Abstract

Background: Colon cancer, a colorectal cancer, is the third most common epithelial malignancy in the world. Family history, bloody stool, palpable mass, anemia, and abdominal MSCT are symptoms and signs of colon carcinoma.Objective: To determine the relationship between the 5 variables and the incidence of colon carcinoma at Dr. Kariadi Hospital, Semarang in 2016.Methods: a Cross-sectional observational analytical study using medical record (RM) and complementary primary data. The inclusion criteria werethe complete medical record, and clinical diagnosis of suspected colon carcinoma. Data obtained from the department of Anatomy Pathology/PA (11,794PA results) were traced to the medical record section (46 patients with suspected colon carcinoma). The incomplete data were confirmed by: contacting the patient/family, obtaining the archive in the laboratory and radiology resulting in 27 patients meeting the inclusion criteria. Analysis was done using chi-square test, Spearman-Kendall bivariate correlation, and logistic regression.Results: Abdominal MSCT was moderately associated with colon carcinoma (p = 0.003; r = 0.488), while family history, bloody stool, palpable mass, and anemia were not associated with colon cancer. Analysis between predictors of outcome: Bloody stool was moderately associated with anemia (p = 0.006; r = 0.411), and anemia was weakly associated MSCT (p = 0.035; r = 0.351). Abdominal MSCT was the predictive factor for colon carcinoma (p = 0.021).Conclusion: Abdominal MSCT was found to be associated with the incidence of colon carcinoma. Bloody stool was associated with anemia, and anemia was associated with abdominal MSCT. MSCT was the predictive factor for colon cancer.

Abstract

Cigarette smoke consists of a mix of chemical substances in the form of gases and dispersed particles. Recently, more than 4000 compounds presented incigarette smoke have been isolated. Most of these compounds are toxic to our body’s cells. Toxic gases including carbon monoxide (CO), hydrogen cyanide(HCN), nitrogen oxides, and volatile chemicals such as nitrosamines, formaldehyde are found in in cigarette smoke. besides toxic compounds, cigarettesmoke also containsfree radicalsincluding peroxynitrite, hydrogen peroxide, and superoxide. These free radicals may accelerate cellular damage due tooxidative stress. Targets od free radical attacks include DNA, protein and lipids. The harmful chemicals in form of gases and volatile substances in cigarettescause multiple genetic mutations. the combination of genetic mutations and DNA damage lead to genetic instability and it may cause cancer. OxidativeDNA damage caused by cigarette smoke can be identified with the presence of 8-oxoguanosine used as one of the biomarkers for oxidative DNA damage.Increased concentration of 8-oxoguanosine in DNA has an important role in carcinogenesis and triggers tumor cells. both active and passive smokers havebeen reported to have an elevated concentration of 8-oxoguanosine in their lung tissue and peripheral leukocytes as well as for passive smokers. This paperprovide informations and understanding of the effects of smoking on the genetic stability, especially in the DNA molecule.

Abstract

Introduction: Hypertension is the most common risk factor for heart failure causing an increase in N-Terminal-Pro Brain Natriuretic Peptide (NT-proBNP) level and a decrease in left ventricular ejection fraction (LVEF). Objective: To investigate the effect of hypertension on NT-proBNP level and LVEF in patients with chronic heart failure.Methods: This cross sectional study takes 29 people with hypertension stage-1 (HTD-1) and 27 people with hypertension stage-2 (HTD-2) as sample. The left ventricular ejection fraction and NT-proBNP level obtained from medical records are analyzed using independent t test and Mann Withney test, followed with Spearmen correlation between NT-proBNP level and LVEF.Results: Mann-Whitney test indicates that the mean value of NT-proBNP level of HTD-2 is significantly higher compared to that of HTD-1, p = 0,000. On the contrary, the independent t test shows that LVEF in HTD-2 is lower than that of HDT-1, p = 0.001. The Spearman correlation analysis conducted on the NT-proBNP level and the LVEF results in value of -0.651, p = 0.000. The cut-off-point value of NT-proBNP level is 1511pg/ml (sensitivity 0.82% and specificity 0.80%) and the value of area under the ROC curve > 0.7. The left ventricular ejection fraction has an area under the ROC curve > 0.7 with a cut-off-point value of 39% (sensitivity 0.67% and specificity 0.69%).Conclusion: The NT-proBNP level and the LVEF of HTD-2 are significant higher and lower respectively compared to those of HTD-1. The NT-proBNP level and LVEF also show a strong negative correlation. The cut-off-point value of NT-proBNP level and LVEF can be utilized to compare the stages of hypertension in patients with chronic heart failure.

