Abstract

Food service by the nutrition unit to patients in hospitals is a complex service and requires special attention. Nutrition and diet that enter the patient's body are one of the supports for the recovery of the patient's health. However, plate waste, which is an indicator of patient satisfaction in food service by the nutrition unit, in case hospitals shows a fairly high figure. This study aims to find out the factors that cause food complaints from patients in the nutrition unit and observe what resources have been possessed by Panti Waluya Hospital in its potential to develop services. This research uses qualitative methods. The study was conducted using the Focus Group Discussion (FGD) method in each unit in the hospital and in-depth interviews with five inpatients to explore their satisfaction and expectations of food service in the hospital. The results of the interviews were then transcribed, and then thematic analysis was carried out. The results of the study showed that there were factors that caused patients to feel hesitant or dissatisfied with food service by the nutrition unit in the hospital, which included the factors of man, money, machine, and method. Through better education, it is hoped that it can increase patient satisfaction with food service by the nutrition unit by knowing the causative factors that cause patient dissatisfaction.

Abstract

Waiting time is one of the factors that affect patient perception of the quality of hospital services, and has an impact on patient satisfaction levels. The process of discharge from hospitalization is one of the factors that can cause low patient satisfaction. The purpose of this paper is to describe various understandings of the discharge process in hospitals X. This study uses qualitative methods. Data collection was carried out through unstructured observations and in-depth interviews with staff regarding the process of discharge of patients from hospitalization. This observation was carried out to observe the process of discharge of patients until the patient received medication using a time-motion study. In-depth interviews were conducted with related units such as inpatient units, inpatient pharmacies, inpatient cashiers, and inpatient registration places. The discharge of insurance patients has a different flow from other payment models. The time to discharge patients with insurance is longer because there is a confirmation process from the inpatient registration unit to the insurance company to get the final guarantee. This process takes an average of about 1 hour and is the longest method of patient discharge. This process involves the inpatient registration unit in charge of contacting the insurance to obtain final financing from the insurance.

Abstract

The implementation of the pharmaceutical inventory system at Dr Saiful Anwar Hospital (RSSA), SI PAISUCEN or Sistem Informasi Pharmaceutical Supply Chain with an integrated Pharmaceutical Supply Chain Management (PSCM) approach, still encounters several problems. For this reason, RSSA management modified the inventory system. This research aims to describe the development of the system and find out the results. This study uses a quantitative descriptive approach conducted on the pharmacy inventory system of Dr. Saiful Anwar Hospital. Secondary data in the form of the annual report of Pharmacy Department in 2021-2022 is used to evaluate the effectiveness of the implementation of SI PAISUCEN. The development of SI PAISUCEN features for the 2021-2022 period is carried out in six categories of activities including planning, procurement, receipt, use, reporting, and evaluation. The development of the SI PAISUCEN feature showed positive results in the form of a reduction in costs due to near-expiration by 82.7% for drugs and 69.1% for medical devices, faster information presentation, more accurate data, and better control of stock of goods. The success of the development of SI PAISUCEN can increase the effectiveness and efficiency of pharmaceutical management in hospitals in general.

Abstract

This study aims to analyze the causes of damage to hospital facilities even though they are still newly used. The research was conducted in one of the private hospitals in Malang City using descriptive qualitative methods by conducting a series of observation activities, focus group discussions (FGDs), in depth interviews with 7 people both previous directors, managers and unit heads in the past period and the current period. Identification of factors affecting damage to building facilities and other utilities in the hospital are: 1.) there is no comprehensive monitoring system on a regular basis from the IPSRS unit, 2.) there is no guide to the construction of facilities in the hospital so that units that should be able to participate in the planning and supervision team of the construction process cannot be carried out in an integrated manner, and 3.) The absence of an inventory system of work equipment IPSRS unit so that equipment is often lost due to various factors. It is expected that by knowing the factors that affect damage to facilities in the hospital, management can anticipate both minimizing the potential for damage at this time, and preventing these factors in the future to make improvements.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a growing global health problem. Early screening and a deeper understanding of the risk patient profile are critical for effective management. This study aims to analyze the profiles of patients at risk for COPD based on PUMA scores and FEV1/FVC results from six public health centers in Malang City. This study used an analytical-observational design with a cross-sectional approach. Samples were purposefully selected from patients at risk of COPD in six public health centers in Malang. PUMA scores were measured using a questionnaire. Data was collected from November 2022 to April 2023 at six public health centers in Malang City. A total of 117 research subjects participated, comprising 58.97% male and 41.02% female, with an average age of 47 years. Most subjects were active smokers (43.50%). Based on PUMA scores (cutoff >6), 22.03% were at risk of COPD, while 77.97% were not at risk. FEV1/FVC results were >0.7 in 71 subjects (60.68%) and <0.7 in 46 subjects (39.31%). Among subjects at risk of COPD, 92% had FEV1/FVC < 0.7. There were 117 subjects screened for the early detection of COPD. The results of the PUMA* score can help health practitioners in identifying patients at risk of COPD.

Abstract

Digital media provides opportunities to expand into new markets, offer services and implement new online communication techniques. Patients as service consumers increasingly use the internet or other digital technologies to get the right information. There is not much research on the opportunities of using social influencers as a hospital marketing strategy. Therefore, this research aims to find out the opportunities that can be used by hospitals in the use of social media as a marketing tool with social influencers. The research method used is a literature study. The literature search in this literature review uses databases in the form of Google Scholar, Emerald, Elsevier, and Proquests. From the search results, articles are selected and grouped based on relevance to digital marketing topics and social influencers. A total of 10 articles were reviewed to obtain results in accordance with the purpose of the literature review. The results of the research show that digital technology marketing with social media influencers can provide good opportunities for hospitals. Social media influencers can create interesting content for hospitals so as to build mutually beneficial relationships with influencers whose characters are in accordance with the brand image or hospital. The results of this research can be used by hospital management as a basis for the development of social media with the help of social influencers as a means for hospital marketing.

