Abstract

Pregnancy is a period of fetal growth and development accompanied by physiological and immunological changes in the mother, making her more susceptible to infections, including COVID-19. Pregnancy accompanied by COVID-19 infection increases the risk of complications for pregnant women. This study aims to determine the prevalence of post-Caesarean section complications in pregnant women with COVID-19. The sampling method employed purposive sampling by observing medical record data at Dr. Ramelan Surabaya Hospital during the period from March 2020 to March 2023. Out of a total of 100 samples, there were 58 confirmed cases of COVID-19, with 8 cases (32.0%) of cardiovascular complications, consisting of 6 cases (75.0%) of PPH complications, and 2 cases (25.0%) of surgical wound infection complications. The prevalence of respiratory system complications included 13 cases (52.0%), with 9 cases (69.2%) of pneumonia complications and 4 cases (30.8%) of Acute Respiratory Distress Syndrome complications. One case (4.0%) of sepsis and 3 cases (12.0%) of maternal death complications were identified. The results of the data analysis using the Contingency Coefficient test showed a p-value of 0.001 (p < α), indicating a significant relationship between COVID-19 infection and an increased prevalence of complications in pregnant women. In conclusion, the study found 25 cases (43.1%) of post-Caesarean section complications in pregnant women with COVID-19, indicating an increased prevalence of complications in pregnant women with COVID-19 post-Caesarean section.

Abstract

ABSTRAK Stres merujuk pada kondisi karena adanya ketidaksesuaian antara harapan dan keinginan, di mana tuntutan dan kapabilitas seseorang untuk memenuhinya tidak sejalan. Diketahui bahwa prevalensi stres pada mahasiswi kedokteran cenderung tinggi karena tekanan yang mereka hadapi, sehingga menimbulkan disparitas dalam tingkat stres antara mahasiswi fakultas kedokteran dan psikologi. Penelitian ini bertujuan untuk mengetahui hubungan antara stres dengan gangguan siklus menstruasi pada mahasiswi Fakultas Kedokteran dan Fakultas Psikologi Universitas Hang Tuah. Metode penelitian menggunakan pendekatan cross-sectional dengan teknik simple random sampling. Penelitian melibatkan 92 mahasiswi Fakultas Kedokteran dan 59 mahasiswi Fakultas Psikologi Universitas Hang Tuah.. Hasil analisis statistik menggunakan uji Spearman, uji Tabulasi Silang, dan uji Kendall's tau-b menunjukkan adanya hubungan yang signifikan antara stres dengan gangguan siklus menstruasi (uji Spearman dan uji Tabulasi Silang: p = 0 dan tingkat signifikansi α = 0,05) dengan nilai koefisien korelasi sebesar 0,412. Temuan ini tidak mengindikasikan adanya hubungan signifikan antara stres dengan fakultas (uji Kendall's tau-b: p = 0,342 dan α = 0,05). Selain itu, tidak terdapat hubungan yang signifikan antara gangguan siklus menstruasi dan fakultas (uji Kendall's tau-b: p = 0,440 dan α = 0,05). Abstract

Preface : Anencephaly is one of the most common forms of neural tube defects and is the second most serious congenital abnormality after congenital heart defects. The overall prevalence of Anencephaly in the world is 5.1 per ten thousand births. Case report : The first patient aged 26 years came without complaints when an ultrasound in the second trimester revealed Anencephaly fetal abnormalities and the pregnancy was terminated with vaginal delivery. The second patient aged 28 years came without complaints when an ultrasound in the first trimester revealed Anencephaly fetal abnormalities and a vaginal delivery was carried out. Discussion : Anencephaly is an abnormality in fetal development characterized by the absence of skull bones accompanied by a lack of most or all of the fetal brain tissue. The main risk factors are lack of folic acid consumption which can occur due to drugs that inhibit folate absorption, the body's increased need for folic acid, and lack of food intake. Early detection requires ultrasound, AFP and acetylcholinesterase examination in the second trimester. The recommended prevention is the consumption of folic acid from preparation for pregnancy until the time of pregnancy.

