Abstract

Hypertension is one of the risk factors that can increase the occurrence of depression. Generally, people diagnosed with hypertension have a difficult experience and also often experience somatic symptoms that cause a low quality of life for patients. All of these factors can cause people diagnosed with hypertension to tend to be easier to get psychological pressure, especially depression. The Objectivein in this research is to determine the relationship between the duration of hypertension with the rate depression in hypertension patients at the heart and vascular polyclinic of the Surabaya Jemursari Islamic Hospital. This type of research is analytic observational with purposive sampling. Respondents in this study were hypertension patients who were treated at the cardiovascular and vascular disease polyclinic at the Surabaya Jemursari Islamic Hospital, amounting to 67 people. Collecting data using a depression rate questionnaire Patient Health Questionnaire (PHQ-9). The result is known that from 67 respondents it was found that the majority had hypertension for 1-5 years, namely 37 respondents (55.2%), 6-10 years as many as 12 respondents (17.9%) and more than 10 years as many as 18 respondents (26.9 %). And as many as 13 respondents (19.4%) had no symptoms of depression, 50 respondents (74.6%) had mild depressive symptoms, 4 respondents (6%) had mild depression and there were no respondents with moderate or severe depression. After the data was processed using the chi square test with a confidence probability of p value=0.05 to determine the relationship between the two, the p-value was 0.106, because the p-value>0.05. Then the research hypothesis is rejected. The conclusion is there is no relationship between the duration of hypertension with the rate of depression in patients with hypertension at the heart and vascular polyclinic of the Surabaya Jemursari Islamic Hospital

Abstract

Pulmonary tuberculosis (TB) remains the second leading cause of death from a single infectious agent. The number of people with pulmonary TB in the world still increased, and TB treatment success percentage in Indonesia has not reached the target until now. Genetic polymorphisms are defined as DNA variations seen in ≥1% of the population. The study of genetic polymorphisms can help identify disease risk, predict drug efficacy and safety, and determine the right chemotherapeutic agent with a dose that suits each patient. Various genetic polymorphisms associated with pulmonary TB susceptibility have been found, including polymorphisms of the NRAMP1, HLA-DRB1, vitamin D receptor, TNF-α, and IFN-γ genes. In addition, several genetic polymorphisms associated with TB treatment were also found, such as SLCO1B1, NAT2, SLC2A9, and ABCB1 genes. Detection of genetic polymorphisms in each individual can help predicts TB risk and improve TB treatment success and reduce side effects that may occur due to TB treatment.

Abstract

Preface : Anencephaly is one of the most common forms of neural tube defects and is the second most serious congenital abnormality after congenital heart defects. The overall prevalence of Anencephaly in the world is 5.1 per ten thousand births. Case report : The first patient aged 26 years came without complaints when an ultrasound in the second trimester revealed Anencephaly fetal abnormalities and the pregnancy was terminated with vaginal delivery. The second patient aged 28 years came without complaints when an ultrasound in the first trimester revealed Anencephaly fetal abnormalities and a vaginal delivery was carried out. Discussion : Anencephaly is an abnormality in fetal development characterized by the absence of skull bones accompanied by a lack of most or all of the fetal brain tissue. The main risk factors are lack of folic acid consumption which can occur due to drugs that inhibit folate absorption, the body's increased need for folic acid, and lack of food intake. Early detection requires ultrasound, AFP and acetylcholinesterase examination in the second trimester. The recommended prevention is the consumption of folic acid from preparation for pregnancy until the time of pregnancy.

Abstract

ABSTRACT Background: Skin hydration and sebum are crucial for maintaining the integrity of the stratum corneum. Abnormalities in the stratum corneum lead to skin inflammation such as seborrheic dermatitis, characterized by symptoms like dandruff, oily yellow scales, erythroderma, affecting individuals across various age groups. Purpose: To analyze skin moisture and sebum in patients with Seborrheic Dermatitis. Method: This research employs an observational analytical approach using a case-control design. The total sample size comprises 87 individuals, including 37 patients with seborrheic dermatitis from the Skin Health, Gender, and Cosmetic Health Center in Makassar city, and 50 healthy controls. Results: The correlation between skin moisture levels and seborrheic dermatitis is significant (p-Value 0.000). The majority of patients exhibit dry skin (64.9%), whereas none of the controls experience the same. A small proportion of patients have normal skin (8.1%), in contrast to the controls (100%). Patients with slightly moist skin (27%) are absent among the controls. Similarly, the correlation between sebum levels and seborrheic dermatitis is significant (p-Value 0.000). The majority of patients exhibit excessive sebum (91.9%), unlike the controls. Few patients have normal sebum levels (8.1%), contrasting with the controls (100%). Conclusion: In patients with seborrheic dermatitis, the majority present with dry skin moisture (≤29%) compared to healthy individuals with normal moisture levels (30-60%), and oily sebum levels (≥34%) compared to healthy individuals with normal sebum levels (16-33%). There is a significant relationship between skin moisture and sebum levels and the occurrence of seborrheic dermatitis (p-Value 0.000). Keywords: skin moisture, skin sebum, seborrheic dermatitis

