Abstract

Changes that occur in the elderly include physical changes, including physical disorders such as Rheumatoid Arthritis (RA). RA can be triggered by several stress factors, such as surgery and trauma, and often appears suddenly. The highest incidence of rheumatism is in the elderly, this shows that rheumatism increases with age. One of the non-pharmacological therapies to reduce rheumatic pain is rheumatic exercise. Qualitative research type, descriptive design with case study. Number of samples in the study of 5 respondents with total sampling technique. The data was analyzed and presented in the form of a frequency distribution table. Results: Respondent characteristics based on age showed that the majority were over 60 years old (80%). All respondents were female (100%), and the majority worked as farmers (60%). The average highest pain scale before rheumatic exercise was 4.6 and the highest after 3 days of rheumatic exercise was 3.3. Rheumatic exercise therapy can reduce the pain scale in elderly patients suffering from Rheumatoid Arthritis.

Abstract

Elderly individuals are defined as those who have reached the age of 60 years and above. According to the March 2022 Susenas data, 10.48 percent of the population consists of elderly individuals. One of the main challenges faced by the elderly is making decisions regarding their place of residence. The data shows that 7.10% of elderly individuals live alone, 22.07% live only with their spouse, and 33.66% live with their nuclear family. This study uses a quantitative approach with a cross sectional approach. Data collection was carried out in June 2024 using an instrument, namely a questionnaire sheet distributed directly. The sampling technique is purpose sampling with a sample of 93 elderly respondents aged >60 years, able to communicate well verbally and in writing, willing to be respondents. The variables measured were economic conditions, health conditions, social welfare and the availability of health facilities which were analyzed with data analyzed by the chi-square test. The results indicate that poor economic conditions (82.8%), health status assessed by MMSE and the KATZ index as healthy (100%), and blood pressure measurements as healthy (88.2%) are factors influencing the decision. High social support (84.9%), availability of supporting health care facilities (87.1%), and choosing to live with a spouse or family (92.5%) were also significant. There is a significant relationship between economic conditions (p= 0.0001, OR [95% CI] = 1.778 [1.154-1.239]) and social support (p= 0.0001, OR [95% CI] = 0.500 [0.296-0.884]) with the choice of residence. No significant correlation was found between health status (p= 0.690, OR [95% CI] = 1.093 [1.023-1.168]) and the availability of health care facilities (p= 1.000, OR [95% CI] = 0.880 [0.097-8.018]) with the choice of residence. Economic conditions and social support are significantly related to the choice of residence. Family support and attention to the elderly can improve their well-being.

Abstract

Phlebitis is a nosocomial infection caused by microorganisms and is characterized by clinical symptoms experienced by patients for 3x24 hours during the treatment period in the hospital. Appropriate handling both pharmacological and non-pharmacological in phlebitis can help minimize pain and injury in children. Compress is an alternative non-pharmacological treatment that can be done using several types of compresses, namely warm compresses, 0.9% NaCl compresses and aloe vera gel compresses. This type of research is quantitative research with descriptive analytic research design. The sample used amounted to 15 respondents children who are treated in the aster room of Prof. Dr. Margono Soekarjo Purwokerto Hospital with incidental sampling technique. The main variable in this study is the degree of phlebitis. This research data analysis is carried out interactively, in which there are three components, namely data reduction, data display, and conclusion drawing. The degree of phlebitis in children decreased every day after giving warm compresses, 0.9% NaCl compresses and aloe vera gel compresses and did not cause side effects during the intervention process with an average decrease on days 2 and 3. The results of giving warm compresses to 5 respondents showed a decrease in the degree of phlebitis with an average value of degree of phlebitis 3 to degree of phlebitis 2. While giving 0.9% NaCl compress 1 respondent recovered from phlebitis with a degree score of 0 from the previous degree score of 2. For the administration of aloe vera gel compresses on 5 respondents showed a decrease with an average value of degree of phlebitis 2 to 0 and 4 of the total 5 respondents given the intervention showed a degree score of 0 or recovered from phlebitis. Giving compresses helps reduce the degree of phlebitis in children so that it can be used as an alternative intervention against the incidence of phlebitis in children.

