Abstract

Accreditation is the principal external quality-assurance instrument for primary health centres (puskesmas), yet the foremost challenge lies in sustaining a quality culture after the survey concludes. Although leadership is recognised as a key structural factor, the relational dynamic between the head of the health centre and the administrative head as a single managerial unit remains rarely examined. Objective: To comparatively analyse how managerial leadership alignment shapes the sustainability of post-accreditation quality culture in two puskesmas with contrasting territorial characteristics and accreditation status. Methods:A qualitative comparative case study was conducted in two puskesmas (urban and rural) within one regency in Jambi Province. Data were gathered through in-depth interviews, non-participant observation, and document review, then analysed thematically using the Donabedian framework. Results: The urban centre exhibited alignment between leadership and administration—personal exemplarity, consistent presence, and a single line of command—that supported the internalisation of quality values. The rural centre experienced misalignment rooted in a structural gap of overlapping job descriptions, producing dual instructions, staff confusion, and weakened sustainability. Conclusion: The alignment of managerial relationships is a crucial structural determinant in the sustainability of a quality culture; the clarity of formal authority distribution needs to be reinforced.

Abstract

This study aims to analyze the effect of the use of game-based Gimkit learning media on the historical thinking skills of eleventh-grade students. The background of this study is based on the low historical thinking skills of students who still tend to be oriented towards memorization and lack of active student involvement in analyzing historical events. The study used a quantitative approach with a quasi-experimental method through a nonequivalent control group design. The research sample consisted of 43 students divided into an experimental class of 21 students and a control class of 22 students. The experimental class received learning using game-based Gimkit media, while the control class received learning assisted by PowerPoint media. Data collection techniques were carried out through historical thinking ability tests using pretest and posttest instruments. Data analysis was carried out using descriptive statistical tests, N-Gain calculations, and Independent Sample t-tests with the help of SPSS version 21. The results showed that the average N-Gain of the experimental class was 0.7115 in the high category, while the control class obtained an average of 0.3892 in the medium category. The results of the hypothesis test showed a calculated t-value of 20.518 > t-table 2.019 with a significance value of 0.000 < 0.05, thus the alternative hypothesis was accepted. These findings indicate that the use of game-based Gimkit learning media has a positive and significant impact on improving students' historical thinking skills. Thus, Gimkit can be used as an alternative, innovative history learning medium to improve students' historical thinking skills.

Abstract

umbar Herniated Nucleus Pulposus (HNP) is one of the leading causes of low back pain that may reduce patients' quality of life. A high Body Mass Index (BMI) is believed to increase axial loading on the spine, accelerate intervertebral disc degeneration, and worsen the severity of HNP. Magnetic Resonance Imaging (MRI) is considered the gold standard for objectively assessing the severity of HNP. This study aimed to analyze the relationship between body mass index (BMI) and the severity of lumbar herniated nucleus pulposus (HNP) based on magnetic resonance imaging (MRI) radiological findings at RSD Gunung Jati Cirebon in 2024. This research used a quantitative observational analytic design with a cross-sectional approach. Data were obtained from medical records of patients diagnosed with lumbar HNP who underwent MRI examination between January and December 2024. The sample was selected using total sampling, resulting in 44 eligible patients. The independent variable was BMI, classified as normal, overweight, and obesity class I, while the dependent variable was the severity of lumbar HNP based on MRI radiological interpretation, classified as mild, moderate, and severe. Data were analyzed using univariate analysis and Spearman correlation test. The results showed that most patients were in the overweight category, with 19 patients (43.2%), followed by obesity class I in 16 patients (36.4%) and normal BMI in 9 patients (20.5%). The most frequent HNP severity was severe, found in 21 patients (47.7%). Spearman correlation demonstrated a significant relationship between BMI and lumbar HNP severity (p < 0.001; r = 0.775). In conclusion, higher BMI is strongly associated with more severe lumbar HNP based on MRI findings.

Abstract

This research aims to examine the Mipit Pare tradition as an effort to preserve local cultural values in the lives of the people of Ciampel Hamlet. Mipit Pare is a tradition that is carried out before the implementation of the rice harvest as a form of gratitude for the agricultural products obtained and as a form of respect for cultural values that have been inherited from generation to generation. This research uses a qualitative approach with a field descriptive method. Data was obtained through participatory observation and in-depth interviews with traditional leaders, farmers, the younger generation, communities, and religious leaders in Ciampel Hamlet in May 2026. The results of the study show that the Mipit Pare tradition not only functions as a ceremonial activity before harvest, but also contains various cultural values that are important for peoples lives. These values include gratitude, mutual cooperation, togetherness, respect for nature, and the preservation of the cultural identity of the local community. This tradition also acts as a means of inheriting social values and norms to the younger generation so that the sustainability of local culture can be maintained in the midst of the development of the times and modernization currents. In addition, the implementation of the Mipit Pare tradition is able to strengthen social solidarity and strengthen relationships between community members through joint involvement in various series of traditional activities. Thus, the Mipit Pare tradition has a strategic role in maintaining the sustainability of local culture while strengthening the social cohesion of the people of Ciampel Hamlet. Therefore, efforts to preserve this tradition need to continue to be carried out as part of strengthening cultural identity and local cultural heritage.