Abstract

Transurethral resection of the prostate (TURP) is a surgical procedure that is the gold standard in treating moderate to severe cases of BPH. Although TURP has been reported as an effective surgical procedure, there are several medical risks that may arise due to TURP procedures. The use of fluid irrigation, a key characteristic of the TURP, can lead to an acute decrease in serum sodium (Na) levels. The conditions of hyponatremia and low plasma osmolarity are directly correlated with the risk of intravascular hemolysis due to increased serum potassium, which then causes hyperkalemia and acidosis. This study aims to examine the serum sodium levels in Benign Prostatic Hyperplasia (BPH) patients undergoing the TURP procedure. This research data is secondary data obtained from medical records at RSI Jemursari in 2022 - 2023. This research was analyzed using a descriptive analytical method to provide an overview of serum sodium levels in BPH patients undergoing the TURP procedure. The results from 20 study samples indicated that the average value of Na levels before TURP was 136.96 mmol/L, while the sodium levels after TURP was 138.02 mmol/L. It was concluded that there was no relationship or difference in sodium levels before and after TURP (p=0.501; 95% CI -4.307 – 2.179).

Abstract

Non-communicable diseases (NCDs) have become the focus of world leaders, especially diabetes mellitus. (DM). Through inflammatory processes such as the formation of free fatty acids and adipokine imbalance, adipose tissue contributes to increased insulin resistance. The city of Surabaya has the highest number of DM patients in East Java, with 102,599 DM patients in 2017 and 115,460 DM patients in 2018. The objective of this study is to determine the relationship between knowledge and attitudes towards the prevention of the progression of type 2 diabetes mellitus in the working area of the Made Health Center in Surabaya. This research uses a cross-sectional approach with the chi-square statistical test method. The results of this study indicate that there is a relationship between knowledge and HbA1c levels (p = 0.035) and for the relationship between attitude and HbA1c levels (p = 0.888). This study can be concluded that there is a relationship between knowledge and HbA1c levels and there is no relationship between attitude and HbA1c levels.

Abstract

Telemedicine services are a part of technological developments in health services which make it easier for patients to get treatment. The doctor-patient relationships continue to change and evolve along with the development of technology. It is necessary for doctors to keep up with the technological development and should be able to utilize the information and communication technology to improve the quality of the services. Medical students are expected to be able to prepare themselves to face technological developments, one of which is providing telemedicine services. The goal to be achieved in this research is to identify challenges and recommendations from Ciputra University Faculty of Medicine students regarding readiness for providing the telemedicine services. The method used is descriptive qualitative and uses the cross-sectional approach. The results of data analysis show that the majority of Ciputra University Faculty of Medicine students have knowledge of what telemedicine services are, used, and also studied about the telemedicine services. The results show that Ciputra University Faculty of Medicine students agree that there are challenges and recommendations to telemedicine services. Abstract

Background: Diabetes Mellitus is a condition characterized by consistently high blood glucose levels due to a chronic metabolic disorder. Type 2 diabetes mellitus accounts for 90% of all cases. According to the 10th edition of the International Diabetes Federation report, Indonesia ranks fifth in the world for the number of type 2 diabetes patients, with 19.5 million affected individuals. Efforts to address the rise in type 2 diabetes cases require effective management to maintain normal blood glucose levels. Currently, oral hypoglycemic drugs are the preferred treatment for type 2 diabetes. However, potential side effects can raise concerns among patients. As a result, alternative approaches, such as herbal therapy, may help reduce the prevalence of type 2 diabetes. Momordica charantia L. is known for its hypoglycemic properties. Methods: The literature search is conducted in PubMed, ScienceDirect, and ClinicalKey databases. The articles used were those published in the last 10 years, and the keywords included "type 2 diabetes," "Momordica charantia L.," "bitter melon," and "blood glucose." Results: Momordica charantia L. contains many compounds with antidiabetic properties. These compounds include polysaccharides, peptides, terpenoids, saponins, alkaloids, flavonoids, and others. Momordica charantia L. contains the insulin-mimetic compound mclRBP-19, which is associated with a decrease in FBG and HbA1c levels. Triterpenoids have been found to stimulate GLUT-4 translocation in muscle and fat cells, as well as inhibit PTP1B activity. Additionally, charantin is known to inhibit DPP4 activity. Conclusion: Momordica charantia L. has the potential to lower blood glucose levels in type 2 diabetes patients through its bioactive components.

