Abstract

Introduction: Brain tumors are rare with a worldwide incidence of 296,851 cases worldwide. A large tumor size indicates a long duration of silent growth, allowing more time for seizures to develop. The aim of this study was to determine the correlation between tumor location and size in patients with intracranial tumors and the type of seizure. Methods: This study used a cross-sectional design in patients who encountered seizure and had been diagnosed with intracranial tumors undergoing treatment at H. Adam Malik Hospital Medan We used Fisher Exact analysis test to determine the relationship of seizure generation type in intracranial tumor patients based on tumor location and Chi-Square analysis test to determine the relationship of seizure generation type in intracranial tumor patients based on tumor size. This study included 43 patients with intracranial tumors accompanied by seizures. Results: Statistical analysis showed insignificant results between tumor location and type of seizure with  p value of 0.543 was obstained, indicating that the relationship between tumor location and type of seizure was not significant (p < 0.05). The results of statistical analysis showed a significant result between tumor size and type of seizure with p value of 0.048. Conclusion: There was no association between tumor location in intracranial tumor patients and the type of seizure. However, there was a relationship between tumor size in intracranial tumor patients and the type of seizure.

Abstract

Introduction: Red cell distribution width (RDW), a measure of erythrocyte size variability, is associated with acute inflammation. Elevated RDW predicts adverse outcomes in cardiovascular diseases like acute myocardial infarction. This study investigated the relationship between RDW and coronary lesion severity, assessed by the SYNTAX Score, in patients with non-ST-elevation acute coronary syndrome (NSTE-ACS) undergoing coronary angiography. Methods: This retrospective analytical study included NSTE-ACS patients who underwent coronary angiography at H. Adam Malik General Hospital Medan. Baseline characteristics, laboratory parameters (including RDW), and angiography results were collected. The association between RDW and SYNTAX Score was analyzed. ROC/AUC curve analysis assessed RDW's predictive ability for coronary lesion severity, determining sensitivity and specificity. Results: In 160 patients, RDW was significantly associated with SYNTAX Score (p<0.001). TIMI score, GRACE score, and number of involved arteries also correlated with lesion severity. Conclusion: RDW is a promising predictor of coronary lesion severity in NSTE-ACS patients undergoing coronary angiography.

Abstract

Introduction: RPP is used as a marker of cardiac function. RPP reflects the function of the heart in fulfilling oxygen demand stably and continuously during activity. Slow decline of HRR is associated with risk factors for atherosclerosis, similar to hypertension and dyslipidaemia There has not been much research on HRR and RPP in coronary heart disease. The aim of this study is to examine the relationship between RPP and syntax score in stable coronary heart disease. Methods: This study is an observational analytic study with a cross sectional design to determine the relationship between HRR and RPP to the severity of coronary lesions using the syntax score in stable angina pectoris patients. Sampling was carried out at the Haji Adam Malik Hospital Medan from February. HRR and RPP scores were obtained from cardiac exercise testing and syntax scores were obtained from coronary angiography after validation by an interventionist. EF data were taken from echocardiography results validated by consultant echocardiographers. Statistical data analysis using computer statistical tools, p value <0.05 was considered statistically significant. Results: From angiographic data in samples with syntax score ≥ 23, there were 23 (100%) LAD, 21 (91.3%) LCx, and 22 (95.7%) RCA involvement. In samples with Syntax score < 23, there were single vessel lesions as many as 11 (40.07%), two vessels as many as 12 (44.4%), and three vessels as many as 4 (14.8%). From the test results, there was a significant relationship between HRR and RPP and SYNTAX score with a p value <0.05 (<0.001) in both variables. Conclusion: There was a significant association in HRR scores and RPP scores between groups with SYNTAX scores ≥ 23 and SYNTAX scores < 23.

