Abstract

Introduction: Sixty percent of femoral hernias are characterized by incarceration and strangulation. Jejunal pseudo diverticulum are rare and usually asymptomatic. It may cause chronic obstruction of small bowel and can lead to an acute perforation.Case presentation: An 85 – year – old woman presenting with 1 week history of generalized abdominal pain, with episodes of vomiting, fever, and history of 6 months of reponible femoral hernia and 2 weeks strangulated of femoral hernia. An abdominal X-ray displayed multiple dilated loops of the small bowel, coil spring sign and intra peritoneal free air. This  patient underwent a laparotomy and hernioraphy, which identified single perforated jejunal pseudo diverticulum 50 cm from ligamentum of treitz orally from strangulated of ileal on femoral hernia site, and associated fecal contamination. The management for this case was perforation repaired with diverticulectomy, simple closure, and extensive washout of intraperitoneal cavity. The non tension femoral hernia repair was performed with monofilament, macroporous MESH.Conclusion: chronic intestinal obstruction caused by femoral hernia in the elderly can lead a performed of intestinal pseudo diverticulum and lead to significant morbidity and mortality. This could be suspected in those presenting with cramping abdominal pain and altered bowel habits.Keywords: hernia, femoral, strangulated, pseudo diverticulum, perforated, management

Abstract

The most common hypertension is essential hypertention influenced by genetic factors. Autonomicic nervous activity is influenced by several factors including genetic. This study aimed to determine the relationship between family history of hypertension and autonomic nervous system activity. In this observational analytic cross sectional study, out of 124 a students of the Medical Faculty, University Islam Sultan Agung Semarang 44 students were included using simple stratified method consisting of 22 men and 22 women aged over 20 years. The subjects were asked for family history of hypertension, orthostatic and valsalva examination. 25 students (56.8%) had a family history of hypertension and experienced imbalance of autonomic nervous activity. Out of 19 students (43.2%) without a history of hypertension, 10 people (22.7%) had a balance of autonomic nervous system activity and 9 people (20.5%) experienced imbalance of autonomic nervous system activity. Fisher correlation test results showed a significant association (p < 0.05) and correlation test of Lamda resulted in p value of 0.100, indicating a very week positive correlation. The study concludes that there was a correlation between a family history of hypertension and on autonomic nervous system activity with a very weak relationship.

Abstract

Background:a study conducted among 11 hospitals in DKI Jakarta in 2004 showed that 9.8% in-patient had nosocomial infections, the most common way in which germs spread is through personnel’s hands. Thus, hand washing and antiseptics play a significant role in the prevention of nosocomial infections Triclosan and Cidastat are the two antiseptics used in the Sultan Agung Islamic Hospital. The purpose of this study was to find out the difference in the number of germs on nurse’s hands using Triclosan and Cida-stat.Design and Method: This experimental study using pre and post test control group design used three treated groups; group A (treated with aquabidest), group B (treated with Triclosan), Group C (treated with Cidastat). The samples were palms of 12 nurses for in-patients in the Baitul Syifa ward and Arrijal of Sultan Agung Islamic Hospital.Result: One way anova showed a statistically significant different among the three treated groups with p value of 0.020 (<0.05). Post Hoct Test to compare the number of germs before and after hand washing using Triclosan and Cida-stat resulted in no significant difference (>0.05). The paired T test showed a significant difference between Triclosan and Cidastat group.Conclusion: There was no significant different in the number of germ before and after hand washing using Triclosan and Cida-stat antiseptics (Sains Medika, 2(2):163-169).

Abstract

Background: Hyperglycemia is one of the signs of diabetes mellitus, the disease is still being suffered by the people of Indonesia. Oral antidiabetic drugs mostly give unwanted side effects, then the experts developed a system of traditional medicine for diabetes mellitus. The research aimed to determine the effect of ginger juice on blood glucose levels and histopathological of pancreatic beta cells in male wistar rats induced by alloxan.Design and methods: An experimental study with a post-test only control group design, the study subjects white male wistar rats. Rats were divided into 5 groups, each group consisted of 5 rats, the group I was the control group (no treatment), group II is diabetic group (negative control), group III is Ginger + diabetes group, diabetes group is a group IV + Ginger + glibenclamide at a dose of each half, the group is the group of diabetic + V Glibenklamid.Results: The mean blood glucose levels were highest 172.02 ± 21.40 mg / dl is the K II. Lowest mean glucose levels were 112.72 ± 14.49 mg / dl for KV. Score the highest rates of pancreatic beta cell damage that is at the K II, while the lowest in the K V. Conclusion: Ginger Juice effect on blood glucose levels alloxan-induced rats. Ginger juice histopathologic effect on pancreatic beta cells alloxan-induced rats (Sains Medika, 4(2):165-173).

