Abstract

Disabilitas merupakan salah satu sumber permasalahan hidup bagi penderita kusta yang dapat mengganggu kehidupan sosial dan ekonomi mereka. Sebagian besar masyarakat merasa jijik sehingga penderita kusta dijauhi. Penderita kusta juga tidak dapat bekerja lagi akibat disabilitas yang membuat mereka mengalami kerugian ekonomi yang cukup besar. Penelitian ini dilakukan di Rumah Sakit Kusta Naob yang merupakan satu-satunya rumah sakit khusus kusta di NTT. Hingga saat ini, belum ada data terkait dampak sosial dan ekonomi pada pasien kusta di NTT terutama bagi mereka yang mengalami disabilitas. Penelitian ini bertujuan untuk mengidentifikasi dampak sosial dan ekonomi akibat disabilitas kusta pada pasien kusta dari perspektif pasien. Penelitian ini merupakan penelitian kualitatif. Pengumpulan data dilakukan dengan observasi, wawancara mendalam, dan dokumentasi. Subjek dipilih secara purposive sampling. Data yang dikumpulkan dianalisis dengan menggunakan aplikasi Open Code 4.03. Hasil analisis menunjukkan bahwa sebagian besar subjek mengalami dampak negatif sosial dan ekonomi akibat disabilitas. Namun, dari hasil penelitian didapatkan juga bahwa ternyata subjek yang memiliki tingkat disabilitas tinggi tidak mengalami dampak sebesar mereka yang tingkat disabilitasnya rendah dikarenakan adanya pemahaman yang baik terhadap kusta, baik masyarakat maupun pasien. Oleh karena itu, diperlukan edukasi serta keterlibatan berbagai pihak untuk membantu mengurangi dampak sosial dan ekonomi yang dialami pasien.   Disability is one of the sources of life problems for leprosy sufferers that can disrupt their social and economic lives. Most people are disgusted by leprosy, so lepers are shunned. Leprosy sufferers are also unable to work due to disability, which makes them suffer considerable economic losses. This study was conducted at Naob Leprosy Hospital, which is the only specialized leprosy hospital in NTT. Until now, there has been no data related to the social and economic impact of leprosy patients in NTT, especially those with disabilities. This study aims to identify the social and economic impacts of leprosy disability on leprosy patients from the patients' perspective. This research is a qualitative study. Data were collected through observation, in-depth interviews, and documentation. Subjects were selected by purposive sampling. The results of the analysis showed that most subjects experienced negative social and economic impacts due to disability. However, it was also found that subjects with a high level of disability did not experience as much impact as those with a low level of disability due to a good understanding of leprosy, both in the community and patients. Therefore, education and involvement of various parties are needed to help reduce the social and economic impact of the patient.

Abstract

Metastatic brain tumors, surpassing primary brain tumors in prevalence by tenfold, are the most common brain neoplasms, emphasizing the role of early diagnosis in improving the patient's quality of life. This study aimed to identify the characteristics of brain tumor metastases at Cipto Mangunkusumo National Referral Hospital.  With consecutive sampling, a cross-sectional design study using secondary brain tumor data was conducted at dr. Cipto Mangunkusumo National Referral Hospital from January 2016 to December 2020. Characteristics data were presented in percentages, and bivariate analysis was conducted using Chi-square or Fisher's test. Among 222 subjects (mean age 50.9 ± 10.4 years, 60.8% women), metastases were predominantly supratentorial (50.6%), multiple (65.8%), and associated with mass effects (51.4%), particularly midline shift (69.6%). Primary tumors were predominantly lung (36.5%) , breast (34.2%), and 55.9% experienced metastases in other organs. Brain metastases manifested metachronously in 67.6% of cases, with primary breast tumors showing a higher propensity for metatentorial and infratentorial metastases, while primary lung tumors showed a synchronous onset. The study highlighted associations between mass effects, loss of consciousness -, and poor performance in subjects. Primary lung and breast tumors exhibited the highest incidence of brain metastases.

