Abstract

Cancer related anorexia and cachexia significantly impair nutritional status, quality of life, and treatment outcomes. While appetite stimulants like megestrol acetate and dronabinol have shown effectiveness, their adverse effects limit long-term use. Cyproheptadine, a first-generation antihistamine and serotonin antagonist, has emerged as a potentially safer alternative. This review aims to evaluate critically synthesize previous research findingson clinical studies, randomized trials, and retrospective analyses on cyproheptadine’s efficacy and safety as an appetite stimulant. It has shown consistent benefits across various populations, including children with cystic fibrosis, patients with nonorganic failure to thrive, and adults with cancer-related cachexia. Improvements include significant appetite score increases (e.g., K-ESAS change of –2.42 vs. –2.03 with placebo; p = 0.0307), average weight gains of ~24 g/day, and improved BMI and nutritional markers such as albumin and hemoglobin. The most common side effect was mild drowsiness, while serious adverse effects like hepatotoxicity were rare. However, concerns remain about off-label overuse in community settings, often due to lack of patient education and regulatory oversight. Overall, cyproheptadine appears to be an effective and well-tolerated short-term option for appetite stimulation. Nonetheless, more research is needed to establish long-term safety, efficacy, and appropriate dosing guidelines.

Abstract

Organophosphate poisoning poses a significant health threat due to its ability to induce oxidative stress, cellular damage, and neurotransmitter disruption. Pharmacovigilance plays a crucial role in monitoring and preventing adverse effects associated with medical products, including responses to toxic exposure. This study aims to evaluate the protective role of N-acetylcysteine (NAC) against organophosphate-induced toxicity through a comprehensive literature review. The method employed includes a review of eight experimental studies conducted between 2017 and 2023, involving both in vivo (animal models) and in vitro (cell cultures and human samples) approaches. The results indicate that NAC significantly reduces oxidative stress biomarkers, enhances antioxidant enzyme activity, suppresses apoptosis, and restores inhibited cholinesterase activity. Moreover, NAC improves histopathological alterations in organs exposed to organophosphates, including the liver, kidneys, brain, and testes. In conclusion, NAC demonstrates strong therapeutic potential as a standalone or adjunct agent in managing organophosphate toxicity through its antioxidant, anti-inflammatory, and cytoprotective mechanisms.

Abstract

Polycystic Ovary Syndrome (PCOS) is a complex endocrine disorder affecting reproductive-aged women and is often accompanied by insulin resistance and hyperandrogenism. Metformin, a first-line antidiabetic agent, has been widely used in the treatment of PCOS due to its ability to improve insulin sensitivity, regulate menstrual cycles, reduce androgen levels, and promote ovulation. This literature review aims to critically evaluate and synthesize previous research findings on metformin and the influence of genetic and metabolic factors on individual responses in PCOS patients. Nine clinical studies published in the last five years were analyzed. The findings indicate that metformin consistently improves hormonal and metabolic parameters, particularly in overweight or insulin-resistant patients. Combination therapies, such as metformin with pioglitazone, liraglutide, or exenatide, were shown to be more effective than monotherapy in managing metabolic dysfunction and reproductive abnormalities. However, gastrointestinal side effects and interindividual variability in therapeutic response remain significant challenges. These results suggest that personalized treatment strategies based on patient phenotype and tolerance may optimize outcomes. Further research is needed to establish predictive markers and refine therapeutic regimens for long-term PCOS management.

