Abstract

Children with Autism Spectrum Disorder (ASD) face unique challenges in building attachment relationships, due to ASD children experiencing impairments in communication, social interaction, and emotional regulation. This study aims to identify effective intervention strategies to improve the quality of attachment between parents and children with ASD through a Systematic Literature Review (SLR) approach. The review findings indicate that several interventions have proven effective in enhancing attachment between parents or primary caregivers and children with ASD, including the following interventions: DIR/Floortime, Focused Playtime Intervention (FPI), Parent-Child Interaction Therapy (PCIT), Circle of Security (COS), Positive Behavior Support (PBS), play training, massage therapy, and psychoeducation. The results of these interventions show that changes occur before and after the intervention, including strengthening two-way communication, increasing parental sensitivity, and improving the child's emotional responses. These findings have practical implications for parents, educators, and therapists in designing attachment-based approaches to support the social and emotional development of children with ASD. Responsive and supportive designing attachment-based approaches to support the social and emotional development of children with ASD. Responsive and supportive relationship-based interventions can build secure attachments between children with ASD and their parents and serve as an important reference in attachment-based parenting and therapy practices.

Abstract

Providing passive ROM to the extremities can help improve blood circulation, improve respiratory function, and support the recovery of consciousness as assessed using the Aldrete Score. The purpose of this study was to determine the effect of providing early passive ROM mobilization of the extremities on changes in the Aldrete score in patients after general anesthesia. This study used a pre-experimental design with a one-group pretest-posttest approach. The study sample consisted of 57 respondents. The intervention provided was early passive ROM mobilization. The Aldrete score was measured before and after the intervention. Data analysis used the Wilcoxon signed rank test with a significance level of α = 0.05. The results showed that the average Aldrete score before the intervention was 5.89 and after the intervention increased to 9.26. The results of the statistical test showed a p value = 0.0001 (p <0.05), which means that there is a significant effect of providing early passive ROM mobilization of the extremities on increasing the Aldrete score after general anesthesia. From this study, it can be concluded that there is an effect of providing early passive ROM mobilization of the extremities on increasing the Aldrete score after general anesthesia.

Abstract

Industrial dust exposure is a hazardous work environment that can potentially cause respiratory problems in workers, particularly in industrial sectors with production activities that generate high levels of dust particles. This study aims to examine the relationship between industrial dust exposure and respiratory problems in workers through a systematic review. The method used was a qualitative descriptive literature study of scientific articles from 2021-2026 obtained through Google Scholar based on inclusion and exclusion criteria, then systematically analyzed. The results of the study indicate that most studies found a significant relationship between dust exposure and respiratory problems, especially in work environments with dust levels exceeding the threshold limit (TLV). Common symptoms include coughing, shortness of breath, phlegm production, and decreased lung function. In addition to dust levels, other factors such as duration of exposure, length of service, smoking habits, and the use of personal protective equipment (PPE) also play a role in increasing the risk. However, several studies showed insignificant results, which are suspected to be influenced by the use of proper PPE and an effective work environment control system. These findings confirm that dust exposure is a significant, multifactorial risk factor, necessitating comprehensive control efforts through a hierarchy of controls, environmental monitoring, and regular health checks to prevent long-term impacts on worker health.

Abstract

Background: Class II restoration according to the classification of Greene Vardiman Black is one of the procedures in dental conservation that often presents a challenge for operators or dentists during treatment. Class II restorations have their own level of difficulty, particularly during the placement of restorative materials into the cavity. One of the possible failures in Class II restorative treatment is marginal leakage, which can lead to microleakage at the restoration margins. Purpose: To report the success of class II restoration with composite resin using tofflmire matrix. Case report: A 23-year-old female patient came with a complaint of cavities, the complaint was felt since 1 year ago in the lower left back tooth, there was no pain and had never been treated. In the case, preparation and restoration were carried out using composite resin with isolation using tofflmire matrix, applied using an Incremental technique. Discussion: Class II posterior tooth restorations have challenges such as leakage due to the techniques and materials used. Composite resin is chosen because of its good adaptation, esthetics, and wear resistance. Incremental techniques are preferred because they reduce polymerization stress and provide optimal curing results. Tofflemire matrix is ​​used for stability and soft tissue protection.Conclusion: The selection of composite resin with an Incremental technique and the use of a Tofflemire matrix provides more optimal, aesthetic, and minimal risk of failure results for class II posterior tooth restorations.

