Abstract

Introduction: Indonesia ranked seventh in the prevalence of diabetes mellitus in the world. Improved glycemic control has been proven to prevent the diabetes and its complications. Adherence to treatment, weight regulation with benchmarks for Body Mass Index (BMI) indicators, activity or exercise habits, diet, and smoking history play an important role in glycemic control. Objective: The aim  of  this study was to identify factors affecting glycemic control in outpatient with diabetes mellitus.Methods: A cross sectional study  among 32 diabetes mellitus patients.This study was conducted from April 2019 to September 2019 at Rumah Sakit Nasional Diponegoro (RSND), Semarang. The glycemic control status were assesed by HbA1c. Factors such medication adherence, diet adherence, smoking status, and physical activity were assesed with a structured questionnaire. Anthropometric measurement done to get the BMI status. Data were analyzed with bivariat analysis with chi square test (p<0.05).Result: The factors that were significantly have relationship with glycemic control status are diet adherence (p=0.046), and medication adherence (p=0.040). Physical activity, smoking status and BMI doesn’t have significant relationship with glycemic control status.Conclusion: Factors affecting glycemic control of diabetes mellitus patients in this study were diet adherence and medication adherence. History of smoking, BMI, and physical activity were not factors that influenced the control status in diabetes mellitus patients in this study.

Abstract

Introduction: Hearing loss and tinnitus may occur in pilots due to exposure of the high noise level (ranges from 104-110 dB) generated by aircraft/helicopters. In addition, flying time, acoustic trauma, diabetes mellitus, hypertension and smoking are risk factors of hearing loss. Objective: to identify the factors associated with hearing loss in the crew of the Indonesian Army Aviation Center, SemarangMethods: in the cross-sectional research, 114 military helicopter aircrews aged 20-58 years, working for the Army Aviation Center in Semarang were included. The exclusion criteria were history of previous ear surgery, infection of the outer and middle ear, the use of ototoxic drugs (kanamycin, cisplatin and carboplatin). The level of hearing loss and tinitus was assessed by otoscopy and audiometry. Whereas, tinnitus was obtained from the history taking. Data were analyzed using chi-square test and fisher's exact and Yates’s correction.Results: One hundred and fourteen subjects (mean age of 26.54 + 4.72) were included. Several factors such as flight time (p = 0.698), acoustic trauma (p = 0.151), diabetes mellitus (p = 0.596), smoking (p = 0.222), hypertension (p = 0.356) were not associated with sensorineural hearing loss. Several factors such as flight time (p = 0.706), acoustic trauma (p = 0.5160), diabetes mellitus (p = 0.789), smoking (p = 0.495), hypertension (p = 0.112) were not associated with tinnitus.Conclusion: There is no association between flight time, acoustic trauma, diabetes mellitus, hypertension, and smoking and hearing loss.

Abstract

Introduction: The medical education world-wide is adapting competency based learning; it is high-time that the medical syllabus is inculcated with integrated-medical ethics training. An educational training program with a pre- and post-test method of analysis has better student outcome with respect to retention, thinking and understanding. Objective: To assess the understanding and application of medical ethics in students after structured module based training.Methods: One hundred and fifty students of 1st professional Bachelor of Medicine and Bachelor of Surgery (MBBS) were provided a 10 hours’ duration module based teaching of basic principles and core concepts of medical ethics in patient care. The assessment was conducted using a pre and post analysis questionnaire having 15 multiple choice questions with one correct response. Data were analysed using paired student’s t-test for comparing pre- and post-test scores. P < 0.05 was considered as significantResults: Total post-test correct responses were highly significant than pre-test responses. Gender wise, both males and females- post-test response significantly improved (p<0.001), indicating that there is improvement in understanding the principles and key concepts of medical ethics provided during the training. Upon completion of the training, most students strongly agreed that the training has inspired them towards the moral code of conduct, attitude and behavioural change and it provided necessary information on medical ethics.Conclusion: Our study demonstrates that the structured medical ethics training module yielded a significant enhancement of not only the knowledge and understanding level of the students but also in their perception, attitude and behavioural interest about the importance of ethical medical practice in the future patient care.

