Abstract

Diabetes mellitus in increasing at an alarming rate and has become a global challenge. Diabetes mellitus type 2 is agroup of metabolic diseases characterized by hyperglycemia resulting from defects in insulin secretion, insulin actionor both. One of the most complications of diabetes is diabetic neuropathy. Diabetic neuropathy refers to damage ofmotor nerves, sensory, and autonomic system. The aim of this study is to identify patients at risk for diabetic neuropathywith monofilament examination in internist polyclinic Arifin Achmad General Hospital from May to July 2017.Monofilament is an inexpensive, practical tool in everyday use and can be used in early detection of sensation loss inthe foot to prevent diabetic ulcer. This study is descriptive research with direct examination of patient diabetes type 2.There were 73 samples that fulfilled the inclusion criteria. Profile patients with diabetes in the polyclinic are female morethan male is 60,3%. The age group 56-65 years is the most age group of DM patients in the polyclinic is 52.1%.Duration of diabetes is 5-10 years (52.1%). The results of monofilament examination in patients with type 2 DM arescore 0-3 as many as 19 people (26%), score of 3.5-5 as many as 27 people (37%), and score> 5,5 as many as 27 people(37%).

Abstract

Hypertension is the second largest of the 10 diseases on an outpatient at a hospital in Indonesia. The poor complianceto therapy of hypertension is a major cause of uncontrolled blood pressure. The aim of this study is to determinecompliance antihypertension on patient at DR. Sardjito hospital Yogyakarta. This study was designed with aretrospective cohort study design using a database of participants claimed prescribing health insurance (ASKES) inthe DR. Sardjito hospital using antihypertensive drugs. Compliance measured with medication possession ratio (MPR).Data was analysed by chi- square and logistic regression statistic. From 8.011 patients, compliance of antihypertensivedrugs during the first year is 7,6%. Analysis for compliance showed that the type of antihypertensive diuretics aremore compliance compare with angiotensin II receptor blockers, ACE inhibitors, calcium channel blockers and betablockers. Combination therapy (20,7%) is more compliance than monotherapy ( 4,1%), as well as drugs administration1 time a day (8,2%) is more compliance than drug administration 2 times (2,5%) and 3 times a day (3%). Theproportion of compliance in antihypertensive users at DR. Sardjito hospital classified as less good. Compliancepattern indicates that therapy is not continuous, the longer the use of antihypertensive therapy, the higher thediscontinuous therapy.

Abstract

The learning approach is the way to achieve their learning objectives. Learning approach can influence the OSCE value. This study aimed to analyze the relationship between learning approach and OSCE value. This study was analytic descriptive with cross-sectional design. The subjects were all the medical student University of Riau in the second year. Study Process Questionnaire (SPQ) which have been translated to Indonesia language was used in this study. OSCE value before remedial data were analyzed using Kruskal-Wallis. The majority of students (93.1%) using the achieving approach and there were no relationship between learning approach and OSCE value. lue.

Abstract

Staphylococcus aureus (S.aureus) remains to be the most important cause of post operative wound infection. Nursescould become reservoirs to transmit S.aureus through contaminated hands transiently, or through colonized nose.Strain polymorphism could be determined by Restriction Fragment Length Polymorphism (RFLP), using coa gene andrestriction endonuclease enzyme Alu1. There were 30 isolates of S.aureus’s infection, and 20 isolates taken from handsand nose of the nurses in charge. From 50 isolate positive S.aureus, PCR results showed single and multiple bandswithin 300 to 900 base pairs (bp) in length, and multiple bands within 200 to 600 bp. Five out of 30 patients (17%)showed no PCR-RFLP similarity with any of the nurses. Ten out of 15 nurses which hands were positive for S.aureus,has PCR-RFLP similarity with some patients. There was only 1 out of 5 nurses which nose was positive for S.aureus,showed PCR-RFLP similarity with some patients. Statistically, the proportion of the similar PCR-RFLP between thosesamples in this study is 0.12 (12%). Conclusion: Nurses had 12 % PCR-RFLP similarity for S.aureus with post operativewound infection.