Abstract

Introduction: Some studies indicated that antlions (Myrmeleon sp.) is traditionally used by many people in Indonesia as a diabetes treatmenter. However. reseach has not been done to find the effect of antlion extract in pancreas, an important organ in diabetes mellitus pathophysiology. Objective: To prove the effect of antlion extract towards histopathological feature of pancreas in diabetic mice.Methods: In the post test only control group design, 25 Swiss mice were divided into five groups. Normal group (Nor-G), Negative control group (Neg-G), and the other 3 groups were intraperitoneally injected with Streptozotocine 200 mg/kg to induce diabetic state. In Neg-G had no antlion treatment, whereas in At-14, At-28, and At-56 were treated with antlion extract at the dose of 14, 28, and 56 mg/kg BW respectively for 24 days. Pancreas then collected and prepared to be examined under microscope with 400x magnification. The score of pancreatic islet injury was determined by assessing structural damage and inflammatory cells infiltration. Kruskal-Wallis and Mann-Whitney were adopted as statistical analysis.Results: Mann-Whitney analysis indicated that the score of pancreatic islets injury in At-56 was significant lower compared to that of Neg-G, p<0.05. The score of pancreatic islets injury in At-56 also significant lower compared to that of At-14 and At-28, p<0.05. In contrary, when compared to that of Nor-G, the pancreatic islets injury in At-56 was not signicantly lower, p>0.05.Conclusion: Antlion extract treatment at the dose of 56 mg/kg BW was capable of reducing pancreatic islet injury in diabetic mice induced by streptozotocine.

Abstract

Tethered Cord Syndome (TCS) refers to a group of neurological disorders related to malformations of the spinal cord, pulling of the spinal cord at the base of spinal canal. Tethered cord syndrome can be seen at any age but most often during childhood. A few children complain of diffuse pain in lower extremitiesor urological symptoms, 20% - 30 % of the patients will have a neurogenic bladder. A 9 years old girl, complained to have a flank pain and eneuresis. A mass is palpable at right flank. Ultrasound revealed duplex hydronephrosis and hydroureter with trabeculated bladder. Voiding Cystouretrography showed grade V vesicoureteral reflux (VUR) and neurogenic bladder appearance. Lumbosacral MRI demonstrated tethered cord with adjacent lipoma and spina bifida (Sains Medika, 4(1):89-96).

Abstract

Introduction: The soursoup leaves extract (Annona muricata) has widely been used as traditional medicine for cancer. No studies have been conduct to investigate the safety of the extract. Objectives: The purpose of the study was to investigate the acute oral toxicity test of soursoup leaves extract (Annona muricata) on Swiss mice’s liver and kidney.Methods: Twenty four mice were divided into 4 groups. Group I was control group, while group II-IV was given soursoup leaves extract as single dose orally via sonde. The mice were obsereved until day 7 to determine the LD50 and at the end were terminated to collect the liver and kidney. The organs later were made into histopathology slides. The slides read with light microscope. The data analyzed with ANOVA and was considered significant at p<0.05.Results: All mice were alive during the 7 days observation and no mice showing the toxic spectrum after the dosing. Microscopically, no damage on the liver and kidney organ among the groups.Conclusion: The LD50 of soursoup leaves extract is more than 2000 mg/kgBW. This result indicate that the extract is practically non toxic and do not damage the liver and kidney.