Abstract

Fibromyalgia (FM) is a multifactorial chronic pain syndrome that significantly affects patients' quality of life. Despite the widespread use of gabapentinoids such as gabapentin, their off-label use in FM treatment remains controversial due to inconsistent efficacy and safety concerns. This systematic review aims to map the current state of knowledge in the field. of off-guideline use of gabapentin use in FM by synthesizing findings from four selected studies published between 2020 and 2025. The studies reviewed include clinical and preclinical trials assessing gabapentin monotherapy, its extended-release formulations, and combinations with osteopathic manipulative medicine (OMM) or natural compounds like sulforaphane. Results suggest that combination therapies offer superior pain relief compared to gabapentin alone. Notably, extended-release gabapentin demonstrated improved adherence and reduced side effects. Furthermore, mirogabalin, a novel gabapentinoid, was found to enhance both pain control and cognitive function ("fibro fog") in animal models. However, reported side effects such as dizziness and drowsiness, along with limitations in study design and sample size, emphasize the need for further randomized controlled trials. This review highlights the evolving role of gabapentinoids in FM treatment and underscores the importance of integrated therapeutic strategies for optimal patient outcomes.

Abstract

Tuberculosis (TB) remains a major public health problem in Indonesia, with treatment success highly dependent on patients’ adherence to anti-tuberculosis drug regimens. However, adherence is often hindered by adverse drug reactions (ADR), including nausea, vomiting, gastrointestinal disturbances, loss of appetite, and other symptoms that discourage patients from completing therapy. This systematic literature review aimed to analyze the impact of ADR on medication adherence among TB patients in Indonesia. Literature searches were conducted in Google Scholar, PubMed, and ScienceDirect databases using predefined keywords, with inclusion criteria covering articles published between 2020 and 2025, written in English or Indonesian, full-text available, and focusing on the relationship between ADR and treatment adherence. From the 22 eligible studies reviewed, the majority reported a significant relationship between ADR and adherence, while several studies indicated no significant effect. The most frequently reported ADR were gastrointestinal disorders, headaches, joint pain, and skin reactions. In addition, psychosocial factors such as family support, knowledge, and patient attitudes were also identified as influencing adherence, with family support emerging as the most dominant factor. Overall, ADR were found to negatively affect adherence in many cases, potentially increasing the risk of multidrug-resistant TB. Strengthened patient education, proactive counseling, and closer monitoring from healthcare providers are recommended to improve adherence and treatment outcomes.

Abstract

Chronic diseases such as type 2 diabetes mellitus and hypertension remain highly prevalent worldwide, including in Indonesia. The geriatric population is particularly vulnerable because of their susceptibility to polypharmacy, which increases the risk of adverse outcomes. This study aimed to evaluate polypharmacy among geriatric patients, especially participants of the PROLANIS, using a Systematic Literature Review (SLR). A literature search was conducted through Google Scholar following PRISMA guidelines. Of 40 articles identified, only three met the inclusion criteria, consisting of publications from the last 10 years indexed by SINTA and SCOPUS. The review included 350 geriatric patients, most aged ≥60 years and predominantly female. The main chronic conditions were hypertension and diabetes mellitus, with Lisni et al. (2025) specifically reporting type 2 diabetes. Polypharmacy was strongly associated with drug interactions, where moderate-severity combinations such as glimepiride-metformin (18.6%) were most frequent, and pioglitazone-clopidogrel was categorized as high-severity. These interactions mainly increased the risk of hypoglycemia, while others reduced antidiabetic effectiveness. Utaminingrum et al. (2015) emphasized polypharmacy in the broader context of irrational drug use, including self-medication and antibiotic overuse. Factors linked to polypharmacy included the number of prescribed drugs and education level, while age and gender showed no significant associations.

Abstract

Multidrug-resistant tuberculosis (MDR-TB) remains a major challenge in global tuberculosis control due to low treatment success rates, long therapy durations, and a high risk of adverse effects. The development of new therapy regimens and additional strategies continues to be evaluated to improve treatment efficacy and safety. This systematic literature review was conducted by examining four randomized controlled trials published between 2015 and 2025, namely the studies by Tang et al., Zhang et al., Ruan et al., and Nyang’wa et al. Articles were selected based on their relevance to the topic of efficacy and adverse effects of MDR-TB therapy regimens. Extracted data included culture conversion rates, treatment success, recovery time, and safety profiles. This review shows that innovations in MDR-TB therapy regimens, whether through the addition of new drugs, the use of adjuvant herbal therapies, or the development of short-course oral regimens—can enhance efficacy while improving treatment safety profiles. These findings support the implementation of new regimens as more effective and safer therapeutic alternatives, with the potential to improve patient adherence in MDR-TB management.