Abstract

Eschericia coli penghasil extended-spectrum beta-lactamase (ESBL) memiliki tingkat resistensi tertinggi terhadap sebagian besar agen antimikroba dan menjadi penyebab utama kematian terkait resistensi bakteri teerhadap antibiotik. Tujuan penelitian ini ialah menganalisis faktor risiko dan mortalitas kasus infeksi E. coli  penghasil ESBL. Penelitian ini menggunakan desain analitik kasus-kontrol dan  dilakukan di RS Murni Teguh Medan. Sampel penelitian meliputi 174  spesimen pasien rawat inap dengan isolat E. coli pada tahun 2022, yang dibagi menjadi kelompok penghasil ESBL (135 spesimen, 77,6%) dan bukan penghasil ESBL (39 spesimen, 22,4%). Data dianalisis  dengan uji chi-square dan regresi logistik. Hasil regresi logistik menunjukkan perbedaan bermakna pada riwayat penggunaan antibiotik (p=0,003; OR=3,9) sebagai faktor risiko kejadian infeksi E. coli  penghasil ESBL. Selain itu, komorbiditas  merupakan variabel paling bermakna sebagai penyebab mortalitas pada kejadian infeksi E. coli (p=0,007; OR=8,2). Terdapat perbedaan bermakna pada mortalitas antara ESBL dan non-ESBL (p=0,028;=OR 5,9). Infeksi E. coli penghasil ESBL memiliki risiko yang bermakna terkait riwayat penggunaan antibiotik dan komorbiditas, dengan mortalitas yang jauh lebih tinggi dibandingkan kelompok non-ESBL. Hasil tersebut menegaskan pentingnya penguatan program penatagunaan antibiotik di rumah sakit untuk mencegah dan mengendalikan resistensi antimikroba.

Abstract

COVID-19 is a viral infection that affects not only the respiratory system but also other organs of the body. Ocular manifestations due to COVID-19 infection and its vaccination have been reported, including cases of neuro-ophthalmology. This may be due to the neurotropic and neuroinvasive characteristics of the virus. This case series aims to investigate the characteristics of neuro-ophthalmology cases that may function as a guideline and warning to the examiner. We found ten COVID-19 infection cases, consisting of five optic neuritis cases with two pediatric patients, two patients with cavernous sinus thrombosis (CST), two patients presenting with visual field defects, and one patient with sixth nerve palsy. Vaccine-related-wise, we found two patients presenting with optic neuritis, one patient with CST and one with visual field defect, all with a history of getting vector virus vaccine. The majority of the patients in this report were male, with two pediatric patients in total. The most common neuro-ophthalmology manifestations related to COVID-19 infection were optic neuritis, followed by CST, visual field defect, and cranial nerve palsy. Optic neuritis was also the most common case pertaining to vaccination, followed by CST and visual field defect.

Abstract

The abnormal genetic makeup of a genetic disease will be reflected in the dermatoglyphic pattern. Early detection and diagnosis of autism spectrum disorder (ASD) are necessary to help ASD individuals receive early intervention so that delays in communication, social, and behavioral development can be addressed. This review aims to determine whether loop fingerprint patterns can be a component of ASD risk screening. Systematic search on a database and a manual handsearching from articles which were not indexed in the major electronic databases to identify articles reporting the association of fingerprint patterns with ASD. Data extracted from each research article included author name, study characteristics, sample characteristics, ASD diagnosis criteria, fingerprint scanning method, and frequency of fingerprint findings. After the selection process, three articles were obtained for analysis. This review found that two studies reported that the ASD group had a significantly higher frequency of loop patterns on the left index finger. Additionally, one study reported that the ASD group had a higher frequency of arch patterns on all fingers of the right and left hands. This systematic review concludes that the loop fingerprint pattern on the left index finger may be an additional component in screening for ASD risk.

Abstract

Primary hyperparathyroidism is a common endocrine disorder that often requires parathyroid-dectomyas a curative measure. However, post-operatively, patients are susceptible to severe and prolongedhypocalcemia, known as post-parathyroidectomy hypocalcemia syndrome (HBS), which can prolong the lengthof stay and increase the cost of care. Preoperative use of bisphosphonates has been proposed as a potentialtherapy to reduce the incidence of HBS. This EBCR aims to determine the effectiveness of preoperativebisphosphonate administration in reducing the risk of HBS in patients with primary hyperparathyroidismundergoing parathyroidectomy. A systematic literature review conducted through three electronic databasesand critical analysis using FAITH tools from the Central of Evidence-based Medicine, University of Oxford,showed that pre-operative administration of bisphosphonates significantly reduces the occurrence of HBSwith an overall NNT (Number Needed to Treat) of three. However, further research is needed to validatebisphosphonates’ effectiveness and safety in this context. Overall, these EBCR suggest that bisphosphonatescan be considered as part of a preventive strategy for HBS in patients with primary hyperparathyroidismundergoing parathyroidectomy.

