Abstract

Stroke is classified into two main types based on its pathophysiology, namely ischemic stroke and hemorrhagic stroke. Ischemic stroke, also known as non-hemorrhagic stroke, occurs due to reduced blood and oxygen to the brain and often results in motor impairments such as hemiparesis, postural control deficits, abnormal gait patterns, and decreased selective movement ability. Damage to the basal ganglia plays a signicant role in movement coordination disorders following stroke. This study aimed to determina the effectiveness of physiotherapy management using the New Bobath Concept in improving postural control, gait patterns, and selective movement ability in a patient with left hemiparesis caused by ischemic stroke unvolving the right posterior basal ganglia. This study employed a case study design. Physiotherapy intervention was conducted using New Bobath Concept over six therapy session within a four-week period. Postural control was assessed using a postural grid, gait pattern changes were analyzed through gait pattern analysis, and selective movement ability was evaluated using the Modofied Motor Assessment Scale (MMAS). The results demostrated improvements in postural control and gait patterns, indicated by a reduction in circumduction during walking. Associative reaction movement began to decrease, although they were still briefly observed during the stance and swing phase. Selective upper extremity movement abiluty also improved, with MMAS scores increasing from 28 at the initial assessment (T1) to 42 at the final assessment (T6). These findings suggest that physiotherapy intervention using New Bobath Concept is effective in improving postural control, enhancing gait patterns, and increasing selective movement ability in patients with hemiparesis following ischemic stroke. The New Bobath Concept can be considered a relevant therapeutic approach in post-stroke rehabilitation to optimize functional recovery.

Abstract

Pathogenic bacteria that often cause health problems include Escherichia coli. This bacterium is a Gram-negative bacterium naturally found in the human digestive tract, but some strains are pathogenic and can cause diseases such as diarrhea, urinary tract infections, and infections in the digestive tract. Antibiotic resistance encourages the need to search for alternative antibacterial sources that are safer and more effective. The use of natural materials, particularly medicinal plants, has become one of the widely developed solutions because they are relatively easy to obtain and have lower side effects. Therefore, this research is expected to potentially provide a drug candidate from natural materials. This study showed results of antibacterial activity with high effectiveness, namely at a 50% concentration of 19 mm, while 25% concentration was 17 mm and 12.5% showed 12 mm. It is known that a clear zone above 20 mm indicates that the inhibitory power is very strong. A zone of 16-20 mm indicates that the bacteria's inhibitory power shows that the bacteria are strong in inhibiting bacterial growth. Moderate inhibitory power in bacterial growth is indicated by a size of 11-15 mm, and if the diameter is below 10 mm, it indicates that the bacteria's inhibitory power is weak. In this study, the results showed very good outcomes, with strong inhibitory power, making it a potential alternative in handling diseases caused by E. coli. These results support the potential of the test material to be further developed as an antibacterial agent. To achieve better research results, it is expected that future researchers can conduct tests on other types of bacteria.

Abstract

ISPA is the most common disease suffered by toddlers, namely 78% of toddlers come to health services with ISPA incidents every year, the number of toddlers treated with ISPA incidents is 42.91%. Based on routine reports from the ISPA Sub-Directorate, the ISPA incident is 20.06% in Indonesia and the ISPA incident rate in West Sumatra according to the 2023 Indonesian Health Profile was found to be 29.9%. ISPA cases at the Lubuk Kilangan Health Center in 2024 were found to be 2,319 cases. The purpose of this study was to determine the relationship between the level of maternal knowledge and the environment with the incidence of acute respiratory infections (ARI) in toddlers aged 1-5 years at the Lubuk Kilangan Health Center. This type of research is quantitative with a cross-sectional approach conducted at the Lubuk Kilangan Health Center on February 17-22, 2025. The population of this study was 368 with a sample of 79 using the accidental sampling technique. Data collection using questionnaires, analyzed univariately and bivariately using the Chi-Square test p < 0.05. The results of the Chi-Square test of the relationship between the level of knowledge and the incidence of ISPA in toddlers showed ρ-value = 0.000 < 0.05. More than half of toddlers live in a non-risk environment (95%), more than half of toddlers are exposed to cigarette smoke (91%). The conclusion is that there is a relationship between the level of maternal knowledge and the incidence of ISPA at the Lubuk Kilangan Community Health Center. It is expected that the health center can improve health education for mothers of infants as well as efforts to improve the environment in order to reduce the incidence of ARI in children aged 1–5 years.

