Abstract

Hypertension becomes a problem in old age, Elderly people with hypertension often experience decreased cognitive function and have an impact on quality of life . Management of hypertension with problems of decreased cognitive function and quality of life can be done with non-pharmacological therapy. One of the non-pharmacological therapy that can be applied to the elderly who have hypertension is chair based exercise. The purpose of this study was to see the effect of chair-based exercise on cognitive function and quality of life in the elderly with hypertension.The design of this study used quasy experiments with pre and post control groups with a large sample of 16 respondents in each group. The sampling technique used is consecutive sampling with independent variables, namely chair based exercise, Dependent variable in this study were cognitif function and quality of life. The research instruments used are Instrumen mini mental state exam (MMSE), quality of life instrument for elderly with hypertention. The research was carried out at the Budi Sejahtera Banjarbaru Elderly Social Protection and Rehabilitation Institution (PPRSLU). The statistical test used in this study was wilcoxon signed test. The test results found that There was an effect of chair based exercise on the cognitive function of the elderly in the intervention group, but there was no effect in the control group. There was an effect of chair based exercise on the quality of life of the elderly in the intervention and control groups. In elderly with hypertension, nurses need to help the elderly so that the problem of hypertension is resolved and able to fight disability. Chair based exercised can be applied for the kognitif function and quality of life.

Abstract

Introduction & Objectives: Cryptorchidism is one of the most frequent congenital birth defects in male children and is present in 2–4% of full-term male births. The study aims to validate the associated genomic information to pick up and catalog life expectations with our community's associated syndromes and pathologies. Materials & Methods: The study was carried out between the 1st of June 2014 and the end of December 2018. G-banding of metaphase chromosomes and high-resolution karyotype analysis was performed in all patients using 5 DNA probes, ordered in the deletion intervals, and correlated with the cytogenetic map of the chromosome and sequence tagged size (STS) in the molecular study of microdeletion and microduplication using fluorescence in situ hybridization (FISH). The data were translated to the Statistical Package for Social Science (SPSS) version 22, Excel 2016 . Results: Out of 124 managed and operated on patients, genitourinary anomalies were the most common associated pathologies in 44% of cases, and hypospadias was the most common in 21.77%. Developmental delay and intellectual disabilities with speech disorders are considered the second one 24.19%. Y chromosome microdeletions were observed in 75% of all, the most considered one Y q1.2 D in 45.16%, p-value < 0.05. X p11.2 micro duplications were observed in 8.78% of the cases. Higher ascent of the testis presented with more microdeletion (intra-abdominal 35.48% of the patients) p-value < 0.05.  With the STS, sY84 & sy86, and the loci, DYS 273 & 148 the azoospermia factor A region (AZFa) was estimated in 39.51% of the patients. Conclusions: Genomic information and screening are recommended for bilateral   cryptorchidism as the chromosomal microdeletions and microduplications have been associated with a spectrum of pathologies, considering this with a catalog of the multidisciplinary team and complete disease network to follow up the clinical pictures after surgery.

Abstract

This study analyzes the legal principles underlying insurance coverage and economy risk classification in modern insurance practices. The principles of insurance law: utmost good faith, insurable interest, indemnity, subrogation, proximate cause, are important foundations in regulating the relationship between the insurer and the insured. On the other hand, risk classification plays a crucial role in the assessment, premium determination, risk selection, and risk management by insurance companies. The method used is a literature study with a normative approach, examining regulations, legal doctrines, scientific literature, and practices in the insurance industry. The results of the study indicate that a deep understanding of the principles of insurance law and risk classification can improve transparency, contractual fairness, moral hazard risk mitigation, operational efficiency in the insurance industry. Risks in insurance are classified into two main categories, namely pure risk which is related to the possibility of loss alone and speculative risk which involves the possibility of profit or loss. The six basic principles of insurance law, insurable interest, utmost good faith, proximate cause, indemnity, subrogation, contribution are the foundation for the formation and implementation of a valid insurance contract. The development of digital technology, the use of big data, artificial intelligence, machine learning in the insurance sector, has brought significant changes to risk classification methods, underwriting, and claims assessment processes. In conclusion, a comprehensive understanding of legal principles and risk classification is essential to ensure legal protection, increase public trust, and support the sustainability of health and general insurance practices in the modern era for Indonesia.

