Journal of Indonesian Specialized Nutrition (JISN) is an online journal act as the nutrition and scientific publication platform published by Ridwan Institute. Abstract

Iron is an important nutrient, required to support tissue oxygen delivery, cell growth and differentiation regulation, and energy metabolism. Body iron levels are mainly controlled by regulation of iron absorption in duodenum and proximal jejunum, allowing absorption to be accurately matched to unregulated losses. Since iron bioavailability often reduced, dietary iron absorption is controlled by cellular and systemic factors to ensure that overall body iron levels are maintained at adequate levels. A better understanding of the mechanism for iron absorption and factors influencing its absorption and bioavailability is important to avoid iron deficiency or iron overload. There are complex regulatory frameworks managing iron absorption, transportation, storage, and recycling. It is able to provide enough iron for critical body functions and react relatively quickly as iron demands increase, but mechanisms must also be in place to restrict iron absorption once the body is overwhelmed with iron. Several factors promote and impede iron absorption, such as phytate and ascorbic acid, respectively. The danger of iron deficiency for the world's population is of great significance, it is important to introduce effective strategies to tackle this issue through nutrition programs; food iron supplements; iron medication supplements; and probiotic, prebiotic, and symbiotic approaches.

Abstract

Indonesia has projected to accelerate stunting reduction to 14% by 2024. Multiform interventions and collaboration among institutions or organizations have already been made to tackle the underlying determinants of stunting. This paper review was to explore the traditional food that may potentially help to accelerate progress towards stunting reduction. The results of an expert meeting of the “Declaration of Consensus Nutrition and Hydration based on Traditional Foods” suggested developing an innovative strategy to support the Government of Indonesia in National Strategy stunting reduction. The essential of the first 1000 days of a child’s life particularly in feeding practice to infants aged 6 months with a required on rich macro and micronutrients, home-cooked and derived from traditional foods. Traditional foods have relatively acceptable and affordable with complete nutrients. Growing evidence shows the relationship between local foods and child nutrition status.

Abstract

The prevalence of food allergy in children is increasing globally, with cow's milk allergy beingthe most common in infants and increasing from 2% to 3% in developed countries. However, data oncow's milk allergy in infants is limited in Indonesia. This study used a mixed methods approach using anonline survey and in-depth interviews. The survey was conducted from September 2021 to March 2022.Qualitative analysis, all interviews were recorded and transcribed. Data were analyzed using simplethematic analysis. The results showed that a total of 93 pediatricians and 104 midwives participated inthe survey. However, among the 93 pediatricians, only 56 (60.2%) completed the survey. Meanwhile, 11midwives (10.6%) were excluded because they had no experience in handling CMA cases. The majorityof CMA cases occurred in children under two years old, and most were treated by pediatricians. In termsof diagnostic tests, 51.8% of pediatricians recommended all specific IgE tests, prick tests, and provocationtests to diagnose allergies. More than 80% (46/56) of pediatricians chose to avoid all types of cow's milkproducts in the mother and child's diet as the main treatment. Meanwhile, only 22.1% of midwives didthe same. Milk choice will differ depending on the severity of CMA. Soy milk formula is preferred bymidwives for patients with low-level and moderate to severe allergies. Meanwhile, pediatriciansrecommend extensive hydrolysate formula for low severity patients and amino acid-based formula formoderate to severe.

Abstract

Current guidelines recommend extensively hydrolyzed formulas (eHF) as the first choice for infants with mild to moderate cow’s milk allergy (CMA) symptoms. Different eHF producers may apply different technical processes that can influence the end characteristics of the products, the efficacy and effectiveness in CMA management. Thi s review aims to summarizes references and reports to indicate the importance factors that should be considered in selecting eHF available in the market for CMA treatment. The most important factor is the clinical evidence on tolerance level of the eHF formula above 90% among allergic children or the efficacy and effectiveness in CMA management. Other factors are the composition of protein molecular size in the end product and the long history of usage of the product in the population with positive feedback from more than 90% healthcare professionals that could indicate as the real-world evidence.

Abstract

The field of specialized nutrition plays an important role in improving people's health and quality of life. Indonesia has various nutritional problems that need to be addressed, including malnutrition, overnutrition, and other nutritional disorders. The need for journals as a publication platform in the field of specialty nutrition in Indonesia is very important. The purpose of this study was to assess the need for journals as a publication platform in the field of specialty nutrition in Indonesia. This research uses a qualitative method with a case study approach. The object of this research is the journal of Indonesian specialized nutrition (JISN). Data collection techniques in this study used literature study and interviews. The data that has been collected is then analyzed in three stages, namely data reduction, data presentation and conclusion drawing. The results showed that JISN facilitates authors, academics, students, and health professionals to share their scientific work in the field of community and clinical nutrition, as well as other related fields including epidemiology, community nutrition, family medicine, health economics, and other public health research. The journal is also a platform to provide learning and recommendations in the field of specialty nutrition and related data for the development of good health practices for the people of Indonesia and the world. These needs are the basis for journal managers to improve the quality and reach of journals in the field of specialty nutrition in Indonesia.

