Abstract

Mental health issues of concern in Indonesia are severe mental disorders, including schizophrenia, which have a significant impact on patients and society. This study aims to evaluate the effectiveness of the implementation of the Implementation Strategy (SP) to control violent behavior by verbal means in managing the risk of violent behavior in patients with mental disorders at Soerojo Hospital. The research method is a descriptive case study with a nursing process approach. Data collection was carried out by means of observational interviews, and patient medical design documentation studies. This research instrument uses a nursing care format. The subjects in this case study are schizophrenic patients at risk of violent behavior at Soerojo Hospital. The results of the study showed that the training to recognize risks and control violent behavior physically, verbally, and spiritually was effectively applied to patients, as seen in Mr. F who successfully controlled violent behavior by following the SP to control violent behavior in a verbal way. The implementation of this strategy reduces the incidence of violent behavior, improves safety, and improves the quality of life of patients. This study also highlights the importance of training and education for health workers as well as the role of family support in managing violent behavior. In conclusion, the implementation of SP I, II, and III at Soerojo Hospital is effective and can be a model in dealing with the risk of violent behavior in patients with mental disorders, so that it is expected to improve the quality of mental health services in Indonesia.

Abstract

Acute inflammation in pneumonia is the result of an acute respiratory infection that is usually accompanied by coughing, shortness of breath and inflammation of the lung parenchyma. This can lead to increased secretion production and often causes the main problem of ineffective airway clearance. The purpose of this scientific work is to determine the effect of simple eucalyptus oil vapor inhalation therapy in reducing the frequency of breathing in children with pneumonia. The main ingredient in eucalyptus oil is the compound cineole (eucalyptol) which is known for its anti-inflammatory, antioxidant, bronchodilating (relieving the airway), antiviral and antimicrobial effects. This effect of cineole also has a mucolytic effect (dilutes phlegm) so that the secretion becomes thinner and easier to remove. The method used is a case study by going through the stages of the nursing process including assessment, diagnosis, intervention, implementation and evaluation. The purpose of this scientific work is to determine the effect of simple steam inhalation therapy on reducing the frequency of breathing. The nursing care process was carried out for 3 days starting April 22, 2024 - April 24, 2024. The results of the nursing care process that has been carried out, it is found that simple steam inhalation therapy with eucalyptus oil is effective for reducing the frequency of breathing in patients with airway clearance problems.

Abstract

Planning of pharmaceutical preparations, medical devices and BMHP (Medical Consumables) is the initial stage to determine the type and amount of pharmaceutical preparations, medical devices and BMHP that suit needs. Procurement is an activity to realize planned and approved needs, through the purchase, production/manufacture of pharmaceutical preparations, and donations/dropping grants. This type of research uses research Descriptive Qualitative With research methods aim to make a picture or description of a situation objectively. This research was conducted using a semi-structured interview method that was included in the category Indepth Interview Namely free interviews so that the implementation is wider when compared to structured interviews. This research was conducted at RSU Muhammadiyah North Sumatra in December 2023. Based on the results of research on drug planning and procurement at the Muhammadiyah Hospital in North Sumatra using the consumption method carried out based on sales patterns or depleted stocks in pharmaceutical warehouses. The distribution of drugs from pharmaceutical warehouses to pharmaceutical installations is often delayed so that pharmaceutical services to patients are disrupted and cause patients to wait a long time. The obstacles seen in drug procurement are still not optimal, this is because there are empty and slow-moving drugs. The conclusion in this study is the planning and procurement of drug availability at the Muhammadiyah Hospital North Sumatra in improving the pharmaceutical service system to be better, planning is carried out to avoid drug buildup and expiration. ABC method analysis should be applied so that the data is more accurate, not only using the consumption method.

