Abstract

Trigeminal neuralgia (TN), sometimes referred to as tic doloureaux or Fothergill disease, is a facial nerve disorder that is widely recognized as one of the most excruciating human ailments. It is characterized by sudden, severe, and recurrent episodes of facial pain that can significantly impair a patient’s quality of life. Oral medications are commonly used as first-line therapy; however, they fail to provide long-term relief in approximately half of patients with trigeminal neuralgia. This condition often leads to repeated clinical visits and the need for alternative therapeutic approaches. A 26-year-old woman presented with complaints of severe, intermittent, and sharp pain localized to her left cheek, consistent with the clinical features of trigeminal neuralgia. Several minimally invasive and surgical treatment options are available for managing this condition, including radiofrequency procedures and microvascular decompression. However, these approaches still carry certain risks and may not be acceptable to all patients. Due to its lower risk profile, ultrasound-guided peripheral nerve block can serve as an effective and safer alternative treatment, particularly for patients who refuse or are not suitable candidates for invasive or surgical interventions.

Abstract

Background: Acne vulgaris is one of the most common skin problems and is closely associated with the activity of Propionibacterium acnes. Long-term use of antibiotics may lead to bacterial resistance; therefore, alternative antibacterial agents derived from natural products are needed. White rice (Oryza sativa L.) is known to contain bioactive compounds such as flavonoids, alkaloids, terpenoids, tannins, and saponins, which potentially exhibit antibacterial activity. Objective: This study aimed to evaluate the antibacterial activity of ethanol extract of white rice against the growth of Propionibacterium acnes. Methods: This research was conducted as a laboratory experimental study using a post-test only control group design. The ethanol extract of white rice was prepared by maceration using 70% ethanol. Antibacterial activity was evaluated using the disc diffusion method with extract concentrations of 100%, 50%, 25%, 12.5%, and 6.25%. Clindamycin was used as a positive control, while sterile distilled water served as a negative control. The parameter observed was the diameter of the inhibition zone. Result: The results of phytochemical screening showed that the ethanol extract of white rice (Oryza sativa L.) contains flavonoids, alkaloids, and terpenoids. Antibacterial activity test showed that the ethanol extract of white rice at concentrations of 100%, 50%, and 25% was able to inhibit the growth of Propionibacterium acnes by forming an inhibition zone, while at concentrations of 12.5% and 6.25% no inhibition zone was formed. The highest inhibition zone diameter was found at a concentration of 100%. The ethanol extract of white rice at concentrations of 100% (7.65 mm), 50% (6.77 mm), and 25% (6.15 mm) was able to inhibit the growth of Propionibacterium acnes, and was classified as having moderate inhibition. In contrast, at concentrations of 12.5% and 6.25%, the ethanol extract of white rice did not show any inhibitory activity detected in vitro using the disc diffusion method. The results of statistical analysis showed a p value <0.001, which means there was a significant difference in the diameter of the inhibition zone between the treatment groups. Conclusion: In conclusion, the ethanol extract of white rice exhibits antibacterial activity against Propionibacterium acnes in a concentration-dependent manner; however, its inhibitory effect remains lower than that of clindamycin.

Abstract

Knee septic arthritis is an orthopedic emergency that requires prompt and appropriate management to prevent permanent joint damage and systemic complications. Inflammatory markers such as erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) are commonly used for diagnosis and monitoring of treatment response; however, their prognostic value after surgical debridement remains variable. This systematic review aimed to evaluate the role of ESR, CRP, and platelet count as prognostic indicators following irrigation and/or surgical debridement in patients with knee septic arthritis. A literature search was conducted in January using PubMed, Google Scholar, ScienceDirect, Wiley Online Library, SpringerLink, and Semantic Scholar, following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Study quality was assessed using the National Heart, Lung, and Blood Institute (NHLBI) tools. Fifteen studies meeting the inclusion criteria were qualitatively analyzed. The findings indicate that elevated preoperative CRP levels and delayed postoperative CRP reduction are associated with unfavorable clinical outcomes, including failure of infection eradication and the need for repeat debridement. Although ESR is consistently elevated during the acute phase, its slower decline limits its short-term prognostic utility. Platelet count may increase as part of the systemic inflammatory response but shows inconsistent prognostic value.

