Abstract

Aging brings with it the consequence of an increase in various cardiovascular diseases, one of which is hypertension. Long-term use of antihypertensive drugs can cause various side effects, one of which is mental changes, thus non-pharmacological therapies such as music therapy are needed. There are many types of music therapy that can be applied, one of which is traditional music (gamelan). There are inconsistencies in previous research, so a combination of Balinese gamelan selonding music therapy with VR or Virtual Reality is needed. VR has become one of the leading innovations in the field of health and wellness, especially in the context of therapy and rehabilitation. The purpose of this study is to determine the effectiveness of VR-based Balinese gamelan selonding music therapy in reducing anxiety and blood pressure in the elderly with hypertension. The research method uses a quasi-experimental pre-post control group design. The sampling technique is purposive sampling according to inclusion and exclusion criteria. The instruments in this study used an anxiety questionnaire, tensimeter, MP3/handphone, earphones and a VR Box. The results of this study show that the provision of VR-based Balinese gamelan selonding music therapy can reduce anxiety and blood pressure in the elderly.

Abstract

Nanoemulsions using vegetable oils as excipients have gained attention for their ability to improve the delivery and stability of bioactive compounds in pharmaceutical and food applications. This study aims to systematically compare various methods for preparing nanoemulsions with vegetable oil excipients. A narrative literature review was conducted, focusing on articles published between 2021 and 2025 from Google Scholar, PubMed, and ScienceDirect. The population comprised all relevant scientific articles, with purposive sampling based on inclusion criteria such as active DOI and accessibility. Data were analyzed qualitatively using content analysis to identify, group, and compare preparation methods, physicochemical characteristics, and product stability. Results indicate that high-energy methods like ultrasonication and high-pressure homogenization produce nanoemulsions with smaller, more uniform droplets and greater stability, while low-energy methods such as spontaneous emulsification are simpler but may yield less stable products. The choice of vegetable oil and surfactant significantly affects droplet size and stability. In conclusion, optimizing both the oil type and preparation method is essential for developing effective nanoemulsion-based products.

Abstract

Hypertension is a major global health problem with increasing prevalence and significant risks of complications. This study aims to compare the effectiveness of Captopril 25 mg and Amlodipine 5 mg in lowering blood pressure among hypertensive patients. An analytical observational study with a cross-sectional design was conducted at Waruroyom Community Health Center, involving 70 patients divided equally into two groups. Data were collected using structured questionnaires, medication adherence sheets, and calibrated digital blood pressure monitors. Univariate and bivariate analyses were performed, including paired T-tests and Mann-Whitney tests. The results showed that both Captopril and Amlodipine effectively reduced systolic and diastolic blood pressure. However, Amlodipine 5 mg demonstrated a significantly greater reduction in diastolic blood pressure compared to Captopril 25 mg, while both drugs had similar effects on systolic blood pressure. The conclusion is that Amlodipine may be preferred for patients requiring greater diastolic blood pressure control, although both drugs are effective for systolic reduction.

Abstract

Total Knee Replacement (TKR) is a surgical procedure aimed at replacing damaged knee joints due to osteoarthritis or other conditions causing pain and dysfunction. This study aims to describe the clinical functional outcomes after TKR based on age and gender among patients at Royal Prima General Hospital, Medan. A descriptive cross-sectional design was used, involving 30 patients who underwent TKR between May and April 2025. Data were collected from medical records and assessed using the Oxford Knee Score (OKS). Statistical analysis was performed to evaluate the distribution of functional outcomes. The results showed that most patients were aged 71–82 years (53.5%) and female (76.7%). Younger patients (56–60 years) achieved better clinical function (Excellent category), while older patients (71–82 years) tended to have Moderate scores. Females demonstrated better clinical outcomes than males. The study concludes that age and gender influence post-TKR clinical function, with younger and female patients showing superior outcomes

Abstract

Background: Acute appendicitis is a common surgical emergency characterized by inflammation of the appendix, requiring timely diagnosis to prevent complications. The Alvarado score is a simple, inexpensive clinical tool designed to aid early diagnosis, while histopathological examination remains the diagnostic gold standard. This study explores the relationship between preoperative Alvarado scores and postoperative anatomical pathology results at Royal Prima Marelan Hospital in 2024. Methods: This cross-sectional analytic study analyzed medical records of 89 patients diagnosed with acute appendicitis who underwent appendectomy from January to December 2024. Data on demographic characteristics, Alvarado scores, and histopathological outcomes were extracted. Chi-square tests assessed correlation, while sensitivity and specificity analyzes evaluated the diagnostic performance of the Alvarado score. Results: Most patients were aged 15–24 years (55.1%) and female (56.2%). Alvarado scores ≥7 were observed in 85.4% of cases. Histopathology confirmed acute appendicitis in 95.5% of patients and chronic inflammation in 4.5%. A significant association was found between Alvarado scores and pathology results (p=0.000; OR=24.485). The Alvarado score demonstrated high predictive value for acute appendicitis. Conclusion: There is a strong correlation between preoperative Alvarado scores and postoperative pathological findings in acute appendicitis. The Alvarado score is a practical tool to improve diagnostic accuracy and reduce unnecessary surgeries. Future research should validate these findings with larger, prospective studies.

Abstract

Background: Medical students are exposed to high academic demands, which often lead to poor sleep quality and may impact cognitive function and academic achievement. This study aimed to analyze the relationship between sleep quality and grade point average (GPA) among medical students at Prima Indonesia University, class of 2023. Methods: A quantitative analytic approach with a cross-sectional design was used. The population included all active students from the 2023 cohort, with 162 respondents selected by simple random sampling. Sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI), and GPA data were obtained from official academic records. Data analysis involved univariate and bivariate statistics, with normality tested using Kolmogorov–Smirnov and correlation assessed by Spearman's rank test. Results: The majority of students (63.6%) had poor sleep quality (PSQI > 5), and the average GPA was 3.29. Spearman analysis showed a moderate negative correlation (rₛ = –0.505, p < 0.05) between sleep quality and GPA, indicating that poorer sleep quality was associated with lower academic achievement. Conclusion: Poor sleep quality significantly affects academic performance in medical students, highlighting the need for sleep hygiene education and time management interventions.

