Abstract

Beta thalassemia occurs a decreasing of beta chain synthesis. In individuals with Beta Thalassemia, there can be a total loss of the β globin chain (Thalassemia Major) or a partial loss of the β globin chain (Thalassemia Minor). In 2019, there are more than 10,531 Thalassemia patients in Indonesia, and estimated 2,500 newborns with Thalassemia every year in Indonesia. The study was case report, Patient An. N, 8 years old, has been diagnosed with Thalassemia and has fears of growing complaints and does not improve because of late blood transfusion. Limited knowledge about the patient's health condition such as risk factors of thalassemia, complications of thalassemia, prevention of thalassemia, food that need to be avoided, also lack of motivation and encouragement for families to bring their children for blood transfusion routinely are factors that cause problems that occur in patients and families. Holistic and comprehensive management of An. N’s problems has been done by providing counseling to improve the patient's family knowledge.

Abstract

Gamma-Glutamil Transferase (GGT) enzyme is a widely distributed transpeptidase in various body tissues. This enzyme is located in cell membranes and cytoplasm of hepatocytes. In addition, the expressions of the GGT enzyme is found in the kidneys, lungs, pancreas, blood vessel endothelium, nerve cells, and blood plasma. It has been used routinely in clinic to diagnose liver damage caused by alcoholism. The normal value of plasma GGT activity is 0–40 U / l. Normally, the GGT activity in blood plasma is low, but liver damage or bile ducts obstruction will increase it. There were significant association between the GGT activities in plasma and various diseases such as cardiovascular disease, metabolic syndrome such as diabetes mellitus and obesity, neurological and neurodegenerative diseases and others various diseases. Related to the measurement of GGT enzyme activity is cheap and easy to do, it's very potential to be developed for marker to diagnosis, prognosis, and further evaluation of various diseases. Further studies are needed to apply it for to be an accurate biomarker for various diseases

Abstract

The development and use of contact lenses is increasing rapidly, both in developed and developing countries. In line with that many contact lens users are one of the names of infections in the eye. One of the causes is contamination of contact lens solution. To identify the types of microorganisms found in contact lens solutions in students of the Faculty of Medicine, University of Lampung in 2018. This study used a descriptive observational research type. The study was conducted at the Faculty of Medicine, University of Lampung in June 2018. Samples were contact lens solution, with a total sample of 30 samples. The sample was taken to the Laboratory of Microbiology, Faculty of Medicine, University of Lampung. Samples were identified by bacterial culture, gram staining and biochemical tests. Identification of B. subtilis (22%), Pseudomonas aeruginosa (11%), Staphylococcus aureus (8%), Serratia sp (7%), Klebsiella sp (7%), E.coli (2%) . Of the 60 samples studied, it was found that there were no bacterial growths in 26 samples (43%), and there was bacterial growth in 34 samples (57%)

Abstract

Coronavirus disease 2019 (COVID-19) is the first viral outbreak pneumonia in Wuhan, China, in December 2019, caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-Cov-2). WHO reported 2.241.359 positive cases of COVID-19 patients worldwide, with a cumulative death toll of 152.551 men by April 19, 2020. As in 18 April 2020, Indonesia itself hit 6.248 positive cases with a total of 535 deaths, according to the Indonesian Health Ministry. SARS-Cov-2 is a coronaviral (β) beta group virus. In order to enter the target cell, SARS-CoV 2 uses the ACE Angiotensin conversion enzyme (ACE) 2. This can reduce the function of ACE2 which changes Angiotensin 2 into Angiotensin (1-7). The disrupted production of Angiotensin (1-7) can cause a vasodilator effect that is not optimal, so the condition of people with hypertension can be worsen. The risk of SARS-Cov-2 infection is higher among those suffering from hypertension with the drug Renin-Angiotensin-Aldosterone System inhibitor, due to the expression of ACE2. Overexpression of ACE2 may make it easier for SARS-CoV-2 to penetrate the body.

Abstract

Indonesia ranks third in most cases of tuberculosis (TB) in the world. This is due to the low knowledge about TB and the low level of medication taking. Therefore, the role of family doctor is needed to manage patients holistically from various aspects. This study is a descriptive study of case reports. Primary data were obtained through alloanamnesis, physical examination and home visits. Secondary data were obtained from the patient's medical record. Patient Mr. A, 21 years old, came for routine TB treatment and has been diagnosed two months ago. The patient has poor nutritional status. Problems needed by patients are lack of physical activity, lack of patient knowledge of TB, lack of family support and smoking habits. Regarding pharmacological and non-pharmacological intervention in patients is education about the disease for patients and families. The evaluation results of the intervention are that patients who have begun physical and family activities have supported for routine treatment. But patient’s family still have the habit of smoking indoors. Holistic management of TB patients is needed. The patient's level of knowledge about the disease and the importance of the family knowing the schedule for treatment and a healthy lifestyle determine the success of TB treatment. Patient and Family took doctor’s advice well after intervention, but lifestyle changes such as smoking habit haven’t done optimally

Abstract

Diabetes mellitus (DM) is a metabolic disease that occurs due to abnormal insulin secretion, abnormal working insulin or both. One of the risk factors for DM is insulin resistance, which is the inability of insulin to perform the function of uptake and glucose utilization. The second most common cause of type 2 diabetes is dyslipidemia, or which is called diabetes dyslipidemia. In dyslipidemia there is an increase or decrease in plasma lipid fractions. Lipid fractions that play a role in the pathogenesis of DM are triglycerides (TG) and high density lipoprotein (HDL). Triglycerides are lipid fractions consisting of three fatty acid molecules, which are produced from free fatty acids (FFA) in the liver and some in the intestine. High-density lipoproteins are complex metabolic systems that has function as macrophages to remove cholesterol from artery walls. One examination that can be an indicator of insulin resistance is the ratio of triglycerides and high density lipoprotein cholesterole (HDL-C) levels. Measurements were made by dividing the concentration of TG with HDL-C, with the TG/HDL-C ratio having a risk is more than 3. The relationship between the TG/HDL-C ratio and the incidence of Type 2 DM is estimated to be due to β cell dysfunction. The TG/HDL-C ratio is used as a marker of β cell lipotoxicity which results in a decrease in insulin secretion and an increase in β cell apoptosis due to increased triglyceride concentrations. Hypertriglyceridemia and low levels of HDL indicate the occurrence of diabetes dyslipidemia.

