Abstract

Background: Personal hygiene is a fundamental healthy-living behavior that should be established during school age to prevent infectious diseases. A preliminary study at SD Negeri 060831 Medan revealed that 63% of students did not wash their hands with soap, 47% did not brush their teeth twice daily, and 40% neglected nail and hair hygiene. Conventional lecture-based education was considered insufficient, indicating the need for a more engaging approach such as visual storytelling. Objective: To analyze differences in personal hygiene behavior (knowledge, attitude, and practice) before and after visual storytelling education among school-aged children. Methods: A pre-experimental study with one-group pretest-posttest design. All 50 students of grades III–V were included through total sampling. Instruments consisted of a 20-item knowledge questionnaire (Guttman scale), a 20-item attitude questionnaire (Likert scale), and a 20-item practice observation checklist (Likert scale), previously validated with Cronbach’s α of 0.81, 0.75, and 0.71 respectively. Bivariate analysis used the Paired Sample t-test. Results: The proportion of respondents in the good category increased for knowledge (40.0% to 56.0%), attitude (56.0% to 58.0%), and practice (40.0% to 78.0%). Statistical tests demonstrated significant differences for knowledge (p = 0.001), attitude (p = 0.005), and practice (p < 0.001). Conclusion: Visual storytelling education is associated with improvements in personal hygiene behavior among school-aged children across knowledge, attitude, and practice domains. This method may serve as an alternative health-promotion medium to support the School Health Program (UKS) in primary schools.

Abstract

This study aims to examine the relationship between adolescents' knowledge about HIV/AIDS and HIV/AIDS prevention behavior at SMA Swasta Katolik Santa Maria Medan. Adolescents remain a high-risk group for HIV/AIDS transmission due to limited reproductive-health knowledge and exposure to risky behaviors, while national data show that Medan City records the highest number of HIV cases in North Sumatra Province. This research employed a descriptive-correlational design with a cross-sectional approach. The population consisted of 294 students in grades X–XII, and 75 respondents were recruited using the Slovin formula with a purposive sampling technique. Data were collected using a closed-ended questionnaire adapted from a previously validated instrument, then analyzed through univariate and bivariate analyses. The results showed that most respondents were female (54.7%) and aged 17 years (69.3%). The majority of respondents had a moderate level of knowledge about HIV/AIDS (58.6%) and moderate prevention behavior (66.6%). Bivariate analysis produced a correlation coefficient of 0.249 with a p-value of 0.045 (<0.05), indicating a statistically significant but weak positive relationship between adolescents' knowledge and HIV/AIDS prevention behavior. These findings imply that continuous and systematic health-education programs at school are needed to strengthen adolescents' knowledge and preventive behavior toward HIV/AIDS.

Abstract

Type 2 Diabetes Mellitus (T2DM) is a chronic non-communicable disease with a steadily rising global and local prevalence, in which unhealthy lifestyle choices play a critical role. This study aimed to analyze the association of lifestyle factors (dietary pattern, physical activity, sleep pattern, stress, and smoking habits) with the incidence of T2DM at Stabat Public Health Center. A quantitative study with a cross-sectional design was conducted on 1,123 T2DM patients recorded in 2025, with a sample of 92 respondents selected using Slovin's formula. Data were collected through questionnaires and analyzed using univariate, bivariate (Chi-Square), and multivariate (multiple logistic regression) analyses. Bivariate analysis revealed significant associations between T2DM incidence and dietary pattern (p = 0.002; OR = 8.333), physical activity (p = 0.000; OR = 13.839), sleep pattern (p = 0.008; OR = 4.059), stress (p = 0.000; OR = 11.420), and smoking habits (p = 0.021; OR = 3.235). Multivariate analysis confirmed that physical activity was the most dominant lifestyle factor associated with T2DM incidence (p = 0.008; OR = 10.091; 95% CI: 1.820–55.947). All evaluated lifestyle factors are significantly associated with T2DM, with physical inactivity identified as the primary determinant. Stabat Public Health Center is recommended to strengthen promotive-preventive programs focusing primarily on physical activity and comprehensive healthy lifestyle interventions.

