Abstract

Pancreatic cancer ranks as the seventh most common cause of cancer-related death, according to the 2020 Global Cancer Statistics. This cancer can cause blockages in the duodenum or bile ducts, often requiring bypass laparotomy to relieve gastrointestinal obstructions caused by pancreatic head tumors and to restore disrupted digestive flow. Postoperative pain is common, with major surgeries typically causing more intense pain. Data shows that 70-80% of surgery patients experience postoperative pain, and 10-30% develop chronic pain. This study examines the effects of deep breathing relaxation and finger-hold therapy on changes in vital signs and pain levels. The treatment was applied for 8 minutes over 3 days, with pain monitored through vital signs. This method combines deep breathing with finger holding for 2 minutes per finger. Results showed a pain reduction from a scale of 6 to 3 on the Numeric Rating Scale, along with changes in vital signs. This intervention can be beneficial for managing postoperative pain in laparotomy patients

Abstract

Hospitalization stress is a condition such as anxiety, sadness and fear that children and their families experience while undergoing treatment in hospital. The short-term impact that can occur due to anxiety and fear that is not treated immediately is rejection of treatment and care, which can result in an increase in days of care, and cause the child's condition to get worse. The aim of this research is to reduce anxiety due to hospitalization in children using puzzle play therapy. This research method uses a case study method carried out on children aged 4 and 6 years for 3 days with a duration of 15 minutes per meeting. The puzzle that will be given to children aged 4 years is 9 pieces, while children aged 6 years are given 12 pieces. The puzzles given to the two children were in the form of cute pictures. Before and after administering puzzle playing therapy, characteristics and anxiety scales were assessed using the Face Image Scale assessment. The results of the intervention that had been carried out for 3 days showed a decrease in the anxiety scale in the case of the 1st child who experienced a decrease in anxiety from a scale of 4 to 3. Meanwhile in the case of the 2nd child there was a decrease from an anxiety scale of 3 to 2. The conclusion is that there is effectiveness of puzzle playing therapy for reduce anxiety due to hospitalization in children. It is hoped that this puzzle play therapy can be applied in hospitals, especially in children's rooms, to minimize anxiety in children during hospitalization.

Abstract

The incidence of treatment discontinuation among tuberculosis (TB) patients in DKI Jakarta has been increasing each year. In 2016, a total of 78 patients, or 21.2% of the total number of patients, discontinued their treatment. This study aims to analyze the effect of continuous education through audiovisual media on the knowledge and attitudes regarding the prevention of treatment discontinuation among TB patients.This research employed a quasi-experimental design with a pretest-posttest control group. The inclusion criteria were: patients diagnosed with tuberculosis, patients undergoing treatment for newly diagnosed TB, and patients who had access to social media platforms. The study was conducted from May 20 to July 5, 2024, in the Jatinegara and Kramat Jati areas of East Jakarta. The data instruments used included demographic questionnaires, knowledge questionnaires, attitude questionnaires, and educational modules. A total of 52 respondents participated, with 26 in the intervention group and 26 in the control group. The sampling technique was purposive sampling. Data analysis was performed using SPSS, with the Friedman test followed by the Wilcoxon test. Statistical analysis revealed a p-value of 0.000, which is less than 0.05.  Conclusion: There is a significant effect of continuous audiovisual education on the prevention of treatment discontinuation in TB patients, leading to improvements in both knowledge and attitudes. Further research is recommended to be conducted on TB patients with drug resistance and to include confounding factors such as smoking habits.

Abstract

Usually patients will feel psychological and physiological stress before surgery which can increased heart rate, blood pressure, and breathing. One of the operations causes excessive anxiety is laparotomy. From the National Social Security Council, (2022) in 2020 laparotomy surgery participants in Indonesia was 12,537 and in 2021 it became 13,131. Anxiety in pre-laparotomy patients caused by psychological readiness for surgery that has not yet occurred. Butterfly hug developed by therapists Luciana and Janero. This therapy provides a comfort, reduces physical pain and increases a sense of calm through the comfort of a hug. This study aims to analyze the effect of butterfly hug on reducing pre-laparotomy surgery anxiety. This research method is a Quasy Experiment Pre-test – Post-test with control group. The dependent variable is pre-laparotomy patient anxiety and butterfly hug as an independent variable. The study population was pre-operative patients who experienced anxiety with a sample of 41 respondents. This research instrument uses a modified butterfly hug SOP and a zung self-anxiety scale questionnaire to assess anxiety. Data analysis uses Wilcoxon test. Statistically, the results obtained a p-value of 0.000 <0.05 which indicates that an effect of butterfly hug therapy on reducing the level of anxiety in pre-laparotomy patients

Abstract

Diabetes mellitus is a public health problem that is of concern to both developed and even developing countries because one in two elderly people suffer from diabetes mellitus. Chronic hyperglycemia in synergy with other metabolic abnormalities in diabetes mellitus patients can cause damage to multiple organ systems, leading to the development of disabling and life-threatening health complications, microvascular (retinopathy, nephropathy, and neuropathy) and macrovascular complications. The level of patient compliance in taking medication is one of the factors that determines the success of diabetes mellitus therapy. This study aims to determine the emotional support of the family on the level of adherence to taking diabetes medication in elderly people with diabetes mellitus. The sample in this study was 86 people using a non-probability sampling technique with a consecutive sampling approach. The research design used was descriptive correlation with a cross-sectional approach. The data analysis test used is univariate, bivariate with the Chi Square test. The research results show that there is a significant relationship between family emotional support and adherence to taking diabetes medication in elderly people with diabetes mellitus (p=0.001). Conclusion: Family emotional support for patients is very necessary for compliance to achieve successful diabetes mellitus therapy so that it indirectly plays a role in stabilizing blood glucose levels in diabetes mellitus patients.