Abstract

Atrophic rhinitis or ozena is a chronic infection of the nasal cavity characterized by progressive atrophy of the nasal mucosa and turbinate bones accompanied by the formation of foul-smelling nasal crusts. This condition is more commonly found in females, particularly during puberty, and is frequently associated with populations of low socioeconomic status and poor environmental sanitation. The exact etiology of atrophic rhinitis remains unclear; however, several factors are believed to contribute, including chronic infection by *Klebsiella ozaenae*, vitamin A and iron deficiency, chronic sinusitis, hormonal imbalance, collagen diseases, trauma, and genetic predisposition. Common clinical manifestations include nasal obstruction, thick greenish nasal discharge, crust formation, epistaxis, anosmia, and *foetor ex nasi*, which is often more noticeable to people around the patient. Anterior rhinoscopy typically reveals widened nasal cavities, turbinate atrophy, thin and dry nasal mucosa, as well as purulent secretions and greenish crusts. Diagnosis is established through history taking, ENT physical examination, and supporting investigations such as microbiological culture, histopathological examination, radiological imaging, and paranasal sinus CT-scan. Management may be conservative with antibiotics, nasal irrigation, nasal drops, vitamin A supplementation, and iron preparations, or surgical in severe cases or those unresponsive to conservative therapy.

Abstract

A Normocytic normochromic anemia is one of the most common types of anemia found in geriatric patients with chronic diseases or acute bleeding. Upper gastrointestinal bleeding (UGIB) may cause continuous blood loss resulting in decreased hemoglobin levels. This case report discusses a 72-year-old woman who presented to the Emergency Department of Cut Meutia Regional General Hospital with complaints of black stools for four days prior to admission accompanied by epigastric pain, nausea, weakness, and pallor. The patient had a history of long-term use of analgesic and anti-gout medications due to chronic joint pain, which was suspected to be a risk factor for UGIB. Physical examination revealed pale conjunctiva and tachycardia, while laboratory findings showed hemoglobin of 7.1 g/dL, hematocrit of 23.3%, MCV of 82 fL, MCH of 29 pg, and uric acid level of 11.56 mg/dL, leading to the diagnosis of normocytic normochromic anemia caused by UGIB accompanied by gout arthritis. The patient received intravenous fluid therapy, proton pump inhibitors, antifibrinolytic agents, gastroprotective therapy, packed red cell transfusion, and other supportive treatments. During hospitalization, clinical improvement was observed, including cessation of gastrointestinal bleeding, reduced epigastric pain, and increased hemoglobin level to 8.6 g/dL. This case highlights the importance of early diagnosis and comprehensive management in geriatric patients with anemia caused by UGIB and comorbid gout arthritis.

Abstract

Hypertension is one of the most common chronic diseases encountered in primary healthcare and may lead to serious complications if left uncontrolled. This case report aims to describe the management of a 48-year-old female patient with grade II hypertension through a family medicine approach at Nisam Primary Health Center, North Aceh Regency. The patient presented with intermittent headaches for one week, which worsened after physical activity and were accompanied by numbness in both legs. She had a five-year history of hypertension with poor adherence to antihypertensive medication. Physical examination revealed a blood pressure of 165/70 mmHg, leading to a diagnosis of grade II hypertension. Comprehensive management was conducted using patient-centered and family-centered approaches, including education regarding hypertension, lifestyle modification, DASH diet implementation, physical activity, medication adherence, and family support through home visits and family folders. The interventions were expected to improve blood pressure control, prevent complications, and enhance the patient’s quality of life. A family medicine approach plays an important role in comprehensive hypertension management at the primary healthcare level.

