Abstract

Medicine storage is an essential component of pharmaceutical quality assurance because inappropriate storage conditions and inadequate inventory practices may affect medicine quality, safety, and availability. This study aimed to analyze the implementation of medicine storage practices in the pharmacy warehouse of Dr. Hasri Ainun Habibie Regional Hospital. A qualitative descriptive approach was employed. Data were collected through direct observation, interviews, and document review involving three informants consisting of the Head of the Pharmacy Installation, the pharmacy warehouse officer in charge, and pharmaceutical technical staff. Data were analyzed descriptively by comparing observed practices with applicable Good Storage Practices (GSP) principles. The findings showed that medicines were generally organized according to type, dosage form, storage requirements, and specific categories, with FEFO applied for stock rotation. Incoming medicines were checked based on purchase documents, quantity, batch number, expiration date, and physical condition, while stock cards supported inventory monitoring. The storage room was maintained in a clean and orderly condition, with adequate ventilation and lighting, temperature control, and protection against insects and rodents. Storage cabinets and medicine refrigerators were also maintained properly, and expired medicines were separated from active stock. However, LASA medicines were not consistently provided with specific labels, and some procedures required greater standardization to ensure consistent implementation. The study concludes that medicine storage practices were generally implemented appropriately, although improvements are needed in LASA identification, standardization of operational procedures, continuous monitoring, and personnel competency to strengthen pharmaceutical quality assurance.

Abstract

Penelitian ini bertujuan untuk menganalisis penerapan Standar Operasional Prosedur (SOP) Keselamatan dan Kesehatan Kerja (K3) di ruang radiologi RSUD Dr. M.M. Dunda Limboto. Penelitian menggunakan pendekatan kualitatif dengan desain deskriptif. Informan berjumlah lima orang yang terdiri atas wakil direktur, kepala unit K3, kepala instalasi radiologi, dan dua staf radiologi. Data dikumpulkan melalui observasi dan wawancara, kemudian dianalisis secara deskriptif melalui tahapan pengumpulan, reduksi, penyajian data, dan penarikan kesimpulan. Hasil penelitian menunjukkan bahwa penerapan SOP K3 telah didukung oleh kebijakan tertulis, audit dan evaluasi internal, penggunaan alat pelindung diri (APD), pembatasan akses ruang radiasi, penerapan prinsip ALARA, pemantauan dosis radiasi, perlindungan ruangan, pemeriksaan kesehatan, serta pemeriksaan dan kalibrasi peralatan secara berkala. Informan menyatakan bahwa tidak terdapat insiden maupun kendala serius dalam penerapan SOP selama periode penelitian dan bahwa petugas secara umum mematuhi ketentuan keselamatan kerja. Meskipun demikian, penerapan SOP tetap memerlukan pemantauan dan evaluasi berkelanjutan untuk memastikan konsistensi kepatuhan serta mengidentifikasi potensi kesenjangan antara prosedur dan praktik di lapangan. Rumah sakit disarankan memperkuat observasi kepatuhan, dokumentasi keselamatan, sosialisasi SOP, pelatihan, dan simulasi keadaan darurat guna meningkatkan efektivitas pengendalian risiko kerja di ruang radiologi.

Abstract

Human resource (HR) competence is an important factor in supporting the effective management of modern administrative systems in hospitals. This study aimed to analyze HR competencies in managing the modern administrative system at Bhayangkara Hospital Gorontalo. A qualitative descriptive approach was employed, with data collected through observation, in-depth interviews, and documentation. Informants were selected purposively based on their involvement and knowledge of hospital administrative system management. Data were analyzed through data reduction, data display, and conclusion drawing and verification, with triangulation used to strengthen data credibility. The findings indicate that HR competencies are reflected in technological capabilities, managerial skills, communication competence, adaptability and continuous learning, and collaboration and system integration. HR competence is supported by employee placement according to educational background and job requirements, continuous training, and the utilization of digital applications such as Astina and the Hospital Management Information System (SIMRS). Technological competence is demonstrated through system monitoring, data security measures, system evaluation, vendor coordination, and technical support. Digital administration has contributed to reducing the use of paper, ink, and stationery by approximately 70–80%. Managerial competence is reflected in task prioritization, time management, problem solving, and coordination, while communication and continuous learning support service continuity and adaptation to technological changes. However, some administrative processes remain manual, and administrative personnel still require IT assistance for certain technical problems. The study concludes that HR competencies generally support modern administrative system management, but continuous competency development, digital literacy improvement, and stronger integration between administrative and IT personnel are needed.