Abstract

The utilization of Solar Power Plants (PLTS) for micro, small, and medium enterprises (MSMEs) is an effective solution to provide an independent, environmentally friendly, and sustainable electrical energy source. This study aims to design and develop an Internet of Things (IoT)-based solar power system for MSME carts that is capable of supplying electrical energy while providing real-time monitoring of electrical parameters. An experimental method was employed through hardware and software design, system assembly, and performance testing. The proposed system consists of a solar panel, solar charge controller, battery, inverter, voltage and current sensors, and a NodeMCU ESP8266 microcontroller for transmitting monitoring data to an IoT platform. System testing was conducted from 08:00 to 17:00 to evaluate variations in voltage, current, and panel temperature under different levels of solar radiation. The results indicate that the solar panel voltage gradually increased and reached a maximum value of 20.0 V at 12:00, before decreasing as solar radiation declined in the afternoon. The Battery Charge Regulator (BCR) maintained a relatively stable output voltage despite fluctuations in the input voltage from the solar panel. Furthermore, the IoT-based monitoring system successfully displayed voltage, current, and temperature data accurately, continuously, and in real time through a monitoring interface, enabling users to monitor system performance remotely. These findings demonstrate that the proposed IoT-based solar power system operates effectively as an independent energy source and supports more efficient energy management for MSME carts.

Abstract

Hand hygiene is a simple and effective practice to prevent the transmission of infectious diseases and improve public health. A preliminary survey conducted in Soko Village, Lamongan Regency, revealed that many community members were unable to perform the six-step hand hygiene procedure correctly. One strategy to improve this ability is health promotion using interactive video media, which presents educational material in an attractive, clear, and easy-to-understand format. This study aimed to determine the effect of health promotion through interactive video media on the hand hygiene ability of the community in Soko Village, Lamongan Regency. This study employed a pre-experimental design with a one-group pre-test and post-test approach. The population consisted of 84 people, and 70 respondents were selected using purposive sampling. The intervention was conducted over three weeks. Data were collected using a hand hygiene ability questionnaire and analyzed with the Wilcoxon test because the data were not normally distributed. The results showed that the mean pre-test score before the intervention was 2.87 and improved to 1.77 in the third-week post-test. The Wilcoxon test showed significant differences in the first week (p=0.001), second week (p=0.000), and third week (p=0.000), indicating that interactive video-based health promotion significantly improved the community's hand hygiene ability.

Abstract

Perkembangan teknologi Internet of Things (IoT) telah mendorong pengembangan sistem monitoring yang mampu menyajikan informasi secara real-time pada berbagai aplikasi kelistrikan, termasuk sistem Pembangkit Listrik Tenaga Surya (PLTS). Salah satu komponen utama dalam sistem PLTS adalah Solar Charge Controller (SCC) yang berfungsi mengatur proses pengisian baterai dari panel surya serta melindungi baterai dari kondisi overcharge dan overdischarge. Namun, sebagian besar Solar Charge Controller komersial masih memiliki keterbatasan pada sistem monitoring karena informasi parameter kelistrikan hanya ditampilkan melalui layar LCD sehingga proses pemantauan harus dilakukan secara langsung di lokasi perangkat. Penelitian ini bertujuan untuk merancang dan membangun Sistem Monitoring Parameter Solar Charge Controller (SCC) Berbasis Lokal Webserver yang mampu menampilkan parameter kelistrikan secara real-time tanpa memerlukan koneksi internet. Penelitian menggunakan metode Research and Development (R&D) dengan merancang sistem monitoring menggunakan ESP32 DevKit V1 sebagai pusat akuisisi dan pengolahan data, dua sensor INA226 untuk mengukur parameter tegangan, arus, dan daya pada sisi panel surya maupun baterai, serta LCD 16×2 sebagai media monitoring lokal. Data hasil pengukuran diproses oleh ESP32 kemudian dikirimkan menuju Local Web Server melalui jaringan Local Area Network (LAN) sehingga dapat diakses menggunakan browser pada komputer maupun smartphone melalui alamat IP lokal. Dashboard monitoring menampilkan informasi berupa tegangan panel surya, arus panel surya, daya panel surya, tegangan baterai, arus pengisian baterai, daya pengisian baterai, status pengisian, grafik perubahan parameter secara real-time, serta tabel data hasil pengukuran. Hasil pengujian menunjukkan bahwa sistem monitoring mampu menampilkan seluruh parameter kelistrikan secara real-time dengan waktu pembaruan data kurang dari satu detik. Selain itu, Local Web Server dapat diakses secara bersamaan oleh beberapa perangkat yang berada pada jaringan lokal tanpa menggunakan layanan cloud maupun koneksi internet. Berdasarkan hasil penelitian, sistem yang dikembangkan mampu memberikan kemudahan dalam proses monitoring parameter Solar Charge Controller secara real-time, stabil, efisien, dan mudah diimplementasikan pada berbagai aplikasi PLTS skala kecil maupun menengah.

