Abstract

Background: Healthcare service quality is a critical determinant of patient satisfaction and clinic performance, particularly in primary healthcare settings in developing countries. Patient satisfaction serves as a key indicator to assess whether healthcare providers meet patients' expectations and needs. Objective: This study aims to analyze the quality of healthcare services based on patient satisfaction at Ghaniya Clinic, Medan, North Sumatra, Indonesia, using the SERVQUAL model encompassing five dimensions: tangibility, reliability, responsiveness, assurance, and empathy. Methods: A descriptive-analytical cross-sectional study was conducted from September to December 2025. A total of 150 outpatients were recruited using purposive sampling. Data were collected using a validated structured questionnaire based on the SERVQUAL instrument. Descriptive statistics, Customer Satisfaction Index (CSI), and Importance-Performance Analysis (IPA) were employed. Results: The overall patient satisfaction rate was 72.7%. Empathy scored the highest mean satisfaction (3.82 ± 0.51), while tangibility received the lowest score (3.41 ± 0.64). The Customer Satisfaction Index (CSI) was 76.3%, indicating a "Satisfied" category. IPA analysis revealed that responsiveness and physical facilities were the primary priority improvement areas. Conclusion: Healthcare service quality at Ghaniya Clinic is moderately satisfactory. Significant improvements are required in responsiveness and tangibility dimensions to achieve higher patient satisfaction. Systematic management interventions targeting staff responsiveness training and facility upgrading are strongly recommended.

Abstract

Background and Purpose: Adverse events in hospitals remain a persistent global patient safety challenge, with nurses' knowledge and attitudes identified as key determinants of safety culture quality. This study aimed to investigate the relationship between nurses' knowledge and attitudes toward patient safety and the implementation of patient safety culture in inpatient wards of Mitra Sejati General Hospital, Medan, Indonesia. Methods: A cross-sectional correlational study was conducted among 85 inpatient ward nurses recruited via total sampling. Inclusion criteria included nurses actively serving in inpatient wards for >= 6 months who consented to participate; nurses on administrative leave or student nurses were excluded. Data were collected using three validated instruments: a 30-item patient safety knowledge questionnaire (Cronbach's alpha = 0.83), a 25-item attitude scale (Cronbach's alpha = 0.81), and the Hospital Survey on Patient Safety Culture version 2.0 (HSOPSC v2.0; Cronbach's alpha = 0.87). Data were collected from September 1 to October 31, 2025. Ethical clearance was obtained from the Health Research Ethics Committee, Universitas Sumatera Utara (No. 451/TGL/KEPK FK USU/2025). Spearman rank correlation and multiple linear regression analysis were performed using SPSS version 26 (alpha = 0.05). Results: The majority of participants were female (69.4%), aged 31-40 years (43.5%), and had completed Bachelor of Nursing education (52.9%). Patient safety knowledge was predominantly in the adequate category (41.2%), with mean 72.4 +/- 11.8. Attitudes were predominantly positive (61.2%), with mean 74.6 +/- 9.3. Patient safety culture implementation was mostly in the adequate category (52.9%), with mean 71.8 +/- 10.5. Significant positive correlations were found between knowledge and PSC implementation (rs = 0.524; p < 0.001) and between attitude and PSC implementation (rs = 0.612; p < 0.001). Multiple regression revealed that both variables together explained 44.1% of variance in PSC implementation (R2 = 0.441; F = 32.33; p < 0.001). Conclusion: Both nurses' knowledge and attitudes toward patient safety are significantly associated with the implementation of patient safety culture. Attitude demonstrated a stronger association than knowledge. Hospital management should prioritize routine patient safety training and a culture-building approach to strengthen both cognitive and attitudinal dimensions of patient safety among inpatient nurses.

