Abstract

Hirschsprung disease (HD) is a congenital anomaly due to intestinal aganglionosis starting from the internal anal sphincter and spreading to the proximal area with varies extension of the segment. Definitive treatment for this abnormality is to perform surgery to remove the aganglionic bowel and can be carried out in one-step and multi-step procedure. In the process of wound healing after surgery, it involves inflammatory mediators as a marker of whether there is a stress response from the endocrine system such as neutrophil-lymphocyte ratio (NLR). This study aims to compare NLR as an inflammatory biomarker in one-step and multi-step procedure of hirschsprung disease patients at Arifin Achmad Hospital. The design of this study was cross-sectional using medical records and using total sampling for number of samples. Age, gender, and method of surgery were evaluated by univariate and bivariate to determine whether there were statistical differences. Seven HD patient taken from Mei-October at Arifin Achmad Hospital taken for evaluated. The results showed that the most samples were in < 1-month-old (42.9%) and male (71,4%) for gender. The average number of NLR was in 7.33 after one-step procedure and 2.77 for multi-step procedure. This number of NLR both not statistically different neither for the post-operative examination nor the comparison between procedure.

Abstract

Pregnancy is a condition of multisystemic changes in a woman's body to adapt to the presence of the fetus. Physiological changes also occur in the kidneys and urinary tract and persist until the postpartum period. Chronic Kidney Disease (CKD) is a condition of kidney damage in which the estimated glomerular filtration rate (eGFR) is less than 60 mL/min for three months. In general, CKD occurs in 10-14% of the population. About 1 in 750 pregnant women has chronic kidney failure grades 3-5. Despite its low incidence, CKD in pregnancy can cause severe Fetomaternal complications.

Abstract

Superior Vena Cava Syndrome (SVCS) includes various clinical signs and symptoms due to external compression and intrinsic obstruction of superior vena cava (SVC) itself or the superior cavo-atrial junction that cause reducing of blood flow. Infection becomes main etiology in previous decades but development in antibiotics treatment and improvement of socio-economyc condition made incidence of SVCS due to infection decreased. SVCS due to malignancy recently reach 60-90% of overall cases. 78-85% of SVC obstructions due to malignancy are caused by lung cancer and 80% of it is right lesion. Historically SVCS is considered as an oncology emergency condition and become one of a few indications for palliative radiotherapy. This literature review will explain the pathophysiology, etiology, diagnosis and management of SVCS with the aim of providing timely and effective intervention to treat the causes of this syndrome so that it can significantly relieve symptoms and improve the quality of life of patients with SVCS.

Abstract

Dengue Hemorrhagic Fever (DHF) is a potentially fatal disease that threatening global health, including Indonesia. Various efforts have been made to control DHF, one of them is by means of vector control. Household based insecticides are widely used by the community in controlling insect vectors, including Ae.aegypti as the main vector of DHF. This study aimed to describe the use of household insecticides in the Tangkerang Barat Sub District, Marpoyan Damai District. This study was conducted in September-November 2022 with a total of 100 respondents. The sample selection technique used was simple random sampling from 4 RWs. Data collection was carried out using guided interviews with 4 questions to respondents who used insecticides. Data analysis was carried out using univariate analysis. This study showed that almost all respondents (98%) used household insecticides, the most widely used type of formulation was aerosol (51%), active ingredient was dimeflutrin 0.02%; cypermethrin 0.10% (25.5%), used once a day (75.5%) and used at night (98%). Improper use of household insecticides can have negative effects on the environment, humans, and the occurrence of resistance. Mosquito Nest Eradication needs to be further improved in controlling Ae.aegypti.

Abstract

Steroids can diminish inflammation and forestall cardiac impairment in cardiac surgery cases, encompassing coronary heart surgery; however, this approach remains a topic of ongoing discussion. The present study investigated the ramifications of administering steroids in coronary heart surgery patients, focusing on the variations in CRP and Troponin I level at Arifin Achmad General Hospital in Riau Province. Employing a cross-sectional design, the study retrospectively reviewed medical records of individuals who had undergone coronary heart surgery between January 2018 and September 2020 at Arifin Achmad Hospital in Riau Province. Comparisons between Troponin I and C-Reactive Protein (CRP) levels were drawn contingent on the specific administered steroid (methylprednisolone or dexamethasone). The study further examined secondary outcomes, encompassing myocardial contractility, duration of ventilator employment, and Intensive Care Unit (ICU) length of stay, distinguishing patterns based on the steroid type. Among the 42 analyzed patients, the methylprednisolone and dexamethasone groups exhibited significant increases in troponin I and CRP levels (p<0.05). In contrast, changes in myocardial contractility pre- and post-surgery remained consistent across both groups—similarly, no substantial difference in ventilator or ICU stay duration. As the conclusion, dexamethasone and methylprednisolone have not given any effect in preventing increases in troponin I, CRP levels and myocardial contractility early after coronary heart surgery.

