Abstract

Diabetes mellitus is a metabolic disorder characterized by hyperglycemia. Most causes occur due to impaired insulin secretion, impaired insulin action,  impaired carbohydrate, fat, protein metabolism. The results of the Basic Health Research in 2018 showed that the prevalence of DM in Indonesia is 8.5% or around 20.4 million people diagnosed with diabetes mellitus. In Jakarta (Kemenkes RI, 2018). It is estimated that a person with diabetes has a 25% lifetime risk of developing diabetic ulcers (International Diabetes Federation, 2020). One of the common nursing problems in DM clients is ineffective peripheral perfusion. Buerger Allen Exercise is one of the active movement exercises that aims to improve peripheral perfusion to the lower extremities. This case report aims to analyze nursing care in the application of Buerger Allen Exercise interventions in clients with Diabetes Mellitus. The research method used is descriptive research in the form of a case study with a sample of 1 person conducted for 3 consecutive days with a duration of 12 minutes in the morning and evening in the internal medicine inpatient room. The results of the evaluation after the intervention of the Buerger Allen exercise found a change in ABI values on the first day of the right ABI value: was 0.87 and the left ABI: was 0.80, on the second day the right ABI value was 1.2 and the left ABI was 1.03, on the third day the right ABI value was 1.01 and the left ABI: 1,0. For CRT, pulse on the extremities, and edema there were no significant changes. While in the acral there is a change to warm acral on the lower extremities. This shows the effectiveness of the Buerger Allen exercise on lower extremity peripheral perfusion, especially in increasing ABI values

Abstract

Hypertension is an increase in systolic blood pressure ? 140 mmHg and diastolic blood pressure ? 90 mmHg, Mostly hypertension occurs without signs and symptoms, so patients do not realize they have been diagnosed with hypertension, that's why hypertension is known as the Silent Killer. South Jakarta ranks 3rd out of 6 regions in DKI Jakarta with a prevalence of hypertension based on doctor's diagnosis of 10.92%. Foot massage is one of  complementary therapy that is safe and applicable that aims to lower blood pressure. The scientific work aims to apply foot massage to lower blood pressure in hypertensive patients. The method used is case study with a sample of 1 patient in  3  consecutive days for 30 minutes with 12 steps  during 15 minutes in each foot. The results after giving foot massage there is a decrease in blood pressure from the first day to the third day, the average systolic pressure dropped by 6.4 mmHg and diastolic pressure dropped by 5mmHg. This shows the effectiveness of foot massage on lowering blood pressure. Through this study, foot massage is an alternative solution to efforts to reduce blood pressure non-pharmacologically. This study can be a reference and initial source of information for further research, in addition to further research is expected to look at other aspects such as anxiety levels and sleep patterns of patients.

Abstract

Diabetes mellitus is a collection of symptoms in a person caused by an increase in blood glucose levels due to decreased insulin secretion. To overcome the instability of blood glucose levels can be through two ways, namely pharmacological and non-pharmacological therapies. One of the non-pharmacological techniques is relaxation that can stabilize blood glucose levels, the type of relaxation therapy that has high effectiveness in reducing blood sugar levels is progressive muscle relaxation therapy. This research design is a type of descriptive observational research in the form of a case report. The research time was from May 15 to June 3, 2023. The population of this study were all male patients with DM who were admitted to the Interne room in Padang Hospital. The sample amounted to 2 people taken by purposive sampling technique. Data collection techniques using interview methods, physical examination and observation of DM patients with blood sugar checks for the application of progressive muscle relaxation therapy to stabilize blood glucose levels. The evaluation results of the intervention given to patients showed the effect of progressive muscle relaxation therapy in stabilizing the blood glucose levels of Diabetes Mellitus patients in the Male Interne room of in Hospital.  The suggestion of this study is that progressive muscle relaxation techniques can be considered to be an alternative non-pharmacological intervention that can be done by nurses to reduce blood glucose levels of patients with Type 2 DM.
