Abstract

Catamenial pneumothorax is a pneumothorax that occurs repeatedly and spontaneously in women, recur within 24 to 72 hours before or after menstrual onset. Catamenial pneumothorax is one of the symptoms of Thoracic Endometriosis Syndrome (TES). This pneumothorax is rare with an incidence of 3-6% in women aged 30-35 years. Pneumothorax occurrences often associated with endometriosis. Pathomechanics of pneumothorax associated with endometriosis is discussed in several literature in four mechanism, that is physiological theory, transition, metastasis-microembolism, and through the diaphragm pathway. The typical clinical cause of pneumothorax is related to menstrual time, usually associated with chest pain that most often occurs (90%), followed by dyspnoea (31%) and cough (rarely). Catamenial pneumothorax in almost all cases occur unilateral on the right side. This case is a 30-year-old woman with a catamenial pneumothorax with endometriosis. The case report explained the relationship between endometriosis and the incidence of catamenial pneumothorax. In this case, clinical characteristic of pneumothorax in patients occurs when entering or in the menstrual cycle, and there is clinical improvement after the management according to catamenial pneumothorax. Therapy given in the case consists of pharmacological therapy to reduce tightness, insertion of water sealed drainage (WSD), and minimally surgical technique namely video-assisted thoracoscopic surgery (VATS).

Abstract

From World Health Organization (WHO) in 2016 asserted that in 2015 around 23 million or about 54% people deaths caused of Congestive Heart Failure (CHF). Based on data from the Lampung Provincial Health Office in 2016 with inpatient data on heart failure clients increased with a percentage of 42.6%, mostly in old age occurred 50% (60-70years). Implementation of family doctor services to patients by identifying risk factors, clinical problems, and management with a patient centered approach and family approach. The study conducted was Case Report. Primary data were obtained through history taking and physical examination by making home visits, filling family folders, and filling patient files. The assessment is based on a quantitative and qualitative holistic initial diagnosis, process, and end of visit. Based on a holistic diagnosis, the patient suffers from heart failure with grade II hypertension. Complaints felt by the patient are heartburn, chest palpitations, weakness, dizziness, decreased appetite, and fatigue. Based on social functions, patients have limitations in heavy activities. The intervention was carried out non-medically through education and counseling with a family approach and medically by giving Digoxin, Furosemide, Bisoprolol dan Clopidogrel (CPG). During the visit during the control, the patient's concern about heart disease and hypertension they experienced was reduced and the patient understood the disease. Heart failure accompanied by hypertension occurs mostly in old age. The diagnosis of CHF accompanied by Grade II Hypertension in this case is consistent with the theory and critical review of recent research. There are internal and external risk factors triggering the emergence of CHF. This is stated by several theories which are a source of reference. The role of the family is important in the care and treatment of sick family members.

Abstract

The Pentavalent (DPT-HB-Hib) immunization is one of advanced immunization programmed by the goverment to prevent diseases such as diphteria, pertussis, tetanus, B-type hepatitis, and B-type Haemophilus influenzae. The lowest advanced pentavalent immunization coverage in Bandar Lampung is located at Labuhan Ratu Health Center (26,9%). Several factors are related to pentavalent immunization administration such as mother’s knowledge, mother’s attitude, mother’s job status, affordability of health services, and the role of health workers. Purpose of this research is to find out the factors related to advanced pentavalent immunization in the working area of Labuhan Ratu Public Health Bandar Lampung. This research used analytic observational method with cross-sectional approach. The sample of the research is 42 respondents in which live at work area of Labuhan Ratu Public Health Bandar Lampung city through purposive sampling method. The result of univariate analysis was 42,9% of respondents had complete advanced Pentavalent immunization status and the other 64,3% respondents had incomplete status. The result of bivariate analysis showed some variables had a correlation with advanced pentavalent immunization status such as mother’s knowledge (p-value 0,029), mother’s attitude (p-value 0,022) and mother’s job status (p value 0,014). Meanwhile, some variables had no correlation with advanced pentavalent immunization status such as mother’s education (p-value 0,384), affordability to health services (p-value 0,344) and the role of health workers (p-value 0,571). The conclusion is mother’s knowledge, mother’s attitude, and mother’s job status are correlated to advanced Pentavalent immunization in the working area of labuhan ratu public health Bandar Lampung city 2018.

