Abstract

Antiretroviral treatment is one of the biggest parts of the treatment for people infected with HIV. This study aims to determine the effectiveness of drugs and the cost efficiency of ARV combination drug therapy. The study used a non-experimental research design that was observational to determine the Cost Effectiveness Analysis of 2 combinations of ARVs, namely the combination of TLD (Tenofovir + Lamivudin + Dolutegraph), and the combination of TLE (Tenofovir + Lamivudin + Efavirenz). Data collection was carried out retrospectively from the medical record data of patients who started ARV treatment in the period 2021 to 2023. Total of 189 patients were actively treated as outpatients at VCT polyclinics and 64 who met the inclusion criteria were used as study subjects. The effectiveness of treatment uses the parameters of the Viral Load test results. Treatment is said to be effective if the Viral Load test result is <40 copies/mL. Based on the results of the Viral Load test, the TLD combination was more effective (75.9%) compared to the TLE combination (66.7%). Based on Cost Effective Analysis, the combination of TLD (Tenofovir + Lamivudin + Dolutegrafir) is better with an ACER value of IDR 39,871 compared to the combination of TLE (Tenofovir + Lamivudin + Efavirenz) with an ACER value of IDR 68,599.

Abstract

Telemedicine has emerged as a transformative solution to enhance healthcare accessibility and efficiency, especially during the COVID-19 pandemic. Despite its benefits, its adoption and sustainability face significant barriers, including high initial costs, inconsistent reimbursement policies, digital divides, and operational challenges. These issues disproportionately affect rural and underserved populations, limiting equitable access and efficiency in healthcare delivery. This study aims to assess the financial and operational impacts of telemedicine, with a particular focus on hybrid telehealth models that integrate virtual consultations with in-person care. It investigates the role of government subsidies in addressing financial barriers and ensuring long-term sustainability. Telemedicine improves healthcare accessibility but faces economic, technological, and social barriers. Future research must explore underserved areas, hybrid models, and digital literacy, while policymakers should enhance infrastructure and implement inclusive policies. The study underscores the transformative potential of telemedicine in reshaping hospital financial models while promoting equitable access to care. It highlights the necessity for targeted strategies and government support to overcome existing barriers, ensuring that telemedicine serves as an effective and inclusive healthcare solution. Future efforts should focus on optimizing subsidy allocation to maximize both economic and healthcare benefits.

Abstract

Human Immunodeficiency Virus (HIV) is a disease that is contagious to humans so that it can lower the immune system and be susceptible to diseases. Drugs given to treat HIV infection and improve the quality of life of People With HIV/AIDS (ODHA) are antiretrovirals (ARVs). Adherence to taking antiretrovirals is an important factor in the success of therapy and reducing the amount of HIV virus in the body of ODHA, so that a high level of adherence has a fairly good or undetectable viral load result. The purpose of this study is to find out if there is a relationship between compliance with viral load levels. The research method was conducted with a cross sectional design, starting from May to June 2024. The data used are the results of the MMAS-8 questionnaire that has been validated and filled out directly at the Putri Ayu Health Center in Jambi City and the results of the viral voad level laboratory. Sampling with consecutive sampling. The data obtained in the analysis by bivariate analysis used the Cramer's V with a degree of significance of p-value <0.05. The results in this study were obtained by 11 (52.4%) respondents with a high level of compliance so that the results of viral load were not detected, there were 10 respondents with a moderate and low level of compliance so that 8 (38.1%) respondents had a detected viral load result and 2 respondents were not detected. There was a significant relationship with adherence to taking antiretroviral drugs to viral load levels with a result of 0.000 p-value <0.05 or 5%.

Abstract

Globalization caused changes in the job market and gave rise to a new segment in the job market called the Gig Economy. Gig Economy is a general term used to describe a job market dominated by freelancers or temporary jobs centered on the internet. This study uses a quantitative research method. This research data uses primary and secondary data with the distribution of a questionnaire of 100 millennial generation respondents in the city of Wattampone. The sampling technique uses the Random Sampling technique. The results of this study show that (1) The job market has a significant effect on the Gig economy. (2) Technology has a significant effect on the gig economy. (3) The job market and technology have a simultaneous effect on the Gig Economy.

