Abstract

Introduction: Subcutaneous mycosis (SM) is a fungal infection involving dermis and subcutaneous tissue, which can disseminate slowly through systemic blood or lymphatic circulation. The subacute or chronic infection usually found in workers of rural tropical and subtropical area. SM occurs due to trauma along with other predisposing factors such as sex, genetic and occupation.Aim: To identify the types of SM, its clinical findings, laboratory work-up and the treatment at the Dermatomycology Division, Department of Dermatology and Venereology dr. Cipto Mangunkusumo National Hospital between the year 1989 and 2013.Method: This retrospective study collected data from medical records and case reports of patients with SM who came at Department of Dermatology and Venereology dr. Cipto Mangunkusumo National Hospital (1989-2013).Results: We found 16 cases of SM, i.e. subcutaneous mucormycosis (5 cases), eumycetoma (4 cases), actinomycetoma (4 cases) and chromoblastomycosis (3 cases). There was a greater number of male than female patients ratio (3:1) and mostly were in the age group of 25-44 years. The direct microscopic examination did not reveal any fungal element, except for black spora in chromoblastomycosis (1 case). The culture revealed Basidiobolus ranarum in subcutaneous mucormycosis (5 cases), Nocardia transvalensis in actinomycetoma (1 case) and Phialophora sp. in 1 case of chromoblastomycosis. On histopathological examination, we found fine granules of actinomycetoma, sulphuric granules of actinomycetoma, coenocytic hyphae of subcutaneous mucormycosis, eosinophilic granule in 1 case of mycetoma and hyphae with black spore in chromoblastomycosis.Conclusion: SM is still a rare disease, comprehensive management of SM needs supporting laboratory work-up, particularly the histopathological examination.

Abstract

Allergic contact dermatitis (ACD) is a cutaneous inflammatory reaction caused by contact with specific exogenous allergen to which a person has developed allergic sensitization. Nickel was reported as one of the most common causes of ACD worldwide, particularly affecting females. It is associated with a variety of nickel-containing products, from jewelry to detergent. A 32-year-old female was presented with pruritic eruption consist of erythematous papules and vesicles on both hands and extensor forearms since 2 weeks before admission. The patient is a housewife who did handwashing clothes and kitchenware with detergent and soap every day. At first, there were erythematous papules on the right extensor forearm, then the patient wore rubber gloves to protect hands when washing. However, the lesions spread to the left extensor forearm and there were also vesicles on both hands. There was history of localized reactions to metal items, the latter one was 3 weeks ago after wearing a metal necklace. The clinical presentation were multiple erythematous papules and vesicles, erythematous and hyperpigmented macules. The patch test using Chemotechnique Diagnostics AB and square chamber showed a 2+ reaction to nickel sulphate. The patient was educated to avoid the causative allergen and treated with oral antihistamine and topical corticosteroid. The patient showed significant clinical improvement after 2 weeks. Nickel as metal catalyst used in detergents is found in small concentration, but can lead to ACD in an already sensitized individual. The lesions persist despite the use of gloves because nickel is absorbed through rubber material.

Abstract

Aims: This study investigated the seroprevalence and sexual risk behavior factors associated with syphilis and herpes simplex virus type 2 (HSV-2) among men who have sex with men (MSM), in Manado, Indonesia. Methods: Serum samples and questionnaires regarding their socio-demographic and sexual behaviour were collected from 60 MSM in Manado. Samples were screened for anti-HSV-2 IgG for herpes, the Venereal Disease Research Laboratory (VDRL) and the Treponema Pallidum Haemagglutination (TPHA) test for syphilis. A cross sectional method was used in this study. Results: Of 60 MSM (median age was 20 years), 35% had homosexual orientation and 65% had bisexual orientation. First sexual intercourse of most participants (48.3%) within 16 to 18 years old. In the last 12 months, most participants (55%) had only one male sex partner. There were 63.3% and 16.7% participants performed oral and anal intercourse. About 23.3% participants used condom consistently, but only 35.7% used compatible lubricant. Seropositivity of VDRL and TPHA was detected in 5% participants and anti-HSV-2 IgG in 23.3% participants. The positive serology test for syphilis was associated with sexual intercourse mode (p= 0.001). Significant association was found between anti-HSV-2 IgG seropositivity and condom-use behaviour (p=0,028). Conclusions: The study demonstrated 5% and 23.3% positivity of serology test for syphilis and anti-HSV-2 IgG respectively. Seropositivity of syphilis was correlated with oral and anal intercourse while HSV-2 with no condom-use behaviour among MSM in Manado, Indonesia.