Abstract

Background: Dietary patterns strongly influence Body Mass Index (BMI), a key anthropometric measure for nutritional status. Medical students face dietary disruptions from academic stress and irregular schedules, increasing obesity risk. Objective: To examine the association between dietary patterns and BMI among Universitas Prima Indonesia medical students, class of 2022. Methods: A quantitative correlational cross-sectional study was conducted on 198 randomly sampled students. A 15-item dietary pattern questionnaire (Cronbach's α = 0.959) and measured weight and height data were used. Data were analyzed with univariate statistics, the Kolmogorov–Smirnov normality test, and Spearman correlation. Results: Fifty-one percent of students had good dietary patterns, while 61.1% exhibited non-normal BMI (overweight or obese). Spearman analysis showed a strong negative correlation between dietary patterns and BMI (r = –0.639, p < 0.001). Conclusion: Better dietary patterns are significantly associated with normal BMI. Interventions promoting healthy eating and continued nutritional education are recommended.

Abstract

The present case report describes the clinical features, diagnostic findings, and management of a 61-year-old female patient diagnosed with Chronic Suppurative Otitis Media (CSOM) of the right ear, benign type. CSOM is a persistent inflammation of the middle ear characterized by continuous or recurrent ear discharge through a tympanic membrane perforation lasting more than two weeks. The patient presented with right ear discharge for approximately four months, initially serous and later purulent with occasional blood, accompanied by ear pain, fullness, tinnitus, and mild dizziness. Physical examination revealed a central perforation of the tympanic membrane with mucopurulent discharge, supporting the diagnosis of benign-type CSOM. Treatment consisted of aural toilet and systemic antibiotic therapy using levofloxacin 500 mg twice daily combined with cetirizine 10 mg once daily for one week, alongside education to maintain ear hygiene and avoid water exposure. Clinical improvement was observed with resolution of otorrhea and no further complications. Prognosis was good (ad vitam bonam), while functional and curative outcomes were moderately favorable (dubia ad bonam). This report emphasizes the importance of early diagnosis, adequate antimicrobial therapy, and patient compliance in achieving a dry and safe ear condition while preventing recurrence or long-term hearing impairment in CSOM.

Abstract

Ossification of the ligamentum flavum (OLF) is characterized by the replacement of ligamentum flavum tissue with mature laminar bone accompanied by hypertrophy. Although rare, its highest prevalence is reported among East Asian populations, predominantly affecting males under 50 years old, and most commonly found in the lower thoracic region. Clinically, OLF presents as a slowly progressive myelopathy, yet its precise pathogenesis remains unclear. Diagnostic imaging modalities include radiography, computed tomography (CT), and magnetic resonance imaging (MRI). This case report describes a 42-year-old female diagnosed with thoracolumbar OLF based on MRI examination. The patient presented with spastic paraparesis and hypoesthesia from both feet to dermatomes Th10–11 following a history of trauma. Lumbar radiography showed only lumbal spondylosis, while thoracolumbar MRI revealed nodular thickening of the ligamentum flavum from Th6–7 to Th12–L1, resulting in spinal canal stenosis and compression of the spinal cord. Additional degenerative findings included superior endplate degeneration of L4 and multiple intervertebral disc bulging. Neurological deficits indicated myelopathy and radiculopathy due to compression of the lateral corticospinal and spinothalamic tracts. The absence of spinal cord edema on MRI suggested a chronic onset. OLF frequently coexists with vertebral degenerative abnormalities, as observed in this patient. Based on the radiologic characteristics, the lesion represented several morphological types including extended, enlarged, fused, and tuberous forms. Laminectomy performed in this case resulted in gradual neurological recovery. In conclusion, OLF is a rare but important cause of progressive thoracic myelopathy that may lead to irreversible neurological impairment. MRI evaluation plays a crucial role in preoperative assessment of spinal canal stenosis and spinal cord compression.

Abstract

The present study reports a comparative evaluation of the antidiabetic potential of ethanol extracts from Syzygium polyanthum (bay leaves) and Moringa oleifera L. (moringa seeds) based on their inhibitory activity against the α-glucosidase enzyme. Diabetes mellitus, a chronic metabolic disorder characterized by hyperglycemia due to impaired insulin secretion or action, can be managed through enzyme inhibition that delays carbohydrate hydrolysis. The investigation was conducted for five weeks using an in vitro experimental method, in which both plant materials were extracted via maceration with 70% ethanol. The α-glucosidase inhibition assay was performed spectrophotometrically at 410 nm, and the half-maximal inhibitory concentration (IC₅₀) was determined to compare the inhibitory capacities of both extracts. Phytochemical profiling revealed that bay leaf extract contains flavonoids, tannins, saponins, and steroids, while moringa seed extract contains flavonoids, alkaloids, and steroids. Results showed that bay leaf extract exhibited strong α-glucosidase inhibition with an IC₅₀ value below 100 ppm, indicating a significant potential as a natural antidiabetic agent. In contrast, moringa seed extract demonstrated weaker activity with no substantial inhibition observed at the tested concentrations. Based on comparative analysis, bay leaf extract shows a higher inhibitory efficiency, suggesting its potential application in herbal antidiabetic formulations. Further studies are recommended to explore the optimal concentration range and bioactive constituents of moringa seeds.
