Abstract

Medical informatics as a basic medical science that participates in formal medical decision-making processes is integrated with the formation of medical databases and administrative data with features of automating work process technology in health care institutions. This study aims to analyze and find out the analysis of the application of health information systems to health insurance payments. This research is a literature review sourced from several online sites such as Google Scholar, Pubmed, and Refseek by including keywords “Pharmaceutical Preparations”, “Support”, “National Health System”. The inclusion criterion used was the implementation of a health information system for payment of health insurance using the PRISMA plot diagram. The results of this study found that the implementation of health information systems for health insurance payments in health facilities is still not optimal. Many factors have affected the overall operation of the system, where the quality and quantity of human resources in handling health information systems is still lacking, the lack of facilitating facilities and infrastructure sometimes still causes disruptions and requires improvement

Abstract

An integrated referral system (Sisrute) is an internet-based technology that connects patient data from lower service levels to higher or equivalent service levels (horizontally or vertically) to simplify and speed up the referral process. Sisrute has been officially implemented since 2016 but there are still limited studies on Sisrute implementation. The purpose of this study is to find out the limits of introduction of Sisrute in Indonesia. The author conducted a literature review based on journals and conference papers in the last 5 years (2018-2023). There were 8 articles that met the inclusion criteria. The results of the review found that Sisrute implementation constraints consisted of 3 aspects, namely organization or governance, human resources (users), and application system features. The organizing aspect includes leadership commitment; person responsible; reporting and evaluation; standard operating procedure and special Sisrute team; Sisrute referral acceptance flow; supporting facilities; Sisrute information media; referral hospital commitment; and update of Sisrute component information data. User aspects include response time or speed of response from referral hospitals; completeness of referred patient data; system use skills and training; as well as user perceptions regarding convenience, interest, benefits and supporting circumstances. Aspects of application systems include server or network issues; appearance and completeness of system components; mobile version system features; and utilization of application features.

Abstract

Long waiting time is one component that has the potential to cause patient dissatisfaction. If the waiting time is long, it will reduce patient comfort and affect patient utility in the future. The method used in this research is Literature Review, namely by collecting and processing research data that has existed before with the 2018-2022 range. The results of this study found that there was a waiting time for drugs, both concoctions and standard drugs, that were not in accordance with the minimum service standards, so that waste was found. There are various types of waste that are found resulting in long drug waiting times such as too many processes to the long process of submitting drug prescriptions to pharmaceutical installations, which causes many inefficient and ineffective activities to arise. Based on this, there is a waiting time for drugs, both concoctions and standard drugs, that are not in accordance with the minimum service standards, so that waste is found. Thus the length of waiting time in preparing the drug is influenced by several factors such as speed and completeness of patient data up to re-submission which takes a lot of time.

Scabies is a parasitic infectious disease (Sarcoptes scabiei var. hominis) which is still a public health problem in the world, including Indonesia. Scabies can be transmitted through direct and indirect contact with the greatest prevalence in children, densely populated populations, poor sanitation and hygiene. Symptoms and signs of scabies include papules, vesicles, pustules or nodules accompanied by itching at night with a predilection for a thin epidermal layer. This study is a case report that aims to examine the accuracy in diagnosing and providing pharmacological and non-pharmacological (communication, information and education) treatment of scabies patients. Primary data were obtained through anamnesis (by autoanamnesis and alloanamnesis with the patient's family members) and physical examination by conducting home visits, filling out the family folder, and filling out patient files. The assessment was carried out based on the initial holistic diagnosis, process and end of the visit both quantitatively and qualitatively. In the study, a 10-year-old boy patient was found at the Pediatric Polyclinic at the Dewantara Health Center with complaints of itching all over the body, especially between the fingers, since the last 8 months. Complaints of itching worsen at night so that it disturbs the patient's sleep. The patient has two siblings who live in Dayah with the same complaint and occasionally stay at home with the patient. The patient's mother also had the same complaint. On physical examination, HR: 88x/I, RR: 18x/I, temperature: 36.5°C and nodules were found in the left palmar region of the manus, and canaliculi in the form of white straight lines with papular tips. Management in the form of communication, information and education on environmental sanitation and hygiene as well as pharmacological treatment.

According to the WHO classification, COVID-19 patients could be classified with severity as uncomplicated illness, mild pneumonia, severe pneumonia, acute respiratory distress syndrome (ARDS), sepsis and septic shock.Case illustration: A 25 year old female, with a chief complaint of shortness of breath and cough, history of cesarean section surgery with indications of severe preeclampsia and hysterectomy for uterine atony. weeks previously the patient was confirmed positive for COVID-19 without symptoms and tested negative through the Polymerase Chain Reaction (PCR) follow up examination. The Chest X-Ray showed the right lung atelectasis, bronchitis and minimal pleural effusion. CT scan of the chest showed a ground glass opacity in almost all bilateral lung segments with septal thickening in according with the picture of COVID pneumonia with cardiomegaly. The patient was intubated and put on a ventilator due to acute respiratory failure and the drugs include antiviral (oseltamivir), chloroquine, antibiotic (meropenem) and steroid (methylprednisolon) intravenous.Discussion: Post COVID-19 fibrosis is one of the emerging complications of COVID 19 pneumonia and ARDS. This case report highlights ARDS in patient with Pulmonary Post Covid-19 and its challenges in diagnosis and treatment. The combination treatment of mechanical ventilation and drugs such as antiviral, chloroquine, antibiotic, steroid intravenous could give the good outcome for the patient of ARDS due to Pulmonary Fibrosis Post COVID - 19

Abstract

Malnutrition is a form of malnutrition caused by inadequate food intake and recurrent infectious diseases. This study aims to determine the relationship between LBW and the incidence of wasting in children aged 6-23 months in Pagedangan District, Tangerang Regency in 2020. The research design used was cross-sectional. Malnutrition (wasting) is measured using the indicator body weight according to body length (BB/PB). Data collection was carried out by anthropometric measurements (body weight and length) and questionnaire interviews with respondents (mothers of research subjects). The results of the study showed that 17.0% of children aged 6-23 months in Pagedangan District, Tangerang Regency experienced wasting. Out of 153 children aged 6-23 months, 52.9% did not reach the minimum acceptable diet, 32.0% had a deficit in energy intake, and 52.9% had a deficit in protein intake. The results of the chi-square analysis showed that the incidence of LBW had a significant relationship with malnutrition (wasting). Multiple logistic regression tests show that energy intake is the dominant factor for wasting in children aged 6-23 months in Pagedangan District, Tangerang Regency in 2020 (OR=5.616; 95% CI: 1.193-26.438).
