Abstract

Elderly go through an aging process, which is marked by various declines, especially in dynamic balance. To address this issue, researchers offer a solution in the form of balance exercises, namely Heel Raise Exercise (HRE) and Chair-Based Exercise (CBE). Heel Raise Exercise (HRE) and Chair-Based Exercise (CBE) are balance exercises done through lower limb muscle contractions and are considered safe and easy for older adults to do. The aim of this study is to test the comparative effectiveness of HRE and CBE on dynamic balance in older adults. This research uses a quasi-experimental design with a pre-test and post-test for two groups, with 15 samples each, namely group A and B at Banjar Graha Shanti, West Denpasar District, Bali, who met the inclusion criteria. The sampling technique used is purposive non-probability. The instrument for measuring dynamic balance used the Time Up and Go Test (TUGT). HRE and CBE exercises were given 3 times a week for 4 weeks. The results of the Paired Samples T-Test pre-test and post-test TUGT in both groups showed a p-value of 0.000 (p<0.05). The results indicate that there was an effect on the improvement of dynamic balance before and after the treatment in both groups. The result of the second hypothesis test, the Independent Samples T-Test between groups, obtained a p-value of 0.048 (p<0.05), which means there was a significant difference between groups in dynamic balance. Group B (CBE) had a higher percentage than Group A (HRE), which was 40.940%. It can be concluded that both exercises have a significant effect on improving dynamic balance, but CBE is more effective than HRE in improving dynamic balance in the elderly at Banjar Graha Shanti, West Denpasar, Bali.

Abstract

Hebephrenic schizophrenia is characterized by disorganized thinking, flat or inappropriate affect, and disorganized behavior. This condition typically appears in late adolescence to young adulthood and is associated with a poor prognosis due to early onset and social functioning. Severe disorganization can lead to persistent decline in social and occupational functioning, especially if treatment is inadequate or not sustained. A 20-year-old man came to the Emergency Department in Hospital accompanied by his family, complaining of unusual behavior. The complaints began in 2019, including disorganized behavior such as talking and laughing to himself without any apparent stimulus, irrelevant responses when spoken to, climbing roof tiles, jumping from the second floor, picking up cigarette butts along the street, taking money without permission, and leaving the house naked. The patient had undergone several months of psychiatric treatment in 2021, but therapy was discontinued. A mental status examination revealed severe disorganized thinking and behavior, flat affect, impaired attention and cognitive function, and auditory hallucinations characterized by talking and laughing to oneself. The patient was diagnosed with hebephrenic schizophrenia and received pharmacological therapy consisting of the atypical antipsychotics Clozapine 1 x 100 mg and Quetiapine 2 x 200 mg combined with Valproic Acid 2 x 500 mg, Lorazepam 2 x 1 mg, and Trihexylphenidyl 2 x 2 mg. Additionally, psychosocial interventions were provided, including psychoeducation. Severe disorganization in hebephrenic schizophrenia is a clinical challenge that requires a sustained multimodal approach encompassing pharmacological therapy and psychosocial interventions. Continuity of pharmacological therapy and psychosocial interventions is crucial for improving social functioning and preventing relapse.

Abstract

This study aimed to analyze the implementation of physical ergonomics, determine the level of employee work comfort, and examine the effect of physical ergonomics on employee work comfort at PT Matahari Putra Makmur. This research employed a quantitative method using a survey approach. Data were collected through questionnaires, observations, and documentation involving 10 respondents. The data were analyzed using descriptive statistics, simple linear regression, partial t-test, and the coefficient of determination with Microsoft Excel. The results showed that the average score of physical ergonomics was 21.30, while the average work comfort score was 20.10. The simple linear regression analysis indicated a positive relationship between physical ergonomics and work comfort, with a correlation coefficient (r) of 0.849, indicating a very strong relationship. The partial t-test revealed that the calculated t-value (4.546) was greater than the t-table value (2.306), indicating that physical ergonomics had a positive and significant effect on employee work comfort. The coefficient of determination (R²) was 72.1%, meaning that physical ergonomics explained 72.1% of the variation in employee work comfort, while the remaining 27.9% was influenced by other factors outside the scope of this study. Therefore, improving the implementation of physical ergonomics is expected to create a safer, more comfortable, and more productive work environment.