Abstract

The World Health Organization suggests that total body fat is one of the main risk factors for metabolic syndrome which is the most significant cause of morbidity and mortality in the world. The excessive fat mass harms the function of the heart, lowers the oxygen uptake by working muscles. Excess of body fat may decrease physical fitness. This research aims to know the correlation of total body fat with physical fitness index in medical students of Universitas Islam Indonesia. This research used a cross-sectional study design. The research subjects consisted of 45 male medical students of Universitas Islam of Indonesia under 25 years old. Total body fat of its subject is measured by Bioelectrical impedance analysis method (Karada scan), while physical fitness index is measured by the Harvard Step Test. This study is tested by Spearman correlation. Mean of TBF (%) value was 23.68 ± 6.29. Mean of physical fitness index levels was 60.05 ± 31.27. Spearman correlation test has a result r = -0.314 with a significance of p = 0.038 (p<0.05), that means total body fat and physical fitness index has a significant negative correlation. There was a significant negative correlation between total body fat and physical fitness index

Abstract

Introduction: Hospital is one of the important links in the service delivery system to the community so that the hospital is expected to provide information to support the provision of timely and accurate services. To improve the quality of hospital services, an up to date information system is needed. The user satisfaction on information system can be used to evaluate the quality of an organization's information system. Objective: to evaluate the quality of the pharmacy management information system (PMIS) at Sultan Agung Islamic Hospital (SAIH), Semarang, and determine the priority of area for improvements.Methods: In this cross-sectional qualititative study, the responses of all employees to questionnaire were obtained; these employees worked at the pharmacy of Sultan Agung Islamic Hospital in Semarang. The PIECES method was applied. The data analysis included gap and customer windowResults: The gap value on the dimension of performance, information, economic, control, efficiency, service was -1.10, -0.82, -0.28, -0.22, 0.01, -0.62, respectivelyConclusion: The user’s unfulfilled expectation on PMIS at SAIH including dimensions of performance, information, economy, control, service, and efficiency. Priority areas for quality improvement of PMIS at the Sultan Agung Islamic Hospital’s phamacy included the menu display, system responsiveness, the completeness of the system, no error occurs, the alert system of the drug stock, the responsiveness of the IT department to the problem of PIMS use.

Abstract

Introduction: Hemodialysis is the most preferred Renal Replacement Therapy (RRT) options for End-Stage Kidney Disease (ESKD) patients. Although it is safe and beneficial for patients, several complications may occur during hemodialysis process. One complication that is rarely found in the hemodialysis process is visual impairment. Visual impairment or even sudden blindness occured during or within 24 hours post hemodialysis. Objective: To describe the occurring sudden blindness as a rare complication in patients undergoing hemodialysis and its etiology.Brief Case: 19 years old man with ESKD undergoing hemodialysis for 1 month with a complaint of sudden blindness in both eyes occurring <24 hours post hemodialysis.Discussion: Fast and precise efforts are needed to diagnose the etiology of blindness, so that prevention and appropriate treatment can be immediately taken. Multidisciplinary teamwork is needed to diagnose the etiology of visual impairment.Conclusion: Sudden blindness in this case may be caused by the 3rd degree hypertensive retinopathy caused by malignant hypertension and Posterior Reversible Encephalopathy Syndrome (PRES)

Abstract

Introduction: It has been known that  the infestation of Trichuris muris egg in the intestinal mice may induce a host immune ‎response to eliminate the worms. Worms antigen will stimulate host immune response in the intestinal ‎secondary lymphoid organs. One of the secondary lymphoid organs in the intestinum is GALT (gut-‎associated lymphoid tissue). Worm antigen activate naive T cells and naive B cells in GALT and ‎trigger differentiation of T cells into Th2 and stimulate B cell to produce gut IgE. However a structural change of GALT structure is still poorly understood.‎ Objective: This study aims to find  influence of embrionic Trichuris muris egg infestation to the structural ‎changes of mice intestinum GALT.Methods: This research uses true experimental method with post-test only with control group design. Mice are grouped into 3 groups. The first group was given 40 Trichuris muris embryonic egg peroral, second group was given 200 eggs, and third was control group. On the 30th day after treatment, the mice were sacrificed and intestinal GALT structural change has been analyzed (p<0.05). Results: There was no effect of Trichuris muris embryonic egg infestation on the structure of mice intestinum GALT. However, there is a unique finding of GALT in the form of a Peyer’s patch in the basal plica transversalis colon proximalis of 1 mice given egg T. peroral murmus, at low doses or high doses. Conclusion: There was no effect of Trichuris muris embryonic egg infestation on the structure of mice intestinum GALT
