Abstract

Introduction: As various pterygium morphologies have been advocated as contributing factor on corneal astigmatism, little support in the literature available in establishing techniques in measuring pterygium thickness as clinical indicator.Objective: The aim of this study was to describe a quantitative method in determining pterygium thickness using anterior segment optical coherence tomography (AS-OCT).Methods: Anterior segment imaging was performed using enhanced high resolution cornea (EHRC) of Visante™ AS-OCT in 120 primary pterygium eyes. Prior to imaging, corneal topography assessment was performed on each pterygium eye in order to identify its topographic location. Based on topography mapping, three meridians (in degrees) were selected as close as possible to the pterygium border, which signify the demarcation of pterygium from the cornea. Reliability testing between intra and inter-observer of AS-OCT imaging modality was examined using intraclass correlation and scatter plot.Results: The overall (n = 120) mean and standard deviation of pterygium thickness EHRC of AS-OCT modality were 0.48 ± 0.10 mm (confidence interval: 0.45 – 0.50). EHRC of AS-OCT also showed excellent intra and intergrader reliability in measuring pterygium thickness with intraclass correlation of 0.997 (confidence interval: 0.994 – 0.998).Conclusions: EHRC of AS-OCT imaging modality is a better choice in assessing pterygium compared to traditional slit-lamp biomicroscopy. This tool is applicable for future work related to better understanding on the role thickness in pterygium morphology, its progression and prediction of induced corneal astigmatism and visual impairment due to pterygium.

Abstract

Hepatitis B virus has been a problem in developing countries. The transmission of HBV is through the exposure to the blood or secret of the infected patients. Woman has 3 times higher risk to get HBV than men. The pleminary observation revealed that in Pondok Pesantren Putri QoyyimYogyakarta 20-30 students shared a room. It also showed poor hygene that might lead to environmental based transmission. No sepesific sympthom for HBV infection in the early phase causes people unaware of getting infected. This study was aimed at finding out factors associated with HBV case among students in Pondok Pesantren Putri Ibnul Qoyyim Yogyakarta. This was an observational study using Cross Sectionl approach including 97 female students. The data were collected using interview, observation and blood testing to find out the marker of possitive HbsAg. The independent variable was blood transfussion history, the use of shrynge, piercing , permanent tattoo, accupunture, mouth cavity examination or dentist, family history of hepatitis,the tooth brush sharing or nail cutter.The resut showed that out of 97 female students subjected to blood test, 2 had a possitive HbsAg. Univariate analysis showed that factors associated with hepatitis B is the family history (OR=7.636; 95% Cl= 0.445-130.985 p=0,002). Multivariate analysis showed that family history of hepatitis B was significantly associated with the hepatitis B incidence (OR= 5. 704; 95% Cl= 0. 287-113. 276, p=0.004). Family history of hepatitis B is the risk factor for hepatitis B incidence in Pondok Pesantren Putri Ibnul Qoyyim Yogyakarta. Health education on the risk of geeting infected by Hepatitis B virus should be in the first priority.

Abstract

INTRODUCTION: The increasing number of multidrug-resistant Pseudomonas aeruginosa worldwide need to be continuously monitored to prevent its transmission. This study reviewed multidrug-resistant Pseudomonas aeruginosa in Southeast Asia.METHODS: Pubmed database was searched (January 2013-March 2018) for relevant articles using specific names of all eleven Southeast Asian countries. The articles retrieved were screened and analyzed to meet the inclusion criteria.RESULTS: A total of 17 articles met the inclusion criteria. Vietnam had the most published data included in this study, East Timor, Cambodia, and Laos had none. There are variations among Pseudomonas aeruginosa resistance in Southeast Asia. Thailand had complete data on antibiotic resistance and had the highest resistance rate of almost all antibiotics. Colistin had the lowest resistance rate among all antibiotics.CONCLUSIONS: Colistin had the lowest resistance rate among all antibiotics. Another study with more extensive data should be done to validate this study. Future research should be explicitly providing detail data results for antimicrobial resistance analyses.