Abstract

PT Rivercape Indonesia has been failing to meet its annual production target of 23,000 units on Final Assembly Line F1, with actual output reaching only 20,667 units and an estimated revenue gap of USD 7,821,288.71. This study aims to identify the root cause of this shortfall, formulate improvement alternatives, and select the most appropriate alternative using an Assembly Line Balancing (ALB) approach supported by a Current Reality Tree (CRT) and the Analytic Hierarchy Process (AHP). The research method combined direct observation, focus group discussions, and internal company data collected from January to February 2026. The existing-condition analysis shows that the line operates with 26 workstations and a cycle time of 335 seconds, resulting in a Line Efficiency of only 74.39% and a Balance Delay of 25.61% (categorized as bad according to Yin, 2016). CRT analysis confirmed the root cause as the absence of a mechanism to optimize workstation utilization. Three improvement alternatives were evaluated against a target cycle time of 301 seconds: Alternative 1 (TSALBP Type 2) achieved a Line Efficiency of 82.79%; Alternative 2 (TSALBP Type 1) achieved the most substantial improvement, with a Line Efficiency of 93.90% across 23 workstations; and Alternative 3 (UALBP) achieved a Line Efficiency of 93.59% with a U-shaped layout. Using AHP with a Consistency Ratio of 0.011, Alternative 2 was selected as the most preferred strategy with the highest composite score of 0.500. Implementation of the selected alternative is projected to recover approximately USD 6.42 million of the existing revenue gap through a four-phase, nine-month implementation plan.

Abstract

This study aims to identify potential hazards, analyze the level of occupational accident risks, and provide risk control recommendations for plastic bottle press machine operators at CV Barokah Kayane using the quantitative Hazard Identification and Risk Assessment (HIRA) method. This research employed a quantitative descriptive approach, with data collected through field observations, interviews, documentation, and risk assessments based on likelihood and severity parameters. Risk values were calculated to classify risks into low, medium, and high categories. The results identified five major hazards in the plastic bottle pressing process. Two work activities were classified as high risk, namely feeding plastic bottles into the press machine due to the risk of hand entrapment and arranging compressed plastic bales due to the risk of being struck by heavy bales. Three other activities were categorized as medium risk, including the use of cutters that may cause lacerations, liquid spills from plastic bottles that may result in slips, and machine noise that may lead to hearing impairment. Risk control recommendations were developed based on the hierarchy of controls, including the preparation and implementation of Standard Operating Procedures (SOPs), the use of personal protective equipment (PPE), improvements in work methods, proper manual lifting techniques, routine workplace housekeeping, and increased supervision of operator compliance. The study concludes that the quantitative HIRA method effectively identifies and prioritizes occupational accident risks and can serve as a basis for developing risk control programs to improve occupational safety and health at CV Barokah Kayane.

Abstract

Basic immunization is an effective measure for preventing infectious diseases in infants. The completeness of basic immunization is influenced by various factors, one of which is family support. This study aimed to determine the relationship between family support and the completeness of basic immunization among infants aged 0–11 months in the working area of ​​the Bahu Community Health Center (Puskesmas) in Manado City. A cross-sectional study design was employed with a sample of 61 respondents selected using total sampling. Data were collected via questionnaires and analyzed using univariate and bivariate methods, specifically Fisher's Exact Test. The results showed that 55 respondents (90.2%) received good family support, while 6 respondents (9.8%) received poor family support. A total of 55 infants (90.2%) had received complete basic immunization, whereas 6 infants (9.8%) had not. Bivariate analysis yielded a p-value of 1.000 (p > 0.05), indicating no significant relationship between family support and the completeness of basic immunization among infants in the Bahu Community Health Center's working area. It is concluded that the completeness of basic immunization in this study was not associated with family support and was likely influenced by other factors, such as access to healthcare services, education provided by health workers, and the implementation of the immunization program.