Abstract

Cockroach caused intestinal problem due to its mechanical vectorial capability. It can carry many parasites, withdiverse susceptibility patterns. Its number in the enviromenment determine the potential risk of health hazard tohuman. This cross-sectional, entomological survey-study aimed to determine its prevalence with emphasize on vectorialcapability, public heath implication and comparison of several traditional baits/ attractants used to trapped cockroach.During 4 months of sampling, 434 cockroaches were successfully trapped, and the prevalence of intestinal parasiteisolated was 77.9%,which comprise of several types of parasite. Cockroach also more attracted to brown sugar usedas baits/attractants. These findings confirmed the importance of keeping hygiene/sanitation practices that must bemaintain all the time.

Abstract

Squamous cell carcinoma is a nonmelanoma skin malignancy derived from suprabasal keratinocytes. Although completeremission can occur after surgery, patients with advanced stage have poor prognosis. Some cases of high risk canmetastasis. Lymph node’s metastasis occur in approximately 4% of cases with 1,5% mortality rate. Difficulties inidentifying high risk cases and the absence of management guideline are contributing factors in patient mortality.Staging is a process to determine the presence and progression of cancer.

Abstract

Management in patients with type 2 diabetes mellitus (DM) is aimed diabetic control required antidiabetic drugs(OAD). The aim of this study was to know data about the profile of diabetics control in patients with type 2 diabetesmellitus followed in department of endocrinology Arifin Achmad General Hospital Pekanbaru. The type of researchwas descriptive. The samples are 96 people included all patients with type 2 DM according to inclusion criteria. Theidentity patients and patients knowledge were determined using questionnaire, used of antidiabetics drugs and diabeticcontrol were detemined using medical record. The characteristics of patients with type 2 DM were most at group ofage 50-59 years (45,8%), female sex (58,3%), duration of DM 1-5 years (52,1%), complication of diabetic neuropathy(25%) and frequency of the Doctor once a month (64,6%). The used OAD predominant oral antidiabetic drugs (83%)combinations sulfonylureas and biguanid (73,7%), combinations oral antidiabetic drugs with insulin (14%) arecombinations biguanid with long acting insulin (69,2%) and intensive insulin (3%) are long acting insulin with rapidacting insulin (66,6%). Diabetic control of patients with type 2 DM are fasting blood glucose, post prandial 2 hoursblood glucose, total cholesterol, LDL and HDL was bad (68%, 74%, 46,9%, 61,5% and 70,8%). This researchshowed that used of antidiabetic drugs was not satisfactory of diabetic control.

Abstract

The aim of fluid therapy during hemodynamic resuscitation is to increase global blood flow and increase flow to the microcirculation and therby, and thus oxygen availability for cellular respiration. Effect of volume expansion on microvascular perfusion might not always be in coherence with its macrocirculatory. It should be stressed that the total amount of volume administered during fluid resuscitation is believed to be a major determinant of outcome in critically ill shock patients. The purpose of this review is to evaluate the boundaries of macrohemodynamic approach to fluid administration and the integrated with microcirculatory.

Abstract

One of atherosclerosis risk factor is dislipidemia. One of plant that is expected to overcome dislipidemia is shallot(Allium ascalonicum L.). The aim of this research was to know the effect of shallot’s extraction water on the plasmalipid profile of mice. Groups with treatment consist of yolks group, and shallot water extraction divided into threetreatmen level ; 20%, 40% and 80%. Parameters that were measured were total cholesterol level, LDL, HDL andplasma triglyceride of mice in the end of treatment. Simvastatin and aquadest were separately used as positive andnegative control. This research proves that shallot’s extortion water has effect in decreasing the level of total cholesterol,LDL and triglyceride and also has effect in increasing level of HDL plasma with 80% of concentration as the bestconcentration.

Abstract

Patients with urinary catheter have an increased risk to get bacteriuria that could be develop to urinary tract infection(UTI). The aim of this study was to know sensitivity pattern and detection of ESBL in bacteria that isolated from urinof patient with urinary catheter more than 48 hours at Petala Bumi Hospital Pekanbaru. Thirty urin spesiments wasgrowthed in blood agar and MacConkey, incubated for 18 – 24 hours. Identification bacteria was done by some ofbiochemical test, sensitivity test was done by Kirby Bauer method, and ESBL detection done by double disc method.Predominantly bacteria isolated was Gram negative (90%). Gram-negative bacteria was 77.7% sensitive tochloramphenicol and fosfomycin. Gram-positive bacteria was 100% sensitive almost all antibiotic tested. Based onscreening test, this study found E. coli (40%) and Klebsiella sp, Acinetobacter sp and Pseudomonas sp (20%) werepossible produce ESBL, but ESBL was not found based on confirmatory test.