Abstract

Introduction: The act of intubating with an endotracheal tube is a definitive airway management technique that provides maximal protection against the occurrence of aspiration of stomach contents or fluids. Some studies recommend the use of a head pillow to improve the laryngoscopic view by enhancing the occipito-atlanto-axial angle and enlarging the submandibular space. The aim of this study was to find out comparison of the level of difficulty of intubation in intubation techniques with and without the use of pillows. Methods: The design of this study was a randomized cross-sectional study with two research groups conducted to compare the difficulty levels of intubation using the technique with and without a pillow. Results: Patients without a pillow often required a little assistance during intubation, while the majority of patients using a pillow were easily intubated. However, no significant difference was found between the two groups (p>0.05). Patients intubated without a pillow had an average score of 15.9±1.6, while those intubated with a pillow had an average score of 10.8±1.8, showing a significant difference between the two groups (p<0.05). Using the Cormack-Lehane score, most patients without a pillow were at grade 2, while the majority of patients with a pillow were at grade 1. A significant difference was found between the two groups (p<0.05). Conclusion: Patients intubated without a pillow often required a little assistance, while the majority of patients intubated with a pillow were easily intubated; however, no significant difference was found between the two groups (p>0.05).

Abstract

Introduction: Leukaemia is a blood cell malignancy that manifests as young cells in the peripheral blood. In developing countries, ALL account for 80% of leukemia cases. Despite the implementation of contemporary intensive therapy, remission is attained in 98% of patients. LDH have found to have benefit in diagnosing and may be prognostic factor especially in predicting remission in ALL. Methods: This observational cohort study took place at the pediatric ward of RSUP H. Adam Malik – Medan, from August 2022 to February 2023. Subjects meeting the study criteria were assessed to know the relationship between LDH between the initial phase and remission phase of ALL. Multivariate logistic regression analysis was then conducted for the combined scores. Results: We found significant differences in LDH and blast levels between the initial phase and remission phase of ALL (p<0,001).  However, the multivariate logistic regression analysis yielded statistically non-significant results (p > 0.05). Conclusion: In this study, we found a significant differences of LDH and blast between the initial phase and remission phase of ALL. LDH and blast may be a promosing prognostic factor to assess ALL.

Abstract

Introduction: Chronic obstructive pulmonary disease is associated with acute exacerbations that can lead to respiratory failure and as major cause of morbidity and mortality worldwide. The Aim of study was to explore survival rate of COPD patients with type II respiratory failure Methods: Observational research with retrospective cohort design using data from medical records of chronic obstructive pulmonary disease patients with type II respiratory failure treated at H. Adam Malik General Hospital Medan for 10 months from November 2021 to September 2022. Analyzed of independent and dependent variables used Chi Square test. To obtain most dominant independent variable that influence dependent variable, a multiple logistic regression test is used. Results: There were 21 patients (50%) who alived until end of research but also 21 deaths occured. Survival rate was 50% with a median survival time 14 days (CI 95%: 7.178-20.822 days). Survival rate of COPD patients with type II respiratory failure who do not routinely received therapy was 16.7% with a median survival time 4 days (CI 95%: 42.962-5.039). Survival rate of COPD patients with type II respiratory failure who routinely received therapy was 75%. Based on log rank test, there was a significant difference median length of life between patients who did not and who routinely received therapy (p <0.001). Conclusion: The survival rate in COPD patients who regularly use inhaler therapy is significantly higher than patients who do not routinely use inhaler therapy.

Abstract

Introduction: DCM is characterised by myocardial structural and functional abnormalities not caused by ischemia. One cause is hyperthyroidism. Hyperthyroidism can lead to a hyperdynamic circulatory state, increasing cardiac output and metabolic demands, which can ultimately result in heart failure. Case: A 35-year-old woman presented with complaints of acute shortness of breath that worsened at night, accompanied by bilateral lower limb oedema and palpitations. Her medical history revealed hyperthyroidism diagnosed in 2021, but the patient was not compliant with treatment. Physical examination showed low blood pressure (84/60 mmHg), tachycardia 110 Beats Per Minute (BPM), and elevated Jugular Venous Pressure (JVP). Auscultation detected fine bilateral crackles and mitral regurgitation. Bilateral lower limb oedema. Echocardiography showed global hypokinesia with an Ejection Fraction (EF) of 30%, consistent with DCM. Laboratory tests revealed hyponatremia, elevated creatinine, and significantly increased liver enzymes. Initial management included intravenous saline infusion, continuous dobutamine infusion, and high-dose furosemide drip, which did not respond to furosemide. Acetazolamide was introduced as an additional diuretic. The patient also received ramipril, spironolactone, and enoxaparin for comprehensive cardiovascular and electrolyte imbalance management. This case highlights the importance of recognising the multifactorial nature of heart failure, particularly in patients with hyperthyroidism. Conclusion: This case illustrates the complexity of treating DCM with hyperthyroidism and the need for individualized therapy to optimize patient outcomes. The addition of acetazolamide proved effective in addressing the inadequate response to furosemide, emphasizing its role in enhancing diuretic response.