Abstract

Tujuan Penelitian: Perubahan biologis selama kehamilan dapat menjadi penctus terjadinya gangguan psikologis mulai dari yang ringan hingga gangguan jiwa. Penelitian ini memberikan gambaran kejadian depresi pada ibu post partum yang melahirkan bayi kembar (gemeli) di RSUDD Dr. Soeroto Ngawi selama bulan Januari 2016-Maret 2016. Metode : Penelitian deskriptif ini menggunakan kuesioner Edinburgh Postnatal Depresion Scale (EPDS) pada 12 ibu post partum yang melahirkan gemeli di Ruang Nifas Wijaya Kusuma dan kontrol nifas di Poli Hamil/Kandungan Rumah Sakit Umum Dr. Soeroto, Ngawi selama bulan Januari 2016-Maret 2016. Hasil : Sebagian besar responden (75%) berpendidikan SD-SMP, 67% responden berusia 20-24 tahun, 67% responden tidak bekerja, 58% tidak pernah kontrol rutin selama kehamilan. Enam responden (50%) menyatakan jarang mengalami gangguan tidur, 33% menyatakan bahwa hampir setiap waktu mengalami gangguan tidur karena kecemasan yang dialami, 17% tidak pernah mengalami gangguan tidur. Mayoritas responden (58%) tidak pernah mengalami kejadian/peristiwa berat pada satu tahun sebelum hamil. Berdasarkan hasil Kuesioner EPDS didapatkan 58% responden kemungkinan tidak mengalami depresi, 25% responden kemungkinannya sedang untuk mengalami depresi, 17% responden kemungkinannya tinggi untuk mengalami depresi. Kesimpulan : Sebagian besar responden post partum gemeli yang kemungkinan mengalami depresi berasal dari kelompok usia 20-24 tahun, mempunyai anak lebih dari tiga, dan tidak rutin kontrol di poli RSUDD Dr. Soeroto Ngawi. Oleh karena itu diperlukan kerjasama lintas sektor untuk mencegah terjadinya depresi pada ibu post partum gemelli.

Abstract

Kanker serviks masih menjadi masalah kesehatan masyarakat yang signifikan, terutama di negara berpenghasilan rendah dan menengah, sehingga upaya skrining (deteksi dini) risiko kanker serviks, salah satunya berupa IVA(Inspeksi Visual Asam asetat) penting untuk upaya. Penelitian ini bertujuan mengevaluasi dampak intervensi konseling berbasis Model Keyakinan Kesehatan (Health Belief Model) dalam meningkatkan pengetahuan dan kemauan guru serta staf sekolah untuk menjalani skrining kanker serviks dengan metode Inspeksi Visual Asam Asetat (IVA). Intervensi berupa sesi konseling dilaksanakan di delapan sekolah, dan pengetahuan serta sikap peserta diukur sebelum dan sesudah edukasi. Data hasil penelitian disajikan dalam bentuk tabel, diagram dan dianalisis dengan uji statistik paired-student's T test. Hasil menunjukkan adanya peningkatan skor pengetahuan yang signifikan secara statistik, dari 65,2 ± 12,4 menjadi 84,7 ± 9,8 (p < 0,001). Persentase peserta yang menyatakan kesediaan untuk menjalani skrining IVA juga meningkat dari 58,3% menjadi 87,5% (p = 0,003). Namun demikian, cakupan skrining aktual hanya naik dari 12% menjadi 14% (p = 0,08), masih jauh dari target partisipasi sebesar 70%. Kesimpulan penelitian ini adalah diperlukan strategi komprehensif yang tidak hanya fokus pada peningkatan pengetahuan, tetapi juga mengatasi hambatan psikologis, budaya, dan akses pelayanan. Pendekatan multifaset yang melibatkan edukasi berkelanjutan, dukungan sosial, peran aktif tenaga kesehatan masyarakat, serta peningkatan aksesibilitas layanan skrining diyakini dapat mendorong partisipasi lebih luas. Kesimpulannya, intervensi konseling dapat meningkatkan kesadaran dan kemauan, namun dampaknya terhadap cakupan skrining masih terbatas sehingga perlu didukung langkah strategis yang lebih menyeluruh.

