Abstract

Latar Belakang: Down syndrome merupakan gangguan perkembangan yang terjadi sebagai akibat dari kegagalan pembelahan sel pada embrio, yang seharusnya membuat dua salinan kromosom 21 namun menghasilkan tiga salinan, sehingga bayi baru lahir memiliki 47 kromosom, bukan 46 pada umumnya. Fisioterapi berperan dalam mengatasi  problematik pada down syndrome seperti gangguan impairment, disability, dan activity limitation sehingga pasien dapat beraktivitas secara mandiri. Isi: Penelitian ini bertujuan untuk mengetahui bagaimana proses penatalaksanaan fisioterapi dengan pemberian intervensi metode bobath pada pasien down syndrome. Metode: Desain yang digunakan laporan kasus (case report) dengan melakukan anamnesis, pemeriksaan meliputi inspeksi, palpasi, ROM, pemeriksaan khusus joint laxity, sensory profile, dan GMFM-88, serta intervensi fisioterapi dengan menggunakan metode bobath, dan evaluasi fisioterapi. Hasil: Setelah dilakukan 3 kali tindakan fisioterapi terdapat peningkatan tonus otot, pada saat berdiri dengan fiksasi hip pelvic anak sudah stabil, perubahan respon anak pada permasalahan gangguan taktil, namun belum ada peningkatan pada kemampuan fungsional pada anak. Kesimpulan: Metode bobath efektif diberikan kepada anak down syndrome.   Kata kunci: Down syndrome; Fisioterapi; Tonus postural; Bobath; Gross Motor Function Measure

Abstract

Background. FHP is a condition where the head shifts forward past the vertical line of the body. In recent years, the prevalence of musculoskeletal disorders in adolescents has increased, which is thought to be due to the use of school bags. Purpose To determine the relation between bag load and the incidence of FHP in students. Method. This study was conducted with a population of students in grades 10-12 who were measured for 5 school days. The determination of the sample of this study used the purposive sampling method and obtained a sample of 300 people. This study measured the burden of students' bags and measured CVA and TFL with the APECS application. Result. In this study, the results showed that the average weight of student's bags during a week was 2.07 kg. The weight of the bags carried by women tended to be heavier than men. In the CVA measurement when wearing a bag, there was an increase in students who experienced FHP. In the TFL measurement when wearing a bag, there was a variation and increase in the degree of TFL, compared to the neutral position. Conclusion. The weight of the bag and the incidence of FHP have a significant relation with the weak category. The comparison between the average body weight and the average weight of the bag carried by students in one week is categorized as normal. Abstract

Background: Life-Space Mobility (LSM) is a concept that assesses a person's mobility based on the range of movement and participation in daily activities. Meanwhile, the Life-Space Assessment (LSA) is one of the LSM measurement tools that was initially developed for the elderly population and is now being applied to post-stroke patients. However, a more in-depth review is still necessary regarding the psychometric properties of LSA in post-stroke patients. Objective: This literature review aims to evaluate the psychometric properties of LSA in post-stroke patients and review its potential application in Indonesia. Methods: A literature search was conducted on the PubMed database, and Google Scholar with keywords related to LSM, LSA, stroke, and psychometric properties. Studies that met the inclusion criteria were then evaluated using the psychometric property quality criteria from Terwee et al. (2007). Results: Of the 822 articles found, five studies were selected based on the inclusion and exclusion criteria. LSA showed good content validity, with adequate internal consistency (Cronbach's alpha 0.73–0.85). Construct validity varied, with several studies showing weak correlations with other measuring instruments. Reliability is generally high (ICC > 0.70), while responsiveness and floor/ceiling effects still require further research. Conclusion: LSA is a potential measurement tool to assess LSM in post-stroke patients. However, further research is needed regarding its validity and responsiveness, especially in the context of the Indonesian language and culture, to ensure its accurate and relevant application.

Abstract

Background: Constraint-induced movement therapy (CIMT) has demonstrated effective as a neurorehabilitation intervention for addressing "learned non-use" and enhancing functional independence in stroke patients. However, its application to lower extremities (LE-CIMT) remains relatively unexplored. Several studies have shown inconsistent results, particularly regarding the effects of LE-CIMT on balance and lower extremity function in post-stroke patients. Objective: To evaluate the impact of LE-CIMT on balance and lower extremity function in post-stroke patients. Method: A literature review was conducted by searching for relevant articles in PubMed, Physiotherapy Evidence Database (PEDro), Cochrane Library, and Google Scholar from December 2024 to February 2025. Selected articles were appraised using the PEDro scale to assess the level of bias in randomized controlled trials. Results: Seven articles meeting the inclusion and exclusion criteria were identified, all with moderate levels of bias. Data extraction revealed significant improvements (p<0.05) in balance and lower extremity function following LE-CIMT intervention. These improvements are attributed to mechanisms of cortical reorganization. Conclusion: LE-CIMT demonstrates positive effects on balance and lower extremity function in post-stroke patients. Abstract

Background: The prevalence of Chronic Obstructive Pulmonary Disease (COPD) in Indonesia has shown a significant increase from year to year. A hallmark of COPD is chronic obstruction of the airways. Rehabilitation in patients with COPD is necessary to prevent further complications and to restore respiratory function and daily activity capacity. Objective: This case report aims to explore the physiotherapy management of functional activity limitations caused by respiratory dysfunction due to shortness of breath and coughing resulting from COPD. Method: This program was carried out directly on a respondent with respiratory dysfunction characterized by shortness of breath and coughing due to COPD. The therapy was administered in three sessions. Assessments included muscle strength using the Manual Muscle Test Grading, dyspnea using the Borg Scale, lung volume through thoracic cage expansion and metline measurements, risk of pressure sores using the Braden Scale, and functional activity using the Barthel Index. Results: After three therapy sessions, there was a decrease in dyspnea scale from 2/10 to 0.5/10. Additionally, lung volume showed an increase in the xiphoid process area with a difference of 2 cm, although it had not yet reached the normal category. Conclusion: Physiotherapy management in a patient with Chronic Obstructive Pulmonary Disease (COPD) demonstrated positive outcomes. Through the application of techniques such as breathing control, pursed-lip breathing, segmental breathing, and diaphragmatic breathing over three sessions, there was a significant reduction in shortness of breath and an improvement in lung volume.
