Abstract

Background :Smoking is one of the habits that is inherent in humans because it is addictive. Active smokers in Indonesia have increased significantly in 10 years. Smoking has a negative impact on health, one of which is changes in salivary flow rate. Changes in salivary flow rate in smokers can cause complications in the airway when performing surgery using inhalation general anaesthesia. The purpose of this study was to determine the relationship between smoking status and salivary flow rate in patients with inhalation general anaesthesia. Method : This type of research is observational analytic with a cross sectional approach. The sample in the study was 35 respondents with 4 inclusion criteria. The sample in this study was taken with purporsive sampling technique. Data collection stimulated saliva by suction method. Data analysis using univariate and bivariate analysis, namely the Chi-Square test. Results : The results showed that inhalation general anaesthesia patients were more smokers with 24 respondents (68.6%) and respondents who experienced changes in salivary flow rate obtained the most respondents who experienced hypersaliva as many as 15 respondents (42.9%). The results of bivariate analysis using the Chi-Square test showed that 11 respondents (31.4%) who smoked experienced hypersaliva with a p-value of 0.000 <0.05. Conclusion : There is a relationship between smoking status and salivary flow rate in inhalation anaesthesia patients, namely the increase in salivary flow rate in smoking respondents with inhalation general anaesthesia.

Abstract

Abstract Background: Fenofibrate is a lipophilic drug that is used to treat hypercholesterolemia and hypertriglyceridemia in patients.. The low solubility of fenofibrate can be improved by creating a phenofibrate transfersome delivery system. The objective of this research was to determine the effect of phosphatidylcholine 90G, Tween 80, with sonication on fenofibrate transfersomesand to obtain the optimum composition of phosphatidylcholine 90G, tween 80, with sonication on critical parameters including particle size, zeta potential, absorption efficiency. For formula optimization, the factorial design method was applied. Phosphatidylcholine 90G composition, tween 80, with sonication were used as independent variables, with particle size, zeta potential, and sorption effectiveness as dependent variables. Method: The optimization was carried out with critical parameters of the fenofibrate transfersomes tested including particle size, zeta potential, and adsorption efficiency. The critical parameter test data were analyzed using factorial design. Result: The results showed that there was an effect of phosphatidylcholine 90G, tween 80, and sonication on fenofibrate transfersomes provide effect by including particle size, zeta potential, and sorption efficiency. Conclusion: The optimum proportion of phosphatidylcholine 90G was 95%, tween 80 was 5%, and sonication was 33,39 time. The transfersome ability of fenofibrate was good in the optimum formula in the particle size test was 23,675 nm and zeta potential 52,231 mv and the entrapment efficiency was 96,915 %. Abstract

Stem cell-based therapy is currently believed to be the future of medicine. The type of stem cell that is widely used in cell-based therapy is mesenchymal stem cells (MSCs). In stem cell-based therapy, the MSCs that will be used mostly need to be expanded first by culturing the cells. The serum that is often used in MSCs culturing is a fetal bovine serum (FBS). However, animal components should be avoided in clinical applications and Good Manufacturing Practices (GMP). FBS is not adhering to GMP, which compromises the safety and effectiveness of cell-based therapies. Therefore, other supplements are required for cultures without animal serum constituents. Platelet-rich plasma (PRP) is a plasma fraction derived from blood with a platelet concentration of 3-5 times higher than in blood. Several studies have shown that PRP contains various growth factors that can increase the proliferative and differentiation capacity of MSCs which is equivalent to or even better than FBS. PRP also has the advantage that it can be an autologous or allogeneic product from human peripheral blood, so it is free from animal substances. Therefore in the future PRP has great potential as an alternative to FBS both in culture expansion and in MSCs processing.