Abstract

Introduction: Aging is a biological process that can induces changes to the structural integrity and physiological function of skin. Structural changes of skin are a result of dermal atrophy, decreased collagen, the loss of subcutaneous fat, the loss of inherent elasticity, and increased melanogen.1,5,6 From the definition itself, a stem cell characterized by its ability to self-renew and also have an ability to differentiate along multiple lineage pathways.2,4 ADSC which is a kind of mesenchymal stem cell from human adipose tissue, have some potential properties as stem cells derived from bone marrow.4,7 Objective: The aim of this study was to verify the comparison effect of ADSCs and their secretome for skin aging treatment. Methods: ADSC were isolated from lipoaspirates obtained from healthy donors after obtaining written consent and ethical approval using liberase enzymatic digestion, washed with steril PBS and centrifuged. After the phase, ADSC were seeded directly after isolation with liberases and cultured, then produced the conditioning media (AAPE) by hypoxia amplifies.3,4,6,7 We analysed the ADSCs (2,5 x 105 cells) and their secretomes (0,1ml in 0,4ml NaCl 0,9%) by subcutaneous injection on the back of a rat (with range age 48 weeks), and followed began every two weeks after injected until six weeks and stained with Van Giesson staining, for measured the density of collagen.3,5,6 Results: Collagen density was increased after ADSC injected to the skin. From statistical analysis showed there was a significant result of collagen density compared to control group and AAPE (p<0.05). We conclude that ADSC had anti-aging potential or regenerative by stimulating collagen synthesis of dermal fibroblast. ADSC will be as new modality treatment for aging skin in future.

Abstract

Leg ulcers one common problem in the adult population to geriatric with a prevalence about 1-2%. The pattern of life which tends to be sedentary, less activity, obesity, increased the incidence of leg ulcers. The duration of healing and recurrence occurs in majority of patients contributed the decline in quality of life for patients with venous ulcers. Diagnosis, latest treatment and prevention are essential to discuss.

Abstract

Background: The clinical manifestation of Darier-White disease, an autosomal dominant genodermatosis, are greasy hyperkeratotic papules in seborrheic regions with nail abnormalities and mucous membrane changes due to a defective sarcoendoplasmic calcium pump. The clinical appearance and unpleasant odor in some sites of the lesions may cause psychosocial disturbances and pose a major morbidity. Case Illustration: We report a case of a 20-year old female with clinical manifestations of multiple erythematous to hyperpigmented hyperkeratotic papules, multiple flesh-colored keratotic papules with red and white longitudinal bands on dystrophic fingernails, as well as plantar pits. Skin punch biopsy was performed to support the diagnosis. Urea-containing emollients and acitretin at a dose of 0.3 mg/kg/day were prescribed for the patient. Discussion: Our case showed the classic clinical manifestations of Darier’s Disease. The skin punch biopsy result is also consistent with Darier’s disease. We provided the patient with emollients and acitretin, a systemic retinoid, which are found to be the most effective treatment for DD. While no relationship is established between Darier’s disease and other medical problems, the patient developed anxiety and social isolation due to the clinical appearance and odor, creating a negative impact on her quality of life. Abstract

Background: Demodicosis is a disease, caused by parasitisation of the opportunistic parasites from the acariasis group – Demodex mites. This article presents a comparative study of two methods (light microscopy of skin scrapings and confocal laser scanning in vivo microscopy) for identification of Demodex mites on the facial skin in acne and rosacea patients. The use of confocal laser scanning in vivo microscopy in dermatology today is considered as one of the most promising methods. Methods: A total of 90 subjects were included in the study, comprising 30 patients with acne and rosacea complicated by demodicosis, 30 patients with acne and rosacea not complicated by demodicosis, and 30 healthy volunteers. All patients were examined by scraping of the skin, eyebrow and/or eyelash epilation and confocal laser scanning in vivo microscopy. Results: The specificity of light microscopy of skin scrapings was 65.5%, while the specificity of confocal laser scanning in vivo microscopy for the diagnosis of demodicosis was 68.8%. Conclusion: The study showed advantages of confocal laser scanning in vivo microscopy compared to the traditional method of investigation.