Abstract

Digital transformation in health services through Electronic Medical Records (EMR) is expected to improve service efficiency, information accuracy, and patient experience. Successful EMR implementation depends not only on technology but also on the fit between human and organizational factors. This study aimed to analyze EMR implementation based on the Human, Organization, Technology-Fit (HOT-FIT) model in relation to patient satisfaction at Koncara Public Health Center, Purwakarta Regency. This observational analytic quantitative study used a cross-sectional design. A total of 100 patients were selected using purposive sampling. Data were collected using a four-point Likert questionnaire measuring Human, Organization, Technology, and Patient Satisfaction. Data analysis included univariate analysis, normality testing, Spearman correlation, and multiple linear regression using IBM SPSS Statistics. Human was strongly correlated with patient satisfaction (rho=0.793; p<0.001), Organization also showed a strong correlation (rho=0.701; p<0.001), and Technology had the strongest correlation (rho=0.844; p<0.001). Multiple linear regression showed significant partial effects of Human (B=0.287; p<0.001), Organization (B=0.213; p=0.004), and Technology (B=0.457; p<0.001). The overall model was significant (F=127.282; p<0.001) with R²=0.799, and Technology was the dominant predictor (β=0.499). Patient satisfaction was statistically associated with the fit among Human, Organization, and Technology. The health center should prioritize EMR system stability and quality while strengthening staff competence and service governance.

Abstract

Stroke is a non-communicable disease that remains a leading cause of death and disability worldwide. The risk of stroke is influenced by several modifiable factors, including a history of comorbidities, stress levels, and physical activity. This study aimed to analyze the relationship between a history of comorbidities, stress levels, and physical activity with the incidence of stroke among patients at RSU Royal Prima Medan in 2026. A quantitative method with a cross-sectional design was employed. The sample comprised all 53 stroke patients treated between January and April 2026, selected through total sampling. Data were collected using questionnaires and analyzed univariately and bivariately using the Chi-Square test at α = 0.05. Results showed that the majority of respondents were over 56 years old (67.9%), male (54.7%), and had ischemic stroke (52.8%). Bivariate analysis revealed p-values of 0.509 for comorbidities, 0.027 for stress levels, and 0.402 for physical activity. Stress levels were significantly associated with stroke incidence, whereas comorbidities and physical activity were not. Efforts to control chronic diseases, manage stress, and adopt a healthy lifestyle remain essential to reduce stroke risk.

Abstract

Refractive errors are a major cause of visual impairment among medical students and require appropriate knowledge, attitudes, and practices for effective management. This study aims to compare the knowledge, attitudes, and practices of medical students from the 2022 and 2023 cohorts at the Faculty of Medicine, Hang Tuah University, Surabaya, regarding refractive error correction. This analytic observational study employed a cross-sectional design. A total of 140 active medical students from the 2022 and 2023 cohorts were recruited using quota sampling. Data were collected through a validated self-administered online questionnaire distributed via Google Forms. Comparative analysis was performed using the Mann–Whitney U test with a significance level of α < 0.05. The results demonstrated a statistically significant difference in knowledge levels between the two cohorts (p < 0.001), with students from the 2022 cohort exhibiting higher knowledge scores. However, no significant differences were observed in attitudes (p = 0.086) or practices (p = 0.891) between the two groups. Although the majority of students exhibited positive attitudes toward refractive error correction, preventive and corrective practices in both cohorts remained suboptimal.

Abstract

Breast milk is a source of important nutritional elements for newborns for growth and development and increases the body's immunity to disease. Papaya fruit (Carica papaya) contains lactagogue, a substance that can increase or facilitate milk production, and is rich in vitamin A, polyphenols, and steroids that stimulate the prolactin reflex. This study aimed to determine the effect of giving papaya fruit on the smooth production of breast milk in postpartum mothers at the Gunung Sahilan Garden Clinic, division 2 area, Gunung Sahilan District, Kampar Regency. The research used a Pre-Experimental method with a One Group Pretest Posttest design, in which one group was observed before treatment (pretest) and after treatment (posttest). The sample consisted of 12 postpartum mothers taken by total sampling, and milk volume was measured using a breast pump and an observation sheet. Data were analyzed using the Shapiro Wilk normality test and the Paired T-Test with a significance level of a = 0.05. The results showed that milk production was normally distributed (pretest P = 0.51 and posttest P = 0.58, both > 0.05), with a mean increase from 163.3 ml before to 256 ml after consuming papaya. The Paired T-Test obtained P = 0.00 < 0.05, meaning there was an effect of papaya fruit consumption on the smooth production of breast milk in postpartum mothers.