Abstract

Refractive errors, notably myopia, are highly prevalent in Asian countries, with a notable increase observed in the pediatric population. This surge is attributed to lifestyle changes such as less time outdoors and increased exposure to digital screens and near-work activities, which are known to elevate myopia risk. A retrospective review of medical records was conducted at the Department of Ophthalmology, dr. Cipto Mangunkusumo Hospital, Jakarta, Indonesia, from 2020 to 2022, identifying 290 eyes of 146 children with refractive errors. Myopia emerged as the most prevalent refractive error, affecting 53.4% of eyes in this study. The median age of the participants was 8 years old, with 56.8% being females and 65.8% residing in Jakarta. Early detection and management of refractive errors are crucial to prevent irreversible visual impairment in children. Further multi-centered studies are recommended to comprehensively understand pediatric refractive errors in Indonesia.

Abstract

Hemofilia adalah penyakit genetik berupa gangguan pembekuan darah yang dapat memengaruhikualitas hidup. Perawatan seumur hidup anak dengan hemofilia membutuhkan pemahaman mendalamterhadap beban yang dialami orang tua selaku pengasuh dan pendampingnya. Penelitian ini bertujuanuntuk mengetahui beban para orang tua pasien anak dengan hemofilia. Penelitian ini menggunakan desainkualitatif dengan pendekatan studi kasus. Penelitian dilakukan pada Juni-Oktober 2023 di tempat domisiliorang tua di Yogyakarta dan RSUP Dr. Sardjito untuk wawancara narasumber tenaga kesehatan Empatnarasumber utama adalah ibu dari anak penyandang hemofilia. Triangulasi data didapatkan dari narasumberlain yaitu dua tenaga kesehatan yang sering berinteraksi dengan anak dari narasumber utama. Pengambilandata menggunakan wawancara semi terstruktur. Analisis data menggunakan metode analisis tematik. Hasilpenelitian menunjukkan bahwa beban orang tua dari anak penyandang hemofilia di Yogyakarta meliputibeban ekonomi terkait biaya transport, makan, dan internet untuk anak saat perawatan di rumah sakit, bebanpsikis yang berkaitan dengan perasaan bersalah dan kecemasan terkait kehidupan anak, beban medis akibatkesulitan dalam penyuntikan obat karena obesitas pada anak, beban sosial yang berhubungan dengan verbalbullying dan rendahnya pengetahuan lingkungan sekitar terkait hemofilia. Selain beban yang dirasakan, orangtua juga mendapat dukungan dari berbagai pihak seperti Jaminan Kesehatan Nasional, dukungan sesamaorang tua pasien hemofilia, dan pendampingan psikologis dari para tenaga kesehatan.

Abstract

The visual system has complex connections with all brain areas and can be affected by a brain tumor. Neuro-ophthalmological examinations are important because early diagnosis and managing small tumors will improve outcomes. This study aimed to investigate brain tumors' neuro-ophthalmic (NO) manifestation and the affecting factors. A cross-sectional study using total sampling was conducted in patients undergoing pre-operative examinations from January 2014 to December 2019 in dr. Cipto Mangunkusumo National Referral Hospital. Variables studied in this paper are brain tumor characteristics, NO manifestations, and the relationship between them. Analysis was conducted using chi-square or Fisher's exact test and post hoc analysis. All subjects presented with NO manifestations, with fundoscopic abnormalities (72.9%) as the most common manifestation. The relationships between the tumor type with visual field disturbances (p=0.002) and multiple tumor lesions with funduscopic abnormalities (p=0.043) were significant. Lesion location also had a significant relationship with all NO variables (p<0.05). The NO manifestations were found in all subjects, predominantly the fundoscopic abnormalities. The tumor type, number of lesions, and location of lesions had a significant relationship with NO manifestations.