Abstract

Background: Vitiligo is a skin pigmentation disorder, affecting approximately 1-2% population worldwide. Currently, there are wide varieties of treatment to achieve re-pigmentation, and phototherapy is one of the effective treatments. Narrowband ultraviolet B (NB-UVB) is administered twice a week according to the phototherapy protocol for vitiligo. Some vitiligo patients have difficulty following this schedule, leading to decrease of patient’s compliance. Therefore, we performed study comparing the efficacy of phototherapy frequency, once a week and twice a week NB-UVB in vitiligo. Methods: This preliminary study divided 10 patients into 2 groups of 5 patients each with randomized, double-blind method. Group A was scheduled for NB-UVB phototherapy twice a week while Group B was scheduled for once a week. All patients received topical desoxymetasone cream once daily. Re-pigmentation were compared using Vitiligo Area Scoring Index (VASI). VASI was graded as good (≥25%), moderate (10-24%), and poor (<10 %) responses. Evaluation for melanin index for each lesion was assessed by Mexameter® and cumulative doses of phototherapy were compared. Results: Thirty-seven lesions were treated on face and neck, trunk and extremities. Good responses were obtained in 14.29% group A patients and 13.04% group B patients. The mean of cumulative doses phototherapy in group A was 850 mJ/cm2, whereas group B was 800 mJ/cm2. There was no significant differences (p>0.05) between both groups in terms of re-pigmentation, cumulative doses of phototherapy, and melanin index. Conclusion: In vitiligo, once a week NB-UVB phototherapy appears to be as efficacious as twice a week treatment schedule.

Abstract

Background: Kerion celsi is a severe inflamed deep abscesses form of tinea capitis which mostly occurs in children population. Despite good regular treatment with antifungals, the prognosis of kerion celsi is poor with scarring alopecia. Case Illustration: A 5-year-old boy had history of an alopecia with locally large painful oedema on his scalp a month before treatment. From the physical examination, a painful large area with alopecia was found with abscess covered by thick crust. Pull test showed hair breakage and right retro-auricular lymphatic nodes enlargement was also found. Endo-ectothrix spores were positive in potassium-hydroxide examination and the culture showed colony of Trichophyton mentagrophytes. A surgical debridement was performed to remove the thick crust covering the abscess with alopecia and followed by application of normal saline gauze dressing and terbinafine cream on the top of the lesion for 6 weeks. Systemic micronized griseofulvin was also administered for 6 weeks. Combination treatment of debridement-topical and systemic antifungal gave a good result without scarring alopecia. Discussion: Kerion celsi was diagnosed based on clinical findings and mycological examination. From the laboratory examination, a dermatophyte fungal infection was found. Skin debridement was performed to create a good environment for topical antifungal application and wound healing, with 20mg/kg bodyweight of systemic griseofulvin administered as the first line therapy for tinea capitis. A good prognosis was achieved after those treatments combination. Conclusion: In this case, combination between skin debridement with topical and systemic antifungals gave a better result in treating kerion celsi.

Abstract

Skin aging is a complex biological process in human being, as a result from intrinsic factors (genetic, hormonal, metabolism) and extrinsic factors (UV exposure, pollution, smoking, life style). In postmenopausal women, physiologically, the amount of estrogen are decreased, causing deterioration of their skin’s appearance. Along with the changes that occur, skin care require more attention. A holistic approach can increase epidermal thickness, therefore, reducing the depth of wrinkles, as well as improving skin texture and moisture. There are various treatments available to improve skin appearance due to aging. This EBCR is aimed to compare the efficacy of topical estradiol and topical glycolic acid as skin treatment in postmenopausal women. Articles were searched through Pubmed/MEDLINE, EBSCO, and Cochrane. One randomized-controlled trial by Fuchs KO, et al was obtained and critically appraised. Based on the appraisal, study by Fuchs KO, et al is considered valid, important, and applicable. Both estradiol and glycolic acid show good efficacy and safety for postmenopausal women with signs of skin aging, however estradiol is not considered to have better efficacy than glycolic acid.