Abstract

Background: Platelet transfusion plays a role in preventing and treating bleeding in patients with thrombocytopenia. Transfusion failures can sometimes reach 24-44%. The success of platelet transfusion is assessed using several formulas, including Post-transfusion Platelet Increment (PPI), Percentage Platelet Recovery (PPR), and Corrected Count Increment (CCI). CCI is more accurate and most widely used to assess transfusion success. Transfusion is considered successful if the CCI value one hour after transfusion is ≥ 7.5x10^9/µL or ≥ 4.5 x10^9/µL after 24 hours. Purpose: The aim of this study was to analyze the effectiveness of platelet transfusion, using Corrected Count Increment (CCI) after apheresis platelet transfusion in patients with thrombocytopenia and identify factors that influence this outcome, such as age, gender, and underlying diagnosis at Wahidin Sudirohusodo Hospital Makassar. Methods: This study is a retrospective study by taking secondary data from medical records of patients aged ≥ 18 years, who having weight and height / length data and routine blood counts results for thrombocytopenia <150,000 / µL at Wahidin Sudirohusodo Hospital Makassar from January 2023 to December 2023. The research sample was analyzed using the results of CCI calculations using data processing software. Data analysis using the Kolmogorov-Smirnov test for data normality, the chi Square test, Wilcoxon statistical test results are significant if the p-value <0.05. Results: The results of the data analysis found proportion of CCI after apheresis platelet transfusion was found to be significantly higher (p < 0.001) in the group with increased platelet values. The results of CCI analysis with gender, age, malignant and non-malignant diagnoses showed no significant relationship (p > 0.05). Conclusion: In this study, the effectiveness of apheresis thrombocyte transfusion can be assessed by CCI, which is influenced by height and weight, as well as an increase in platelet values. There is no relationship between gender, age and diagnosis (malignancy and non-malignancy) with CCI values.

Abstract

Menarche is the first menstruation experienced by teenage girls. This period is a transitional period marked by various physical, emotional and psychosocial changes. Efforts to prevent the negative impact on menarche require knowledge about menstruation, one of which is using animated video media and snakes and ladders games. The aim of this research is to identify the influence of health education using the media game Snakes and Ladders and animated videos on the readiness of young women to face menarche. This type of research is quasi-experimental with pre and post test control group design. The research sample consisted of 40 female students using probability sampling with simple random sampling technique. Data analysis used the Paired T-Test and Independent T-Test. The results of the univariate test showed that the pre-test and post-test mean scores in the intervention group were 47.60 and 69.90 and the pre-test and post-test mean scores in the control group were 47.65 and 59.95. Bivariate analysis using the Paired T-Test in the intervention group and control group obtained a result of 0.000 where the p-value <0.05, meaning that there was an influence of health education using snakes and ladders game media and animated videos as well as powerpoint and leaflet media on the readiness of young women in facing menarche. The Independent T-Test showed a result of 0.000 where the p-value <0.05, meaning that there was a significant difference between the mean value of young women's readiness to face menarche in the intervention and control groups. The conclusion that can be drawn is that health education using the media game snakes and ladders and animated videos has an effect on the readiness of young women to face menarche. Animated video media and snakes and ladders games are more effective than PowerPoint and leaflet media in increasing the readiness of young women to face menarche.

Abstract

Introduction: The Hospital Without Walls (HWW) concept has become a strategic approach in the development of health services oriented towards efficiency and affordability, especially in challenging geographical areas such as Talaud Regency. HWW aims to expand the reach of hospital services beyond the physical boundaries of buildings by optimizing technology, cross-sector collaboration, and community participation. This study was motivated by the high number of visits of internal medicine patients to RSUD Mala without going through the referral mechanism from FKTP. This study aims to analyze the application of the HWW concept in internal medicine poly health services, analyze the application of the concept in reducing cold case rates, and identify obstacles and challenges that occur in its application at the RSUD Mala, Talaud Regency. Method: Huberman model. The research was conducted in March-July 2025 at RSUD Mala, Talaud Regency. Data collection techniques were carried out through in-depth interviews with five main informants, namely the hospital director, internal medicine specialist, family doctor, health center, doctor, and patient. Data was also strengthened by observation and documentation as an effort to triangulate sources and methods. Results: The implementation of HWW in RSUD Mala has started partially but has not been systematic. The hospital has started to apply HWW principles such as remote consultation and digital communication, but does not yet have operational SOPs and HR support and management systems in place. Informants see great opportunities in the development of this concept. In the context of cold cases, HWW plays an important role in initial screening through cooperation between primary care facilities and hospitals, education, back-referral, and utilization of telemedicine. Internal barriers include the absence of SOPs, limited facilities, and lack of HR training. External barriers include the absence of BPJS financing regulations and the lack of public understanding of HWW. Conclusion: RSUD Mala has great potential to develop HWW-based services, but requires systemic intervention in terms of policy, human resources, hospital information system development, and public education. The implementation of HWW can improve service efficiency, reduce hospital burden, accelerate case handling, and improve patient satisfaction.

