Abstract

Inaccuracy in diagnosis coding can reduce the validity of morbidity data and lead to potential errors in health service claim tariffs. This study aims to analyze the accuracy level of the principal diagnosis code for Diabetes Mellitus (DM) cases in inpatients based on ICD-10 at RSUD dr. KH. Idham Chalid for the period of January–March 2026. This study employed a descriptive quantitative method with a retrospective cross-sectional design. The study population included all medical records of inpatients diagnosed with Diabetes Mellitus from January to March 2026. Sampling was carried out using a total sampling technique (saturated sampling), resulting in 117 medical record files. Data collection was conducted through document review of medical record recapitulations, which were subsequently analyzed using univariate percentage frequencies. The results showed that out of 117 medical record files, the accuracy rate of the principal diagnosis code reached 86.32% (101 cases). Meanwhile, the inaccuracy rate was recorded at 8.55% (10 cases), and the remaining 5.13% fell into ambiguous cases requiring further review or incomplete data. The primary factor causing code inaccuracy (70% of inaccurate cases) was the incorrect selection of the primary code from secondary diagnoses instead of the principal diagnosis. Based on the results of the study, it was concluded that the quality and accuracy rate of principal diagnosis coding for Diabetes Mellitus cases at RSUD dr. KH. Idham Chalid is fairly good and consistently above 80%. Nevertheless, periodic evaluation of diagnosis documentation completeness by attending physicians and enhancing coders' competencies remain necessary to optimize coding quality.

Abstract

The implementation of Electronic Medical Records is one of the digitalization concepts in improving the quality of hospital information systems. This study aims to determine the overview of medical data security in the Medical Record Installation. This study uses a descriptive qualitative method through observation of the implementation of electronic medical records in the medical record installation, in-depth interviews, questionnaires, and documentation studies, and uses three health information data security standards, namely confidentiality, integrity, and availability. The results of the study indicate that there are still discrepancies with health information security standards, namely confidentiality, where password sharing still exists and password changes have not been carried out periodically. Staff should have their own accounts and passwords created by the IT department, as well as education regarding periodic password changes for medical record staff. In addition, regarding availability, audit trails have not yet been implemented. The IT department needs to conduct an audit trail every six months to monitor data changes, enhance accountability, and prevent misuse. This study concludes that the implementation of electronic medical records at Azra Hospital Bogor is quite good because it accelerates access to medical data; however, the features in the electronic medical record application need to be further developed in order to optimally meet the needs of Azra Hospital.

Abstract

Birth weight is an important indicator of neonatal health and is associated with subsequent child growth and health outcomes. Three maternal factors, namely body mass index (BMI), hemoglobin levels, and maternal educational attainment, have been extensively studied in relation to birth weight; however, findings across populations remain inconsistent. This non-systematic narrative review searched PubMed, ScienceDirect, Google Scholar, and GARUDA for publications from 2021 to 2026 (searches conducted through September 2026), using three separate search groups addressing maternal BMI, hemoglobin levels, and education in relation to birth weight. Twenty studies met the inclusion criteria, including four studies conducted in Indonesia. The findings showed that seven studies on BMI reported associations with birth weight or related growth and birth outcomes, although the direction and outcomes assessed varied considerably. Seven studies examining hemoglobin levels demonstrated heterogeneous patterns of association, with some studies reporting non-linear relationships between hemoglobin levels and birth weight outcomes, while others identified associations only at the anemia end of the spectrum. Six studies on maternal education showed the greatest inconsistency: four reported significant associations with growth or neonatal outcomes, whereas two found no significant association. In conclusion, maternal BMI and hemoglobin levels were associated with birth weight or related growth outcomes, although the underlying mechanisms could not be established with certainty based on the reviewed evidence. Maternal education demonstrated the most heterogeneous pattern, with variations in findings across different populations and social contexts. Not all reviewed studies specifically examined term infants.

Abstract

Total Knee Replacement (TKR) is a surgical procedure performed to replace a severely damaged knee joint caused by osteoarthritis. Patients following TKR commonly experience pain, limited range of motion (ROM), decreased muscle strength, and impaired functional ability, requiring appropriate physiotherapy management. This case study was conducted on a patient following Total Knee Replacement at RSUD Bandung Kiwari who underwent six physiotherapy sessions. Outcome measures included the Visual Analog Scale (VAS), goniometric assessment, Manual Muscle Testing (MMT), and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). The intervention program consisted of ultrasound, ankle pumping exercise, knee extension exercise, and heel slide exercise. The results demonstrated a reduction in pain, improvement in knee joint range of motion, increased muscle strength, and enhanced functional ability in performing daily activities. These findings indicate that the combination of ultrasound and exercise therapy was effective in supporting the rehabilitation process and improving functional outcomes in a patient following Total Knee Replacement at RSUD Bandung Kiwari.

Abstract

Plantar fasciitis is an inflammation of the plantar fascia. The physiotherapy problems identified included: (1) pain in the sole of the foot, (2) muscle spasm of the gastrocnemius muscle, (3) limited range of motion (ROM) in both ankle joints, (4) bilateral ankle muscle weakness, (5) decreased functional activity, and (6) tenderness in the calcaneal area. This study was conducted at Cahya Kawaluyan Hospital using a case study design involving a patient with bilateral plantar fasciitis et causa calcaneal spur who received physiotherapy interventions consisting of infrared (IR), Transcutaneous Electrical Nerve Stimulation (TENS), and exercise therapy, including free active exercise, calf raises, and active stretching. Data collection and analysis were performed using auto history. The instruments used focused on pain, muscle spasm, ROM, muscle strength, and functional activity. After four treatment sessions, the results were as follows: (1) pain decreased from 1.8 at T1 to 0 at T4; (2) muscle spasm decreased from 1 at T1 to 0 at T4; (3) ROM increased, with right ankle dorsiflexion improving from 15° at T1 to 25° at T4, right ankle eversion from 10° to 15°, left ankle dorsiflexion from 10° to 20°, and left ankle eversion from 5° to 15°; (4) muscle strength increased from grade 4 at T1 to grade 5 at T5; and (5) functional activity in daily living improved. The conclusion of this study is that TENS (Transcutaneous Electrical Nerve Stimulation), infrared (IR), and exercise therapy, consisting of free active exercise, calf raises, and active stretching, can reduce the problems associated with bilateral plantar fasciitis et causa calcaneal spur.

Abstract

Pharmaceutical services aim to ensure safe, correct, and rational drug use. However, the practice of purchasing hard drugs without a doctor’s prescription still frequently occurs in the community, often caused by a lack of understanding regarding drug classification and the resulting risks. This study aims to determine the level of community knowledge concerning drug classification, purchasing regulations, and the dangers of using medicines without prescriptions at Hasanah Farma Pharmacy. This research applied a descriptive quantitative method using a survey approach. Samples were taken using accidental sampling technique involving 96 respondents visiting Hasanah Farma Pharmacy during February–March 2026. Data collection used a questionnaire covering aspects of knowledge about drug classification, side effects, risk of resistance, and legal regulations. Data were analyzed descriptively using frequency distribution and percentage, and relationships between variables were tested using Chi-Square statistical test. The results showed that the community’s level of knowledge was generally categorized as moderate (49.0%), followed by low (30.2%), and high (20.8%). Most respondents understood that antibiotics are classified as prescription-only medicines, yet many were still unclear about the duration limit of use and the risk of bacterial resistance. Statistical analysis showed significant relationships between education level, information source, and knowledge level (p < 0.05). In conclusion, public understanding is not yet fully optimal. Active involvement of pharmacy professionals in continuous health education is necessary to foster a culture of rational and safe medication use.
