Abstract

Type 2 Diabetes Mellitus (T2DM) is a chronic disease with high prevalence in Indonesia, include Bantul Regency,Yogyakarta. In September 2024 the number of cases reached 18,520, indicating a high disease burden at the primary healthcare level. At Puskesmas Imogiri 1, most T2DM patients receive metformin therapy, yet the coverage of care and effectiveness of treatment in achieving optimal glycemic control, particularly HbA1c levels, remain challenging in daily practice. This study aimed to assess the effectiveness of metformin dosage in achieving HbA1c <7% in T2DM patients at Puskesmas Imogiri 1. A quantitative study with a cross-sectional design and total sampling technique was conducted among T2DM patients treated at this facility. The variables studied included type and dosage of metformin therapy and HbA1c levels as indicators of glycemic control, with data collected from medical record reviews, including medication history and the most recent HbA1c test results. Of the 54 respondents, 35.2% achieved HbA1c <7%, while the majority remained above the target. Statistical analysis using Fisher’s Exact Test revealed no significant association between metformin dosage and glycemic control status. Likewise, duration of therapy, sex, and age were not significantly associated with HbA1c achievement (p>0.05). In conclusion, this study indicates that there is no significant relationship between metformin dosage, duration of therapy, age, or sex and the achievement of HbA1c targets in T2DM patients at Puskesmas Imogiri 1.

Abstract

Adolescence is a transitional period characterized by physical, psychological, and social changes that have the potential to influence risky behavior, including the transmission of HIV/AIDS. Indonesia continues to experience an increase in HIV/AIDS cases from year to year, with productive age groups, including adolescents, as one of the most vulnerable. Lack of knowledge and low access to information related to HIV/AIDS are factors that contribute to the low awareness of prevention among adolescents. This study aims to determine the description of knowledge, attitudes, and actions to prevent HIV/AIDS among adolescents at SMA Negeri 8 Kupang City using a descriptive design with a cross-sectional approach. The study population was students of SMA Negeri 8 Kupang, with the number of samples determined based on inclusion criteria and using stratified random sampling. The research instrument was a questionnaire that included aspects of knowledge, attitudes, and HIV/AIDS prevention actions, and data were analyzed univariately to describe the frequency distribution of each variable. The results showed that most respondents had knowledge in the moderate category, attitudes that tended to be positive towards HIV/AIDS prevention efforts, but the application of preventive measures in everyday life was still not optimal, indicating a gap between knowledge and real behavior. Suggestions for adolescents include improving knowledge about HIV/AIDS supported by the development of positive attitudes and environmental reinforcement to encourage consistent preventive actions, while school-based health education and the active involvement of health workers are essential in strengthening HIV/AIDS prevention efforts among adolescents.

Abstract

In 2022, there were 2.3 million women diagnosed with breast cancer and 670,000 deaths recorded worldwide. Women aged 30-49 years have a higher potential to have pre-cancerous lesions so that screening is one of the most effective and cheapest methods to detect cancer cells early. Breast self-examination (BSE) is an effective and easy-to-teach screening if other types of screening are difficult to access. Theory of Planned Behavior (TPB) as a theory of behavioral change that focuses on internal cognitive behavioral factors. This study is a systematic review and meta-analysis study using secondary data that has been published in the last 10 years from 3 databases, namely: Pubmed, Google Scholar and Science Direct. The keywords used were “Breast Cancer Examination” OR “Breast Cancer Screening” AND “Theory of Planned Behavior” OR “attitude” OR “perceived behavioral control” OR “self-efficacy”. The articles used were English articles with a cross-sectional design. Articles were collected using the PRISMA flow diagram and analyzed using the Review Manager 5.4 application. A total of 18 cross-sectional articles from Ethiopia, Thailand, Nepal, Rwanda, East Africa and Nigeria were analyzed using meta-analysis. The total number of research subjects was 58,799. The results of the meta-analysis showed that positive attitudes (aOR = 1.74; 95% CI = 1.35 to 2.23; p <0.00) and strong perceived behavioral control (aOR = 1.31; 95% CI = 1.17 to 1.48; p <0.001) increased the utilization of breast cancer screening. Positive attitudes and strong perceived behavioral control can increase the utilization of breast cancer screening.

Abstract

Breast cancer is recognized as a major global health challenge with 2.3 million cases recorded in 2020 and projections indicating a continuous increase until 2040 (WHO), where early detection becomes a crucial preventive measure that can be introduced through breast self-examination (BSE) education, yet the knowledge of adolescent girls regarding BSE remains low, requiring effective health education strategies; this study therefore aimed to assess the impact of health education using demonstration methods on improving BSE knowledge among adolescent girls at SMK Jayabeka 01 Karawang in 2025 by employing a quantitative approach with a pre-experimental one-group pretest–posttest design involving 103 respondents selected through total sampling, utilizing pretest and posttest questionnaires consisting of 20 items, and analyzing the data with the Wilcoxon Signed Rank Test due to non-normal distribution, with the results showing a significant improvement in knowledge after the intervention (p-value = 0.000, p < 0.05) where 85 respondents experienced an increase, 18 showed no change, and none experienced a decrease, while before the intervention most were in the “moderate” category (67%) and afterward the majority (82.5%) moved into the “good” category, thus confirming that health education delivered through demonstration methods is effective in enhancing adolescent girls’ knowledge of BSE and can be recommended as a suitable and sustainable strategy in school-based programs for early detection of breast cancer.

Abstract

Pulmonary tuberculosis (TB) continues to pose a significant worldwide health concern, particularly when associated with complications. Delays in diagnosis and identification of drug resistance in these cases can increase morbidity and mortality. This descriptive analytical study using cross-sectional methodology aims to explain the results of GeneXpert examination, a rapid molecular diagnostic instrument, in patients with pulmonary tuberculosis with complications at Panti Rapih Hospital, Yogyakarta. In 2024, secondary data collection using a total sampling strategy, which included 660 patients with pulmonary and extrapulmonary tuberculosis, resulted in 132 cases of pulmonary tuberculosis and 32 samples that met the inclusion criteria. The findings showed that the sample was dominated by men (56.3%) of productive age (75%), with pleural effusion as the most common complication. The main GeneXpert finding was that more than half of the samples (59.4%) showed "MTB Detected Low" results that were sensitive to Rifampicin. This profile confirms the urgency of using GeneXpert as a rapid diagnostic tool to determine appropriate treatment strategies early, particularly in complex patients at high risk of therapy failure.
