Abstract

Stunting is a long-term growth problem in toddlers caused by malnutrition and requires immediate treatment. One of the main ways to address this problem is through the Supplementary Feeding Recovery (PMT-P) program that uses local food ingredients. This study aims to assess the effectiveness of the PMT-P program in improving the nutritional status of stunted toddlers in Depok Village, Batang Regency, using an input, process, and output approach. This study applied a qualitative method with data collection through in-depth interviews and field observations conducted from April to May 2026. A total of 13 informants were involved, consisting of nutrition program managers, village midwives, village officials, integrated health post (Posyandu) cadres, and mothers of toddlers as sources of triangulation information. The authenticity of the data was tested through source triangulation, then analyzed using descriptive narrative. The results of the evaluation of the input and process components indicate that the availability of cadres, technical competence, and budget collaboration have contributed to program implementation, despite limitations in nutrition education media and animal protein menu variations due to logistical problems with storing fresh ingredients. However, the output and impact components of the program showed positive results, where all recorded stunted toddlers were successfully reached by the PMT-P intervention. In addition, most mothers of toddlers reported that their children were obedient and finished the additional food portions provided regularly. This intervention successfully provided a real positive impact, namely a gradual increase in toddlers' weight. Informants, including village midwives and cadres, perceived changes in the physical condition and improvements in toddlers' weight quite well during the program, in line with the focus of this study which examined the depth of the program implementation process in the field. Thus, it can be concluded that the implementation of the PMT-P program in Depok Village has generally run well in both input and process components, and is perceived to provide positive physical changes in toddlers, although the management of the supply of varied menus and the availability of educational facilities need to be further improved.

Abstract

Organizational culture is an important element in creating a conducive work environment, particularly for healthcare professionals such as nurses. In mental hospitals, organizational culture not only reflects individual perceptions, but also shared values, collective behavior, and interaction patterns that influence the quality of healthcare services. This study aimed to describe the organizational culture among nurses at Aceh Mental Hospital. This study used a quantitative descriptive survey design. The sample consisted of 148 inpatient nurses selected using purposive sampling technique. Data were collected using an organizational culture questionnaire developed based on the organizational culture dimensions proposed by Robbins and Judge, including innovation and risk-taking, attention to detail, outcome orientation, people orientation, team orientation, aggressiveness, and stability. The instrument had undergone content validity through expert judgment and construct validity testing, with Cronbach’s Alpha values ≥0.765 indicating good reliability. The results showed that most nurses had a good perception of organizational culture (92.6%). All subvariables were categorized as good, with the highest percentage in stability (92.6%) and the lowest in outcome orientation (86.5%). The findings indicate that organizational culture at Aceh Mental Hospital has generally developed positively and is supported by collaborative work culture, organizational stability, and the characteristics of nurses. However, this study was limited to descriptive analysis and still has the potential for response bias due to self-report measurement.

Abstract

Blastocystis sp., an intestinal protozoan widely distributed in the human gastrointestinal tract, exhibits a global presence, with prevalence varying by region and generally higher in developing countries compared to developed ones. In Indonesia, reported prevalence varies considerably by region and study population, ranging from approximately 34% to 72% in several regional studies. This protozoan is water-borne and primarily transmitted through cysts via the fecal-oral route. Factors such as poor hygiene, immigration, travel to tropical developing countries, contact with animals, and consumption of contaminated food or water elevate the risk of infection. The pathogenesis in humans remains unclear, as infections may either manifest symptoms or remain asymptomatic, partly due to diagnostic limitations and variation in virulence across subtypes. This review aims to elucidate the characteristics and pathogenicity of Blastocystis sp. subtypes, particularly ST3, which is frequently dominant in symptomatic patients, and to discuss ongoing controversies in the literature.

Abstract

Hypertension is a common chronic condition among the elderly and requires long-term medication adherence to prevent complications. This study aimed to determine the relationship between family support and medication adherence among elderly patients with hypertension at the Outpatient Clinic of Puskesmas Poigar. A quantitative method with a correlational analytic design and a cross-sectional approach was used. The sample consisted of 60 respondents selected through accidental sampling. Data were collected using a modified family support questionnaire and the MMAS-8 medication adherence scale, then analyzed using univariate and bivariate tests with the Chi-square test at a significance level of α = 0.05. The findings showed that most respondents had high family support (41.7%) and high medication adherence (45.0%). The Chi-square test revealed a significant relationship between family support and medication adherence (p = 0.007), indicating that elderly individuals receiving strong family support were more likely to adhere to antihypertensive therapy. These results highlight the essential role of family involvement in improving treatment adherence among elderly hypertensive patients and the need to integrate family-based interventions into community nursing services.