Abstract

Atherosclerosis is a multifocal immunoinflammatory disease of medium and large arteries stimulated by lipids, endothelial cells, leukocytes and intima smooth muscle cells. One of the markers of atherosclerosis is thickening of the tunica media. The administration of antioxidants is expected to inhibit the occurrence of atherosclerosis. Mangrove (Rhizophora sp) is one of plants with a high source of antioxidants. The objective of this study was to analyze the effect of mangrove (Rhizophora sp) fruit extract on the thickness of aortic tunica media of atherosclerotic rats. A total of eighteen rats were used in this study, divided into 3 groups. Group A (standard control) is a group of rats that received a standard diet. Group B (positive control) is a group of rats that received a high cholesterol diet for 3 days. Group C (treatment group), apart from receiving a high cholesterol diet, was also given Rhizophora sp fruit extract at a dose 500 mg/kg b.w/day orally for 3 days. The effect of mangrove (Rhizophora sp) fruit extract on the thickness of the aortic tunica media was evaluated by a histological procedure using the Leica application. This study shows that the thickness of aortic tunica media is higher in the group receiving a high cholesterol diet than the standard control group (0.096 vs. 0.079; P=0.036). The group that received mangrove (Rhizophora sp) fruit extract had relatively the same thickness as the standard group (0.080 vs. 0.079; P=0.999). As the conclusion, Mangrove (Rhizophora sp) fruit extract may inhibit thickening of the aortic tunica media in atherosclerotic rats.

Abstract

Insecticide resistant of Aedes aegypti populations is one of the main problems in controlling dengue hemorrhagic fever vectors because it will produce resistant offspring. This study aimed to determine the insecticide susceptibilty status of Ae. aegypti to 5% malathion, 0,75% permethrin, and 0,1% bendiocarb in Tangkerang Timur Village, Tenayan Raya District, Pekanbaru. This study is a descriptive observational study with a cross-sectional design. The resistance test method in this study was the World Health Organization (WHO) susceptibility test method with impregnated paper (5% malation, 0,75% permethrin, and 0,1% bendiocarb). The larval that were reared into adult mosquitoes were collected from 100 houses in Tangkerang Timur Village. Total adult mosquitoes aged 3-5 days tested in this study were 100  mosquitoes. The test mosquitoes were exposed to impregnated paper for 1 hour and the number of knock down mosquitoes was counted. Mosquito mortality was counted after 24 hours. The results of the susceptibility test in this study showed the mortality of Ae. aegypti to 5% malathion, 0,75% permethrin, and 0,1% bendiocarb were 100%, 85%, and 3.75%. The results of this study indicated that Ae. Aegypti has been resistant to 0,75% permethrin and 0,1% bendiocarb and susceptible to 5% malathion.

Abstract

In 2022, 10,6 million people in the world were infected with tuberculosis and Indonesia ranks second. As many as 8 million of the world’s population died from tobacco smoking in 2021. Cavitary lung lesions are cavities filled with air in the lungs and smoking can directly damage lung parenchyma due its contents so tuberculosis patients who smoke are more risk of developing lung cavities. This study aims to determine association between smoking and formation of lung cavity. This study was an observational analytic study with case-control design using 80 samples consisting of 40 smoking and 40 non-smoker patients. Sample selection used purposive sampling technique and samples were collected from medical records in 2021. Data analysis used chi-square test. It was found that patients with lung cavity were 46,3% and patients who smoked and had lung cavity were 30%. Based on the result of data analysis, it can be concluded that there is a significant relationship between smoking and the formation of lung cavity lesions in tuberculosis patients at RSUP H Adam Malik with p = 0,014 (p < 0,05) and tuberculosis patients who smoke have a risk of 3,115 times to have lung cavity lesions (OR = 3,115).

Abstract

Relatively tight working hours are problems for employees in urban areas, affecting less time for physical activities. About 59% of office workers in Jakarta are classified as sedentary. The high number of sedentary workers can cause productivity problems due to low fitness. Appropriate interventions are needed to overcome it. We have done literature searches from 7 journal databases in the last 20 years, including ScienceDirect, EBSCOhost, PubMed, ProQuest, Scopus, ClinicalKey, and Cochrane. Inclusion criteria include research design in the form of systematic review/meta-analysis/randomized controlled trials, outcomes in the form of physical fitness or work performance, and interventions in the form of workplace exercise or stretching. Three selected articles showed a significant relationship (p-value <0.05) between programmed physical exercise at work and several fitness factors, such as decreased body fat and fat mass, increased lean mass, and increased sit-up and push-up fitness test results. Physical activity at work decreased cardiovascular risk as assessed by VO2max. In conclusion, physical activity during working hours effectively improves health-related fitness. The minimal intervention types needed are aerobics, strength training, and muscle endurance. We recommend providing interventions for workplace fitness and education programs (WFEP).

Abstract

House dust mites (HDM) are essential in causing allergic diseases such as asthma and allergic rhinitis. The main factors affecting the growth and development of HDM are temperature, humidity and food availability. The study aimed to describe the HDM density at SDN 009 students' homes Petapahan, Kuantan Singingi Regency, Riau Province. This research is a descriptive study with a cross-sectional approach. House dust mites were identified using a light microscope directly in the parasitology laboratory; research samples were taken from two locations; bedrooms and living rooms. The results showed that the HDM density was higher in the bedroom than in the living room, with the HDM density in the bedroom being 12.5 mites/gram of dust and the HDM density in the living room being 10.37 mites/gram of dust. The total HDM density is 11.53 mites/gram of dust. HDM density is higher in bedrooms than in living rooms