Abstract

This study explores the differences in knowledge, attitude, and practice (KAP) across gender, experience, residence, and ethnicity among 316 nurses. Understanding these variations is essential to tailor effective interventions that promote better knowledge acquisition and application in the long battle against antimicrobial resistance. A cross-sectional study was conducted with 316 participants (89 males, 227 females) to assess KAP metrics. Data were analyzed using incidence rate ratios (IRR) to explore relationships between demographic factors and KAP scores. Correlation analyses were performed to determine the strength of relationships between knowledge, attitude, and practice across various subgroups, including gender, residence (rural/urban), experience levels, and ethnicity. Significant gender differences were found, with females scoring higher in knowledge (IRR=1.03, p=0.024) and attitude (IRR=1.03, p=0.030) than males. Urban participants had significantly higher knowledge scores than rural participants (IRR=1.19, p<0.001). Experience levels also impacted knowledge, with participants having 3-6 years of experience showing lower scores than those with over ten years of experience (IRR=0.89, p<0.001). Attitude positively correlated with knowledge and practice (p<0.05). The findings reveal notable disparities in KAP across gender, experience, and residence. Females and urban participants demonstrate higher knowledge and attitude, while experience correlates with greater knowledge. Interventions should target rural areas and less experienced individuals and focus on improving attitudes to drive better knowledge and practice outcomes. Tailored educational programs addressing these disparities are recommended for more equitable KAP improvements.

Abstract

Lean management is a dedicated, effective system for resource optimization, time efficiency, waste reduction, and care improvement. We aimed to evaluate the implementation of lean management in our hospital setting. We expect significant improvement in time efficiency, particularly in discharge time, administrative collection, and payment mechanism. We randomly collected 494 patients’ medical records regarding time departure after treatment—308 of 494 representing the period before lean implementation. We performed additional nested randomization stratified by hospital class services (first class, second class, third class, and VIP). The results confirmed substantial time reduction (Mann Whitney test < 0.01 with high effect sizes r-rank biserial = .98) in discharge time in the nursing administration and cashier sections before and after lean implementation. Moreover, these findings were consistent across class services (nested-Mann-Whitney <.0.01). The downward waiting time was visible if the patient was hospitalized in the first, second, third, or VIP class. Implementing lean management reduces to 78% waiting time compared to the period before implementation (95%CI IRR negative binomial regression 77-79%). Lean management practice among nurses slashes patient discharge waiting time by 78%, proving its power to streamline hospital operations and boost efficiency across all service classes.

Abstract

Adolescence is a developmental stage where an individual progresses from the onset of secondary sexual characteristics to sexual maturity. During puberty, noticeable changes included rapid body growth, the appearance of secondary sexual characteristics, emotional fluctuations, and, most significantly for adolescent girls, the onset of menstruation. A lack of support from close family members in managing self-care during menstruation is a reproductive health issue that can lead to health disturbances. The purpose of this study was to determine the relationship between family support and self-efficacy towards menstrual hygiene management in female adolescents in grade XI of SMA N 1 Sidareja. This quantitative study was a cross-sectional correlational design. The study was conducted at SMA N 1 Sidareja from October 2023 to July 2024. The sample consisted of 105 eleventh-grade female students, selected using cluster random sampling followed by systematic random sampling. The research instruments included questionnaires assessing family support and self-efficacy in menstrual hygiene management Data were analyzed using the Chi-Square test. The Chi-Square test results indicated a significant correlation between family support and self-efficacy in menstrual hygiene management among adolescent girls at SMA N 1 Sida`reja (p-value = 0.005). This shows that there is a significant relationship between family support and self-efficacy towards menstrual hygiene management with a correlation that has a positive direction, meaning that the better the family support, the higher the self-efficacy towards menstrual hygiene management. It can be concluded that there is a significant correlation between family support and self-efficacy in menstrual hygiene management among eleventh-grade girls at SMA N 1 Sidareja.

Abstract

Diabetes mellitus (DM) increases the risk of fungal infections, particularly in the urine. The genus Candida is the most common cause of fungal infections in immunocompromised patients, including those with DM. The use of automatic identification tools such as Vitek-2 Compact allows for the rapid and accurate identification of Candida species. This study aims to identify the types of pathogenic fungi present in the urine of DM patients. This descriptive study used a cross-sectional design. Fungal examination were conducted macroscopically and microscopically on 15 urine samples from DM patients cultured on Sabouraud Dextrose Agar (SDA) media. Further identification was performed using the Vitek-2 Compact system to determine the species of fungi in positive samples. Culture results releaved that 12 out of 15 urine samples (80%) showed positive fungal growth. Identification using the Vitek-2 Compact system indicated a dominance of Candida species (66,7%), with Candida guilliermondii (33,3%) being the most frequently found species. Other fungal species identified included Candida krusei (16,7%), Candida famata (16,7%), Trichosporon asahii (16,7%), and Kodamaea ohmeri (16,7%). This study found a high prevalence of Candida infections in the urine of diabetic patients, with Candida guilliermondii being the most frequently identified species. The use of the Vitek-2 Compact system proved to be an effective method for the rapid and accurate identification of fungal species, highlighting its importance in clinical diagnostics. These findings emphasize the need for routine fungal screening in diabetic patients, as well as the importance of early and targeted antifungal therapy to prevent the complications associated with fungal infections, particularly in rural settings where healthcare resources may be limited.