Abstract

Medical oxygen is a critical component of hospital infrastructure, and its effective distribution is essentialfor maintaining continuity of patient care and safety. This study aimed to analyze medical gas distributionmanagement at Prof. Dr. H. Aloei Saboe Regional Hospital, Gorontalo, focusing on facilities and infrastructure,human resources, and the utilization of the Hospital Management Information System (SIMRS). A descriptivequalitative design was employed from August to October 2024. Data were collected through observation, in-depthinterviews, and documentation involving three informants consisting of the Head of the Hospital Facilities andInfrastructure Maintenance Unit (IPSRS), an IPSRS officer, and the Head of the Isolation Room. Data wereanalyzed through data organization, reduction, coding, categorization, cross-source comparison, and descriptiveinterpretation. The findings showed that oxygen distribution was supported by a centralized oxygen installation,reserve cylinders, routine external supply, and demand-based distribution. However, centralized and manualdistribution systems operated simultaneously, requiring direct stock monitoring and manual reporting. IPSRSstaffing was considered adequate for current operations, although afternoon and night shifts had fewer personneland staff had not received specific medical oxygen management training. SIMRS utilization was also limited,with oxygen monitoring, recording, and reporting at the IPSRS level remaining predominantly manual. The studyconcludes that medical oxygen distribution management is operationally functional but requires strengtheningthrough standardized procedures, competency-based training, improved contingency mechanisms, and greaterintegration of oxygen inventory and distribution monitoring into SIMRS to enhance efficiency, responsiveness,and patient safety.

Abstract

Blockchain technology offers potential for improving the security, traceability, and management of patient data in healthcare institutions. However, its practical implementation depends not only on technological capabilities but also on human resource readiness, usability, service processes, data security, and system integration. This study aimed to analyze blockchain-based patient data management at Dr. M.M. Dunda Limboto Regional General Hospital, focusing on staff satisfaction, ease of use, service quality, data security, and system integration. A descriptive qualitative design was employed, with data collected in November 2025 through observation, semi-structured interviews, and documentation. Four informants representing hospital management, human resources, information technology, and medical records were selected based on their involvement in patient data management. Data were analyzed through organization, reduction, categorization, data display, cross-source comparison, and conclusion drawing. The findings indicate that blockchain-based patient data management was perceived to support data traceability, transparency, access control, and the preservation of record histories. The system was considered relatively simple to use, although patient access to information remained dependent on medical personnel. Human resource limitations, particularly IT workload and staff adaptation, were identified as important challenges. Data security was supported by authentication, role-based access, standard operating procedures, and reported encryption mechanisms. However, integration with the broader hospital information system remained incomplete, with some processes still conducted manually. The study concludes that effective blockchain-based patient data management requires not only technological implementation but also continuous staff development, adequate infrastructure, stronger interoperability, and improved integration of information systems.

Abstract

Type 2 diabetes mellitus (T2DM) accompanied by obesity and multiple complications requiresindividualized and integrated nutrition management. This case study aimed to describe the implementation of theNutrition Care Process (NCP) in a patient with obese T2DM, diabetic neuropathy, diabetic gastroparesis, febrilesuspected urinary tract infection, and mild hyponatremia. A descriptive case study was conducted prospectivelyduring hospitalization using the four stages of the NCP: nutrition assessment, nutrition diagnosis, nutritionintervention, and nutrition monitoring and evaluation. Data were collected through dietary assessment,anthropometric and biochemical measurements, clinical examination, medical records, and nutrition-focusedphysical assessment, and were analyzed descriptively by comparing conditions before and during nutrition care.The assessment identified excessive energy and macronutrient intake, frequent consumption of sugar-sweetenedbeverages, limited adherence to previous dietary recommendations, obesity, and severe hyperglycemia.Individualized nutrition intervention was provided through controlled energy and macronutrient intake, dietarymodification, nutrition education, and family involvement. During monitoring, dietary intake progressivelyapproached the prescribed requirements, while random blood glucose showed a marked decline duringhospitalization. Nausea and abdominal pain resolved, and the patient's general clinical condition improved,although obesity and hyperglycemia persisted at the end of observation. The findings indicate that the NCP is afeasible and clinically useful framework for delivering individualized nutrition care in patients with complexT2DM and multiple comorbidities. Continued nutrition counseling, self-management support, familyinvolvement, and multidisciplinary monitoring are needed to sustain nutritional and metabolic improvements afterhospitalization.