Abstract

Introduction: Brown algae (Sargassum duplicatum) contains bioactive compound with natural antioxidant that may potentially decrease lipid peroxidation and protein oxidation process in diabetes mellitus. Objective: to prove the effectiveness of brown algae extract administration (150mg/kgBW; 300mg/kgBW and 450 mg/kGBW respectively) in reducing serum MDA and PCO levels in streptozotocin-induced sprague dawley rat. Methods: This experiment study used the post test only control group design. Twenty eight (28) Sprague dawley rats induced with Streptozotocin (STZ) 40 mg/kgBW (i.p) were divided into four groups (n=7). The first group was diabetic non treated group (control). The second to fourth groups were the diabetic rats which given brown algae extract (150mg/kgBW; 300mg/kgBW and 450 mg/kgBW respectively) for 30 days. Serum MDA and PCO levels were examined by using ELISA method. Data were analyzed using one way ANOVA.Results: There were no significant differences in serum MDA levels among groups (p=0,405). However, serum PCO level in group administered with 450mg/kgBW of brown algae extract decreased significantly compared to control group (p=0.001), group administered with 150mg/kgBW (p=0.001) and 300mg/kgBW (p=0,037). Conclusion: Administration of brown algae extract did not decrease serum MDA levels significantly. Administration of 450mg/kgBW brown algae extract is effective to decrease serum PCO level significantly.

Abstract

Introduction: Untreated cuts in some period of time, caused by various reasons, can be a problem. Alternative therapy used to speed up the healing of the cut is Nigella Sativa (NS).Objective: to investigate the number of fibroblast and collagen by giving 40% and 60% NS seed extract on the cut.Methods: post test only control group design, 48 male balb/c strain rats were divided into 2 large groups, with each cage contained 24 rats. Each group then divided to 4. Each group is subdivided into 4. Nigella sativa seed extract ointment with a concentration of 40% (NS-40) and 60% (NS-60) smeared 3x daily for 7 days (fibroblasts) and 14 days (collagen). Evaluation derived from preparations using HE staining and Masson’s Trichrome respectively.Results: Mann Whitney analysis shows that the number of fibroblasts and collagen in the CG group, NS-40, and NS- 6 0 Higher than CN, P <0.05.The number of fibroblasts in the NS-40 and NS-60 was not significantly different compared to the CG, as well as CN-40 with CN-60, p> 0.05. While the number of collagen in CN-60 were significantly higher than CN-40, p<0.05.Conclusion : administration of Nigella sativa seed extract ointment with a concentration of 40% and 60% in cuts for 7-14 days increase the number of fibroblasts and collagen, resulting in a shorter healing period.

Abstract

Background: MRSA phenotypic detection has been a problem since it was found in 1962. Some studies explain that the diffusion method of cefoxitinand oxacilin disk test can be used to detect MRSA. Objective: knowing if there is difference between cefoxitin disk test and oxacilin disk test to detectMRSA.Method: laboratory experimental research with diagnostic test design. Research was done at the laboratory of medicine faculty of UNISSULA using 24 watch glasses, 12 with MRSA bacteria (Meticillin Resisten Staphylococcus aureus), and the other 12 with MSSA bacteria (Meticillin sensitive Staphylococcus aureus). specimentsThe results were classified into sensitive and resistant category based on CLSI standard (Clinical Laboratory Standards Institute).Hypothesis test using fisher test, with significance level <0,05Results: speciments MRSA detection using cefoxitin disk resulted 12 resistant speciments and no sensitive speciments. The oxacilin disk resulted 9 resistant speciments and 3 sensitive speciments. MSSA detection using cefoxitin disk resulted no resistant speciments and 12 sensitive speciments, oxacilin disk resulted no resistant speciments and 12 sensitive speciments. Diagnostic test was done by CEBM statistic calculator. The sensitivity and specificity value of MRSA sampels using cefoxitin disk were 96,2% & 96,2%, PPV (positive predictive value) 96,2%, NPV (negative predictive value) with was 96,2%. While the oxacilin disk, the sensitivity was 73,1%, specificity 96,2%, PPV 95,0%, NPV 73,8%. The result of fisher test for cefoxitin disk and oxacilin disk was p=0.000 meant there was difference between cefoxitin disk test and oxacilin disk test to detect MRSA.Conclusion: diffusion method in cefoxitin disk is better than oxacillin disk in MRSA detection.