Abstract

Introduction: The increasing trend of e-cigarettes user is often found in productive young people, especially those who are undergoing education. The use of electric cigarettes can cause respiratory complaints such as coughing, coughing up phlegm and shortness of breath. To identify the subject characteristic and to examine association between e-cigarette using period with respiratory symptoms in e-cigarrette male user who are undergoing education in Senior High School and Medical Education in Medan Methods: This study took samples of research subjects using two groups, where each group numbered 15 people, these groups were entered according to the inclusion and exclusion criteria, with the pearson correlation test using the ATSQ instrument to assess respiratory symptoms. Results: The age range of 21-25 years is the most users of e-cigarettes (43.4%), the most education is in the form of professional general practitioner education in this study (43.3%). Respiratory complaints that were often felt by subjects were coughing in the morning which occurred most often 1-2 times a week and shortness of breath during exercise which appeared less than once per week. The pearson test shows r count > r table (0.883 > 0.641) and is positive. Conclusion: There is an association between e-cigarette using period with respiratory symptoms, where the longer you use, the more severe the respiratory symptoms will be.

Abstract

Introduction: Malnutrition is a serious problem in patients with COPD, it interferes with normal skeletal muscle function, with reduced muscle mass and decreased respiratory muscle strength and endurance. It also contributes to respiratory muscle dysfunction, disease severity, and disability progression. Malnutrition is associated with disease severity, associated with more severe exacerbations, and increases the length of hospital stay. Malnutrition in COPD is described with varying prevalence rates ranging from 30-60%. The aim of this study was to analyse the relationship between malnutrition and quality of life in patients with COPD. Method: The research design is an observational analytic conducted from November 2023 to March 2024. The study population was COPD patients who sought treatment at the Pulmonary Polyclinic at RSCPL with a total sample using the consecutive sampling method of 91 people. Manultrition assessment by measuring TB and BW of patients when they arrived and assessing quality of life with the SGRQ questionnaire, Data processing using SPSS by correlating malnutrition with quality of life, degree of shortness of breath, and spirometry. Results: There was a significant positive correlation between malnutrition and quality of life (p=0.001) with the degree of shortness of breath (p=0.014) and spirometry (p=0.001). Conclusion: We found an association between malnutrition and quality of life, degree of breathlessness and spirometry of patients with COPD, further prospective studies in other populations are needed to validate the results of this study.

Abstract

Introduction: Indonesia introduced the NARU lung cancer screening form to assess individual risk levels for lung cancer, classifying them as low, medium, or high risk. This study aimed to analyze the characteristics of low, medium and high risk groups in Medan city. Methods: A quantitative, descriptive cross-sectional design was used, involving 100 individuals from primary health care facilities across 11 areas, who were evaluated using the 9-question NARU form. Evaluation concluded the individuals into low, medium, and high risk of lung cancer. Results: The  majority of the responded high-risk group for lung cancer, the majority were males aged 45-65 years. They had no personal or family history of cancer but were active smokers, worked in carcinogenic environments, and did not live in homes or environments with carcinogenic risks. Additionally, they had a history of COPD and/or TB. In the moderate-risk group, both males and females aged 45-65 years were included, with no personal or family cancer history. They were passive smokers, did not work in carcinogenic environments, and did not live in homes or environments with carcinogenic risks. None had a history of COPD or TB. In the low-risk group, the majority were females under 45 years. They had no personal or family cancer history, were non-smokers, did not work in carcinogenic environments, and did not live in homes or environments with carcinogenic risks. They also had no history of COPD or TB. Conclusion: The study concluded that in Medan, 48% of the population fell into the moderate-risk group for lung cancer, while 31% were in the low-risk group, and 21% were classified as high risk.