Abstract

Corruption is a serious threat to economic stability and government integrity, where cash transactions in the banking system are often used as a means of laundering money proceeds from crime. This study aims to analyze the regulation of cash transaction reporting in banking to prevent corruption and identify preventive measures. This study highlights and analyzes the implementation of Law Number 8 of 2010 concerning the Prevention and Eradication of Money Laundering Crimes in the context of banking transactions. This study uses a normative juridical method with a literature approach, which examines primary and secondary legal materials to identify the effectiveness of banking regulations in preventing corruption. The results of the study show that the mechanism for reporting cash transactions by banks has been strictly regulated through PPATK, but there are still gaps in its implementation that allow corruption to still occur. The conclusion of this study emphasizes the importance of strengthening regulations and supervision of financial transactions, limiting the nominal amount of cash transactions, as well as more effective coordination between banking authorities and law enforcement agencies in eradicating corruption through the banking system. With stricter supervision, the transparency of financial transactions can be improved, thereby preventing the use of banking institutions as a means of laundering money from corruption.

Abstract

Processing organic waste into renewable energy is an effective solution to reduce environmental impact and meet energy needs. This research aims to design and build a manually stirred biodigester using a double propeller system with a capacity of 200 liters. This biodigester is designed to process cow manure waste and rumen as the main raw material, utilizing the abundant and not yet optimal biomass potential. A manual stirring system with two propellers is implemented to improve the mixing efficiency of the substrate, so as to maximize the production of biogas. Tests are carried out to evaluate the performance of the equipment, including the biogas production capacity, the quality of the methane gas produced, and the efficiency of the fermentation process. The results of the study show that the designed biodigester is able to produce biogas effectively with methane levels that meet energy use standards. Innovations in manual stirrers using double propellers have been proven to increase the homogeneity of the substrate, accelerate fermentation times, and increase the volume of gases produced. This biodigester has the potential to be applied on a household and community scale, supporting efforts to use sustainable renewable energy while reducing environmental pollution due to organic waste. This research is expected to be a reference in the development of biodigester technology that is efficient and easy to apply in various regions.

Abstract

Adolescence is an important transition period for women's reproductive health, characterized by a variety of physiological changes including the menstrual cycle. Menstrual cycle disorders, such as dysmenorrhea, oligomenorrhea, and amenorrhea, are common and can affect the quality of life of adolescent girls. This study aims to analyze the prevalence, risk factors, and impact of menstrual cycle disorders in adolescents in Indonesia through a literature reflection of articles published between 2019 and 2024. The results showed that about 67.8% of adolescent girls experienced at least one type of menstrual disorder, with dysmenorrhea being the most common complaint. Factors that affect this disorder include physical activity, stress levels, nutritional status, and body mass index. This research is expected to provide a scientific basis for the development of more effective prevention and treatment strategies for menstrual cycle disorders in adolescents.

Abstract

This study aims to analyze regional asset management and its impact on public services in BPKAD Nabire Regency. This type of research is qualitative with primary and secondary data sources. Data collection was carried out through observation, interviews with 4 BPKAD employees and 2 local communities, as well as documentation. The data analysis method uses qualitative descriptive analysis with the Spradley approach, which includes data collection, field recording, theme identification, and conclusion drawn. The results of the study show that the management of regional assets in BPKAD Nabire Regency through the stages of inventory, assessment, maintenance, reporting, and supervision seeks to improve the efficiency and effectiveness of public services. Despite good management efforts, constraints such as lack of human resources, technological limitations, minimal budgets, lack of transparency, and regulatory challenges hinder the effectiveness of asset management and the quality of public services. In conclusion, to improve the management of regional assets more effectively, improvements are needed in the capacity of human resources, information technology, budget, as well as transparency and accountability. With these steps, it is hoped that it can improve public services and have a positive impact on the people of Nabire Regency.

Abstract

The hierarchical culture in Islamic boarding schools (pondok pesantren) is often seen as an integral part of the education system and the character-building process for students (santri). However, in some cases, this system can contribute to the normalization of bullying, which is perceived as a form of discipline or a mental endurance test. This study aims to analyze how hierarchical structures in pesantren can lead to bullying and how such behavior becomes normalized in students' daily lives. This research employs a qualitative approach, using case studies based on observed incidents. Findings indicate that while hierarchy helps instill values of obedience and discipline, it can also create opportunities for bullying, which is often regarded as a tradition that junior students must endure. Case studies from several Islamic boarding schools in Indonesia, such as Pondok Pesantren Annur 2 Bululawang in Malang and Pondok Pesantren Husnul Khotimah in Kuningan, reveal that bullying persists in various forms. Therefore, it is essential to evaluate the hierarchical system to ensure that positive values are upheld without compromising students' psychological well-being. Additionally, effective preventive measures should be implemented to create a safe and supportive learning environment for all students.