Abstract

Background: Femur fracture is damage to the continuity of the femur bone which can be caused by direct trauma, muscle fatigue, or certain other conditions such as bone degeneration/osteoporosis. Objective: The ORIF technique is a surgical technique used to treat fractures by installing internal fixation in the form of plates and screws or nails. Method: This study employed a case study design involving a 56-year-old patient with post-ORIF proximal femur fracture. Exercise therapy was administered over six sessions within three weeks. Outcome measures included the Visual Analog Scale (VAS), goniometer, Manual Muscle Testing (M-MMT), anthropometric measurements, Berg Balance Scale (BBS), 30-second Chair Stand Test, Harris Hip Score (HHS), and Barthel Index. The results were a reduction in tenderness and movement pain, an increase in lower extremity muscle strength and joint range of motion in the right hip, increased balance, endurance, and increased hip functional ability. However, no changes were found in their daily independence. Conclusion: Exercise therapy given to patients with post-ORIF femur fractures can help reduce tenderness and movement pain, increase lower extremity muscle strength, increase dextra hip ROM, increase endurance, and increase hip functionality. However, there is little change in ADL abilities.

Abstract

Diabetes mellitus is a metabolic disorder characterized by hyperglycemia due to impaired insulin secretion or action. The utilization of natural products as antihyperglycemic agents continues to be explored, including andaliman fruit (Zanthoxylum acanthopodium DC.), an endemic plant from Sumatra known to contain bioactive compounds such as flavonoids and saponins. This study aimed to evaluate the antihyperglycemic potential of ethanolic extract of andaliman fruit using an oral glucose tolerance test in male mice. An experimental design was applied with three groups: negative control (distilled water), positive control (glibenclamide), and ethanolic extract of andaliman fruit at a dose of 20 mg/kgBW. Blood glucose levels were measured before and after glucose induction up to 90 minutes. Data were analyzed using One Way ANOVA followed by the Bonferroni post hoc test. The results demonstrated a significant difference among treatment groups (F(2,6) = 19.344; p = 0.002). Post hoc analysis revealed that the andaliman extract significantly differed from the negative control (p = 0.024), but showed no significant difference compared to glibenclamide (p = 0.196). In conclusion, the ethanolic extract of andaliman fruit exhibits antihyperglycemic potential with statistically comparable effectiveness to glibenclamide in a glucose tolerance model. These findings support further development of andaliman as a potential phytopharmaceutical antidiabetic agent derived from local natural resources.

Abstract

Hospitals require optimal human resource performance, both medical and non-medical personnel, to ensure service quality. The performance of non-medical employees at Bhayangkara Level III Hospital Kendari is influenced by various organizational factors such as work facilities, work motivation, and job satisfaction. Initial observations indicate constraints on inadequate work facilities and complaints related to workload that can affect employee motivation and satisfaction. This study aims to determine the relationship between work facilities, work motivation, and job satisfaction with the performance of non-medical employees at Bhayangkara Level III Hospital Kendari in 2026. This type of research is quantitative research with an observational analytical approach and uses a cross-sectional design. The study population was all non-medical employees at Bhayangkara Level III Hospital Kendari, totaling 108 people, with a sample of 84 respondents taken using a simple random sampling technique. Data were collected through questionnaires and analyzed using the Chi-Square statistical test (and the alternative Fisher's Exact Test). The results showed a significant relationship between work facilities and employee performance (p = 0.001) and between work motivation and employee performance (p = 0.005). However, no significant relationship was found between job satisfaction and the performance of nonmedical employees (p=0.120).

Abstract

Endek woven fabric is one of the artistic treasures of Indonesian culture. One of the processes in the production of woven fabric is the midang process, which involves winding threads onto a loom frame through repetitive layering and stacking of aligned threads. The midang process requires precision, speed, and repetition; the work is monotonous and performed while standing. To produce two to four midang results, artisans require two days. The method used was quantitative descriptive analysis, calculating the percentage (%) of questionnaire results using the Nordic Body Map questionnaire and 30 items of subjective fatigue measured on a four-point Likert scale, averaged from 20 midang artisans. The study results indicate an 83.8% increase in musculoskeletal complaints and a 67.0% increase in work-related fatigue following the work period, indicating that standing work postures affect workers’ conditions. Working in a standing position for prolonged periods on a regular basis can cause foot pain, swelling of the soles, lower back pain, and pain in the neck and shoulders. This can lead to cumulative trauma disorders (CTD). To reduce these issues, short breaks are needed between the morning and afternoon shifts. Short breaks can help minimize complaints while using the midang tool.