Abstract

Iron deficiency anemia (IDA) is a condition of insufficient iron that lowers hemoglobin levels and disrupts erythrocyte formation. This condition commonly occurs in children and can weaken the immune system, thereby increasing the risk of recurrent respiratory tract infections such as bronchiolitis, pneumonia, and Acute Respiratory Tract Infections (ARTI). Children with IDA are also at risk of worsening clinical conditions. This study employed a literature review method with a descriptive-analytical approach. A total of 10 journals (2020–2026) from Google Scholar, PubMed, ResearchGate, Elsevier, and the National Center for Biotechnology Information (NCBI) were analyzed using the Population, Intervention, Comparison, Outcome, Study Design (PICOS) framework and synthesized narratively. The results of the review indicate that iron deficiency anemia is significantly associated with an increased risk of recurrent respiratory tract infections in children. Children with IDA have a 2–5 times higher risk of developing bronchiolitis and pneumonia, and up to 10 times greater susceptibility to recurrent ARTI. This condition is also associated with increased disease severity, characterized by low hemoglobin levels, mean corpuscular volume (MCV), and serum iron, as well as elevated total iron-binding capacity (TIBC). Iron supplementation has been proven effective in reducing the incidence of recurrent infections. In conclusion, iron deficiency anemia is associated with an increased risk and severity of recurrent respiratory tract infections in children. Early detection and iron supplementation are essential to reduce morbidity and prevent infection recurrence.

Abstract

Maternal hypertension remains one of the major contributors to maternal morbidity and mortality in Indonesia and globally. This study aimed to analyze the prevalence, mortality trends, and geographical disparities of maternal hypertension in Indonesia during the 2022–2023 period. The research employed a descriptive design using secondary data obtained from official reports of the Indonesian Ministry of Health, national health surveys, and Maternal Perinatal Death Notification (MPDN) data. Data analysis was conducted descriptively by examining prevalence rates, maternal mortality due to hypertension, regional distribution, and temporal trends across provinces in Indonesia. The findings showed a slight but consistent decline in both the prevalence and mortality rates of maternal hypertension from 2022 to 2023 in most provinces. However, significant regional disparities remained evident. Provinces in eastern Indonesia, including Papua Barat, Nusa Tenggara Timur, Maluku, and several provinces in Sulawesi and Kalimantan, consistently recorded higher prevalence and mortality rates compared to provinces in Java and Bali. Meanwhile, provinces with better healthcare infrastructure and maternal referral systems demonstrated lower mortality outcomes. The study also revealed a strong association between high prevalence and increased maternal mortality, indicating the importance of healthcare accessibility, antenatal care quality, and emergency obstetric services in reducing maternal health burdens. These findings emphasize the need for region-specific maternal health interventions, improved healthcare distribution, early detection programs, and strengthened referral systems to reduce maternal hypertension disparities and improve maternal health outcomes in Indonesia.

Abstract

Bariatric surgery is effective in treating obesity and type 2 diabetes mellitus (T2DM); however, pregnancy occurring within 12–24 months after surgery carries significant risks. This report presents a complex pregnancy case in a patient with a history of sleeve gastrectomy, post-hemithyroidectomy hypothyroidism, and T2DM. A 29-year-old primigravida conceived 2 months after undergoing sleeve gastrectomy, which successfully induced T2DM remission, with HbA1c decreasing from 11% to 5.5%. Although glycemic control and euthyroid status were maintained throughout pregnancy, the patient developed severe preeclampsia at 35–36 weeks of gestation, complicated by intrauterine growth restriction (IUGR) and oligohydramnios. An emergency cesarean section delivered a male infant weighing 2000 grams with Apgar scores of 8 and 9 at the first and fifth minutes, respectively. The neonate required 14 days of NICU care due to respiratory distress syndrome, respiratory failure, and hyperbilirubinemia. Maternal gestational weight gain was suboptimal at only 4 kg, below the recommended range for pregnant women with obesity. This case highlights the importance of adhering to the recommended surgery-to-conception interval following bariatric surgery. Early pregnancy after surgery, combined with multiple metabolic comorbidities, may contribute to adverse maternal and neonatal outcomes. Multidisciplinary management, comprehensive nutritional monitoring, and optimization of comorbidities before conception are essential to improve pregnancy outcomes in this population.

