Abstract

PT. XYZ is a food company that is fully committed to the quality of its products. The company experienced a problem with the defect rate of soes products of 4.4% exceeding the maximum limit set by the company of 0.5%. So that efforts are needed to improve product quality and minimize the number of defective products. This study aims to identify in depth the main factors that cause the high level of product defects and formulate recommendations for improvement to significantly improve product quality. The method used is Six Sigma, which systematically identifies problems (define), measures performance (measure), analyzes root causes (analyze), implements improvements (improve), and controls changes (control). Based on the results of the study, it can be concluded that the defect of broken soes due to nozzle misalignment in the cream filling process is the dominant factor causing product defects. Based on the results of the root cause analysis, a number of recommendations for improvement have been formulated and implemented, which have succeeded in increasing the company's sigma level from 3.874 sigma to 4.082 sigma, thus meeting the set target.

Abstract

The purpose of this study is to identify the factors that cause workplace accidents and to develop a plan to address them. A lack of occupational health and safety, unsafe behavior, and unsafe and unhealthy working conditions have led to accidents during the production process of birthday candles at PD. Sinar Baruna Abadi. To prevent such incidents from recurring, it is necessary to implement controls through an accident prevention plan using the Job Safety Analysis (JSA) and HIRADC (Hazard Identification, Risk Assessment, and Determining Control) methods.The methodology consists of three stages: hazard identification through interviews and field observations, followed by risk assessment by analyzing the likelihood and severity of each potential hazard. The study identified 24 potential hazards in the birthday candle production process, categorized by high, medium, and low risk levels.Planning and control were carried out for five risks with varying values. A risk value of 8 involved activities such as pouring materials and removing candles from molds, with risks of burns and blistered hands. A risk value of 9 was associated with removing candles and dipping them into hot paraffin, posing risks of hand burns and respiratory issues. The highest risk (value 10) occurred during the candle dipping process in hot paraffin, with exposure to hot chemical substances that could cause severe burns.Control measures were focused on risks with values of 9 and 10, including the installation of turbine ventilators to reduce exposure to fumes and vapors.

Abstract

The rapid development of development programs in Indonesia, including in the industrial sector, has brought both positive and negative effects. One of the negative effects is environmental pollution, workplace accidents, and occupational diseases, which are caused by an imbalance between progress and the level of awareness regarding occupational safety and health. To ensure comfort and safety in the workplace, a strong work culture is essential. Various methods and tools are used to improve business quality and productivity. One of the most common approaches is the hierarchy of controls, which includes elimination, substitution, engineering controls, administrative controls, and the use of personal protective equipment (PPE).The leather industry company CV. Sinar Brahma Rasyid in Garut faces several occupational safety issues. A total of 33 workplace accidents occurred in 2021, 44 in 2022, and 59 in 2023. In response to these issues, the Job Safety Analysis (JSA) method and the hierarchy of controls were used to identify potential hazards and risks, followed by the implementation of control measures.The aim of this study is to identify the factors contributing to workplace accidents, assess risks, and provide recommendations for improvement in the production department of CV. Sinar Brahma Rasyid. The research findings indicate several causes of workplace accidents, such as employees not using PPE, carelessness, disorganized factory layout, and a lack of tools to assist in material handling.The risk assessment categorized the risks into two levels: low and medium. One example of a medium risk is a workplace accident that can result in serious injury. To reduce workplace accidents and occupational diseases, the study recommends ergonomic chair design, the addition of transport equipment, the use of hazard signs, machine operation training, and the implementation of PPE usage.

Abstract

Company safety is greatly affected by work accidents. PD. Jaya Rasa is an industry in the food sector that produces macaroni where there are potential hazards that can cause losses to the company starting from the production process area which is the main cause of many accidents at work including lan.Therefore, the purpose of this study is to determine the potential hazards that exist in the production area of PD. Jaya Rasa and determine the level of risk of these hazards and propose recommendations for work improvements to improve work safety. The method used is the hazard identification risk assessment and determining control method which serves to identify potential hazards, assess the level of risk and provide recommendations for hazard control. Hazard identification is carried out through field observations and interviews with company owners and employees in the production process area. While the risk assessment is carried out through the stages of hazard identification and data from how often the accident occurs and the impact of the accident which will be calculated using the scale set by AS/NZS: 2004.The results obtained there are 22 levels of hazard risk including 1 extreme risk (4%), 4High risk (18%), 7 Medium risk (31%) and 10 low risk(45%). Where in each existing work activity of the 5 work activities that have the highest and extreme risk level, namely in the work activities of frying, seasoning and gluing macaroni. Then based on the hierarchy of k3 control, the resulting controls for improvement in PD. Jaya rasa are elimination, substitution, administrative design and personal protective equipment.

Abstract

Rapid development in the industry, driven by improvements technology, has complicated the challenges faced. Within sectors manufacturing, difficulties often arise in arranging the layout finished product warehouse. Based on observations at PT Sipatex, the arrangement of finished products in the warehouse is still not well organized, which results in inefficiency. The time required for picking up products in different areas of the warehouse takes longer, and This also makes it difficult for workers to inspect goods which will be sent to customers. The main challenge is currently facing PT Sipatex is a lack of internal organization product arrangement, which can slow down the delivery process and causing some aisles not to match the size of the equipment material handling, making it difficult for forklift operators in pick up finished products at the warehouse. This research aims to provide recommendations for improving product layout in the fabric warehouse so with a focus on reorganizing material handling in order to deal with buildup of finished fabric more effectively, so can reduce product buildup in warehouses. The method used in warehouse management is an approach shared storage, where products with delivery frequency the highest and the ones that move frequently are placed closer to the entrance to minimize handling movement distance material. This shared storage approach successfully reduces average material handling mileage per week of 380.5 meters to 292.7 meters per week