Abstract

Reconstitution of injectable drugs involved mixing sterile drug powders with appropriate sterile solvents prior to parenteral administration. This critical pharmacy practice was particularly vital in pediatrics where dosing errors and contamination posed significant risks. Reconstitution errors could lead to chemical instability, therapeutic failure, and serious adverse effects including toxicity or nosocomial infections. This study aimed to evaluate the appropriateness of sterile injectable drug reconstitution procedures in the Pediatric Ward of Universitas Sebelas Maret Hospital across five parameters: dosage calculation, solvent selection, solvent volume, post-reconstitution storage temperature, and Beyond Use Date (BUD) assignment. A descriptive observational study with prospective design was conducted among pediatric inpatients from December 2025 to January 2026. Seventy cases of pediatric injectable drug administrations were sampled using saturated (total) sampling technique. Primary data were collected through direct pharmacist reconstitution observations while secondary data came from medical records and sterile preparation worksheets. Compliance was benchmarked against gold standards: Pediatric Injectable Drugs 10th edition and Handbook on Injectable Drugs 17th edition. Perfect compliance (100%) was achieved for dosage, solvent selection (NaCl 0.9%, sterile water), and reconstitution volumes. However, storage temperature compliance (2-8°C for light-sensitive drugs) was only 68%, while BUD documentation aligned with post-reconstitution stability in 70% of cases, leaving 2% critical non-compliance posing patient safety risks. While reconstitution technical execution was excellent, cold chain monitoring and BUD documentation required systemic improvements through regular training, real-time temperature tracking, and stricter Standard Operating Procedures to ensure pediatric pharmacotherapy safety.

Abstract

This comparative analytical study examines the philosophical and pedagogical frameworks of tazkiyat al-nafs (purification of the soul) as conceptualized by two preeminent Islamic thinkers: Abu Hamid al-Ghazali (d. 505/1111) and Abu Ali Ahmad ibn Muhammad Miskawayh (d. 421/1030). Through rigorous examination of their foundational ethical treatises, particularly Al-Ghazali's Ihya' 'Ulum al-Din and Miskawayh's Tahdhib al-Akhlaq, this study elucidates the theoretical underpinnings, methodological approaches, and practical implications of their respective models for character formation. The analysis reveals a fundamental dichotomy between Miskawayh's rationalist-philosophical emphasis on psychic equilibrium and virtue cultivation through habituation, and Al-Ghazali's mystical-spiritual orientation toward divine proximity through systematic purification of the self. While both thinkers operated within the Aristotelian framework of virtue ethics and drew upon the tripartite division of the soul, their divergent epistemological commitments—Miskawayh's prioritization of natural reason and environmental conditioning versus Al-Ghazali's integration of Sufi mysticism with ethical praxis—produced substantively distinct architectures for moral development. This comparative investigation contributes to contemporary discourse on holistic character education within Islamic contexts, demonstrating the continued relevance of classical Islamic ethical philosophy for addressing multifaceted challenges in character formation and spiritual development.

Abstract

Terdapat permasalahan prosedur kerja pengelolaan arsip pada Dinas Perpustakaan dan Kearsipan Daerah Kabupaten Merauke yang berakibat menjadi tidak terarah dalam menjalankan fungsinya. Jenis penelitian yang digunakan dalam penelitian ini adalah penelitian deskriptif dengan pendekatan kualitatif yang akan memberikan gambaran mengenai bagaimana pengelolaan arsip pada Dinas Perpustakaan dan Kearsipan Daerah Kabupaten Merauke, karena metode kualitatif mempelajari data di lapangan secara alamiah dan mengutamakan segi kualitas data. Pengelolaan arsip yang baik menjadi sebuah keharusan pada Dinas Perpustakaan dan Kearsipan Daerah Kabupaten Merauke. Pengelolaan arsip pada Dinas Perpustakaan dan Kearsipan Daerah Kabupaten Merauke belum berjalan dengan baik. Faktor pendukung pada Dinas Perpustakaan dan Kearsipan Daerah Kabupaten Merauke yaitu tersedianya sarana dan prasarana berupa seperangkat alat komputer, ruang penyimpanan arsip, sarana penyimpanan arsip aktif, sarana penyimpanan arsip inaktif, kemudian daftar arsip.