Abstract

Diabetes mellitus is a chronic metabolic disease characterized by inadequate insulin production or the development of insulin resistance, resulting in persistent hyperglycemia. One of the common complications of diabetes mellitus is diabetic ulcer, which develops due to microangiopathy and macroangiopathy that impair blood circulation and tissue healing. Diabetic ulcers can significantly reduce patients’ quality of life and increase the risk of infection, hospitalization, and amputation if not managed properly. This study aimed to identify, describe, and evaluate the characteristics of diabetic ulcers among patients treated at Cut Meutia Regional General Hospital, North Aceh Regency. The study employed a descriptive observational design with a cross-sectional approach involving 50 respondents selected through purposive sampling. Data were collected from patient records and observations of ulcer characteristics. The findings showed that most respondents (34%) had Meggitt-Wagner grade 1 diabetic ulcers. The majority of participants were male (58%) and aged between 45–59 years (66%). Half of the respondents had experienced diabetes mellitus for less than five years before ulcer development (50%). Regarding nutritional status, most respondents had a normal body mass index (30%). All respondents had uncontrolled HbA1c levels (100%), indicating poor glycemic control, while more than half reported a history of smoking (54%). These findings highlight the importance of glycemic control, smoking cessation, and regular foot care in preventing diabetic ulcers and reducing the risk of further complications among patients with diabetes mellitus.

Abstract

Edema is an excessive accumulation of fluid between the cells of body tissues. In patients with chronic kidney disease undergoing hemodialysis, the kidneys are unable to excrete metabolic waste products and toxins, which leads to edema. One non-pharmacological intervention that can be provided is a combination therapy of Ankle Pump Exercise and 30° leg elevation to reduce the degree of edema.This final nursing scientific paper used a case study design. The intervention was carried out on June 5th, June 9th, and June 12th, 2026, at the Hemodialysis Room of Bhayangkara Mohamad Hasan Hospital Palembang. After the Ankle Pump Exercise and 30° leg elevation therapy, the results of edema assessment were: Mr. Z had grade II edema with a depth of 3 mm and a duration of 10 seconds, Mr. G had grade I edema with a depth of 2 mm and a duration of 3 seconds, and Mr. D had grade II edema with a depth of 3 mm and a duration of 10 seconds. The application of EBP through Ankle Pump Exercise and 30° leg elevation therapy was effective in reducing the degree of edema in hemodialysis patients.

Abstract

Nutrition is crucial for children's growth and development. Protein Energy Deficiency (PEM) is a common nutritional problem among elementary school-aged children and can impact their health, growth, and cognitive development. The prevalence of PEM among toddlers in Indonesia reached 17.9% in 2013. Palm Mushrooms, which are rich in macronutrients, can be a solution to this problem. Therefore, efforts are needed to address PEM through the consumption of nutritious foods, such as tortilla chips supplemented with palm mushroom flour. The purpose of this study was to analyze the acceptability and nutritional content of tortilla chips supplemented with palm mushroom flour (Volvariella volvacea) as an alternative snack for school-aged children. This study used an experimental design with a Completely Randomized Design (CRD) method, involving three treatments and two replications. The results of the organoleptic test showed that formulation 2, namely tortilla chips supplemented with palm mushroom flour, received a significant value (p<0.05) for color, aroma, taste, and texture, indicating a significant effect of the addition of palm mushroom flour on tortilla chips. Laboratory and organoleptic tests showed significant values ​​(p<0.05) for moisture, ash, carbohydrate, and protein content, indicating a significant effect of the addition of palm mushroom flour. Future researchers are expected to test the shelf life of tortilla chips made from palm mushroom flour.