Abstract

Background: Analysis of cost-effectiveness is an analysis which comparing therapy effective with its cost. In this research, therapies effective are compared between combination of oral anti-hypertension with cost that paid by patient. This research aims to identify the combination of oral anti-hypertension, which cost is the most costeffective on hypertension patient in RSUD Tugurejo Semarang, period 2007.Design and Methods: This research was a non-experimental using descriptive non-analytic, data collection performed in retrospective from medical record of patient of hypertension in RSUD Tugurejo Semarang, period 2007. Research subject was hypertension patient, which used two combinations that were anti-hypertension and using similar anti-hypertension, at least during 3-month period. Hypertension effective measured by calculating patient amount that blood pressure achieved the target during 2 month, after therapy performed. Cost effectiveness seen based on average cost-effectiveness ratio (ACER) value.Results: The result of analysis of cost-effectiveness using anti-hypertension on hypertension patient in RSUD Tugurejo Semarang, period 2007 for 44 cases has fulfilled the criterion. Female patients were 25 cases and male patients were 19 cases. The most used combination of oral anti-hypertension was class combination of diuretic-ACEI (angiotensin converting enzyme inhibitor) for 36,36%. Cheapest direct medical cost wascombination of ACEI–Beta blocker for Rp. 23.250,10. The biggest therapy effectiveness there was at combination of ACEI –diuretic for 46,88%.Conclusion: The most cost-effective is ACEI – diuretic with ACER value for 623,06 (Sains Medika, 4(2):124-133).

Abstract

Colorectal cancer currently occupies the third position of globally cancer morbidity and is the second leading cause of cancer death after lung cancer. Chemotherapy administration is still the main modality for the colorectal cancer stage I to III. Its usage is limited, since its high resistance and risk of side effects. Thus, exploration and development of novel colorectal anti-cancer drugs, including compounds derived from nature origin is needed. Xanthone becomes one natural compound which may be potentially developed as an anti-colorectal cancer due to its cytotoxic and anti-proliferative activities. The in vitro cytotoxic and anti-proliferative actions of xanthone compounds occur through a variety of mechanisms involving both apoptotic induction and inhibition of cell cycle, while in vivo may reduce the tumor size. Since colorectal cancer is frequently treated in a combination therapy, xanthone compounds have also been studied in a combination use and proven effective as co-chemotherapy with the standard chemotherapy drugs. This paper aims at providing an overview of colorectal cancer, pathology, risk factors and protective factors, as well as discussing the current therapies and potential xanthone compounds as an alternative therapy which may be developed later for the colorectal cancer

Abstract

Background: Immunity is not separated from the role of leukocytes with various types and functions. Tocotrienol is one of the vitamin E group of compounds with antioxidant properties which to play an important role in cell repair. The aims to determine how much of the effect of vitamin E tocotrienols and its combination with ascorbic acid on the types of leukocytes.Design and Method: Preparation of blood lear dosage edge is a technique used to determine the effect of treatment between types of leukocytes. The design of experiment was conducted with three treatment that feed intake vitamin E tocotrienols and ascorbic acid and their combination with the control and 3 repetitions. Quantitative data leukocyte counts obtained by calculating the percentage of each type of leukocytes in each treatment group and the mean of the data obtained compared with normal leukocyte counts. In addition, the data is processed by the leukocyte counts of variance test using a completely randomized design (CRD) and BNT level of 5% to determine the differences between treatments.Result: The administration of vitamin E tocotrienols and its combination with ascorbic acid influenced significantly (P> 0.05) to increase the neutrophil are 31.66% and 33.66%, 24.33% and 22.33% respectively, when compared with controls. On the other hand, there is a decrease in the percentage of lymphocytes are 26.66% and 29.16%, 19.50% and 17.00% respectively, when compared with controls. Vitamin E tocotrienols and combined with ascorbic acid did not give significant effect (P <0.05) the percentage of eosinophils, basophils and monocytes.Conclusion: The administration of vitamin E tocotrienols and its combination with ascorbic acid to give effect to the increase in neutrophils (Sains Medika, 4(1):46-56).

