Abstract

Abstract

Latent Tuberculosis Infection (LTBI) is a condition characterized by the absence of clinical symptoms of active disease, yet it carries the risk of progressing to active tuberculosis (TB), particularly within the first two years following infection. The diagnosis of LTBI relies on immunological tests, such as the Tuberculin Skin Test (TST) and Interferon-Gamma Release Assays (IGRA), each with its own strengths and limitations. To compare the sensitivity and specificity of TST and IGRA in the diagnosis of LTBI in household contacts of drug-sensitive TB patients in Medan. This study is an analytical observational study with a cross-sectional design involving 105 subjects who were household contacts of drug-sensitive TB patients. All subjects underwent chest X-ray, TST, IGRA, and Xpert MTB/RIF examination when indicated. Data were statistically analyzed using the Chi-Square test. Among 105 participants, 93 did not have pulmonary TB. Of 93 subjects, 24.7% tested positive by TST and 32.3% by IGRA. TST's calculated sensitivity and specificity compared to IGRA were 76.7% and 63.5%, respectively. TST demonstrates lower sensitivity and specificity than IGRA in diagnosing LTBI. However, TST remains a valuable screening tool, particularly in resource-limited settings.

Abstract

Abstract

Infidelity committed by a man will have a negative impact on his partner, especially on their relationship, the problem of infidelity also has a fairly complicated and vulnerable process so it must be handled immediately because it has two possibilities between separation or forgiveness, but forgiveness also has 3 possibilities, namely forgiving and responding to the cheating behavior, or forgiving but changing the attitude of the cheated on, or even some who forgive and continue to behave normally. Because women will not fully forgive but still have a flaw in trust and even trauma (trust issue) to men who cheat. So the purpose of this study is so that all women who read my research can have the spirit to rise again from trauma and want to try to give men the opportunity to improve harmony in relationships.

Abstract

Diabetes Mellitus (DM) is a chronic metabolic disease with increased blood glucose levels due to impaired insulin secretion or work. According to the International Diabetes Federation (2021), there are around 537 million people with DM in the world, and the number continues to rise. One of the serious complications is diabetic foot ulcers, which are chronic lesions on the feet due to peripheral neuropathy, ischemia, and infection. This paper discusses the case of a 47-year-old woman with a diabetic ulcer after being hit by a motorcycle exhaust. The wound is enlarged, purulent, swollen, and accompanied by severe pain. 7-year history of DM with Blood Sugar At 632 mg/dL and HbA1C 13.6%. Management includes antibiotics, insulin drip, insulin sliding scale, and debridement when blood sugar is <200 mg/dL. Diabetic foot ulcers arise as a result of a combination of neuropathy, ischemia, and infections exacerbated by chronic hyperglycemia. Minor wounds can become severe infections when glucose control is poor. Assessment of severity uses Wagner's classification and degree of infection to determine therapy. Treatment requires a multidisciplinary approach, including blood sugar control, antibiotic therapy, wound care, and debridement. Prevention through education, glucose control, and regular foot care is important to reduce the risk of complications and amputations.

Abstract

Patients with type 2 diabetes mellitus (T2DM) are at increased risk of developing multidrug-resistant tuberculosis (MDR-TB). In Indonesia, data regarding the characteristics and risk factors for MDR-TB among diabetic populations remain limited. Managing MDR-TB in individuals with T2DM is complicated by potential drug interactions and challenges associated with poor glycemic control. This study aimed to assess the association between glycated hemoglobin (HbA1c) levels and the risk of developing MDR-TB among T2DM patients at Adam Malik General Hospital, Medan. A case-control analytical study was conducted using secondary data from medical records. A total of 120 participants were, including 60 MDR-TB cases and 60 drug-sensitive tuberculosis controls. Variables analyzed included age, sex, education, employment status, body mass index, smoking status, and HbA1c levels. Statistical analyses involved chi-square tests and multivariate logistic regression. Multivariate analysis identified smoking as the strongest risk factor for MDR-TB, followed by poor glycemic control, indicated by elevated HbA1c levels. Smoking substantially increases the risk of MDR-TB among patients with T2DM, with smokers being twelve times more likely to develop MDR-TB. These findings emphasize the need to integrate smoking cessation initiatives into MDR-TB prevention and management strategies targeting diabetic populations.

Abstract

The conventional student attendance process at Rancagong 1 Elementary School still relies on manual recording in class journals, which is considered inefficient and prone to errors. The main problem is the delay in information received by parents regarding their children's attendance, making direct monitoring difficult. This research aims to design and build an Android-based student attendance application with parent monitoring features using the Agile method. The Agile methodology was chosen because it enables iterative development with planning, implementation, testing, and evaluation stages. The resulting application has four main access rights: admin, teacher, student, and parent. Black Box Testing results confirmed that all functionalities work according to the design. Evaluation using the System Usability Scale (SUS) with 30 respondents yielded an average score of 87.58, indicating excellent usability. This application proves effective in digitalizing attendance processes, improving teacher efficiency, and strengthening communication with parents through real-time monitoring features.