Abstract

The design and development of a Battery Management System (BMS) with an Adaptive Current Protection feature based on temperature aims to improve battery safety, reliability, and service life by automatically adjusting the charging and discharging current according to temperature conditions. This study employed an experimental method by developing a system consisting of a temperature sensor, current sensor, Battery Management System (BMS) module, ESP32 microcontroller, and an Internet of Things (IoT)-based monitoring platform for real-time data visualization. System testing was conducted under various battery temperature conditions to evaluate its ability to monitor temperature, regulate current adaptively, and transmit measurement data to the monitoring platform. The results showed that the proposed system was able to accurately detect battery temperature changes and automatically adjust the allowable current according to predefined temperature thresholds. Under normal temperature conditions, the system allowed the maximum operating current to maintain battery performance. As the temperature increased, the current limit was gradually reduced, and the protection mechanism was activated when the battery reached unsafe temperature levels. Furthermore, temperature, current, and voltage data were continuously displayed on the IoT monitoring platform, enabling users to monitor battery conditions remotely in real time. The study demonstrates that the proposed Battery Management System with Adaptive Current Protection effectively enhances operational safety, maintains battery performance stability, and contributes to extending battery lifespan.

Abstract

Exposure to benzene vapors in the downstream oil and gas industry poses a latent carcinogenic risk that threatens workers' respiratory health and hematopoietic systems. Personal Protective Equipment (PPE), including respirator masks, is the last resort in protecting workers from workplace hazards. This study aims to analyze in-depth the dynamics of worker compliance in the use of respirator masks and identify factors that influence their resistance to use in the filling shed area of ​​PT Pertamina Patra Niaga Fuel Terminal Meulaboh. The method used is descriptive qualitative with a purposive sampling approach. Data were collected through in-depth interviews with five informants (three operational workers, one HSE officer, and one supervisor) and participant observation using 20 field items. The results of the study indicate the occurrence of a phenomenon of pseudo-compliance (surface compliance), where workers only wear respirators during formal supervision. Based on the PRECEDE-PROCEED model, this non-compliance is triggered by a multidimensional accumulation: ergonomic barriers and environmental thermal constraints (enabling factors) in the form of tightness, stuffiness, and trapped sweat; cognitive devaluation and risk perception bias due to the asymptomatic nature of benzene (predisposing factors); and the blunting of organizational control functions (reinforcing factors) due to the absence of written administrative sanctions that provide a deterrent effect. This study concludes that safety interventions cannot be carried out partially, but must integrate engineering .techniques on the aspect of respirator comfort, ongoing behavioral education, and strict enforcement of internal regulations.

Abstract

Measles remains a significant public health problem in Indonesia, with an observed increase in cases following the COVID-19 pandemic, partly attributed to a decline in immunization coverage. This Community Diagnosis Study was conducted in Village G, within the working area of Primary Health Center P, Tangerang District, from December 12, 2025, to January 8, 2026. A total of 38 suspected measles cases were recorded between June and November 2025, showing an increasing monthly trend, with the highest number of cases reported in Village G. In the initial stage, priority setting was conducted using the USG (Urgency, Seriousness, Growth) scoring method, followed by problem analysis using the BLUM Paradigm. Prioritization of contributing factors was further determined using a non-scoring Delbecq technique, which identified the lifestyle factor as the priority domain for further analysis using Fishbone Diagram and the 5 Whys method. The root causes identified included: (1) suboptimal measles health education activities; (2) incomplete documentation of measles immunization status among children under five; and (3) limited communication and coordination between Primary Health Center P and Village G authorities.Three interventions were implemented: measles health education sessions, improvement of immunization status recording for under-five children, and strengthening communication and coordination with Village G stakeholders. The evaluation showed that 90.9% of participants met the passing criteria (post-test score ≥70 and an improvement in knowledge of at least 10 points). Additionally, 83.3% of participants were found to be unvaccinated for measles, and communication and coordination between the health center and village cadres were successfully strengthened. This program demonstrated improvements in participants’ knowledge regarding measles, enhanced identification of immunization status, and improved communication and coordination between the health center and village authorities. Continuous health education, catch-up immunization programs, and advocacy to Village G officials are recommended as follow-up strategies. Through these efforts, it is expected that measles immunization coverage will increase, ultimately contributing to a reduction in measles morbidity and mortality in the working area of Primary Health Center P, including Village G, in the medium and long term.
