Abstract

The study purpose was to design, develop, and evaluate an elephant grass chopping machine that integrates a rotary blade mechanism and a pull roll system to improve feeding stability, productivity, and chopping uniformity in ruminant feed processing. Traditional manual chopping methods are labor-intensive, time-consuming, and produce inconsistent particle sizes, which reduce feed efficiency. This research aimed to develop a farm-scale machine with a target capacity of 1 ton/hour that is technically and economically feasible for farmers.Materials and methods. This study used an engineering-based research and development approach consisting of design, fabrication, and performance testing. The prototype was powered by a GX 220 gasoline engine and equipped with strip-type carbon steel rotary blades, a pull roll feeding system, and a pulley–V-belt transmission. Performance testing was conducted using elephant grass at three blade rotational speeds (1,120 rpm, 1,680 rpm, and 2,240 rpm). The observed parameters included chopping time, output mass, chopping capacity, and distribution of chopped sizes.Results. The machine achieved an optimal working capacity of approximately 1,080 kg/hour at a blade speed of 1,680 rpm, meeting the design target. Higher blade speeds reduced processing time and increased the proportion of smaller chopped particles, while lower speeds resulted in longer and less uniform cuts. The pull roll system effectively stabilized material feeding and enabled continuous operation.Conclusions. The developed machine improved labor efficiency, productivity, and feed chopping uniformity, indicating good potential for farm-scale application. However, further design improvements are required to reduce frame vibration, optimize hopper design, and enhance blade mounting safety.

Abstract

Abstract

Background: Tuberculosis (TB) remains a major public health challenge in Indonesia, with approximately 1,090,000 cases reported annually. Achieving the national TB elimination target by 2030 requires optimal treatment adherence; however, medication non-adherence continues to hinder treatment success and disease control. This study aimed to identify factors influencing medication adherence among pulmonary TB patients at Puskesmas Medan Area Selatan. Methods: A cross-sectional analytic study was conducted from January to March 2025 involving pulmonary TB patients registered under the Directly Observed Treatment Short-course (DOTS) program. A total of 87 respondents were recruited using consecutive sampling based on the following inclusion criteria: confirmed pulmonary TB diagnosis, undergoing at least one month of treatment, and willingness to participate. Data were collected using structured questionnaires assessing six independent variables: patient knowledge, family support, healthcare service quality, drug side effects, the role of treatment supervisors (PMO), and socioeconomic status. Medication adherence was measured using the Morisky Medication Adherence Scale-8 (MMAS-8). Binary logistic regression analysis was performed using SPSS version 26.0. Results: Among the respondents, 62.1% demonstrated good medication adherence. Bivariate analysis revealed significant associations between adherence and patient knowledge (p = 0.001; OR = 4.87; 95% CI: 1.89–12.53), family support (p = 0.003; OR = 3.94; 95% CI: 1.56–9.94), healthcare service quality (p = 0.021; OR = 2.87; 95% CI: 1.17–7.03), and drug side effects (p = 0.038; OR = 2.41; 95% CI: 1.05–5.51). Multivariate logistic regression identified patient knowledge as the most dominant factor influencing adherence (Adjusted OR = 5.12; 95% CI: 1.91–13.70; p = 0.001), followed by family support (AOR = 3.78; 95% CI: 1.43–9.98; p = 0.007). Conclusion: Patient knowledge is the strongest determinant of medication adherence among TB patients at Puskesmas Medan Area Selatan. Strengthening structured health education interventions and promoting family-centered support strategies are essential to enhance treatment adherence and accelerate progress toward Indonesia’s TB elimination target by 2030.

Abstract

Background: Hospital Management Information Systems (HMIS) are increasingly recognized as essential tools for improving administrative efficiency in healthcare settings. Rumah Sakit Islam Malahayati (RSI Malahayati) in Medan, Indonesia adopted an integrated HMIS to streamline its administrative processes; however, the extent to which the system has effectively improved service delivery efficiency remains inadequately evaluated. This study aimed to evaluate the implementation of HMIS on administrative service efficiency at RSI Malahayati, Medan, in 2026. Methods: A qualitative study employing a phenomenological approach guided by the Colaizzi method was conducted from September to December 2025. The study was set at RSI Malahayati, a 150-bed Islamic private hospital in Medan, North Sumatra. Purposive sampling was applied to recruit 18 participants, including hospital administrators, information technology staff, medical records officers, and front-line administrative personnel. Inclusion criteria required direct engagement with HMIS for a minimum of six months. In-depth, semi-structured interviews were conducted face-to-face, averaging 55 minutes per session. Interviews were audio-recorded, transcribed verbatim, and analyzed using NVivo 12 software. Data saturation was confirmed at the 16th interview. Trustworthiness was ensured through triangulation, member checking, and peer debriefing, in conformity with COREQ standards. Results: Four major themes emerged from data analysis: (1) Enhanced administrative workflow integration, characterized by reduced manual data entry and inter-departmental communication delays; (2) Barriers to optimal system utilization, encompassing inadequate technical training, system downtime incidents, and user resistance; (3) Impact on patient service responsiveness, reflecting shorter waiting times and improved patient registration accuracy; and (4) Organizational and cultural factors shaping HMIS adoption, including leadership support and Islamic institutional values influencing staff acceptance. Participants consistently acknowledged improvements in data accuracy and reporting efficiency, though persistent technical and human resource challenges limited system performance. Conclusion: HMIS implementation at RSI Malahayati has demonstrably improved administrative service efficiency through enhanced workflow integration and data management. However, sustainable efficiency gains require addressing identified barriers through comprehensive staff training, technical infrastructure reinforcement, and culturally responsive change management strategies tailored to Islamic hospital settings.

Abstract

Background: Stroke remains a leading cause of long-term disability worldwide, significantly impairing patients' functional independence. Family support has been recognized as a critical psychosocial factor that may influence rehabilitation outcomes; however, empirical evidence from Indonesian hospital settings, particularly in North Sumatra, remains limited. The purpose of this study was to analyze the effect of family support on the functional recovery of post-stroke patients at Haji General Hospital Medan in 2026. Methods: A descriptive correlational cross-sectional design was employed. Participants were 87 post-stroke patients recruited through purposive sampling between September and December 2025. Inclusion criteria required patients to have been diagnosed with stroke at least one month prior, be aged 18 years or older, and have a primary family caregiver. Data on family support were collected using the adapted Hensarling Diabetes Family Support Scale (HDFSS), and functional recovery was assessed using the Barthel Index (BI). Ethical clearance was obtained from the institutional review board. Data were analyzed using SPSS version 26, with Spearman's rank correlation coefficient applied to test the hypotheses (alpha = 0.05). Results: Of 87 participants, 52.9% reported high family support and 55.2% demonstrated good functional recovery (Barthel Index score ≥ 61). Spearman's rank correlation analysis revealed a statistically significant moderate positive relationship between overall family support and functional recovery (r = 0.541; p = 0.001). All four sub-dimensions of family support (informational, emotional, instrumental, and appraisal) were significantly correlated with functional recovery (p < 0.05). Conclusion: Family support exerts a significant positive effect on the functional recovery of post-stroke patients. Healthcare providers and hospital management should develop structured family-centered care programs to optimize rehabilitation outcomes for post-stroke patients