Abstract

The study purpose was to design, develop, and evaluate an elephant grass chopping machine that integrates a rotary blade mechanism and a pull roll system to improve feeding stability, productivity, and chopping uniformity in ruminant feed processing. Traditional manual chopping methods are labor-intensive, time-consuming, and produce inconsistent particle sizes, which reduce feed efficiency. This research aimed to develop a farm-scale machine with a target capacity of 1 ton/hour that is technically and economically feasible for farmers.Materials and methods. This study used an engineering-based research and development approach consisting of design, fabrication, and performance testing. The prototype was powered by a GX 220 gasoline engine and equipped with strip-type carbon steel rotary blades, a pull roll feeding system, and a pulley–V-belt transmission. Performance testing was conducted using elephant grass at three blade rotational speeds (1,120 rpm, 1,680 rpm, and 2,240 rpm). The observed parameters included chopping time, output mass, chopping capacity, and distribution of chopped sizes.Results. The machine achieved an optimal working capacity of approximately 1,080 kg/hour at a blade speed of 1,680 rpm, meeting the design target. Higher blade speeds reduced processing time and increased the proportion of smaller chopped particles, while lower speeds resulted in longer and less uniform cuts. The pull roll system effectively stabilized material feeding and enabled continuous operation.Conclusions. The developed machine improved labor efficiency, productivity, and feed chopping uniformity, indicating good potential for farm-scale application. However, further design improvements are required to reduce frame vibration, optimize hopper design, and enhance blade mounting safety.

Abstract

Background and Purpose: Depression and reduced quality of life (QoL) are prevalent but underaddressed conditions among institutionalized elderly. This study aimed to examine the effect of Group Activity Therapy (GAT) on quality of life and depression levels in elderly residents at Panti Asuhan Yayasan Karya Murni, Medan, Indonesia. Methods: A quasi-experimental study with a pre-test–post-test control group design was conducted. Sixty elderly residents (aged ≥60 years) were selected using purposive sampling and equally assigned to intervention (n=30) and control (n=30) groups. Inclusion criteria included residents aged ≥60 years, scoring ≥5 on the Geriatric Depression Scale-15 (GDS-15), and able to communicate. Exclusion criteria included severe cognitive impairment and acute medical illness. The intervention group received eight structured GAT sessions over four weeks (two sessions/week, 60 minutes each), covering music therapy, reminiscence therapy, and light exercise. Quality of life was measured using the validated Indonesian version of the WHOQOL-BREF, and depression was assessed with the GDS-15. Data were analyzed using the Wilcoxon Signed-Rank Test and Mann-Whitney U Test (SPSS v.25, p<0.05). Ethical clearance was obtained from the Institutional Ethics Committee of Universitas Bunda Thamrin (No. 014/EC-UBT/2024). Results: The intervention group showed a significant improvement in QoL (mean pre: 48.3 vs. post: 67.8; p=0.001) and a significant reduction in depression scores (mean pre: 9.7 vs. post: 5.1; p=0.001). The control group showed no significant changes. Between-group comparisons were statistically significant for both outcomes (p=0.001). Conclusion: Group Activity Therapy significantly improves quality of life and reduces depression among institutionalized elderly. Healthcare providers and nursing home administrators should integrate structured group activity programs into routine elderly care to promote psychosocial well-being.

Abstract

Women’s empowerment is one of the key strategies for strengthening family economic resilience, particularly in rural areas. Women play dual roles, not only as managers of household affairs but also as economic actors who contribute to family income. This article aims to provide a general review of the role of women’s empowerment in enhancing family economic resilience in Sugihwaras Barat Village. The approach used is descriptive qualitative, examining various forms of women’s empowerment activities such as micro-enterprises, productive skills development, and participation in community-based economic groups. The findings indicate that women’s empowerment can increase family income, strengthen economic independence, and help families cope with economic pressures. Therefore, women’s empowerment plays a strategic role in creating family economic resilience in Sugihwaras Barat Village.