Abstract

Congenital hypothyroidism is an endocrine disorder in newborns characterized by thyroid hormone deficiency, which can lead to impaired growth and permanent neurological developmental disorders if not detected and treated early. Congenital hypothyroidism screening through the measurement of neonatal Thyroid Stimulating Hormone (TSH) levels is an important strategy for early detection to prevent these long-term consequences. This study aimed to describe the profile of neonatal TSH levels in the implementation of a congenital hypothyroidism screening program at a private hospital. This study employed a descriptive design with a retrospective approach. The data used were secondary data in the form of neonatal TSH examination results obtained from laboratory records during the period of April–July 2024. The sampling technique used was total sampling, including all data that met the inclusion criteria, with a total sample of 175 newborns. Data analysis was conducted using univariate analysis to describe the distribution of neonatal TSH levels based on the reference values for congenital hypothyroidism screening. The results showed that all neonatal TSH levels were within the normal reference range, with values ranging from 2 to 12 µIU/mL. No neonatal TSH levels ≥20 µIU/mL were found that would indicate suspicion of congenital hypothyroidism. These findings indicate that there were no signs of thyroid dysfunction among the newborns screened during the study period. The implications of this study confirm that the congenital hypothyroidism screening program has been implemented effectively as an early detection effort. Therefore, routine neonatal TSH screening should be maintained to prevent delayed diagnosis and to support optimal growth and development in infants.

Abstract

Diabetes Mellitus (DM) is a chronic disease that constitutes a major global and national public health problem, with a continuously increasing prevalence and the potential to cause serious complications if not properly managed. One of the key factors in DM management is patients’ level of knowledge, which plays an important role in shaping self-care behaviors and adherence to therapy. This study aimed to analyze the improvement in knowledge among patients with Diabetes Mellitus regarding disease management through health education and leaflet-based media in Pekaroa Village, Sawa District, North Konawe Regency. This study employed a quasi- experimental design using a one-group pre-test and post-test without control group. The population as well as the sample consisted of all patients with Diabetes Mellitus in Pekaroa Village, totaling 8 individuals. The intervention consisted of health education accompanied by the distribution of educational leaflets on Diabetes Mellitus management. Knowledge was measured before the intervention (pre-test), after the first educational session (post- test 1), and after reinforcement of education (post-test 2). Data were analyzed using the wilcoxon signed ranks test. The results showed that the mean knowledge score increased from 2.50 (SD 0.756) at pre-test to 4.38 (SD 0.744) at post-test 1 and reached the maximum score of 5.00 at post-test 2. The wilcoxon test indicated a statistically significant difference between the pre-test and post-test 1 (p = 0.016), while the difference between post-test 1 and post-test 2 was not statistically significant (p = 0.059), although it demonstrated a trend toward improvement and stabilization of knowledge. It can be concluded that health education, reinforced by the distribution of leaflet media, is effective in improving the knowledge of patients with Diabetes Mellitus regarding the management of their disease. Educational leaflets are recommended as a simple, practical, and easily applicable educational strategy in rural communities to support Diabetes Mellitus control.

Abstract

A healthy home is an important factor in public health because the physical condition of the house greatly affects the comfort and health of the people who live in it. This study aims to explain the ownership status of healthy homes based on the physical condition of the building in Sama Subur Village, Motui District, North Konawe Regency. The research method used is a descriptive design with a quantitative approach. The population studied were all heads of families who actively live in Sama Subur Village, with a sample of 45 heads of families selected through a total sampling technique. Data were collected by conducting interviews using a prepared questionnaire and observing the physical condition of the house which includes the building area, type of floor, walls, roof, ceiling, lighting, temperature, ventilation, use of windows, and the condition of the environment around the house. The data analysis process was carried out descriptively by presenting frequency distribution and percentages. The results of this study indicate that the majority of respondents' homes fall into the category that does not meet the requirements for a healthy home, amounting to 55.6%, while only 44.4% meet the requirements. Although most of the physical indicators of the house, such as waterproof floors (80.0%), tightly closed walls (80.0%), waterproof roofs (84.4%), adequate lighting (91.1%), and available ventilation (86.7%) have been met, there are still several deficiencies in the building area, completeness of the house structure, and cleanliness of the environment around the house. The conclusion of this study indicates that the implementation of healthy homes in Sama Subur Village has not reached an optimal level and requires special attention through increased education for the community and support from various sectors to improve the physical quality of houses and the living environment.