Abstract

Alzheimer's Indonesia (ALZI) reports a significant and growing prevalence of Orang Dengan Demensia Dementia (ODD), which is projected to reach 2 to 4 million individuals by 2050. Dementia is often attributed to cognitive deficits, prompting this study to investigate and compare the Community Mini Mental State Examination (MMSE) scores among elderly professionals, specifically fishermen and non-fishermen, residing in Socah Village, Bangkalan, East Java. Objective: This research aims to elucidate the extent of cognitive impairment in this demographic. Method: An analytical observational study employed a purposive sampling approach, with a total sample size of 96 individuals. The age categories ranged from 45 to 89 years, adhering to the Badan Pusat Statistik 2015 guidelines. Data collection took place during August to September 2022. Results: The results of the Mann-Whitney's test indicate a significant difference in Mini Mental State Examination (MMSE) scores between elderly fishermen and non-fishermen in Socah Village, with a p-value of 0.001 (p < α, α = 0.05). Conclusion: This finding leads to the conclusion that there is a notable disparity in MMSE scores among elderly professionals, highlighting the substantial impact of physical activity on cognitive function.

Abstract

Introduction: Premature rupture of membranes (PROM) remains one of several causes of premature birth that increases both maternal mortality ratio and neonatal mortality rate. Inflammation of fetal membranes leads to release of inflammatory mediators and resulting in weakness of fetal membranes, particularly the amniotic membrane. The amniotic membrane in PROM has a focal weakness, which differs from the membrane in non-premature rupture of membrane (non-PROM) that has a generalized weakness. This research aimed to determine histopathological difference of inflammatory cells infiltration into fetal membranes of PROM and non-PROM.   Methods:  Quantitative observation was applied to this research. Cross-sectional design was used for analyzing data. Primary data was collected from the delivery room and operating theater of Emergency Department in dr. Ramelan Central Naval Hospital (RSPAL dr. Ramelan) Surabaya. Data collection started from August to November 2019 using total population sampling technique. Results: A total of 40 samples histopathologically examined showed that 11 out of 20 (55%) fetal membranes in PROM had polymorphonuclear (PMN) cells infiltration and 9 (45%) did not have PMN cells infiltration, 1 out of 20 (5%) fetal membranes in non-PROM had PMN cells infiltration and 19 (95%) did not have PMN cells infiltration. The prevalence ratio (PR) of PROM based upon the presence of PMN cells infiltration into fetal membranes was 2.85 (PR>1). The significance level of Chi-square test was 0.001 (ρ>0.05). Conclusions: The difference of inflammatory cells infiltration into fetal membranes of PROM and non-PROM was statistically significant.

Abstract

HIGHLIGHTS Vesicovaginal fistula (VVF) cases were primarily caused by hysterectomy, post-caesarean section surgery, and radiation therapy for cervical cancer. Symptoms experienced by patients included urine leakage from the vagina, foul odor, and pain during urination. The success rate of fistula repair using cystography and fistula repair procedures was very high. Patients were able to urinate normally without pain or vaginal leakage.   ABSTRACT Objective: Evaluating the risk factors and outcoumes of surgical interventions in patients with vesicovaginal fistula at Dr. Ramelan Naval Hospital Surabaya, from January 2019 to December 2023. Case Series: There were 6 cases of vesicovaginal fistula that underwent surgical interventions at Dr. Ramelan Naval Hospital Surabaya from 2019 to 2023 that met the inclusion criteria. The majority of vesicovaginal fistula cases occurred as a result of hysterectomy, accounting for 4 cases (67%), followed by caesarean section surgery (16%) and radiation therapy for cervical cancer (17%). Symptoms experienced by the patients included urine leakage from the vagina, foul odor, and pain during urination. The vesicovaginal fistulas that underwent surgical interventions were typically 0.5 to 3 cm in size. Most of the cases involved simple-type fistulas. A total of 5 cases (83%) were successfully repaired. These patients were able to urinate normally, without pain or vaginal leakage. However, 1 case (17%) experienced recurrent fistula with an increase in the number of fistulas and continued to experience pain during urination and leakage from the vagina, particularly during strenuous activities. Conclusion: The vesicovaginal fistula cases at Dr. Ramelan Naval Hospital Surabaya from 2019 to 2023 showed a high therapeutic success rate. Among 6 patients that we studied, the majority of cases were caused by hysterectomy, characterized as simple-type fistulas, with 83% of the cases successfully repaired.