Abstract

Abdominal tumor is a serious medical condition that can impair food intake and lead to severe malnutrition. Inadequate oral intake due to abdominal pain, nausea, and pre- and post-operative conditions significantly contributes to nutritional status deterioration. To report the implementation of standardized nutrition care process in a patient with abdominal tumor presenting with inadequate oral intake and severe malnutrition due to chronic disease. A case study was conducted over 4 days of observation (August 6–9, 2024) at the Teratai Ward, Prof. Dr. Margono Soekarjo Hospital, Purwokerto. Data were collected through comprehensive nutritional assessment including anthropometric, biochemical, physical/clinical, and dietary history using SQ-FFQ and 24-hour recall. Nutritional interventions included stepwise diet progression (liquid, strained, soft), education, counseling, and team coordination. Daily monitoring of food intake, clinical/physical findings, and anthropometry was performed. Mrs. S (63 years old) had severely underweight nutritional status (BMI 16.6 kg/m²) with 11.1% weight loss over 3 months. Initial recall showed only 35.8% of energy requirements were met. Following 4 days of nutritional intervention, intake improved significantly to 80.9% energy and 98.5% protein on day 4. Body weight was maintained at 40 kg with no further loss. A comprehensive standardized nutrition care process was effective in progressively improving food intake and preventing further weight loss in a post-laparotomy abdominal tumor patient.

Abstract

Patients with cardiovascular disease, particularly Chronic Coronary Syndrome (CCS), face high perioperative risks, including myocardial ischemia, arrhythmias, and hemodynamic instability. Appropriate anesthetic management aims to maintain the balance between oxygen supply and demand as well as hemodynamic stability, including through the use of regional anesthesia techniques as a safer alternative. We report the case of a 71-year-old male patient, ASA III, with CCS accompanied by valvular heart disease in the form of mild aortic regurgitation and moderate pulmonary regurgitation, who underwent Percutaneous Nephrolithotomy (PCNL). Anesthesia management utilized a lumbar epidural technique with catheter placement at the L1–L2 level, 6 cm intradural, and administration of 12 ml of 0.75% ropivacaine. The intended block coverage included the T10–L2 viscerotome and the T8–S2 dermatome. Hemodynamic and pain assessments were performed periodically during the intraoperative period and in the post-anesthesia care unit (PACU). The block depth reached the target within 20 minutes, allowing the surgery to begin immediately. During the 2-hour and 25-minute procedure, the patient’s hemodynamics remained stable without complaints of chest pain or shortness of breath, and no additional anesthetic agents were required. In the PACU, the patient’s sensory and motor functions returned to normal with a NRS pain score of 1. The epidural regional anesthesia technique in a patient with CCS undergoing non-cardiac surgery was shown to facilitate hemodynamic stability intra- and postoperative pain control, thereby potentially significantly minimizing the risk of perioperative ischemia.

Abstract

Diarrhea remains a significant public health problem in Indonesia and is still frequently found in many regions with relatively high mortality rates, particularly among children under five years of age. Toddlers are the most vulnerable group to diarrhea, and this condition can lead to death if it is not treated promptly and appropriately. The occurrence of diarrhea in toddlers can be influenced by various factors, one of which is inadequate environmental sanitation. This research aims to analyze the relationship between basic sanitation and the incidence of diarrhea among toddlers in the working area of Pamandati Community Health Center, South Konawe Regency, in 2026. This research employed an analytical survey with a cros sectional design. The sample consisted of 89 partisipants selected using proportional random sampling. Data were collected through observation and interviews using a questionnaire, and were analyzed using univariate and bivariate analysis. The results of the statistical analysis at a significance level of α = 0.05 indicated that there was a relationship between clean water facilities (p-value = 0.001) and healthy latrine facilities (p-value = 0.036) with the incidence of diarrhea among toddlers. Meanwhile, waste disposal facilities (p-value = 0.389) and wastewater drainage systems (p-value = 0.082) showed no association with the incidence of diarrhea. In conclusion, the availability of clean water facilities and healthy latrines is associated with the incidence of diarrhea among toddlers in the working area of Pamandati Community Health Center in 2026.