Abstract

Background: Anemia in pregnancy remains a major maternal health problem worldwide, with an estimated global prevalence of 37–40% and a considerable burden in Indonesia. Poor sleep quality has also been reported to aggravate anemia because it may interfere with erythropoiesis. Sweet potato leaves (Ipomoea batatas L.), a low-cost local food rich in iron and vitamin C, offer potential as a non-pharmacological intervention. Objective: This study examined the effect of Ipomoea batatas L. leaf drink on hemoglobin levels and sleep quality among pregnant women in Saitnihuta Village, Dolok Sanggul, in 2026. Methods: A quantitative quasi-experimental one-group pretest-posttest design was applied to 30 third-trimester pregnant women with mild anemia, selected through total sampling. The intervention consisted of a daily leaf drink for two weeks; data were analyzed using the Wilcoxon Signed Ranks Test. Results: Mean hemoglobin increased from 9.92 g/dL to 11.30 g/dL, with 80.0% of respondents reaching normal levels; good sleep quality increased from 10.0% to 86.7% of respondents. Both changes were statistically significant (p = 0.000). Conclusion: Ipomoea batatas L. leaf drink is an effective, inexpensive, and accessible complementary approach to improving hemoglobin levels and sleep quality in pregnant women, alongside routine antenatal care.

Abstract

Posyandu is a community-based health service post that plays a strategic role in monitoring toddler growth and development, providing immunizations, and preventing stunting. Although the national target for Posyandu visits is set at 85%, the actual coverage has only reached 81%, while Medan City recorded the lowest coverage in North Sumatra at 67.2% with a stunting prevalence of 26.4%. At Simalingkar Public Health Center, monthly visits in the first quarter of 2026 reached only around 7% of the target, thereby increasing the risk of growth faltering. This cross-sectional analytical quantitative study aimed to analyze factors associated with low Posyandu visits among toddlers. A sample of 84 mothers of toddlers was selected from a population of 1,158 using Slovin's formula and purposive sampling, and the data were analyzed through univariate and bivariate Chi-Square tests as well as Odds Ratios (OR) with 95% Confidence Intervals (CI). The results showed that 61.9% of toddlers visited Posyandu regularly, whereas 38.1% did not visit regularly. Maternal knowledge (p = 0.744), health service quality (p = 0.051), and distance to Posyandu (p = 0.064) had no significant effect. Conversely, operational schedule (p = 0.026; OR = 3.375) and healthcare personnel (p = 0.016; OR = 3.009) were significantly associated as dominant risk factors. These findings indicate that improving activity scheduling and enhancing the competence of healthcare personnel and cadres should be top priorities for Simalingkar Public Health Center in increasing Posyandu visits among toddlers.

Abstract

Tuberculosis (TB) remains one of the leading causes of death from infectious disease worldwide and continues to pose a serious threat to children under five, whose immune systems are not yet fully developed. This study aimed to analyze the factors associated with TB prevention efforts among children under five in Dolok Masihul Sub-district, Serdang Bedagai Regency, in 2026, namely maternal knowledge, attitude, education level, home environmental conditions, access to health services, and health worker support. A quantitative observational-analytic design with a cross-sectional approach was used. The study population comprised 3,417 mothers of children aged 0–59 months; 72 respondents were selected through simple random sampling based on the Slovin formula. Data were collected using a structured questionnaire and analyzed through univariate and bivariate statistics using the Chi-square test at α = 0.05. Most respondents had inadequate knowledge (83.3%) and a negative attitude (52.8%), while home environment (55.6%), access to health services (51.4%), and health worker support (55.6%) were largely favorable; 52.8% demonstrated good TB prevention practices. Chi-square analysis showed significant associations between prevention efforts and knowledge (p=0.003), attitude (p=0.004), home environment (p=0.000), access to health services (p=0.000), and health worker support (p=0.005), whereas education level was not significantly associated (p=0.334). Strengthening maternal education on BCG immunization and Tuberculosis Preventive Therapy is recommended.