Abstract

Background: Congestive heart failure is a progressive disease that causes a decreased quality of life, increases the need for medical expenses, and has a long treatment time. The purpose of this study is to analyze the cost of treatment and see the utility index in patients with congestive heart failure at RSUD Dr. Moewardi Surakarta. Method: The research method used a cross-sectional design, data were collected retrospectively from medical records, financial data, and pharmaceutical installations to see data on direct medical costs and utility data using the EQ-5D-5L questionnaire during March-May. The subjects of the study were 78 outpatient heart failure patients at RSUD Dr. Moewardi Surakarta with inclusion criteria, namely heart failure patients aged > 40 years and complete medical record data including age, sex, comorbidities, degree of severity, and drug therapy. Results: Data were analyzed using the Man Whitney U test and the Kruskal Walis H test. The results of this study showed direct medical costs, namely pharmacy (Rp. 2,837,328.00), service fees (Rp. 150,000.00), medical procedures (Rp. 89,717.95), and administrative fees (Rp. 10,000.00). Total direct medical costs Rp. 3,087,045.95. The results of the EQ-5D-5L questionnaire based on each dimension were 78.2% no problems with walking ability, 97.4% no problems with self-care, 59.0% no problems with activities, 34.6% little had problems with pain, and 57.7% had no problems with anxiety/depression. The utility index value is 0.803. Conclusion: The conclusion is that the average direct medical cost is Rp. 3,087,045.95 and the utility index value is 0.803.

Abstract

Background: Back pain in pregnant women is a common problem that is often experienced during pregnancy, especially in the third trimester. Back pain in pregnant women not only interferes with the mother's quality of life, but can also affect fetal well-being and the delivery process. In an effort to address the problem of back pain in pregnant women, various physical therapy methods have been developed. Two methods that have gained attention are Kinesio techniques and Pelvic Roxing Exercises. Method: This research is a quantitative research with a pre-experimental study that uses a one group pretest-posttest design with no control group. The sample of this study was third trimester pregnant women at PMB Latifah and PMB Zainab Jambi City as many as 60 pregnant women. The sampling technique used purposive sampling technique. This research instrument is a questionnaire. Data were analyzed using the mann whitney test. Results: The results showed that in the Kinesia Taping therapy group it was known that before being given Kinesio Taping treatment, the scale of back pain felt by respondents was 67% feeling moderate pain and 33% feeling severe pain controlled. After being given Kinesio Taping, the pain scale felt by respondents was 57% feeling mild pain and 43% feeling moderate pain. In the Pelvic Roxing Exercise therapy group, it is known that before being given Pelvic Roxing Exercise therapy, the scale of back pain felt by respondents is 9% feeling mild pain, 70% moderate pain and 21% severe pain controlled. After being given Pelvic Roxing Exercise therapy, the scale of back pain felt by respondents was 77% mild pain and 235 moderate pain. The results of the mann whitney test obtained p-value = 0.041, p-value <0.05 so that there is a significant difference between the results after kinesio taping and pelvic roxing exercise interventions Conclusion: Based on the results of the study, it can be concluded that there are differences in the scale of back pain in third trimester pregnant women before being given kinesio taping therapy and pelvic roxing exercise with after being given kinesio taping therapy and pelvic roxing exercise.

Abstract

Background: Acute respiratory infections is an important public health problem, especially in developing countries. One of the mainstay drugs to overcome this problem is antimicrobials, including antibacterials/antibiotics, antifungals, antivirals,antiprotozoa. Various studies have found that around 40-62% of antibiotics are used inappropriately, including for diseases that do not actually require antibiotics. The aim of this study was to evaluate rational drug use. Method: This research is a non-experimental analytical research withobservationaldesigncross sectional by reviewing the medical records and prescriptions of ISPA patients at the District Health Center. West Tanjung Jabung using the POR method,Drug Related Problems (DRPs) PCNE version 9.1,Gyssen, as well as improvement strategies using the Hanlon method. The research was conducted at Sukarejo Community Health Center and Merlung Community Health Center in April-June 2023 by taking medical record data with a total of 502 medical records which were used to evaluate antibiotics using the POR, PCNE V9.1,I agree, as well as Improvement strategies Hanlon. Results: The results of the study obtained the use of antibiotics at the Sukarejo Health Center of 26% with an average prescription of 3.9 prescriptions, while at the Merlung Health Center of 37% with an average prescription of 3.5, with the right indication 100%, the right type of drug 100%, the right dose 100%, the right way of administration 100%, the right duration of use 0%. From the results of the evaluation of antibiotics at the Sukarejo Health Center and Merlung Health Center, a hanlon improvement strategy was obtained by providing solutions to make SOPs (Standard Operating Procedures) and SPM (Standard Medical Guidelines) so that rational use of antibiotics could occur. Conclusion: The improvement strategy using the Hanlon method is to create Standard Operating Procedures (SOP) and Quality Assurance Standards (SPM) regarding the use of antibiotics and drugs used at Sukarejo Community Health Center and Merlung Community Health Center. Abstract

