Abstract

Abstract

Acute appendicitis is one of the most common abdominal surgical emergencies encountered in young adults and requires prompt diagnosis and management to prevent complications such as perforation, intra-abdominal abscess, and peritonitis. This study aimed to describe the clinical characteristics, diagnostic process, surgical management, and clinical outcomes of a young adult patient with acute appendicitis through a case report combined with an evidence-based literature review. Case data were obtained from the patient's medical records, including medical history, physical examination, laboratory investigations, imaging studies, operative procedures, and postoperative evaluation, and were subsequently compared with current international guidelines and recent scientific evidence. The findings demonstrate that integrating patient history, physical examination, laboratory parameters, clinical scoring systems, and imaging modalities improves diagnostic accuracy and reduces the rate of negative appendectomies. Appendectomy, particularly through the laparoscopic approach, remains the gold standard treatment because it offers a high therapeutic success rate, a lower risk of complications, a shorter hospital stay, and faster postoperative recovery compared with conservative management. Although antibiotic therapy may be considered in carefully selected patients with uncomplicated appendicitis, surgical intervention continues to provide superior long-term clinical outcomes. Therefore, the implementation of a comprehensive diagnostic approach, timely clinical decision-making, and adherence to evidence-based guidelines are essential for improving patient safety and the quality of surgical care in cases of acute appendicitis.

Abstract

Acne (acne vulgaris) is a skin condition caused by various factors, one of which is infection by the bacterium Propionibacterium acnes. The leaves of the lemon pothos (Epipremnum aureum) are known to contain secondary metabolites such as flavonoids, alkaloids, saponins, tannins, and triterpenoids that have potential antibacterial properties. This study aims to determine the physical quality of an ethanol leaf extract gel formulation of golden pothos, its antibacterial activity against Propionibacterium acnes ATCC 6919, and to identify the optimal gel concentration for inhibiting the growth of this bacterium. The study showed that all gel formulations met physical quality requirements, including organoleptic properties, homogeneity, pH, viscosity, and spreadability. Phytochemical analysis revealed that the ethanol extract of lemon-leaved betel leaves contains flavonoids, alkaloids, saponins, tannins, and triterpenoids. The average inhibition zone diameters against Propionibacterium acnes were 6.1 mm at 4%, 7.4 mm at 8%, and 9.3 mm at 16%, respectively, while the positive control measured 12.1 mm, and the negative control showed no inhibitory effect. The results of the One-Way ANOVA test showed a significant difference among the treatment groups (p = 0.006).

Abstract

Cervical cancer is one of the cancers with a high incidence rate among women. Chemotherapy, as the main treatment, can cause Adverse Drug Reactions (ADRs), which can affect the patient’s quality of life. This study aimed to determine patient characteristics, the incidence of chemotherapy side effects (ADRs), and quality of life, as well as to analyze the influence of chemotherapy side effects (ADRs) on the quality of life of cervical cancer patients at Dr. Moewardi Hospital Surakarta. This was an analytic observational study with a cross-sectional approach. The study sample consisted of 66 patients selected through total sampling. ADR incidence was assessed using the Naranjo Algorithm, severity was graded according to the CTCAE criteria, and quality of life was measured with the EORTC QLQ-C30 questionnaire. Data analysis was performed using univariate and bivariate methods. The majority of patients were aged 41–60 years, had an elementary school education, were housewives, were married, were in stage III, underwent paclitaxel-carboplatin combination chemotherapy, and most experienced ADRs categorized as "probable." A total of 71.2% of patients had a moderate quality of life. Pearson Chi-square test results showed no significant effect of chemotherapy ADRs on the quality of life in cervical cancer patients (p=0.327). Chemotherapy side effects (ADRs) did not significantly impact the quality of life of cervical cancer patients at Dr. Moewardi Hospital Surakarta. The patient's quality of life can be influenced by factors beyond the variables studied, such as family and social support, level of spirituality, Subjective Well-Being (SWB), psychological conditions, levels of anxiety or depression, and economic status.