Abstract

Inpatient services at Al-Fuadi Binjai Hospital aim to provide the best care for patients, but the services provided sometimes cause complaints and dissatisfaction of patients who choose to be hospitalized at the hospital. The patient's decision to choose to be treated at Al Fuadi Binjai Hospital is based on Kotler's decision-making theory and Andersen's health care search theory. The purpose of this study was to analyze the factors that influence patients to choose a hospital for hospitalization. Types of research This is an analytical survey research by design cross sectional. Research conducted at Al Fuadi Binjai General Hospital. The study population was 319 people, and the sample was obtained 178 people. The data used are primary and secondary data. Data analysis in univariate, bivariate using chi-square and multivariate using multiple logistic regression at a 95% confidence level (a=0.05). The results showed that the influencing factors The decision to choose hospitalization at Al Fuadi Binjai General Hospital was culture (values in the family) (p = 0.037), confidence (0.031), quality of health worker services (0.023), motivation (p = 0.008), facilities (p = 0.002), and speed of service (p = 0.000), while the perception variable had no effect (p = 0.206). The dominant variable influencing patients' decision to choose hospitalization at Al Fuadi Binjai General Hospital is the speed of service. Patients who stated that service at Al Fuadi Binjai General Hospital was fast, were 5.2 times more likely to choose readmission than patients who stated that service at Al Fuadi Binjai General Hospital was slow. Fast service is the main reason patients choose hospitalization because it has an impact on the speed of patient recovery. It was concluded that the patient's decision to choose hospitalization at Al Fuadi Binjai General Hospital was influenced by the culture, beliefs, quality of service of health workers, motivation, facilities and speed of service. It is recommended for patients to provide input through the suggestion box on complaints felt during treatment to improve the quality of service so that patients without hesitation decide to re-hospitalize when patients and families are sick.

Abstract

The National Health Insurance (JKN) program, which has been running since January 1, 2014, has been widely benefited by most of the community. This program makes it easy for the public to access health services. However, the implementation of JKN for nine years often faces problems and challenges, one of which is governance. Many parties involved in this JKN program include BPJS Kesehatan as the organizer, the Ministry of Health as a regulator, Health Facilities (hospitals) as service providers and Professional Associations. This research is a qualitative research with a type of program review (governance system). The data collection process is carried out by conducting observations, interviews, analysis and documentation. Data analysis is in the form of an explanation of the situation being studied, while the presentation is in the form of a story explanation. This is in accordance with descriptive research which is the study of observations on the object of research to obtain an overview and actual circumstances. This research has been conducted at Dian Harapan Jayapura Hospital in Papua in February 2024. Based on the results of the study, it was found that inputs, processes and outputs related to the governance of National Health Insurance referral patients at the Outpatient Polyclinic of Dian Harapan Jayapura Hospital Papua were based on an assessment of human resources, facilities and infrastructure, Standard Operational Procedures, constraints and problem solving processes. The results of the study based on the influence of Human Resources show that training related to BPJS has not been carried out evenly for all personnel, including the evaluation of services has not been carried out regularly. The Assessment of Facilities and Infrastructure is considered to have been fulfilled only due to disturbances in the queue machine, the V-Claim application and Scan Finger interference. The assessment of SPO is considered to be available and understood, but there has been no routine evaluation to assess the progress of SPO implementation. The assessment of the obstacles that are often experienced is the disruption of the V-Claim Application and the queue system is still offline because the patient has not used the Mobile JKN Application so that the patient comes early in the morning to take the queue number. In terms of handling problems in stages starting from internal, if necessary, coordination between units or coordination with management with higher authority will be carried out. The conclusion in this study is that Human Resources have a great influence on services, in terms of the number that has been fulfilled, it is still necessary to evaluate the progress of services such as obstacles to delays in the doctor's practice schedule to be corrected according to the agreement so as not to affect service satisfaction. Regarding Facilities and Infrastructure, it is still considered good, it only needs to be added to facilitate online registration by increasing the benefits of the Mobile JKN Application, including the Queue Display facility at the Polyclinic. The applicable Standard Operational Procedures have been implemented, but there has not been a routine evaluation of the implementation of the SPO has been carried out appropriately or still requires additional SPOs to support services. Regarding obstacles and the solution of obstacles, it is considered good. Each officer is already independent in handling problems.