Abstract

Self-medication has become a serous problem among the public due to a lack of knowledge and behavior about self-medication. The percentage of West Kalimantan people practicing self-medication during the last month in 2022 was 86.81%. The purpose of this study was to analyze the level of knowledge and behavior of community self-medication in tiong keranjik village, belimbing hulu sub-district, melawi district and the relationship between the two. This research method is a descriptive cross sectional non-experimental research. The sampling technique used purposive sampling with a questionnaire measuring instrument that had passed ethical review and tested validity and reliability on 30 respondents. Data processing was carried out with the spearman rank correlation statistical test. The results of the study of 100 respondents showed that the level of knowledge of community self-medication behavior is included in the good category by 25%, moderate by 57% and less by 18%. The results of the spearman rank correlation test between knowledge and self-medication behavior showed a significance value of 0.000 with a correlations coefficient of 0.651, and a positive direction. It can be concluded that the majority of people have sufficient knowledge with appropriate behavior and there is a significant relationship between the level of knowledge and self-medication behavior in the community of tiong keranjik village with a strong and unidirectional correlation

Abstract

Penyakit Paru Obstruktif Kronis (PPOK) merupakan penyakit pernapasan progresif yang ditandai dengan keterbatasan aliran udara dengan gejala khas sesak napas, batuk, dan produksi dahak. Prevalensi PPOK di Indonesia mencapai 3,7% dengan kecenderungan peningkatan yang diperburuk oleh faktor risiko seperti kebiasaan merokok dan polusi udara. Data RSUP H Adam Malik Medan menunjukkan peningkatan pasien PPOK dari 283 kasus tahun 2022 menjadi 483 kasus tahun 2023, dan 202 kasus pada enam bulan pertama 2024. Pasien PPOK mengalami penurunan saturasi oksigen hingga 85% yang menyebabkan hipoksemia, gangguan pertukaran gas, hiperinflasi paru, dan penumpukan udara dalam alveolus yang mengganggu kualitas hidup. Pursed lip breathing merupakan teknik pernapasan non-farmakologis dengan menghirup udara melalui hidung dan menghembuskan melalui bibir yang dirapatkan untuk meningkatkan tekanan jalan napas dan mencegah kolaps saluran napas. Penelitian ini bertujuan menganalisis pengaruh pursed lip breathing exercises terhadap saturasi oksigen pasien PPOK di RSUP H Adam Malik Medan tahun 2024. Metode penelitian menggunakan desain pra-eksperimental dengan pendekatan one-group pre-post test design. Sampel sebanyak 10 responden dipilih menggunakan teknik purposive sampling dengan kriteria pasien PPOK composmentis dan saturasi oksigen ≤95%. Intervensi pursed lip breathing diberikan sebanyak 6 kali perlakuan. Pengukuran saturasi oksigen menggunakan pulse oxymetry sebelum dan sesudah intervensi. Data dianalisis menggunakan uji Wilcoxon. Hasil penelitian menunjukkan saturasi oksigen meningkat signifikan dari rata-rata 92,70% (SD=1,160) sebelum intervensi menjadi 99,00% (SD=1,054) setelah intervensi dengan peningkatan rata-rata 6,30%. Uji Wilcoxon menghasilkan nilai p=0,004 (p<0,05) yang membuktikan adanya perbedaan signifikan. Seluruh responden (100%) yang awalnya berada pada kategori tidak normal (<95%) berhasil mencapai kategori normal (>95%) setelah pemberian intervensi. Kesimpulan penelitian menyimpulkan pursed lip breathing exercises terbukti efektif meningkatkan saturasi oksigen pasien PPOK secara signifikan dan direkomendasikan sebagai intervensi keperawatan non-farmakologis yang mudah, aman, efektif untuk mengoptimalkan fungsi pernapasan dan kualitas hidup pasien PPOK.
