 0.245 and the median HbA1c level was 8.6 %. There was no significant correlation between serum magnesium levels and HbA1c with the incidence and severity of diabetic neuropathy (p>0.05). However, there was a significant negative correlation between magnesium levels and HbA1c levels in diabetic neuropathy patients.

Abstract

Pulmonary tuberculosis is a major public health problem in Indonesia, with the number of patients being the second largest in the world after India. Tuberculosis (TB) is caused by the pathogen Mycobacterium tuberculosis, 80% of which are pulmonary TB and the rest are extra pulmonary. Patients with a history of TB become one of the risk factors for lung cancer due to chronic tissue damage. Tuberculosis can also present as tuberculoma which can mimic a lung tumor. We report the case of a 55-year-old patient with complaints of a lump on the back since 3 months ago and weight loss of ± 9 kg in 5 months. The chest X-ray and CT scan showed an impression of a lung tumor and a TTNA examination was performed with the results of inflammatory cell infiltration of lymphocytes, neutrophils and macrophages. Cytology examination from the lump on the back came with the result of a cold abscess. A frozen section-open biopsy was performed with macroscopic of caseating necrosis and histopathology results showed the presence of Langhans cells, typically tuberculosis. Currently, the patient treated with ATD first category regimen for 6 months as definitive therapy of tuberculosis.

Abstract

Hepatitis B is still one of public health problems in the world including in Indonesia. The most vulnerable group to hepatitis B virus exposure is healthcare workers including clinical laboratory technicians. The best way to prevent hepatitis B infection is by getting vaccination. Therefore clinical laboratory technicians are among prioritized subject in getting hepatitis B vaccination. However, hepatitis B vaccination for healthcare workers is not an obligation yet. The purpose of this study was to determine the knowledge, attitude and practice of hepatitis B vaccination among clinical laboratory technicians at X Hospital of Riau Province. This descriptive cross-sectional study was carried out between October 2021 and January 2022. Overall, 38 clinical laboratory technicians were recruited for this study. The Kolmogorov-Smirnov test is used to determine the relationship between knowledge with attitudes and practice. The results showed that 52.6% of respondents had good knowledge, 57.9% had a positive attitude and 68.4% had not been vaccinated against hepatitis B. Based on the results of bivariate analysis, there was no relationship between knowledge and attitude (p>0.05) or practice (p>0.05). Although more than 50% of respondents had good knowledge and attitude, the low number of clinical laboratory technicians in getting the hepatitis B vaccine remains a particular concern.

Abstract

A cutaneous horn is a hard cone-shaped protrusion consisting of dense keratin resembling animal horns, which occurs due to abnormalities in the spinous layer of the epidermis, causing excess keratin accumulation. This disease can be found in any body part, especially in regions exposed to a lot of sunlight, such as the ears. Surgical excision is the treatment of choice for this disease. One case was reported in a male patient aged 75 years with a bulge on the left auricle. The patient underwent surgical excision as management and histopathological examination confirmed the diagnosis of the cutaneous horn with mild-moderate squamous epithelial cell dysplasia. Because of the high association with premalignant and malignant conditions, patients should be followed up regularly to prevent a recurrence.

Abstract

Pott puffy tumor (PPT) is an abscess of the subperiosteal os, frontalis. Frontalis subperiosteal abscess is often found in cranial osteomyelitis. Cranial osteomyelitis is an inflammatory process of the cranial bones, mainly caused by pyogenic organisms. The most common cause of cranial osteomyelitis in developing countries is paranasal sinusitis. One of the causes of PPT is an infection of the frontal sinus. Intracranial complications are the most common complications of PPT, one of which is a cerebral abscess in the frontal lobe. In this case report, the patient complained of forehead swelling that spread to both upper eyelids. There is a history of boils on the left side of the forehead since 3 weeks ago but was not treated medically. On physical examination, there was no neurological deficit. In the initial action, aspiration of the swelling results in the form of pus. Based on the results of a plain head CT scan and 3D skull bones, the results obtained were an appearance of an abscess in the subperiosteal frontal bone and osteomyelitis of the frontal bone. The patient underwent debridement craniectomy and further therapy in the form of intravenous antibiotics. As a follow-up examination, an examination of pus culture and examination of anatomical pathology were carried out, and obtained cranial osteomyelitis results but found no pathogenic germs.

Abstract

Doctors practicing their profession are entitled to legal protection as stipulated in Article 50 of Law Number 29 of 2004 concerning Medical Practice. Hospitals must provide legal protection for doctors working in their hospitals by the hospital's obligations regulated in Article 29 paragraph (1) letter (s) of Law Number 44 of 2009 concerning Hospitals. In Indonesia, a statutory regulation has not explicitly regulated legal protection for doctors. This Literature Review discusses legal protection that hospitals can provide to doctors in practicing medicine. The form of preventive legal protection for doctors by hospitals is in the form of guarantees of legal protection, legal administration, authority for professional practice/clinical care, regulation of clinical practice, quality of medical services, provision of quality health service facilities and infrastructure and stipulation of prohibitions and sanctions. Forms of repressive legal protection for doctors by hospitals are handling complaints and claims, investigations, settlements, and financing of medical dispute cases. Legal certainty regarding the provision of legal protection must be included in the Hospital By-Laws. With legal certainty and regulations regarding legal protection, doctors will be protected and increase loyalty to hospitals.

Abstract

Changes in air quality increase host susceptibility to Mycobacterium tuberculosis. Retrospective cohort study to see the relationship between air pollution levels and climate with tuberculosis cases in Pekanbaru City in 2015 – 2020. It found there is a significant positive correlation between PM10, SO2, CO, O3, NO2, PM2.5 and the number of tuberculosis cases per day. While in climate, temperature and humidity parameters do not provide a significant relationship, while rainfall and wind speed provide a very weak correlation strength with tuberculosis. We conclude there is a positive correlation air pollution and the number of tuberculosis cases per day.

Abstract

Cervical cancer is caused by persistent high-risk type of human papillomavirus (HPV) infection. especially types 16 and 18. The choice of cervical cancer therapy is associated with many side effects, tumor drug resistance, and high costs. A more efficient, safe and effective cervical cancer therapy is needed, such as therapeutic vaccine which targets host factors related to E6 and E7 of HPV oncoprotein. The oncoprotein E6 targets and inhibits p53, whereas E7 inhibits pRb. Therapeutic vaccines can be used to treat cervical cancer in all stage. Currently, this therapeutic vaccine is under investigation in phase I/II/III clinical trials.

Abstract

The current modern era causes lifestyle changes, especially in urban communities, in terms of daily consumption patterns that tend to be accustomed to consuming fast food that contains many additional compounds. A compound commonly known as a food flavoring additive is monosodium glutamate (MSG), a sodium salt that is naturally produced from L-glutamic acid. Apart from its role in enhancing the taste of food, several studies have shown that MSG has toxic effects on human and animal tissues including the reproductive system which may cause infertility. Glutamate in MSG has a direct reaction effect at the cellular level, one of which forms free radicals and causes oxidative stress. The increase in free radicals in the body can cause damage to the organs of the body including the testes. Damage caused by MSG can also occur centrally in the hypothalamus so that it interferes with the hormonal and endocrine reproductive systems. Damage to pituitary cells will inhibit the production of gonadotropin-releasing hormone (GnRH), resulting in a decrease in gonadotropin levels, either luteinizing hormone (LH) or follicle stimulating hormone (FSH) produced by the pituitary gland. This article aims to discuss in detail how MSG affects male fertility.