Abstract

Anemia in pregnancy remains a major public health issue in Indonesia, contributing to maternal and fetal complications. This study aims to assess the effectiveness of iron supplementation in preventing anemia among pregnant women through a systematic literature review using PRISMA guidelines. A total of 12 eligible articles published from 2016 to 2025 were analyzed, focusing on the Indonesian population. The findings consistently demonstrate that iron and folic acid supplementation significantly increases hemoglobin levels and reduces anemia prevalence in pregnant women. However, adherence to supplementation is a persistent challenge. Only 53.4% of pregnant women consumed iron tablets for at least 90 days, with adherence influenced by maternal education, socioeconomic status, access to healthcare, and family support. While most studies confirmed the biological effectiveness of iron supplementation, limitations such as small sample sizes, limited geographic coverage, lack of control groups, and short-term observation periods were noted. To optimize outcomes, a multidimensional approach involving health education, improved healthcare access, and community support is essential. Strengthening adherence and monitoring mechanisms can enhance the long-term impact of iron supplementation in reducing maternal anemia in Indonesia.

Abstract

Psychotropic drugs are essential in the management of mental disorders, yet their use often raises challenges related to rational prescribing, cost, and drug availability. Since the implementation of the Indonesian National Health Insurance (JKN) in 2014, changes in the utilization patterns of psychotropic medications have been observed and require systematic evaluation. This study aimed to review the prescribing patterns of psychotropic drugs in the JKN era, focusing on the most frequently used agents, therapeutic shifts, and their implications for healthcare access and financing. A systematic literature review was conducted using databases including Neliti, Garuda, ResearchGate, and Google Scholar. The search used the keywords “psychotropic,” “JKN,” “BPJS,” and “drug utilization.” Inclusion criteria were observational studies conducted in the Indonesian population between 2014 and 2025. From 124 identified publications, three articles met the inclusion criteria and were analyzed. The studies revealed a shift in the use of antipsychotics from typical to atypical agents, with risperidone as the most frequently prescribed. For antidepressants, selective serotonin reuptake inhibitors (SSRIs), particularly fluoxetine and sertraline, were commonly used, although amitriptyline remained an alternative due to limited drug availability. JKN contributed to improved patient access to treatment but was also associated with cost burden, unequal distribution of medicines, and risks of irrational prescribing. JKN has successfully expanded patient access to psychotropic therapy; however, it also introduced financial and prescribing challenges. Among mental health conditions, bipolar disorder showed a significant difference in annual costs between JKN and non-JKN patients, with higher costs under JKN mainly due to the high utilization of sodium divalproex.

Abstract

This study aimed to characterize the secondary metabolite content of Pulai (Alstonia scholaris (L.) R. Br) stem bark through preliminary phytochemical screening of its methanol extract obtained using ultrasound-assisted extraction (UAE). The research is significant as it provides basic phytochemical information to support the ethnomedicinal use of Alstonia scholaris and to explore its potential as a natural source of bioactive compounds for future phytopharmaceutical development. Dried stem bark from Central Borneo was extracted with methanol using UAE at room temperature with a simplicia-to-solvent ratio of 1:5 and three remaceration cycles, yielding 5.29% w/w crude extract with a yellowish-black appearance. Phytochemical screening was carried out using specific qualitative tests for alkaloids (Mayer, Dragendorff, Bouchardat), flavonoids (Shinoda), phenolics (FeCl₃), saponins (foam test), and steroids/terpenoids (Liebermann–Burchard). The methanol extract tested positive for alkaloids and terpenoids, while flavonoids, saponins, and steroids were not detected. These findings indicate that alkaloids and terpenoids are the dominant secondary metabolites in the methanol extract of Alstonia scholaris stem bark obtained by UAE, and provide a foundational profile for further studies on its biological activities.