Abstract

Kesehatan merupakan aspek penting yang mencakup dimensi fisik, mental, sosial, dan spiritual. Namun, pendekatan kesehatan modern masih cenderung berfokus pada aspek biologis sehingga integrasi dimensi spiritual belum optimal. Penelitian ini bertujuan untuk menganalisis konsep kesehatan holistik dalam perspektif Islam, mengkaji peran ilmu kedokteran dan kesehatan masyarakat, serta menjelaskan peran pendidikan Islam dalam mengintegrasikan ketiga aspek tersebut. Penelitian ini menggunakan metode Narrative Literature Review dengan pendekatan kualitatif-deskriptif. Data diperoleh dari literatur yang dipublikasikan pada tahun 2020–2025 melalui Scopus, ScienceDirect, Google Scholar, serta buku dan dokumen ilmiah yang relevan. Sebanyak 15 literatur utama dianalisis menggunakan teknik content analysis. Hasil penelitian menunjukkan bahwa kesehatan dalam Islam merupakan konsep holistik yang mengintegrasikan dimensi fisik, mental, sosial, dan spiritual. Kolaborasi ilmu kedokteran dan kesehatan masyarakat mendukung terwujudnya pelayanan kesehatan yang komprehensif, sedangkan pendidikan Islam berperan sebagai media transformasi dalam membentuk perilaku hidup sehat melalui integrasi nilai-nilai spiritual, ilmu kedokteran, dan kesehatan masyarakat. Temuan ini diharapkan dapat menjadi kontribusi dalam pengembangan pendidikan Islam berbasis kesehatan holistik.

Abstract

Food safety is an important public health concern, particularly for ready-to-eat meat products that may be exposed to contamination during processing, handling, storage, and serving. This study aimed to presumptively identify Salmonella spp. in meatballs sold in selected snack areas of Gorontalo City based on macroscopic, microscopic, and biochemical characteristics. A descriptive laboratory study with a cross-sectional approach was conducted in June 2024 at the Microbiology Laboratory of Universitas Bina Mandiri Gorontalo. Six meatball samples were collected purposively from four snack-selling areas and examined using Salmonella-Shigella Agar (SSA), Gram staining, and Triple Sugar Iron Agar (TSIA). The laboratory findings were analyzed descriptively by comparing colony morphology, Gram reaction, and biochemical characteristics with the typical characteristics of Salmonella. Five samples (A, B, C, E, and F) showed characteristics consistent with presumptive Salmonella spp., including Gram-negative rod-shaped cells and TSIA reactions characterized by a red slant and yellow butt, with hydrogen sulfide production and gas formation observed in most samples. Sample D showed no bacterial colony growth under the examination conditions. The findings indicate that most examined meatball samples showed bacterial characteristics consistent with presumptive Salmonella spp. Therefore, improved hygiene, sanitation, food protection, and microbiological monitoring are recommended. Confirmatory biochemical, serological, or molecular testing is needed to establish definitive identification.

Abstract

Penelitian ini bertujuan untuk mengetahui gambaran kadar kreatinin pada pasien hipertensi sebagai indikator fungsi ginjal di RSUD Toto Kabila. Penelitian ini menggunakan desain deskriptif observasional dengan pendekatan cross-sectional. Sampel penelitian berjumlah 15 pasien hipertensi yang memenuhi kriteria inklusi dan dipilih menggunakan teknik purposive sampling. Pemeriksaan kadar kreatinin serum dilakukan di Laboratorium RSUD Toto Kabila menggunakan Automatic Chemistry Analyzer Mindray BS-380, sedangkan data dianalisis secara univariat dan disajikan dalam bentuk distribusi frekuensi, persentase, serta deskripsi karakteristik responden. Hasil penelitian menunjukkan bahwa 13 responden (86,7%) memiliki kadar kreatinin dalam kategori normal (0,5–1,3 mg/dL), sedangkan 2 responden (13,3%) memiliki kadar kreatinin abnormal (>1,3 mg/dL). Mayoritas responden berjenis kelamin perempuan (73,3%) dan berada pada kelompok usia 45–59 tahun. Nilai kadar kreatinin berkisar antara 0,54 mg/dL hingga 25,07 mg/dL, dengan dua responden menunjukkan peningkatan kadar kreatinin yang mengindikasikan adanya penurunan fungsi ginjal. Disimpulkan bahwa sebagian besar pasien hipertensi di RSUD Toto Kabila masih memiliki kadar kreatinin dalam batas normal, namun sebagian kecil telah mengalami peningkatan kadar kreatinin yang berpotensi menunjukkan gangguan fungsi ginjal. Oleh karena itu, pemeriksaan kreatinin serum secara berkala perlu dilakukan sebagai upaya deteksi dini dan pemantauan fungsi ginjal pada pasien hipertensi.