Abstract

Patient satisfaction serves as a critical indicator of hospital service quality and overall performance. In Indonesia, government hospitals are required to adhere to public service standards, including routine evaluation through the Community Satisfaction Index (IKM). This study examines patient satisfaction among inpatients at RSUD Maria Walanda Maramis, North Minahasa, using four core service quality dimensions: system and procedure clarity, service completion time, staff competence, and infrastructure. To analyze the relationship between selected service quality variables and the level of patient satisfaction in a Type C public hospital. Methods: This research employed a quantitative, cross-sectional design involving 95 inpatients selected using simple random sampling. Data were collected via a structured questionnaire adapted from Permenpan RB No. 14/2017. Bivariate analysis using the Chi-square test was conducted to determine the significance of associations, and Odds Ratios (OR) were calculated to measure the strength of influence of each variable on satisfaction. Results: All four independent variables showed significant associations with patient satisfaction (p < 0.05). The strongest influence was found in the clarity of service systems and procedures (OR = 14.0), followed by infrastructure (OR = 4.219), staff competence (OR = 3.889), and turnaround time (OR = 3.372). Infrastructure received the highest percentage of positive responses (72.6%), while staff competence received the lowest (51.6%). Conclusion: Patient satisfaction at RSUD Maria Walanda Maramis is significantly influenced by administrative clarity, service efficiency, human resource competence, and facility quality. Enhancing procedural transparency and investing in continuous staff development are crucial strategies for improving hospital service quality and public perception.

Abstract

Background:Stunting remains a significant public health challenge in Indonesia, including in Manado City. Despite various government initiatives, the prevalence of stunting persists due to complex contributing factors such as poor nutrition, inadequate parenting, and limited access to health services.Objective:This study aimed to analyze the implementation of health promotion strategies—specifically advocacy, social support, and community empowerment—in reducing stunting prevalence in the working area of Puskesmas Minanga, Manado.Methods:A qualitative study with an exploratory design was conducted from June to July 2025. Data were collected through in-depth interviews, direct observations, and document analysis involving 10 purposively selected informants: program managers, community leaders, and mothers of under-five children. Data were analyzed using content analysis guided by the Miles and Huberman model, involving data reduction, display, and conclusion drawing.Results:The findings revealed that (1) advocacy was carried out through mini-workshops and coordination with local stakeholders, although policy support and budgeting remained limited; (2) social support involved active participation of religious and community leaders in encouraging healthy practices, yet lacked formal community-based structures; and (3) community empowerment focused on cadre training and health education through monthly Posyandu activities, though constrained by funding and attendanceissues.Conclusion:Health promotion strategies—when applied in an integrated manner—showed potential to reduce stunting prevalence. However, sustainability depends on stronger policy commitment, consistent funding, and wider community involvement. Strengthening intersectoral collaboration and community ownership is essential to enhance the effectiveness of stunting prevention programs.

Abstract

This study aims to analyze factors associated with quality of life of diabetes mellitus patients in the Matani Community Health Center Work Area, Tomohon City. This study specifically examines the relationship between duration of disease, depression, anxiety, stress, and family support on quality of life in diabetes mellitus patients. The study used a cross-sectional design with 168 respondents selected through consecutive sampling. Data were collected through interviews using standardized instruments, the DASS 21, HDFSS, and Euro-QoL. The data were analyzed using the chi-square test and multivariate logistic regression. The results showed a significant association between duration of disease and stress with quality of life in diabetes mellitus patients (p < 0.05). Patients with a duration of >5 years of suffering are 3 times more likely to have a poor quality of life and patients with severe stress are 2 times more likely to have a poor quality of life. These findings indicate the importance of increasing awareness in adherence to diabetes mellitus management, especially for patients who have suffered for a long time and improving psychosocial monitoring services for diabetes mellitus patients, especially regarding stress management, and becoming the basis for strengthening primary health care programs in diabetes mellitus management so that the quality of life of diabetes mellitus patients improves.

Abstract

Health insurance is an important component of the health financing system that aims to ensure equitable and sustainable access to health services. From a health economics perspective, the insurance system is able to mitigate the financial risk of illness while managing the efficiency and distribution of financing collectively. This study examines the development of health insurance models in Indonesia, ranging from traditional, modern, to managed care models, and analyses the challenges and implementation strategies of the National Health Insurance (JKN) programme by BPJS Kesehatan. A qualitative approach based on literature study was used with data sources from scientific journals, national regulations, BPJS Kesehatan reports, and public policy documents.The results show that the traditional model tends to be inefficient because it is prone to overutilisation and fraud. Modern models and managed care provide solutions through quality and cost control. However, the implementation of JKN in Indonesia still faces various challenges, such as delays in claim payments, moral hazard, and financing imbalances. One strategy for system sustainability is through the optimisation of participant fund assets, which can be productively invested to support promotive services, infrastructure, and strengthening the healthcare system. With transparent governance, the results of this asset optimisation can be returned for the benefit of the wider community. This study is expected to be a scientific contribution in supporting health financing policy reforms that are inclusive, efficient, and sustainable.