Background: The healthy teeth nutrition card (Kartu Nutrisi Gigi Sehat) is a development model for parenting patterns to prevent stunting in toddlers. These cards were developed for young children and are efficient for children and health practitioners to carry and play with. It is hoped that parenting patterns in the form of healthy dental nutrition can prevent caries which affects food intake and nutritional status. The aim of the research is to develop a healthy dental nutrition card as a parenting pattern to prevent stunting in toddlers. Method: This research is Research and Development (R&D), carried out at Posyandu Widuri VI, Paal Lima Village, Jambi City. Media and material validation involved 6 experts. The research subjects were 16 toddlers, 4 cadres, 4 community health center staff, 2 PKK people, and 16 mothers of toddlers. The data collected using questionnaires. Results: Healthy dental nutrition cards are very eligible for use to promote dental health as a parenting pattern to prevent stunting in toddlers. Conclusion: It is recommended that future research test the effectiveness of healthy dental nutrition cards

Abstract

Background: Stunting is a nutritional problem due to nutrient deficiency during the first 1000 days of life. Efforts to prevent the occurrence of stunting are essential, considering the impact that will be caused. One way is to provide health education to couples of childbearing age so that they can prepare for their pregnancies well. This study aimed to determine the effect of counseling using explosion boxes to increase the knowledge and attitudes of fertile-age couples. This research uses an analytical, operational study with a quasi-experimental research design with a non-equivalent control group design. This research was conducted by providing health education as a form of intervention that was preceded by measurement of knowledge before and after the intervention. Method: The study used a quasi-experimental method with a non-equivalent control group design. This research sample is a fertile age couple in the working area of the Office of the Religious Affairs of Sirah Pulau Padang,Ogan Komering Ilir Distric (OKI) which amounted to 64 respondents. The sampling method uses purposive sampling.The experiment group with the Explosion Box for media education and the control group with the lecture method which was previously given a pretest first and after 14 days. Furthermore, it was analyzed with a dependent sample t-test and an independent sample t-test. Results: The study showed that there was a significant influence in increasing knowledge both in the experimental group (p = 0,000) and the control group (p = 0,000) and the good attitude in the experimental group (p = 0.003) and the control group (p = 0,000). Furthermore, there was no significant difference between respondents' knowledge in the experimental & control group (p = 0.413) with a difference of 0.78 and the attitude of the experimental & control group (p = 0.607) with a difference of 0.98. Conclusion: Counseling with an explosion box is more effective in increasing the knowledge and attitudes of fertile-age couples, even though there is no statistical difference between an explosion box and a lecture. Abstract

Background: MMR and IMR in Indonesia are still high compared to the targets set by the government and SDGs. One way to address this is through the implementation of 24-hour PONEK. The implementation of PONEK is important to note because quality PONEK implementation can reduce MMR and IMR. This study purpose was to provide a more comprehensive understanding of the implementation of PONEK in hospitals Method: This type of research is a literature review using Google Scholar, PubMed, and Scopus databases with keywords implementasi OR pelaksanaan OR penerapan AND PONEK OR EmONC OR CEmONC. The inclusion criteria were the implementation of PONEK in Indonesia in all types of hospitals, using Indonesian and English, publication year range 2012-2022, original research. Results: The lack of health worker training was the main problem with PONEK program inputs. As for the PONEK program process, communication within the team was good and the implementation of monitoring and evaluation has gone well. As for the output of the PONEK program, there are no studies detailing the achievement of targets. Conclusion: The implementation of the PONEK program has been running, but there are still some things that have not been implemented properly. Further research is needed to evaluate the output of PONEK implementation.