Abstract

Therapeutic drug monitoring (TDM) is used in drug therapy for selected drugs with narrow therapeutic index, or a broad range of kinetics variation, or drugs with strong correlation between plasma concentration and clinical effects or toxicity. Aspirin is one of the widely used old generation drugs, with a broad range of activity spectrum: analgesic, antipyretic, anti‐inflammatory, and antiplatelet. As anti‐inflammatory drug, aspirin directed as therapy for many of chronic inflammation, such as rheumatoid arthritis. As an antirheumatic agent, aspirin shows a wide variation in the pharmacokinetic of aspirin metabolism, and has a narrow therapeutic index because it effectives at concentration of 10‐30 mg/dl, but at more than 40 mg/dl intoxication occurs (metabolic acidosis, hyperpnea, and death). Those are why aspirin use as antirheumatic agent need therapeutic drug monitoring, so that doctors can guarantee that plasma aspirin concentration is in the therapeutic range, to achieve optimilization of therapy with minimal side effects. This article reveals about use of aspirin as antirheumatoid drugs and why therapeutic drug monitoring is needed in aspirin use (Sains Medika, 4(2):210‐226).

Abstract

Introduction: The administration of antioxidant combination has been shown to be more effective than that of individual antioxidant in male infertility.Objective: to determine the effect of different combination of antioxidants (vitamin/vit C, vit E, glutathione and zinc) on the number of Sertoli and Leydig cells, sperm quality and texpression of caspase-3 in rats exposed to cigarette smoke.Methods: This was an experimental study using a post test only control group design. Thirty six rats were randomly divided into 6 groups to receive one of the following combinations of antioxidants: vit C and E, glutathione, zinc (T4-G); vit C and E, glutathione (T3-G); vit C and vit E (T2-G); Glutathione, zinc (T1-G); control positive (CP-G) cigarette smoke exposed only, normal (CN-G). The dose of vit C, vit E, glutathion and zinc was 9 mg/day, 1.8 IU/day, 1.8 mg/day, 0.2 mg/day respectively. All groups were exposed to cigarette smoke (3 cigarettes/day for 21 days), except for the CN-G group. On day 22, sperm samples were taken. Testicular tissue was taken for measurements of sperm quality, sertoli and leydig cell.numbers.Results: There was a significant difference in mean number of Leydig cells, sperm motility, sperm count and caspase-3 between groups (p<0.05). Post hoc LSD showed that the administration of combination of four antioxidant agent increasing the number of Sertoli cells and sperm morphology. The results also showed that group treated with 4 antioxidants combination had the highest number of Leydig cells, sperm motility, sperm counts and the lowest expression of caspase-3.Conclusion:The combination of vitC, vit E, glutathione and zinc affects the number of Sertoli and Leydig cells, sperm quality, expression of caspase-3 in rats exposed to cigarette smoke .

Abstract

Background: Jinten hitam (Nigella sativa L.) oil is a liquid made from Jintem hitam seeds which has been known to contain Thymoquinone having anti-cancer efect. Many cancer patients turn to cheaper alternative therapy including the traditional therapy. The objective of this experimental was to evaluate anti-cancer effect of Jinten hitam seeds oil in adenocarsinoma mammae C3H mice.Design and Methods: In this xperimental research with pre post test randomized control group design for 21 days, 24 C3H mice with tumor were divided into 4 group. K-I serves as a control given standard diet and aquades, K-II given standard diet, aquades and first dose of Jinten Hitam oil (0.1 ml/days), K-III given standard diet, aquades and second dose of Jinten Hitam oil ( 0.2 ml/days) and K-IV given standard diet, aquades and third dose of Jinten Hitam oil ( 0.3 ml/days). The volume was recorded at the day 21.Results: mean of the effect of Jinten hitam oil effect in adenocarcinoma mammae for the four group were 158.83 mm³, 259.73 mm³, 191.13 mm³, K-IV 126.27 mm³ respectively. Pair t-test and Wilcoxon test showed significant difference between First dose pre-First dose post and Second dose pre-second dose-post (p<0.05) and Anova test showed no significant difference between K-I K-II, K-I K-III, K-I K-IV, K-II K-III, K-II K-IV, K-III K-IV (p>0.05).Conclusion: Jinten Hitam oil has effect on the volume of Adenocarcinoma mammae in C3H mice (Sains Medika, 3(2):129-134).