Abstract

Cataracts are the cause of blindness which accounts for approximately half of all blindness in the world, especially in developing countries. This is because in developing countries the level of knowledge related to this disease is still low. Awareness of the importance of checking eye health regularly is still low. From this figure, cataract is the highest cause of blindness, about 81 percent. The method used is case report. Primary data were obtained through autoanamnesis and alloanamnesis, physical examination and home visits, to complement family data, psychosocial and environmental data. Assessment is carried out based on a holistic diagnosis from the beginning, process, and end of the study qualitatively. Based on a holistic diagnosis, it was found that an 88-year-old patient complained of wanting to check the condition of his right and left eyes because there was foggy vision in both eyes, the right eye looked more foggy than the left. On eye localis status, eye examination found: The lens of the right eye is cloudy, mature, with a shadow test - with 6/20 vision, and the left eye is immature cloudy with a shadow test + with a vision of 1/60. Internal risk factors in the form of lack of sufficient knowledge of the disease he suffered. The evaluation results showed a strong desire to perform cataract surgery and awareness of the importance of therapy for hypertension. Cases of elderly patients in the form of cataracts and hypertension. Selecting the patient's daughter-in-law as PMO and patient companion. Mrs. M has dared to perform cataract surgery. The patient's management, namely lens extraction, was appropriate. There has been a change in knowledge of Mrs. M, Mr. H, Mrs. U, and Mr. S regarding the disease suffered by the patient and how to respond to it because of the counseling carried out by the researcher.

Abstract

Bacterial vaginosis is a disease that is a public health problem, especially in women, both non-pregnant and pregnant women. Bacterial vaginosis is caused by an excess and overgrowth of anaerobic bacteria such as Gardnerella vaginalis, Ureaplasma urealyticum, Mycoplasma hominis, Mobiluncus sp., Prevotella sp., and other anaerobic bacteria. Bacterial vaginosis is characterized by the presence of an itchy greyish white discharge with a fishy odor, an increase in the pH of the vaginal fluid and is found clue cells on Whiff's test using KOH10%. Management of bacterial vaginosis is in the form of antibiotics with the drug of choice being metronidazole 500 mg orally twice a day for 7 days. In pregnant women, bacterial vaginosis has been reported to increase the incidence of abortion, premature rupture of membranes, preterm labour, low birth weight, and intrauterine infection. Therefore, early diagnosis and appropriate management of bacterial vaginosis can reduce maternal and infant morbidity and mortality.

Abstract

Students have dense activities so they often forget to do exercises that cause declining body health. Jump rope and jogging exercises which are types of aerobic exercise are practical exercises that can improve the functional performance of the body especially the functional capacity of the pulmonary. The purpose of this aerobic exercise research is to determine differences in the functional capacity of the pulmonary of students who did jogging and jump rope. RCT research with 35 students of the Faculty of Medicine, University of Lampung which was divided into a control group and an intervention group. The intervention group had a jogging exercise dose with requency 2 times a week, intensity 40%-90% Hearth Rate Reserve (HRR), time 30 minutes, aerobic type and jump rope exercise dose with frequency 4 times a week, intensity 70%-75% Maximum Hearth Rate (MHR), time 5 minutes, type aerobics. The mean VO2 Max value of the exercise group was higher than the control group and had significant differences. The mean VO2 Max jump rope group was higher than the jogging group but there was no significant difference between them. There was no significant difference between the jump rope and jogging exercise groups but there were significant differences between the exercise training groups and the control group so that jump rope and jogging exercises were considered to increase VO2 Max.

Abstract

Osteosarcoma is a musculoskeletal malignancy that often occurs in pediatric patients requiring multidisciplinary management. Management in the form of amputation surgery (limb-salvage surgery) with a combination of chemotherapy or radiotherapy. Post-amputation complications that are often experienced by patient is postsurgical pain. Persistent Postsurgical Pain (PPSP) in pediatric patients is influenced by risk factors such as age, gender, preoperative preparation (pain intensity, anxiety, pain efficacy), intraoperative measures (site of surgery, potential for nerve damage, administration of analgesia regimens, technique of anesthesia), and postoperative management. The incidence of PPSP in osteosarcoma postamputation patients is related to Phantom Limb Syndrome with varying incidence and duration. This syndrome is a phenomenon that often occurs after post-amputation procedures in the form of sensation (Phantom Limb Sensation) or pain (Phantom Limb Pain) due to loss of part of the body. Phantom Limb Pain (PLP) is an effect and complication that occurs after amputation surgery in pediatric osteosarcoma patients. The PLP pathomechanism is related to the interaction of the Central Nervous System (Cortical Remapping Theory and Proprioceptive Theory) and the Peripheral Nervous System (Dorsal Root Ganglia and nocireceptor stimulation). Management of PLP consists of neuropathic pain medications such as antidepressants, opioids, anticonvulsants, N-methyl-D-aspartate (NMDA) receptor antagonists and other anesthetic agents.
