Abstract

Background: Type 2 diabetes is a metabolic disorder characterized by elevated blood glucose levels because the body is unable to utilize insulin effectively. The incidence of type 2 diabetes increases with age and unhealthy lifestyle. Uncontrolled blood glucose levels in patients can lead to various complications, including microangiopathy, which is a problem in large blood vessels (hypertension). Studies state that type 2 diabetic patients with hypertension have a 7 times greater risk of developing terminal renal failure (ESRD) and 2-4 times of cardiovascular disease, such as myocardial infarction, stroke, or death, compared to normotensive type 2 diabetic patients of the same age. Objective: This study aims to identify the association of blood sugar levels with hypertension in patients with type 2 diabetes mellitus. Method: This study uses quantitative cross-sectional method using observation sheet. The sample in the study was 85 respondents selected by purposive sampling technique. Normality test using Kolmogorow-Smirnov test and for bivariate statistical test using Spearman Rank test. Result: The results showed 53 respondents (62.4%) experienced hyperglycemia, with complications of grade 1 hypertension as many as 33 respondents (38.8%). This result shows that the higher the blood sugar level, the more at risk of hypertension. Conclusion: The conclusion of the study showed a significant relationship between blood sugar levels and hypertension in patients with type 2 diabetes with a p-value of 0.000 (<0.05).

The sample in this study was 40 patients in the inpatient room of Natar Medika Hospital, with a purposive sampling technique. The statistical test in this study used the Wilcoxone test. Results: The results of the univariate analysis obtained results from 40 respondents before being given socialization of the implementation of discharge planning, the most was less than 30 people (75%), after being given socialization, the most was good with 35 people (85.5%), the results of the bivariate analysis showed a p-value of 0.000 <0.05 there was an effect of socialization on the implementation of structured discharge planning at Natar Medika Hospital in 2024, and the implementation of structured discharge planning at Natar Medika Hospital has been running optimally. Conclusion: The suggestion from this study for the management of Natar Medika Hospital is to carry out a supervision process related to the implementation of discharge planning for nurses so that structured discharge planning activities when patients are treated at the hospital until the patient is declared home can run optimally.

Abstract

Background: Hypertension is a degenerative disease that is increasingly common in the community. One of the main problems causing the high prevalence of hypertension is the low awareness and knowledge of the community, which has an impact on non-compliance with treatment. Many people think that hypertension is simply overcome by taking headache medication, which is certainly not appropriate. This phenomenon also occurs in Candirejo Village, Biru-biru District.Purpose: The purpose of this study was to examine the effect of tomato juice consumption on changes in systolic and diastolic blood pressure in patients with hypertension. Methods: This study involved 96 respondents aged 30-65 years who suffered from essential hypertension. The study was conducted from October to December 2023. Respondents' blood pressure was measured five minutes before consuming tomato juice, and then measured again at 30, 60, and 90 minutes after consuming tomato juice. The tomatoes used were ordinary fruit tomatoes consumed by the community. Results: The results of statistical analysis showed the effect of tomato juice administration on reducing systolic and diastolic blood pressure. The largest decrease in blood pressure occurred at 30 minutes after consumption of tomato juice. Conclusion: Giving tomato juice can reduce systolic and diastolic blood pressure in people with hypertension, with the biggest decrease occurring 30 minutes after consumption. These results are expected to benefit the community and can be an alternative solution in the treatment of people with hypertension. This research was carried out from July 22 to September 7 2024. Results: The research that was carried out found that 85 (90.5%) respondents were in the good category in implementing discharge planning. Meanwhile, 88 (93.6%) respondents underwent re-control after hospitalization. The statistical test results obtained p-value = 0.002, which means p<α = 0.005. Conclusion: There is a relationship between the implementation of structured discharge planning and patient compliance with post-hospitalization follow-up. It is hoped that the results of this research can increase nurses' motivation in implementing discharge planning better, so that patients do not experience rehospitalization before it is time for control patients.