Abstract

Dermatitis atopik adalah penyakit kulit kronis dengan prevalensi global yang terus meningkat. Vitamin D3 berperan penting dalam modulasi sistem imun dan fungsi barrier kulit sehingga dapat menjadi terapi tambahan untuk menurunkan keparahan gejala dermatitis atopik. Penelitian ini merupakan tinjauan sistematis dengan menggunakan pedoman PRISMA. Artikel yang dianalisa mencakup penelitian observational dan Randomized Controlled Trial (RCT) yang mengevaluasi efek suplementasi vitamin D3 terhadap dermatitis atopik. Pencarian artikel dilakukan pada PubMed, Scopus, dan Google Scholar dengan menggunakan kata kunci "vitamin D3 supplementation" AND "atopic dermatitis". Seleksi artikel melibatkan penilaian relevansi, telaah teks lengkap dan konsensus antar peneliti. Sebanyak 16 artikel dianalisis, terdiri dari 10 studi observasional dan 6 RCT. Studi observasional menunjukkan adanya korelasi negatif antara kadar vitamin D serum dan tingkat keparahan dermatitis atopik. RCT melaporkan bahwa suplementasi vitamin D3 secara signifikan mengurangi gejala dermatitis atopik, memperbaiki fungsi barrier kulit, dan menurunkan inflamasi. Kombinasi vitamin D3 dengan probiotik dan Zinc menunjukkan efektivitas yang lebih baik dalam mencegah dermatitis atopik pada bayi. Vitamin D3 berpotensi menjadi terapi tambahan yang efektif untuk mengurangi keparahan gejala dermatitis atopik, terutama pada anak-anak. Diperlukan penelitian lebih lanjut untuk mengevaluasi efektivitas jangka panjang dan menentukan dosis yang optimal pada berbagai populasi.

Abstract

Abstrak Sindrom Ovarium Polikistik (PCOS) merupakan gangguan endokrin yang sering dijumpai pada wanita usia reproduktif, ditandai oleh anovulasi, hiperandrogenisme, dan morfologi ovarium polikistik. Kondisi ini juga sering berkaitan dengan resistensi insulin dan peningkatan indeks massa tubuh (BMI), sehingga penting untuk mengevaluasi hubungan antara BMI, HOMA-IR, dan kadar insulin. Penelitian ini bertujuan untuk membandingkan karakteristik klinis dan metabolik antara wanita sehat dan penderita PCOS, serta menganalisis hubungan antara BMI, HOMA-IR, dan kadar insulin pada kelompok PCOS. Penelitian observasional analitik ini menggunakan desain potong lintang dengan total 24 subjek (12 sehat, 12 PCOS). Data dikumpulkan melalui pemeriksaan klinis dan laboratorium, dan dianalisis menggunakan uji Chi-Square untuk data kategorik, serta uji T dan korelasi Pearson untuk data numerik. Hasil menunjukkan perbedaan signifikan antara kelompok sehat dan PCOS dalam hal anovulasi, hiperandrogenisme, dan morfologi ovarium polikistik (p = 0,000). Selain itu, kelompok PCOS menunjukkan BMI dan HOMA-IR yang lebih tinggi secara bermakna (p = 0,003 dan p = 0,000). Pada kelompok PCOS, terdapat korelasi positif yang signifikan antara BMI dan HOMA-IR (r = 0,711; p = 0,010), serta antara BMI dan insulin, maupun HOMA-IR dan insulin. Disimpulkan bahwa wanita dengan PCOS memiliki profil metabolik yang lebih buruk, dengan hubungan yang kuat antara peningkatan BMI dan resistensi insulin.   Kata kunci: Sindrom Ovarium Polikistik, Indeks Massa Tubuh, Resistensi Insulin   Abstract Polycystic Ovary Syndrome (PCOS) is an endocrine disorder that affects women of reproductive age. It is often associated with insulin resistance and increases in body mass index (BMI). This study aims to analyze the correlation between BMI, HOMA-IR, and insulin levels in women with PCOS. This study also compares clinical and metabolic characteristics of these women with those of healthy women. This cross-sectional observational study involved 24 participants consisting of 12 women with PCOS and 12 health women. Clinical and laboratory data were analyzed using Chi-Square test, t-test, and Pearson correlation. The PCOS group showed significantly higher rates of anovulation, hyperandrogenism, and polycystic ovarian morphology (p = 0.000), as well as increased BMI and HOMA-IR (p = 0.003 and p = 0.000). Significant positive correlations were found between BMI, HOMA-IR, and insulin levels in the PCOS group.
