Abstract

Diarrhea is a disease caused by pathogenic microorganism infections, one of which is E.coli, which is a serious problem because it can cause death. One alternative treatment utilizes natural ingredients. This experimental study aims to test the effectiveness of pumpkin (C.moschata) extract processed using the steaming and infusion methods on the growth of E. coli bacteria. The yellow squash samples used were sourced from the Larangan market in Sidoarjo, with criteria of freshness and yellow color. The inhibition test was conducted using the Kirby-Bauer method, repeated three times, with extract concentrations of 25%, 50%, 75%, and 100%, a positive control of chloramphenicol, and a negative control of sterile distilled water. The results showed that all extracts exhibited increased antibacterial activity with increasing concentration, with the largest inhibition zone at a concentration of 100%, namely 12.55 mm for steamed pumpkin flesh and 11.61 mm for steamed pumpkin seeds.

Abstract

Background: Implant contraception is one of the Long-Acting Reversible Contraceptives (LARCs) with high effectiveness in preventing pregnancy. Despite its numerous advantages, the use of implants has not been optimal due to various factors, one of which is the subjective experiences of acceptors during use. An in-depth understanding of acceptors' experiences is crucial for improving family planning services and implant coverage in Indonesia. Objective: This study aims to identify and analyze the experiences of acceptors in using implant contraception through a review of various published qualitative phenomenological studies. Method: This study employed a literature review design with a qualitative phenomenological approach. Articles were searched through PubMed, Google Scholar, Garuda, and National Journal Portal databases using keywords such as "pengalaman akseptor implan", "kontrasepsi implan kualitatif", "implant contraception experience", and "phenomenology family planning". Inclusion criteria included journal articles published between 2020-2025, written in Indonesian or English, and discussing acceptors' implant experiences. A total of 23 articles met the inclusion criteria. Results: From the literature review, five main themes of implant acceptors' experiences were identified: (1) reasons for choosing implants, (2) experiences during the insertion process, (3) perceived side effects, (4) psychological and social responses, and (5) satisfaction and continuation of use. The most frequently reported side effects included menstrual cycle changes, weight gain, and headaches. Husband support and healthcare workers were key factors in implant use continuation. Conclusion: Acceptors' experiences in using implant contraception are influenced by physical side effects, social support, counseling quality, and access to healthcare services. Improving the quality of pre-insertion counseling and comprehensive management of side effects is urgently needed to increase implant use continuation.

Abstract

Tuberculosis (TB) is an infectious disease that remains a major global public health problem and may lead to various complications, one of which is secondary spontaneous pneumothorax (SSP). Pneumothorax in TB occurs as a result of extensive damage to the lung parenchyma and pleura, potentially becoming a life-threatening condition that requires prompt management. This article aims to report and discuss a case of secondary spontaneous pneumothorax caused by pulmonary tuberculosis. A 29-year-old male presented to the emergency department with acute worsening dyspnea accompanied by right-sided chest pain, hemoptysis, decreased appetite, and a history of previous TB treatment. Physical examination revealed signs of respiratory distress with asymmetrical right chest wall movement, hyperresonance on percussion, and decreased breath sounds on the right side. Chest radiography and thoracic CT scan demonstrated active pulmonary tuberculosis with right-sided pneumothorax. Xpert MTB-RIF assay detected Mycobacterium tuberculosis with rifampicin sensitivity. The patient was diagnosed with secondary spontaneous pneumothorax due to recurrent pulmonary tuberculosis and received emergency management with tube thoracostomy followed by anti-tuberculosis therapy according to national guidelines. This case highlights the importance of early recognition of pneumothorax as a serious complication of tuberculosis, particularly in patients with a prior history of TB, and emphasizes the need for prompt diagnosis and comprehensive management to reduce morbidity and mortality.

Abstract

Chronic Kidney Disease (CKD) may lead to various complications, including cognitive impairment. The causative factor is the toxic substances accumulation in the body, including serum ureum and creatinine. This study aimed to determine the relationship between serum ureum and creatinine levels with cognitive function in CKD patients. Samples were taken using a consecutive sampling technique on 46 CKD patients who were hemodialyzed at RSUP Dr. M. Djamil Padang. Cognitive function was assessed using the Montreal Cognitive Assessment Indonesian version (MoCA-Ina). Serum ureum and creatinine were measured with a spectrophotometer. Data were analyzed using the Chi-Square test. Results showed that CKD patients with cognitive impairment were more likely to be older than 50 years old (54.2%) and male (58.3%), had at least a high school education and above (70.8%), were accompanied by hypertension (75%), and without diabetes mellitus (91.7%). The median serum ureum and creatinine levels in patients with cognitive impairment were 124 mg/dL and 10.05 mg/dL. The relationship between serum ureum and creatinine levels with cognitive function had a 0.039 and 0.768 p-value. There is no relationship between the characteristics of the sample, including age, gender, education level, hypertension, and diabetes mellitus with cognitive function in CKD patients. There is a significant relationship between serum ureum levels and cognitive function, but a non-significant relationship between serum creatinine with the cognitive function of CKD patients.

Abstract

Patients with Chronic Kidney Disease (CKD) require hemodialysis due to a decrease kidney function. This hemodialysis procedure can have psychological impacts, one of which is depression. Suspected contributing factors include age, gender, education, duration of hemodialysis, comorbidities, and socioeconomic status. This observational analytical study utilized a cross-sectional design involving 47 CKD patients in the Hemodialysis Unit of Dr. M. Djamil Padang Hospital. The sample was selected using consecutive sampling. Depression screening was assessed with Patient Health Questionnaire (PHQ-9). Results indicate 61.7% of CKD patients experience depression, notably prevalent in individuals aged over 51 years (62%), females (55.17%), with lower education (51.1%), undergoing hemodialysis for over 6 months (75.85%), with hypertension comorbidity (86.2%), without diabetes mellitus comorbidity (96.55%), and with low socioeconomic status (75.86%). Chi-square analysis identified age (p=0.018) is a significant factor, while gender (p=0.432), education level (p=0.693), duration of hemodialysis (p=1.000), hypertension comorbidity (p=0.692), diabetes mellitus comorbidity (p=1.000), and socioeconomic status (p=0.455) are not significantly related. This study concluded that age is a significant factor related to the occurrence of depression in CKD patients, while gender, education level, duration of hemodialysis, comorbidities, and socioeconomic status do not have a significant relationship with the occurrence of depression in CKD patients.