Abstract

The high prevalence of scabies can be caused by poverty, low levels of education, poor hygiene, availability of clean water and density of residents. Diagnosis and treatment are carried out with a holistic and comprehensive family medicine approach. Primary data were obtained through history taking, physical examination and home visits, and secondary data from the patient's medical records. Patient's case a 51-year-old has suffered from scabies (ICD 10 - B86) that has recurred three months ago, but the patient still has a functional degree of 1 in carrying out daily activities. Internal factors in patients are poor personal hygiene, towels and bed linen do not wash regularly even though it has been used other than by family members, as well as curative treatment patterns. External factors in patients are low family income (ICD10-Z59.6), lack of home lighting and ventilation, densely populated home environments, lack of cleanliness in the home environment, and low patient and family knowledge about scabies. The holistic and comprehensive eradication approach uses a family medicine approach in the form of education about the causes, transmission, treatment and prevention of diseases. Scabies disease is difficult to eradicate if it is not accompanied by changes in clean and healthy lifestyle.

Abstract

The World Health Organization (WHO) was informed on December 2019 about a coronavirus pneumonia outbreak in Wuhan, Hubei province (China). Subsequently, on March 12, 2020, 125,048 cases and 4,614 deaths were reported. Coronavirus is an enveloped RNA virus, from the genus Betacoronavirus, that is distributed in birds, humans, and other mammals. WHO has named the novel coronavirus disease as COVID-19. Currently, there is no vaccine or specific antiviral drug for this deadly disease. More than 80 clinical trials have been launched to test coronavirus treatment, including some drug repurposing or repositioning for COVID-19. Viral infections caused by coronavirus could evoke cytokine expression leads to activation of pulmonary capillary endothelial cells, neutrophil infiltration and increased oxidative stress. Vitamin C, an important component of cellular antioxidant systems, is beneficial for critical care management. Vitamin C can also kill and prevent viral replication. Several studies have shown the administration of vitamin C orally and intravenously can reduce the increased risk of complications, reduce the severity, overcome symptoms and improve the prognosis of patients with COVID-19.

Abstract

People from the lower middle class have consume used cooking oil patterns to save costs but can cause liver damage. Used cooking oil contains free radicals which cause liver damage. Black garlic can be a liver protective because it contains high antioxidant substances that can capture free radicals and prevent oxidative reactions. Flavonoids have a role to capture free radicals to stabilize fat peroxy radicals. Flavonoids have the ability to modulate cell signaling pathways that can regulate various cell processes, for example in growth, proliferation, and apoptosis. Black garlic has hepatoprotective potential to block the effect of tert-Butyl Hydroperoxide (t-BHP) oxidant on cell death, lipid peroxidation, oxidative stress, and inflammation in rat hepatocytes and can be an excellent natural candidate in the development of adjuvant therapies and healthy food for liver protection from damage due to the consumption of used cooking oil.

Abstract

Chronic obstructive pulmonary disease (COPD) is a slow progressive and irreversible or partially irreversible lung disease. Risk factors for COPD include exposure to cigarette smoke, air pollution, exposure to dust and smoke in the work environment. The goal of COPD management is to reduce symptoms, prevent acute exacerbations and decreased lung function. This study is a case report with data obtained through history taking (autoanamnesis and alloanamnesis), physical examination and home visits to complete family data, psychosocial and environmental data. Patient Mr. J 49-year-old diagnosed with COPD. Patients have aspects of internal risk in the form of a history of smoking for 16 years, lack of patient knowledge about the illness, feeling bored and stressed out of not working. The external risk aspects are in the form of lack of family knowledge about the disease and its complications as well as curative treatment patterns, a history of vehicle smoke exposure at work for 27 years. Home visits were done three times to intervene in the form of education and counseling in patients and families about the causes, risk factors, clinical symptoms, prognosis, to prevent complications. In the evaluation, it was found that the patient had shown improvement. The diagnosis and management of these patients has been done holistically, patient centered, family approached, community oriented and based on several theories and current research. In the process of behavior change, Mr. J has reached the trial stage for COPD diet and physical exercise after being given an intervention.

