Abstract

Abstract

Abstract Background: Surgery and anesthesia can affect the immune and neuroendocrine systems which can cause the release of hormones and cytokines as a result of changes in the body's physiological responses. Neuroendocrine pathways to the immune system related to stress responses due to surgery. surgery and a decrease in the number of T lymphocytes causes a shift in the balance between immunosuppressive regulatory T lymphocytes and immune-triggering T helper and cytotoxic T cells in a predominance of regulatory T cells. Apart from causing a decrease in the number of T lymphocytes, it also causes suppression of NK cells and an increase in the number of neutrophils. Research purposes determine the effect of various anesthetic techniques on changes in leukocyte count levels. Method: This research is a type of experimental research with a pre-pos test group design approach. The samples for this study were taken from patients undergoing surgery and anesthesia with inclusion criteria: age 17-65 years, ASA 1-2, no infectious diseases. The sample size is the sampling technique is Consecutive sampling. Sample measurements were taken from venous blood before and after surgery and anesthesia. The parameters measured are leukocyte counts which include leukocytes, basophils, neutrophils, monocytes and lymphocytes. Blood samples were taken before surgery and anesthesia, and 2 hours after surgery and anesthesia using a hematology analyzer. The analytical tests used are the parametric dependent test (paired T-test) or the non-parametric Wilcoxon test and the independent test. Results: There is a significant difference in the count of leukocyte types between general anesthesia and regional anesthesia Conclusion: General anesthesia can increase leukocyte counts significantly compared to regional anesthesia, but eosinophils are not significant in both groups

Abstract

ABSTRACT Nursing services require an effective and efficient nursing management system, so that nursing resources can be directed to produce quality nursing services, one of which is through nursing supervision. Supervision is a personal observation of functions or activities, in the nursing care process, and delegations of functions or activities to maintain accountability and evaluate nursing care appropriately. This research aims to develop a clinical supervision application to improve the quality of implementation of nursing supervision in Jambi city hospitals. Clinical supervision research is designed with an application as guidance and information for room heads, team leaders, and implementing nurses in carrying out clinical supervision so that it can improve the quality of nursing supervision in hospitals. Research design with a mixed methods approach, namely qualitative research on conceptual development and quantitative research used in the product development and testing phases. The research procedure uses analysis-design & development-formative evaluation steps with the Dick and Carey procedure. The research was carried out in three hospitals in Jambi City, namely Abdul Manap Hospital, Bratanata Hospital, and Bhayangkara Hospital Jambi in April - August 2023 with the research subjects being all room heads totaling 16 people and team leaders totaling 24 people. There is a difference in user knowledge about nursing supervision before and after the development of the Android-based clinical nursing supervision educational application with a p-value of 0.000 < 0.005. The model for developing nursing clinical supervision applications using the Dick and Carey learning design approach is a systematic learning design model for developing effective and efficient learning programs. Nursing clinical supervision educational applications can be applied in hospitals by room heads in carrying out supervision in inpatient rooms. Abstract

Background: Non-communicable diseases (NCDs) that cause early death both in the general population and among lecturers are increasing. This study aimed to analyze factors related to NCDs, as well as early death among lecturers. Method: This study design was cross-sectional. The sample was 4 lecturers who died in the 2013-2022 period to identify causes of early death and 75 homebase lecturers for NCDs incidents at the Jambi Ministry of Health Polytechnic. Results: The number of early deaths was 4 people (1.75 person-years) and the incidence of NCDs was 17.3%. Factors associated with NCDs were workload (PR= 8.25; 95% CI= 1.96-34.61), family history (PR= 4.50; 95% CI= 1.83-11.05), stress (PR= 3.98; 95% CI=1.54-10.26) and rest patterns (PR 3.61; 95% CI 1.32-9.87). There was no relationship between diet and physical activity and the incidence of NCDs. The dominant factor was workload after controlling by family history, diet, rest patterns and stress (PR= 5.127; 95% CI=1.02-25.60). Conclusion: Lack of rest, family history, stress and workload increase the risk of NCDs and early death in lecturers. The lecturers must pay attention to their workload, implement a balanced nutritional diet, rest patterns, and stress management, especially for those with a family history of NCDs to prevent NCDs and early death.