Abstract

Low-grade fibromyxoid sarcoma (LGFMS) is a rare soft tissue tumor with deceptively benign histological features but significant potential for local recurrence and distant metastasis. Pelvic LGFMS is extremely uncommon and may mimic gynecological malignancies, leading to diagnostic challenges. This report describes a 39-year-old woman presenting with progressive abdominal enlargement, bloating, early satiety, and dyspnea. Physical and ultrasonographic examinations suggested a malignant ovarian tumor. The patient underwent exploratory laparotomy without preoperative CT scan evaluation. Intraoperatively, multiple solid masses involving the intestines, mesentery, omentum, peritoneum, and ovaries were identified, with the largest measuring 20 × 15 cm. Histopathological examination confirmed the diagnosis of LGFMS. The absence of preoperative cross-sectional imaging contributed to diagnostic difficulty in this case. CT scan and MRI are important for evaluating tumor origin, tissue characteristics, and involvement of surrounding structures. Definitive diagnosis requires histopathology supported by immunohistochemistry, particularly MUC4, and molecular confirmation when available. Comprehensive diagnostic evaluation is essential to avoid misdiagnosis, optimize management, and improve long-term outcomes in patients with pelvic LGFMS.

Abstract

Drowning is one of the leading causes of accidental death worldwide and remains a major challenge in forensic investigations due to the complexity of determining the exact cause and mechanism of death. This study aimed to analyze forensic investigation methods used to determine the cause of death in drowning cases. The research employed a qualitative descriptive literature review approach by analyzing scientific articles, forensic journals, and medical references related to drowning investigations. Data were collected through literature searches from Google Scholar, PubMed, and Portal Garuda using keywords related to forensic drowning investigations. The findings indicate that forensic investigation in drowning cases requires a comprehensive approach involving external examination, autopsy, laboratory examination, histopathology, diatom analysis, toxicology, and crime scene investigation. Typical forensic findings include foam cone, washerwoman’s hands, cutis anserina, emphysema aquosum, and the presence of foreign materials in the respiratory and digestive tracts. Diatom examination and histopathological analysis also play important roles in strengthening the diagnosis of drowning. The study concludes that an integrated forensic investigation based on scientific evidence is essential to accurately determine the cause and manner of death in drowning cases and to support medicolegal processes in Indonesia.

Abstract

Medical students need to take the Competence test as Doctor Profession Program (UKMPPD) to be passed and earned their medical profession. Exams are one of the most common stressors experienced by medical students and the body will respond to these stressors in the form of feelings of depression or anxiety. Anxiety and depression itself can affect student performance during exam preparation. The purpose of this study was to determine the relationship between anxiety and depression on the preparation of the Professional Doctoral Program Competency Test (UKMPPD) for students of the Faculty of Medicine, University of Pattimura. The method used is quantitative analysis with cross sectional research design and total sampling technique. The sample is all the student population of the Faculty of Medicine, University of Pattimura who meet the inclusion and exclusion criteria, totaling 90 respondents. The results showed that the majority of respondents experienced mild anxiety symptoms, which is 37% during the CBT exam and 34% during the OSCE exam. The majority of respondents also did not experience depression, which is 68% during the CBT and 70% during the OSCE and the majority had an optimal level of preparation for UKMPPD, which is 63% during the CBT and 64% during the OSCE. Data analysis using the chi square test showed a significant relationship between anxiety symptom and the level of preparation for UKMPPD both CBT (p=0.030) and OSCE (p=0.012). There was no significant relationship between depression symptom and the level of preparation for UKMPPD for both CBT (p=0.123) and OSCE (p=0.07). It can be concluded that there is a significant relationship between anxiety and preparation for UKMPPD, but there is no significant relationship between depression and preparation for UKMPPD.