Abstract

The professionalism of non-healthcare staff plays a vital role in hospital health services. Their professional conduct contributes to the key service quality indicators of tangibles, reliability, responsiveness, assurance, and empathy, thus affecting the overall quality of healthcare services provided to the public. This study aims to analyze the factors influencing the professionalism of non-healthcare personnel at Dr. Saiful Anwar General Hospital (RSSA). Employing an explanatory research design, the study uses quantitative analysis to examine the effects of employment status, remuneration, and promotion opportunities on staff professionalism. Data were collected through a questionnaire distributed to 281 non-healthcare staff at RSSA and were analyzed using logistic regression. The results indicated that employment status did not significantly affect professionalism (t = 2.938, p = 0.087). In contrast, remuneration (t = 7.586, p = 0.006, B = 0.913) and promotion opportunities (t = 8.211, p = 0.004, B = 0.907) both had a significant positive impact. Collectively, these three factors significantly influenced the professionalism of non-healthcare staff (F = 19.007 > 3.08285; p = 0.000 < 0.05). The findings highlighted the importance for healthcare organizations to prioritize fair compensation structures and clear career advancement pathways to enhance employee morale and service quality. Effective management of these factors is expected to foster higher levels of professionalism, which in turn improves healthcare service quality, patient satisfaction, and operational efficiency.

Abstract

Medication error is a mistake in the prescribing process and a failure in the treatment process that has the potential to cause harm and can endanger patients. During the year 2024, 22 medication errors were found in the pharmacy department of Hospital X. Therefore, this study aims to identify the causes of the high incidence of medication errors in the outpatient pharmacy department of Hospital X. This research uses qualitative methods. Data were obtained from primary data through interviews and Focus Group Discussions with 10 informants. Interviews were conducted with the head of the pharmacy installation along with the pharmacy staff, the outpatient installation, and the director. In contrast, the FGD was conducted with the director, the head of the medical support field, and the head of the pharmacy installation. Next, the data is grouped based on themes. After that, root cause analysis is used to determine the main cause. The study results revealed 7 main factors influencing the high rate of medication errors, namely the lack of implementation of electronic prescriptions, causing doctors to still write manual prescriptions, overprocessing during input by pharmacy staff, incorrect screening and input by pharmacy staff, many new pharmacy staff, insufficient human resources in the pharmacy installation, and pharmacy staff who tend to hesitate and fear when making confirmations. The analysis results show that the main cause of medication errors is the lack of implementation of electronic prescriptions, while the use of manual prescriptions complicates the pharmacists' ability to interpret them. After the implementation of electronic prescriptions at Hospital X, the IKP rate became 0 (no incidents occurred) during the observation period. Research shows that medication errors often occur because pharmacy staff find it difficult to interpret prescriptions from doctors who still use manual prescriptions. Electronic prescriptions need to be implemented immediately, with the preparation of a time schedule, the system and procedures to be used, as well as socialization for doctors.

Abstract

Nurse turnover negatively impacts hospital operations, which will result in cost inefficiencies and increased patient safety incidents. Studies have identified job fit (JF) and perceived supervisor support (PSS) as key factors influencing nurses’ intention to leave (NIL). This study aimed to determine whether JF or PSS has a more dominant effect on NIL in a private hospital in Malang. A quantitative cross-sectional design was used in this study, and data were collected using a 5-point Likert scale questionnaire administered to 70 nurses. A census sampling method was used. The data were then analyzed using linear regression to identify the influence of JF and PSS on NIL. The results showed that JF had a non-significant negative effect on NIL (regression coefficient of -0.226), whereas PSS had a significant negative effect (regression coefficient of -0.745). Standardized coefficient measurement indicated that PSS (-0.617) had a stronger effect on NIL compared to JF (-0.149). These results indicate that PSS is the more dominant factor influencing NIL. Hospitals should prioritize strategies related to PSS to reduce turnover by creating a supportive work environment, fostering supervisors’ ability to meet the socio-emotional needs of nurses, and implementing policies that enhance nurse performance and well-being.

Abstract

Trauma is the main cause of morbidity and mortality, especially due to traffic accidents in Indonesia. Bali, as an international tourist destination, faces a high risk of trauma cases, but epidemiological data is still limited. This study aims to analyze the distribution of trauma cases in the emergency room of Bali Jimbaran Hospital and the relationship between risk factors and patient severity. The research design used a cross-sectional retrospective observational analytical study with a total of 6,600 medical records of emergency room patients for the period November 2024–May 2025, of which 1,714 cases were identified as trauma. Independent variables include the type of incident, time of occurrence, and payment method, while the severity of trauma is a dependent variable. The analysis was carried out using chi-square tests, Spearman's rho, and logistic regression. The results showed that the majority of trauma patients were in the moderate (55.4%) category, followed by severe (23.3%) and mild (21.4%). Most patients were of productive age (19–45 years; 42.8%) with a predominance of general surgery cases (52%). Night time was shown to be significantly correlated with severe trauma (p=0.027), while payment methods were not associated with severity (p=0.890). The majority of patients use the BPJS (37.8%) and self-payment (34.8%). In conclusion, the timing factor plays a role in the severity of trauma, while the financing mechanism does not affect clinical outcomes. These findings emphasize the need for driving safety education, optimizing emergency room data recording, increasing the use of special insurance, and strengthening emergency room capacity in tourist areas.