Abstract

Purpose: To describe three cases of toxic optic neuropathy due to methanol.Method: Case seriesResults: There were 3 cases in Dr. Sardjito Hospital. First case, a male, 28 years old, drunk three glasses of lapen about 1 week before he went to hospital. At his first time he came, his vision was no light perception (NLP) for both eyes. Then he hospitalized for a week and got 48 mg methyl prednisolone orally. After 1 month, his vision of the both eyes didn’t improve. From the sample of his drink, we found methanol 4,38% and ethanol 4,91%. Second case, a male, 46 years old, drunk one glass of lapen about 4 days before he went to hospital. His vision was NLP for both eyes. Like the first case, he got the same management, and after 1 month his vision still didn’t improve. From the sample of his drink, we found methanol 12,41% and ethanol 3,74%. Third case, a male, 21 years old, drunk more than 3 glasses of combination of Vodka, lapen, Kratingdaeng, and beer. The visual acuity were 3/60 for the right eye and hand movement (HM) for the left eye. Like the cases before, he got the same management, and after 1 month his vision had improved became 3/60 for the right eye and 2,5/60 for the left eye. Unfortunately, we didn’t have the sample of his drink.Conclusion: Lapen is Yogyakarta home made beverage which contains with methanol. Methanol is toxic liquid that cause toxic optic neuropathy. Blindness is the most common symptom and permanent (Sains Medika, 3(2):177-184).

Abstract

Background: Diarrhoea is still public health problem and often cause outbreak with various morbidity and mortality. The long onset of acute diarrhoea causes the length of stay that affect the cost. So proper management is needed to shorten inpatient length of stay. Probiotic can make the convalescence of children with acute diarrhoea faster. The aim of the study is to know the lenght of stay difference between underfive year old children with acute diarrhoea treated with and without probiotic.Design and Methods: The study was an analitical study with case control design. Study Sample were all inpatient 6-24 month old children with acute diarrhoea in Kota semarang Hospital since January – December 2007. Data were obtained from medical record in pediatric unit. Data analysis with stastitical test of Mann Whitney.Results: Of 130 samples, the lenght of stay average of < 5 years old children with acute diarrhoea with probiotik was 3,42 days compare 4,03 days children acute diarrhoea without probiotik. It is concluded that there was a significantly difference between two groups acute diarrhoea with probiotik and without probiotik.Conclusion: There was a significant difference between the length of stay for the patients with acute diarrhea with and without probiotic treatment. Mean of the length of stay for under 5 years old baby with and without probiotic were 3.42 days and 4.03 days respectively (Sains Medika, 3(1):78-83).

Abstract

Background: Performance appraisal using Balanced Scorecard on internal business process perspective offers a comprehensive solution for performance appraisal in public organization especially in a hospital. Objective: to study the performance of pharmacy department in Sultan Agung Islamic Hospital using Balanced Scorecard from internal business perspective. Methods: Object of the study is the Sultan Agung Islamic Hospital installation of pharmacy. This research is a descriptive observational study. Data were obtained prospectively and then categorized and grouped as quantitative and qualitative data. Qualitative data were gathered from in depth interview with head of pharmacy department Sultan Agung Islamic Hospital. Quantitative data were gathered from direct observation on drugs prescription surveys.Result : Performance perspective on internal business process include: a. Level of availability of drugs in pharmacy department Sultan Agung Islamic Hospital is good with average 98%, b. Level of outpatients admittance in installation of pharmacy Sultan Agung Islamic hospital indicated the average of 66%, it means that installation of pharmacy have lost customers for outpatients as much as 34%.Conclusion: Performance in pharmacy installation of Sultan Agung Islamic Hospital is not in a good criteria, therefore improvement is needed.

Abstract

Introduction: Dyslipidemia as reflected in the results of lipid profile examination is among the most common abnormalities found in acute ischemic stroke patients. Studies assessing the characteristic of these results may provide a foundation for an effective plan of treatment and prevention to overcome the burden of the disease.Objective: To study the characteristic of the results of lipid profile examination among acute ischemic stroke patients at Dr. Soedono General Hospital, Madiun, East Java.Methods: This research is a descriptive study with a cross-sectional approach using secondary data taken from the medical records of acute ischemic stroke inpatients at the Stroke Unit of Dr. Soedono General Hospital, Madiun, East Java admitted in January-December 2015. Results of lipid profile examination consisting of total cholesterol, LDL, HDL, and triglycerides were categorized as high, normal, and low based on their respective levels.Results: There were 72 subjects selected using simple random sampling. Level of total cholesterol was high in 52.78%, normal in 36.11%, and low in 11.11% of the subjects. Level of LDL was high in 54.17%, normal in 22.22%, and low in 23.61% of the subjects. Level of HDL was low in 31.94%, normal in 50.00%, and high in 18.06% of the subjects. Level of triglycerides was high in 26.39%, normal in 33.33%, and low in 40.28% of the subjects.Conclusion: Total cholesterol and LDL are generally high, HDL is generally normal, while triglycerides is generally low among acute ischemic stroke patients in Dr. Soedono General Hospital, Madiun, East Java.