Abstract

Caesarean delivery is an important medical procedure, and it is important to understand how this surgery may affect the balance of the child's gut microbiota. This study aims to determine the impact of gut dysbiosis on growth and development for children with Caesarean section. The research method used in this study is a literature study method with a qualitative approach, namely data collection by understanding and studying theories from various literatures related to the research. The results showed that gut dysbiosis can lead to digestive disorders, impaired nutrient absorption, and impaired physical growth. The compromised composition of the gut microbiota leads to a weakened immune system that can make children susceptible to infections and diseases, hindering their overall growth. Thus, medical and nutritionist care is needed to overcome the negative effects of gut dysbiosis in children born by caesarean section.

Abstract

Cow's milk protein allergy is a common health problem among children. Children with cow's milk protein allergy cannot consume cow's milk or other dairy products because their bodies react to cow's milk proteins. In an effort to meet the nutritional needs of these children, soy formula is a commonly used option. The purpose of this study was to evaluate the safety of soy milk formula in children with cow's milk protein allergy. This study used qualitative research methods. The data collection technique was done by literature study. The data that has been collected was then analyzed using three stages namely data reduction, data presentation and conclusion drawing. The results showed that soy milk formula is generally safe to be given to children with cow's milk protein allergy. Soy milk formula does not cause significant allergic symptoms in children with cow's milk protein allergy. In addition, soy milk formula also fulfills the nutritional needs of children with cow's milk protein allergy. Soy formula contains all the essential nutrients that children need to grow and develop. Based on the results of the study, soy milk formula can be a safe and effective formula option for children with cow's milk protein allergy.

Abstract

Iron deficiency is a significant global health problem, especially among school children. Iron deficiency has a serious impact on children's health, including cognitive development. The aim of this study was to investigate the impact of iron deficiency on cognitive function of school-aged children. This study used a cross-sectional design. The study sample was school-aged children who met the inclusion criteria. The inclusion criteria were children aged 6-12 years, had hemoglobin <12 g/dL, and had no other chronic diseases. Exclusion criteria were children who were undergoing certain drug therapy, children who had other cognitive developmental disorders, and children who had a history of allergy to iron. Data were collected through interviews, physical examinations, and laboratory tests. Data were analyzed using descriptive statistical tests. The results showed that iron-deficient children had lower cognitive function than non-iron-deficient children. This is indicated by lower scores on cognitive tests, such as memory, attention, and learning ability tests. Therefore, it is important to prevent and treat iron deficiency in school-age children

Abstract

With the increasing participation of women in the workforce, the dynamics of motherhood, especially for mothers who breastfeed, have undergone significant changes. These changes have had considerable implications, prompting an exploration of the challenges and advantages women workers face to balance breastfeeding and their professional commitments. This comprehensive review delves into complex views on breastfeeding among working women. The main objective is to explain the benefits, barriers, and relevant strategies related to breastfeeding in the workplace, with the aim of providing insight into forming an effective support system. The benefits of breastfeeding while working, including decreased absenteeism, increased productivity, and decreased worker turnover rates, are powerful incentives. However, various challenges hinder this practice, including inadequate research, inadequate support on the part of employers, logistical problems such as distance between home and workplace, lack of lactation facilities in the workplace, and decreased milk production during working hours. The discussion emphasized the need for a strong lactation promotion model, consisting of seven essential components, tailored to strengthen breastfeeding support in the workplace environment. To facilitate successful breastfeeding for working mothers, it is recommended to provide options such as part-time work or early return for mothers with babies under six months, coupled with important company provisions such as dedicated lactation support staff, educational materials, and peer groups, which are crucial. The integration of occupational physicians in the proposed lactation promotion is an important step toward achieving high breastfeeding rates among female workers.

Abstract

Every year, there is an increase in the number of female workers in Indonesia, most of whom are of productive age. There are quite a few women who breastfeed. With the role of being both a mother and a worker, quite a few problems arise, one of which is the problem of exclusive breastfeeding as a working mother. Method: The literature search for this article review assignment was carried out during November 2023 by searching on the Google Scholar site and obtaining seven journals from publication years 2014 to 2022. From several journals that have been reviewed, it was found that working mothers who breastfeed have several challenges in providing exclusive breastfeeding, including internal, external, knowledge, and education factors. In terms of internal factors, it can be the stress experienced or the feelings the mother faces—external factors include lack of attention from the external environment. In terms of knowledge, many working mothers still do not understand the importance of breastfeeding while working. Meanwhile, in terms of education, it also influences exclusive breastfeeding among working mothers. Several factors and challenges working mothers encounter in providing exclusive breastfeeding can be prevented and overcome in several ways. This can be done by educating working mothers before and while breastfeeding. Moreover, it can also be used by companies where the mothers work to provide space and time for breastfeeding mothers who want to express breast milk at work.