Abstract

The management of medications in hospital pharmacy storage facilities is vital for maintaining the accessibility, integrity, and safety of pharmaceuticals. If the protocols for storage and distribution do not adhere to regulations, it can result in damage, expiration, shortages, or excess inventory. Thus, it is important to assess how medications are stored and distributed within hospital pharmacy storage facilities. The purpose of this research was to examine the practices for the storage and distribution of medications at the pharmacy warehouse of Dr. Moewardi Regional General Hospital (RSUD) and to evaluate the level of compliance with relevant medication management guidelines. A descriptive approach was utilized, incorporating both qualitative and quantitative techniques. Data collection involved observations using checklists, semi-structured discussions with warehouse staff, and reviews of administrative records. The data analysis was descriptive, comparing observed practices in the field against the defined indicators and guidelines for medication management. The evaluation also included checking medication conformity with the National Formulary and categorizing them by usage frequency. The findings revealed that the medication storage procedures comprised 30 indicators, with 29 (96.7%) meeting the required standards and 1 (3.3%) falling short. For the distribution procedures, 15 indicators were assessed: 13 (86.7%) were implemented, and 2 (13.3%) were not. The alignment with the National Formulary was recorded at 92.37%. The classification based on usage rates revealed categories of "fast-moving" and "slow-moving" medications, which need to be monitored for optimal inventory management. In summary, the medication storage and distribution practices at the pharmacy warehouse of Dr. Moewardi RSUD are largely effective and compliant with standards, though certain indicators require enhancement. Ongoing assessment and inventory management are essential to improve the overall efficiency of medication processes.

Abstract

The implementation of Indonesian Diagnosis Related Groups (IDRG) in the healthcare financing system requires the readiness of Hospital Management Information Systems (SIMRS) and adequate human resource (HR) competencies in managing medical record data, coding, and claims. This study aimed to analyze HR competencies in the readiness to implement SIMRS to support the optimization of IDRG claims at Amanda Hospital, South Cikarang. A descriptive study with quantitative and qualitative approaches was conducted. Data were collected through questionnaires, interviews, observations, and documentation involving 20 informants selected using purposive sampling, comprising medical record officers, coding staff, casemix staff, SIMRS administrators, and information technology personnel. Quantitative data were analyzed descriptively based on the percentage of indicator achievement, while qualitative data were analyzed using the Miles and Huberman model. The results showed that HR competency in the knowledge domain was generally good, with 90% of informants understanding the basic concepts of IDRG, although they still required assistance in its implementation. In the skills domain, 80% of informants were able to operate SIMRS and perform coding according to established procedures. Regarding attitudes, 75% of informants demonstrated commitment and responsibility toward IDRG implementation, while 95% expressed readiness to support its implementation. Management support, training, and cross-unit coordination were reported by 90% of informants, although 25% identified unequal distribution of training as a challenge. HR competency and SIMRS implementation readiness increased from 45% to 90% during January–May 2026. These findings indicate that HR competency is an important component of SIMRS implementation readiness and IDRG claim optimization. Continuous training, technical assistance, and strengthened cross-unit coordination are needed to ensure HR readiness and improve the quality of claim data.

Abstract

Medical logistics management plays a critical role in healthcare facility operations, directly impacting patient safety and budgetary efficiency. This study aims to analyze the medical logistics management system at Bedas Kertasari Regional general Hospital (RSUD) to identify the daily practices and operational processes of the system. Utilizing a qualitative approach with a case study design, data collection was conducted through participatory observation and in-depth interviews with pharmacists, warehouse staff, and hospital asset management personnel. The analysis reveals that while procurement efforts are being made to integrate an e-purchasing system, RSUD Bedas Kertasari still faces several crucial challenges. The primary obstacles include data fragmentation between the planning unit and the pharmacy warehouse, lead time fluctuations of up to 22.7% in deliveries due to geographical conditions, and suboptimal digitalization in the implementation of the First Expired First Out (FEFO) system. For future research, it is recommended to conduct an evaluation study using the Human-Organization-Technology (HOT-Fit) model after the regional public hospital (RSUD) management has fully integrated the Hospital Management Information System (SIMRS) into the logistics unit, in order to assess its effectiveness in reducing lead times.

Abstract

Completion of surgical informed consent forms is an important component of the Electronic Medical Record (EMR) because it relates to administrative aspects, quality of care, and legal protection. However, incomplete entries in several components of the surgical informed consent form were still found at Azra Hospital. This study aimed to determine the compliance rate of surgical informed consent form completion in the EMR at Azra Hospital for the period January – March 2026 based on the completeness of each form component. A descriptive quantitative method with total sampling was used, including 131 surgical informed consent forms. Data were obtained from form recapitulation in Microsoft Excel and the Teramedik application. The results showed that the compliance rate for completing surgical informed consent forms was categorized as good. The component with the highest completeness rate was the patient’s signature at 97,71%, while the lowest completeness rate was found in the family witness name at 78,63%. The study concluded that compliance in completing surgical informed consent forms at Azra Hospital is generally good, but improvement is needed for certain components, particularly the name and signature of the family witness, so that all forms are fully completed according to regulations.