Abstract

This study aims to improve operator discipline in the use of Personal Protective Equipment (PPE) at PT. Baja Kurnia through a participatory ergonomics approach. Quantitative methods were used, involving 48 operators, division heads, and an ergonomics expert in the process of identifying problems and implementing solutions. Based on the hazard and operability study (HAZOP) analysis, 13 potential extreme risks were identified, with the main causes being low knowledge and attitude towards PPE (52.9% each). A visual display design was chosen as the solution because it was easy to understand and feasible to implement. Validation of the solution showed a 29.4% increase in knowledge and a 17.6% increase in positive attitudes toward PPE. These results indicate that visual displays are effective in improving operator work discipline. The practical implication of this study is that visual displays can be part of occupational safety strategies in the manufacturing sector, particularly in strengthening a culture of discipline and accident prevention.

Abstract

Wengku is a semi-finished material used in the production of birdcages. The material used to make wengku is bamboo. The process of making wengku is carried out daily. Over time, artisans have been required to produce more than one bundle of wengku per day. Daily demand does not fall below 3 to 4 bundles. Artisans must find ways to increase production capacity to meet this demand. The high demand can negatively impact the artisans' performance due to fatigue and boredom from repetitive work, resulting in suboptimal production. To improve the efficiency of wengku production, a supporting tool is needed to help artisans become more productive. The purpose of this study is to design an ergonomic bamboo-smoothing tool for wengku production that also increases production capacity. The method used is the Ergonomic Function Deployment (EFD) method, which is an evolution of Quality Function Deployment (QFD). EFD creates a new relationship between customer desires and the ergonomic components of a product. The result of this study is the design of an ergonomic bamboo-smoothing tool for wengku, which can increase production capacity. However, the final tool design differs slightly from the initial concept due to adjustments made based on the available raw materials. Testing of the tool showed that it is functional and effective, increasing production capacity by up to two times. Previously, using a manual process, only one bundle of wengku could be produced per day, but with this tool, production can reach up to two bundles per day.

Abstract

Product quality is one of the main factors that determine the success of a company in meeting customer needs. The main problem found at CV Cemara is the number of defective cracker products with broken and moldy conditions. This study aims to find a quality improvement design to overcome CV. Cemara in the cracker products produced. The data processing method is using Six Sigma with the DMAIC stage with the result of reducing the defect of broken cracker products reduced by 38% or to 11 kg / day. The results of the analysis show that the implementation of the DMAIC cycle and the validation of improvements in making Product Operational Standards (SOPs) in increasing the company's Sigma level, from 2.74 to sigma level 3. This shows that the improvement in making SOPs has successfully overcome the main problems of CV. Cemara.

Abstract

PD.Restu is a company engaged in the production of gelatin. In its production activities, the company always experiences problems related to the quality of the products produced which are caused by a lack of attention to methods, machines and materials in the manufacturing process. The level of defects in the production process still exceeds the specified limits, so quality control is necessary. PD.Restu applies a rework system to damaged products, in which case this will of course require additional costs such as additional material costs, operator costs and rework costs. PD.Restu has many defective products that do not comply with company specifications, the average total defects in gelatin products for the period January-June 2023 is 8%. The aim of this research is to find out the factors that cause the largest defects in gelatin products. This research uses a statistical process control (SPC) approach with check sheet tools, Pareto diagrams, control charts and fishbone diagrams. The research results show that the production process is not yet under control. There are 3 types of defects in gelatin products, namely syneretic defects 45%, non-resilient texture 34% and rough surface 21%. To find out several factors that cause defects, use a fishbone diagram with the results that there are several factors, namely humans, machines and methods. Next, the FMEA stage is to determine the priority scale for the size of the RPN. Based on the results of the RPN, it produces suggestions for improvements by providing training to operators, providing SOPs and evaluating unskilled employees. Then analyze the rework costs for 6 months that arise based on defective products, where there are additional costs from several factors, namely operator costs IDR 35,498,800 and replacement material costs IDR 90,970,594, with total production of IDR 67,480 then the rework cost per 1 bag is IDR 24,325.

Abstract

PT. XYZ is a bakery and pastry company established in 2007. The main problem arises in the production process, where the target number of workers in the marchies division is 104, but only 97 are available. Efforts to reassign personnel from the bagelen team have still not met the target. In addition, employee absenteeism remains an obstacle. To date, the company has never conducted a comprehensive measurement of employee performance. This study aims to measure employee performance comprehensively and improve underperforming indicators using the Human Resource Scorecard (HRSC) and Analytical Hierarchy Process (AHP) methods. The results identified 10 Key Performance Indicators (KPIs): 2 from the financial perspective, 3 from the customer perspective, 3 from internal business processes, and 2 from growth and learning. Based on the AHP results, the customer perspective emerged as the top priority, with the turnover rate as its main indicator. Turnover is in the red category, requiring urgent improvement, while employee absenteeism is in the yellow category, which also needs corrective action. The recommended strategy to reduce turnover is the implementation of a comprehensive onboarding program to help new employees quickly adapt to the company’s culture and work system. Additionally, workload should be evaluated every six months, and the work culture should be strengthened through regular workshops and team-building activities. To address absenteeism, the company can introduce attendance-based incentive programs, analyze the reasons behind absences, improve workplace facilities, and reinforce the attendance policy with clear sanctions and proper leave management.