Abstract

Background: TSG-6, an inflamation-associated protein, forms covalent complexes with heavy-chains from TNF. These protein potentiating the antiplasmin activity of this serine protease inhibitor. The aim of study was to determine and show that TSG-6 are present in Bronchoalveolar Lavage (BAL) from patients with lung TB and heavy smoker.Design and Methods: Experiments conducted in vitro with purified components revealed that TSG-6 had immunodetection in the Human Tracheal Aspirates (HTA) and BAL of heavy smoker and Lung Tuberculosis.Results: Immunoprecipitation revealed that TSG-6 accounts in airways hyperresponsiveness for significant proportion of BAL and HTA, but not in control (p < 0,01). TSG-6 relevant protease inhibitor in airway secretion. In primary cultures of differentiated human airway epithelial, which suggest that TSG-6 may be induced by TNF. Our results suggest that TSG-6 may play an important protective role in bronchial epithellium buy increasing the antiprotese screen on the airway lumen. The present study identifies a novel function of TSG-6 by a distinct mechanism system regulated, enhancement of anti TK activity, and provides a rational for this activity within the context of inflamed airways. These findings correlate with the observed increases in protein expressions in human HTA and BAL.Conclusions: TSG-6 are hyper-expressed in airway epithelial cells in cultures of smoker and lung TB. The interactions of TSG-6 with other molecules are also potentially important to the pathophysiologic aspects of inflammatory airways diseases that remain to be elucidated (Sains Medika, 3(1):31-40).

Abstract

Cryptococcal meningitis incidence has increased along with an increase in incidence of HIV-AIDS. This infection causes increased morbidity and mortality in patients with HIV-AIDS. A rapid diagnosis plays an important role to ensure a prompt therapy of the disease. The cryptococcal polysaccharide antigen test for diagnosis of meningitis is rapid but relatively expensive while culture is time consuming. A 47-year man was admitted to hospital with a headache, fever, nausea, and vomiting and a HIV history for the last 6 months. On physical examination, he was compos mentis, meningeal’s stimuli signs (+), where as on examination of craniales nerves, motor and sensibility was in a normal range. Routine blood was normal, 60 CD4 cells/mm3. Laboratory finding included a clowdy/turbid Cerebrospinal fluid (CSF), low glucose level (CSF glucose 43 mg / dl vs. blood glucose 293 mg / dl), elevated protein concentration (137.1 mg / dl), and polymorphonuclear pleocytosis. India ink stain showed encapsulated yeasts. Cryptococcus sp is the only encapsulated yeast, while C. neoformans is the most common cause of Cryptococcosis in patients with HIV-AIDS. The patient was diagnosed with Cryptococcal meningitis by indian ink staining, and immediately given anti-fungal theraphy.

Abstract

Introduction: The success measurement of management application in an organization or company can be seen from the company or organization’s productivity. Customer perspective used to measure productivity is by looking patients’ satisfaction by measuring importance and performance matrices and customers growth. Objectives: To identify the performance achievement in UMM Medical Center (UMC) pharmacy, by measuring the customers perspectives. Methods: This research is using the research design of non experimental case study, descriptive analysis of the data obtained. Data were gathered retrospectively and prospectively. Retrospective data to figure the customers growth, as for prospective data to find out the customer satisfaction by importance and performance matrix. Results: Results indicated that: a) customers are not satisfied by the service in the pharmacy indicated by total servqual values of -0,227, b) dimensions of tangibles, reliability, responsiveness, and assurance currently are meeting the expectations of the customers and make customers feel satisfied towards the quality of service, c) there is no increase of Customer growth (0%) during year 2014 and 2015.Conclusion: The productivity improvement efforts of Pharmacy in UMC from customers perspective are not achieved optimally, the productivity improvement is needed.

Abstract

Introduction: Clinical reasoning is a thinking process happened when various informations gathered from clinical cases were given or met by medical students, and used for formulating diagnosis and solving patients problems. Experiences and material inputs are important factors affecting clinical reasoning. Various theories explained that students on pre-clinical and clinical rotation level are still on the novice stage, so it is possible to have similar stage on clinical reasoning. On the other hand, it was reported that there are differences in clinical reasoning on different academic stages. Objective: knowing the differences of clinical reasoning on preclinical students and clinical students.Method: observational study with cross sectional design. Respondents of this study were 120 students from medicine faculty of Sultan Agung Semarang University, comprised of: 60 preclinical students whom have been underwent tropical disease modul batch 2011 and 60 clinical rotation students on pediatric stage. Respondents answered 20 items questions in Script Concordance Test (SCT) form on pediatric tropical disease cases. SCT items were prepared by pediatricians, structures and contents were reviewed by medical education experts and concordanced by 10 experts of pediatric unit. Value differences between 2 groups were statistically analyzed using Mann Whitney U on SPSS 16.0, with acceptable error accepted level if p<0.05. Result: non parametric result test by Mann-whitney showed that mean score of SCT on preclinical group were lower (8,57;+ 1,81) than on clinical rotation students 11,49 (+ 1,98) p = 0,000. Conclusion: clinical reasoning competence measured by SCT is higher on clinical rotation students compared to pre clinical students.