Abstract

Hypertension control can be done by compliance with taking medication and a low-salt diet. Many people with hypertension are not compliant with taking medication and forget about a low-salt diet because they feel healthy. In fact, this should not happen because it will affect their blood pressure. The purpose of this study was to determine the relationship between compliance with taking medication and a low-salt diet with blood pressure stability in hypertensive patients. The research method used was quantitative analytical descriptive with a cross-sectional approach. The population in this study were all patients with hypertension who underwent outpatient treatment at the internal medicine. A total of 92 respondents were the sample of this study. Sampling was carried out using a purposive sampling technique. The results used chi square found that there was a relationship between medication adherence and blood pressure stability with a p-value of 0.001 and there was a relationship between a low-salt diet and blood pressure stability in hypertensive patients with a p-value of 0.001. The conclusion is that there is a relationship between medication adherence and a low-salt diet with blood pressure stability in hypertensive patients. Recommendations for patients with hypertension are advised to maintain salt intake and continue taking medication so that blood pressure remains stable.

Abstract

Infant mortality due to tetanus neonatorum infection is 77 per-1,000 live births. The effect of applying topical breast milk method in cord care can accelerate cord detachment and prevent infection compared to other methods. The purpose of applying topical breast milk is to accelerate cord detachment and prevent infection. The method used is a case study with the inclusion criteria of newborns of full term, normal birth weight, being inpatient with the mother and APGAR score of at least seven or more. The number of respondents in this application amounted to one baby, the time of implementation of the application was carried out three days twice a day (morning and evening), observations were made with observation sheets for signs of infection, and umbilical cord release. The results of the application after three days of treatment showed a change in the umbilical cord, the cord turned blackish in colour with a dry texture and no signs of infection. Conclusion The application of cord care using topical breast milk can accelerate the process of cord detachment and prevent infection.

Abstract

Ventilator Associated Pneumonia (VAP) is a nosocomial infection that is often experienced by patients who receive mechanical ventilation for more than 48 hours. Various evidence shows that the prognosis for VAP patients is very poor and actions that can be taken to prevent VAP include monitoring the patient's condition through diagnostic examinations, namely blood gas analysis (AGD). This study aims to determine the relationship between the incidence of VAP and AGD. The research design used was correlational with a cross sectional approach. The sample for this research was 67 patient data using purposive sampling technique. The analysis used the Fisher exact test statistical test. The results of this study involved 67 respondents, with the majority of ICU patients aged between 20 and 60 years (43,3%)  and most of them were men (55,2%). The most common occupation among respondents was self-employment (35,8%). VAP positive result (41,8%) and based on the results of blood gas analysis, it showed that (32.8%) patients had respiratory acidosis, (20.9%) had respiratory alkalosis, (10.4%) had metabolic acidosis, (14.9%) had metabolic alkalosis. The results of the bivariate analysis showed that there was a relationship between the ventilator associated pneumonia variable and the blood gas analysis variable with a p value of 0.00 <? (0.05). In conclusion, patients who are on a ventilator are at risk of developing VAP. Patients on ventilators need to have their Clinical Pulmonary Infection Score (CPIS) checked regularly and need to implement strict VAP prevention protocols.

Abstract

Hallucinations are one of the most common sensory perception disorders in patients with mental disorders, particularly schizophrenia. One non-pharmacological therapy that can be used to reduce hallucination symptoms is occupational drawing therapy. This study aims to analyze the effectiveness of occupational drawing therapy in reducing hallucination signs and symptoms through a literature review method. Literature searches were conducted using the Google Scholar database with relevant keywords and article selection was based on inclusion and exclusion criteria. The results show that occupational drawing therapy significantly reduces hallucination symptoms in patients, both in terms of symptom intensity reduction and changes in patient conditions. This therapy helps patients express emotions, distraction from hallucinations, and improve self-control. Therefore, occupational drawing therapy can be considered an effective non-pharmacological intervention for managing patients with hallucinations.

Abstract

Stunting is a condition of failure to grow due to chronic malnutrition from pregnancy to the age of five, which has an impact on the physical and cognitive development of children. The incidence of stunting in Indonesia ranges from 27.22% to 28.11%. This shows that more than a quarter of the population is stunted. This prevention should be done far before pregnancy. This study was conducted to analyse the relationship between knowledge and stunting prevention attitudes in brides-to-be in Pondok Melati District, Bekasi City. The method used in this study is cross-sectional. Data was collected through a questionnaire filled out by 150 brides-to-be using total sampling. Bivariate analysis was performed using the chi-square test. The results found that there was a significant relationship between the knowledge and attitude of the bride-to-be regarding stunting prevention with a p-value of 0.000 (p-value < 0.05). These results emphasize the importance of education about stunting for brides-to-be, especially related to nutrition before and during pregnancy and the long-term impact of stunting on children.
