Abstract

OBJECTIVE Severe diabetic foot ulcer leads to amputation and is associated with higher risk of mortality. The purpose of this study is to identify clinical correlates of severe diabetic foot ulcers. METHODS The design of the study was cross-sectional survey of medical records on patients treated in Sardjito Hospital from 1 January to 31 July, 2014. Severe foot ulcer was measured as grade 4 and 5 according to Wagner classification. ABI (Ankle Brachial Index) to estimate blood flow to lower extremities and Semmes-Weinstein test using monofilament to detect neuropathy, classified as positive or negative, were done in all patients. HbA1C, serum sodium and creatinine levels were measured during the latest hospital visit. RESULTS The results of this study showed that 77 (36.5%) among 211 patients had severe foot ulcers (grade 4 and 5, Wagner classification). ABI and Semmes-Weinstein monofilament test failed to predict severity of diabetic foot ulcers. HbA1c increased the odds of severe ulcers while higher serum sodium level and higher diastolic blood pressure protected patients against severe ulcers. CONCLUSION Better glycemic control and caution against excessive reduction of diastolic blood pressure, usually due to anti-hypertension medication, should be recommended to prevent the development of severe foot ulcer. Abstract

Autism spectrum disorders (ASD) may affect all spheres of a child’s life. One of the areas of the behavioral spectrum need to be focused is affected. Thus, the study is enthused in impaired emotional regulation (ER) affecting children with ASD. The significance of ER is related to that promoting adaptive behavior in children; indeed, disturbed ER in ASD children leads to emotional and behavioral problems. Commonly children with ASD lack adaptive ER strategies thus they react impulsively and inadequately to emotional stimuli thus it affects their psychological well-being. Though ER in ASD children is intrinsic the nurturing of these children with an understanding of ER can promote better psychological wellbeing. Thus, study objectives to examine a) the impact of ASD on their ER b) the impact of ER on the psychological wellbeing of children with ASD c) To develop awareness among these parents regarding the impact of ER on the psychological wellbeing of their ASD child. d) To propose a new model of parental education about ER and its significance on psychological wellbeing of ASD children. This study takes a base on the keenly examined past evidence on impaired ER in ASD children and its impact on psychological wellbeing. Proposal for aiding ER through parental education has introduced. The conclusion drawn in this study is that the intervention can be more promising with educating parents regarding ER, may help their child to gain maximum from therapeutic intervention. The new proposed model of intervention extends the further scope for research in this regard. Keywords: ASD; Emotional regulation, Parent’s education, Intervention.

Abstract

The aim of this study was to research the comparative effects of ciprofloxacin (CP)  and gentamicin (GT) alone or in combination on the deleterious effects on liver function  and histopathological architecture in rats for  (10) days administration. Doses of 100 mg/kg were used for each of  CP and GT ,and each of both drugs 50mg/kg were combined to estimate the synergistic effect .Thirty two  rats were divided into four groups , group I :control , (1 ml/kg saline orally) group 2: CP  (100 mg/kg orally)  for (10) consecutive days , group 3:  GT  (100 mg//kg) intraperitoneally (i/p)  for (10) consecutive days, group 4: CP  (50 mg/kg orally) plus GT  (50 mg//kg)  i/p  for (10) consecutive days . Blood and liver collected for biochemical and histopathological examination of the liver. The degree of liver injury was determined by measuring biochemical markers, the serum activity of Alanine aminotransferase (ALT), Aspartate aminotransferase (AST) and Malondialdehyde (MDA) levels, as measures of oxidative stress. The results of this study showed that in the serum of the rats in the test groups were found to be significantly higher (p < 0.01) than control group; this effect was found to vary among the groups. These findings are indicative that combination of CP + GT more injury to hepatocytes than GT and CP respectively .The synergistic effects of combination of CP + GT showed the higher levels of liver enzyme and sever liver injury.  Keywords:  Gentamicin, Ciprofloxacin, Combination Hepatotoxicity.

Abstract

Background: Gastric bleeding due to stress-related mucosal damage (SRMD) is a condition that is caused due to erosive gastritis that often occurs in critically ill patients in intensive care. One condition that is known as a risk factor for SRMD is traumatic brain injury. Two common agents used as gastric ulcer prophylaxis are proton pump inhibitor (PPI) and H2 antagonist receptor (H2AR). The goal of this study was to compare the effectiveness of PPI and H2AR administration as prophylaxis of SMRD in patients with TBIs who were treated in the ICU. Patients and Methods: This is a single-blind, randomized, controlled trial with pre and post-test measurements. All subjects were measured for baseline gastric pH before being given gastric ulcer prophylaxis. Gastric pH was measured using a pH-meter. The subjects were divided into two groups: omeprazole group (omeprazole 40 mg every 12 hours) and ranitidine group (received ranitidine 50 mg every 12 hours). The pH levels were measured regularly twice daily for five days.  Results: 56 subjects were involved in this study and divided equally into two groups. For each gastric pH measurement, the pH in both groups did not significantly differ. The optimal gastric pH was achieved in 24 hours after the first administration of gastric ulcer prophylaxis.  Conclusions: The administration of ranitidine or omeprazole is equally effective in maintaining the acidity of gastric acid in TBI patients in ICU. There was no significant difference in the incidence of gastric bleeding in ranitidine and omeprazole groups. Keywords: ICU, Gastric pH, PPI, H2-receptor antagonist.