Abstract

Pneumothorax is a collection of air and gas contained in the pleural cavity. Pneumothorax is divided into spontaneous (primary and secondary) and traumatic pneumothorax. Secondary spontaneous pneumothorax can occur preceded by underlying disease,as in the case of pulmonary tuberculosis (Tb). The most common manifestations in this case, such as dypsnoe, chest pain, coughing, asymmetrical chest wall, and hypersonor. Case report, a 75 years old man was taken to the hospital complained of dypsnea since 3 weeks before admission, accompanied by coughing with green sputum. The patient complained fever and sweating at night 2 weeks before. Physical examination showed asymmetrical chest movement and shape, right fremitus tactile is lagging, hypersonor in the right hemithorax. Complete blood examination showed leukocytes 15.300/uL and radiographic examination showed an avascular lucency shadow with white pleural line and infiltrates with fibrosis in both lungs, leading to secondary spontaneous pneumothorax ec primary lung tb. Patient were given non-medical therapy with oxygenation and insertion of chest tube, meanwhile medical therapy by administering antibiotic and analgesic.

Abstract

In 2016 Tanggamus Regency had 70 cases of infant mortality, under-fives with malnutrition 1,371 cases (3.31%), and 1 case of malnutrition, and the coverage of community participation in under-fives growth monitoring was 75.6% . However , uneven distribution of health workers and low knowledge of health cadres about measuring the nutritional status of children, so the coverage of nutritional status is still low. This study uses the quassy experimental method with the design of one group pretest-posttest design. The sampling technique is total sampling. The research was conducted from April to July 2018, located in Gunungtiga Village, Ulubelu District, Tanggamus Regency, Lampung. The subjects that were obtained were 18 Posyandu cadres. The result of the median value of the knowledge pre-test is 30, while the median value of the post-test knowledge is 80. The results of the bivariate analysis p = 0,000. There were significant differences between before and after training in measuring the nutritional status of children under five to the knowledge of Posyandu cadres in Gunungtiga Village.

Abstract

Morbus Hansen (MH) is a chronicinfectiousdisease caused by Mycobacterium leprae. Morbus Hansen's disease is a national public health problem in Indonesia. The cure rate in Lampung in 2014 is about 83.3% for PB and 60.1% for MB. The cure rate has not yet reached the target release from treatment, which is >90%. Low achievement of MH cure rate can be caused by patients who’re not regularly taking the medication. The lack of MH cure rate can be caused byseveralfactors, which are externalamd internal factors. This study aimed to determine the internal factors that associated with therapy success of Morbus Hansen's disease in Bandar Lampung. This study is an observational study with cross sectional design.The sample in this study isMH patients who take medication to Puskesmas in Bandar Lampung region, about 42 patients. Data then analyzed using chi-square test. The results showed that out of 42 respondents, the success of MH therapy is bigger on the child's age, female gender, higher education, not working, medication adherence, good knowledge and good socio-economic. From the bivariate analysis, it’s known that there is influence of age (p=0,017), knowledge (p=0,030) and socio-economic on the successful of therapy (p=0.002), while gender, education and employment do not affect the success of the therapy (p = 0.190; p = 0.064; p = 0.355). From six internal factors analyzed in this studythere are only age, level of knowledge and socio-economic thatassociated to the successful therapyof patients withMorbus Hansen's disease.