Abstract

Delayed union and nonunion are complications in the healing of long bone fractures that can cause delayed functional recovery, decreased quality of life for patients, and increased need for further interventions. This condition occurs due to disturbances in the bone healing process influenced by various biological and mechanical factors. This literature review aims to identify and analyze risk factors associated with the occurrence of delayed union and nonunion in long bone fractures. This study uses the literature review method thru the process of identification, selection, and analysis of various scientific articles based on inclusion and exclusion criteria. The review results show that risk factors include fracture characteristics such as high-energy trauma, open fractures, soft tissue damage, and bone loss; patient factors such as advanced age, diabetes mellitus, osteoporosis, malnutrition, obesity, and smoking; factors related to the operative procedure such as reduction quality, fixation stability, and timing of definitive fixation; and postoperative complications, especially infections. These factors can disrupt vascularization, osteoblast activity, mechanical stability, and callus formation. It can be concluded that delayed union and nonunion are multifactorial conditions that require early risk identification, optimization of patient conditions, and appropriate fracture management to improve bone healing success.

Abstract

PT. XYZ merupakan suatu perusahaan yang bergerak pada layanan jasa rental dan jasa servis forklift diesel yang mengalami permasalahan tingginya tingkat kerusakan pada sistem hidrolik dan mesin sehingga menyebabkan downtime dan menurunkan produktivitas waktu operasional. Penelitian ini bertujuan untuk mengidentifikasi komponen penting dan menentukan strategi perawatan yang paling efektif menggunakan metode Failure Mode and Effect Analysis (FMEA) dan Reliability Centered Maintenance (RCM). Proses penelitian dilakukan dengan cara observasi, wawancara, dan dokumentasi terhadap data riwayat kerusakan forklift diesel yang kemudian dianalisis menggunakan FMEA untuk mendapatkan nilai Risk Priority Number (RPN) serta RCM untuk menetapkan strategi pemeliharaan. Hasil penelitian menunjukkan terdapat 18 kejadian kerusakan dengan total downtime 84 jam, di mana komponen paling kritis adalah pompa hidrolik (RPN 288), seal silinder hidrolik (RPN 224), injektor (RPN 192), selang hidrolik (RPN 160), dan filter hidrolik (RPN 140). Berdasarkan hasil RCM, strategi pemeliharaan yang direkomendasikan meliputi Scheduled Restoration Task, Scheduled Discard Task, dan Scheduled On Condition Task sesuai karakteristik masing-masing komponen. Penerapan kedua metode tersebut diharapkan mampu mengurangi frekuensi kerusakan, menekan downtime, meningkatkan pendinginan forklift diesel, serta mengoptimalkan biaya pemeliharaan di PT. XYZ.

Abstract

Infection is the leading cause of blindness in developing countries and results in corneal ulcers. If not addressed, increasing tissue damage may result in perforation and visual loss. This case report addresses bacterial corneal ulcers resulting from asphalt exposure. The physical examination indicated a visual acuity of 1/~ in the right eye and 6/6 in the left eye, accompanied with eyelid edema, conjunctival and circumcorneal congestion, a complete corneal epithelial defect, corneal stromal turbidity, and desmetocele. The case was deemed severe, and conjunctival flap surgery was conducted. This case presentation aims to enhance comprehension of corneal ulceration processes and management, particularly in recognizing instances where pharmaceutical treatment is inadequate.

Abstract

This study aimed to formulate and characterize bar soap based on coconut oil and olive oil with the addition of rose essential oil (Rosa hybrida) as an implementation of green chemistry principles. Essential oil was extracted from fresh and dried rose flowers (75 g and 100 g, four replications each) using the Soxhlet method with n-hexane solvent, followed by simple distillation. The best essential oil (fresh flowers, 100 g) was formulated into three soap variations: F1 (100% coconut oil), F2 (100% olive oil), and F3 (50:50 combination), each with 5 mL essential oil added. Characterization included organoleptic, pH, foam stability, antibacterial activity against Staphylococcus aureus and Escherichia coli (well diffusion method), and hedonic tests by 55 panelists. Dried flowers produced a larger oil volume (average 8.8 mL) than fresh flowers (2.45 mL), while fresh flowers produced a sharper aroma. All formulas met SNI 3532:2021 with pH 9.65–10.61. The highest foam stability was achieved by F1 (95.65%), followed by F3 (89.47%) and F2 (87.50%). Antibacterial activity of all formulas was classified as weak (inhibition zones of 1.61–3.11 mm). F3 was the most preferred formula, with the highest estimated selling price (IDR 4,000). The application of green chemistry is reflected in the use of renewable raw materials, relatively safe processes, and biodegradable products.