Abstract

Occupational safety and health are closely related to employee performance, which can ensure the success of a company, as an efficient workplace environment directly influences employee performance. However, occupational safety and health issues in Indonesia have not received sufficient attention. This problem can be observed from the still high rate of workplace accidents in the country, particularly in the Pagar Merbau palm oil plantation. This study aims to determine the effect of occupational safety and health (OSH) on employee performance in the factory area of the Pagar Merbau Palm Oil Plantation. This a quantitative descriptive design with a survey approach using the total sampling method, involving all 60 factory employees as respondents. Data were collected through a questionnaire that had been tested for validity and reliability. The univariate analysis showed that the majority of employees assessed the implementation of OSH as good (93.3%), and most employees demonstrated good performance (86.7%). A simple linear regression analysis revealed that OSH had a significant effect on employee performance, with a significance value (p) = 0.000 and an R Square of 0.208, indicating that 20.8% of the variation in employee performance is explained by the OSH variable. Thus, proper implementation of OSH can significantly improve employee performance.

Abstract

Research on the safety of remdesivir in Indonesian subjects for COVID-19 therapy is still limited and has yielded different results. None of these studies have used the Naranjo algorithm to analyze the causality of remdesivir safety. This study aimed to evaluate the safety of remdesivir in patients with severe COVID-19. Data were collected using medical records of patients with severe COVID-19 at Cengkareng Regional Hospital, Indonesia. The study was conducted retrospectively with a cohort study design. Subjects consisted of the remdesivir group (n=80) and the non-remdesivir group (n=80). Adverse drug reactions (ADRs) were observed, and causality analysis of ADRs was performed using the Naranjo algorithm. The study found 10 ADRs experienced by patients during treatment. ADRs were more common in the remdesivir group than in the non-remdesivir group (58.8% vs 53.8%; p=0.633). Significant events with a high proportion in the remdesivir group were increased ALT (p=0.037) and lymphopenia (p=0.01). Hepatotoxic reactions associated with remdesivir use were associated with P-glycoprotein (P-gp) inhibitor interactions. Decreased lymphocyte counts were closely related to the clinical condition of the study subjects, namely patients with severe COVID-19, and this condition was associated with a poor prognosis. Increased blood triglycerides, lymphopenia, and decreased estimated glomerular filtration rate (eGFR) were categorized as probable. Meanwhile, nausea, melena, QT interval prolongation, increased ALT, increased AST, hyperuricemia, and neutrophilia were categorized as quite possible. The results of this study can be concluded that remdesivir causes liver dysfunction manifested by increased ALT values ​​and decreased lymphocyte counts, which worsen the condition of patients with severe COVID-19.

Abstract

Optimal postpartum care is fundamental to preventing complications, facilitating maternal recovery, and promoting mother’s independence during the puerperium. Accordingly, comprehensive and evidence-based postpartum nursing care is required. This case study aimed to apply holistic nursing care using the nursing process approach in a normal postpartum patient, Mrs N, in Arafah 2 Inpatient Ward of dr. Zainoel Abidin Regional General Hospital, Banda Aceh. The assessment revealed a blood pressure of 121/76 mmHg, pulse rate of 85 beats per minute, respiratory rate of 22 breaths per minute, body temperature of 36.8°C, the patient presented with a first-degree perineal rupture with-healed suture, lochia rubra, and intermittent perineal pain with a pain score of 4 during movement. The identified nursing diagnoses included postpartum discomfort, ineffective breastfeeding, and knowledge deficit related to postpartum care. Evidence-based nursing interventions consisted of breathing relaxation techniques and Qur’an recitation to reduce postpartum discomfort, oxytocin massage and breast care to enhance breast milk production, as well as nutritional counseling—particularly protein intake—and perineal wound care to improve postpartum self-care abilities. The evaluation demonstrated that postpartum discomfort was resolved, ineffective breastfeeding was partially resolved, and knowledge deficit was resolved. Comprehensive and evidence-based postpartum nursing care is recommended to enhance maternal comfort, support successful breastfeeding, and promote optimal recovery during the postpartum period.
