Abstract

Diabetes mellitus is a metabolic disease characterized by hyperglycemia. Several classifications of diabetes mellitus include type 1 diabetes, type 2 diabetes, gestational diabetes, and other types of diabetes. Type 2 diabetes is caused by insulin resistance. Hyperglycemia in type 2 diabetes patients causes cell and tissue damage through oxidative stress processes. One of the main biomarkers of oxidative stress levels that can be analyzed is malondialdehyde levels. To reduce ROS levels, a definitive therapy is needed, which is the administration of exogenous antioxidants, one of which is N-Acetylcysteine (NAC) therapy. NAC works by forming glutathione antioxidants, which aim to fulfill the body's natural antioxidant deficit in overcoming hyperglycemia conditions. This study was a true experimental research study with a pre-posttest research design. The sample used was 20 patient blood serum samples, which were measured for MDA levels before NAC administration. Then, NAC therapy was administered at a dose of 3X400 mg per day for 5 weeks, and MDA levels were measured after the therapy using the Thiobarbituric Acid Reactive Substance (TBARS) method. The results of the study showed that the mean MDA level after NAC administration and the mean MDA level before NAC administration were 2.76 ± 0.251 nmol/ml and 3.28 ± 0.427 nmol/ml, respectively. There was a significant decrease in MDA levels in the blood serum of type 2 diabetes patients with a value of p = 0.000 (p <0.5). Thus, it can be concluded that the use of NAC has an effect on reducing MDA levels in type 2 diabetes mellitus patients

Abstract

Objective: This study aims to determine differences in pathogenic bacterial contamination on door handles based on time, location, and room in the intensive care unit of Dr. M. Djamil Hospital; Methods: This type of research is observational analytic. Samples were taken in 5 intensive rooms with a total of 40 samples. After that, the number of colonies was calculated based on differences in sampling time, door handle location, and room; Results: The results showed that 87.5% of door handles in the intensive care unit of Dr. M. Djamil Hospital were contaminated with bacteria. The bacteria found were S.warneri, S.hom.hominis, A.viridans, S.vitulinus, and S.maltophilia; Conclusion: There is no significant difference in the number of bacterial colonies on the door handles of the intensive care unit of Dr. M. Djamil Hospital based on time, location, and room.

Abstract

Colorectal cancer is the third most common malignancy worldwide. Chronic inflammation, including intestinal protozoan infection, is linked to its risk. This systematic review examined the association between protozoan infection and colorectal cancer. Literature was searched in five databases (2014–2024), screened with PRISMA, and assessed using the JBI checklist. Thirty-two studies were reviewed. Protozoan infection incidence was generally higher in colorectal cancer than in non-cancer or other cancer groups. Blastocystis sp. was the most common species. Eighteen of 22 studies showed significant associations, particularly with Blastocystis sp., Cryptosporidium sp., Pentatrichomonas hominis, and Entamoeba histolytica. Factors involved included demographics, biology, environment, lifestyle, clinical history, and cancer stage. Most studies support a significant link. Opportunistic protozoan infections may worsen colorectal cancer progression, and chronic Cryptosporidium infection may induce preneoplastic lesions.

Abstract

Chronic Kidney Disease (CKD) is a condition where there is structural or functional damage to the kidneys for more than 3 months with or without being followed by a decrease in the estimated Glomerular Filtration Rate (eGFR) and is progressive and irreversible. Hypertension is the most common comorbidity found in patients with CKD which can increase the progression of kidney damage. Objective: To determine the relationship between the stage of CKD and hypertension and the distribution of the frequency of patients with CKD based on age, sex, eGFR value, and hypertension. Methods: This research was an observational analytic study with a cross-sectional design. The research was conducted at the Medical Record Installation of RSUP Dr. M. Djamil from December 2022 - March 2023 used a purposive sampling technique and obtained 33 samples. The research data was secondary data in the form of blood pressure and serum creatinine and then analyzed using the Chi-Square test with a degree of confidence of 95% and p-value <0,05. Results: This study showed that most cases of CKD occurred in the age group 41-65 years (54,5%) and mostly found in men (57,6%). In this study, stage 5 CKD was the most common (72,7%) and the majority of patients with CKD suffered from hypertension (69,7%). The results of data analysis obtained p = 0,090 (p> 0.05). Conclusion: There was no significant relationship between the stage of CKD and hypertension (P=0,090) in adult patients at Dr. M. Djamil Padang in 2020–2021.

Abstract

Diabetic Foot Ulcers are one of the most common, serious, and costly complications of type 2 diabetes mellitus. Diabetic Foot Ulcers have become more common in tandem with the rising prevalence of type 2 diabetes mellitus. Changes in many hematological parameters, such as Prothrombine Time and Activated Partial Thromboplastine Time, are influenced by the mechanism of diabetic ulcer occurrence. Understanding the various aspects of hematological parameters will help in the prevention and treatment of diabetic foot ulcers. This study aims to determine the comparison in the hematological profile in type 2 DM patients with and without diabetic foot ulcers at Dr. M. Djamil Padang hospital in 2019.  This type of research is an observational analytic study with a retrospective design using medical record data of patients at Dr. M. Jamil Padang hospital from August 2020 to March 2021. The sample consisted of the medical records of patients with type 2 diabetes mellitus with foot ulcers and without foot ulcers, respectively, as many as 60 subjects. Bivariate analysis using independent t-test or Mann Whitney test.  The results showed that the mean PT level of type 2 DM patients with diabetic foot ulcers was 12.8417 ± 2.36939, without diabetic foot ulcers 11.997 ± 1.8325 (p <0.001). The mean aPTT of patients with diabetic foot ulcers was 41.2733 ± 35.1686, without diabetic foot ulcers 32.912 ± 13.1263 (p <0.001).  This study concludes that there are significant in between type 2 DM patient with foot ulcer and without foot ulcers.

Abstract

HIV/AIDS has been a global health issue for many years. This chronic condition affects multiple aspects of life and acts as a significant stressor that reduces patients’ quality of life. People living with HIV/AIDS (PLWHA) often face limitations in daily activities, making it difficult to accept their identity and life circumstances. This study aimed to analyze the relationship between self-acceptance and quality of life among HIV/AIDS patients at the VCT Clinic of Dr. M. Djamil General Hospital in 2023. An analytical cross-sectional design was used with purposive sampling, involving 84 participants. Data collection took place from November 2022 to November 2023 using the USAQ and WHOQOL HIV-BREF questionnaires. Data were analyzed using Pearson correlation tests. Results showed a positive and significant correlation between self-acceptance and quality of life (p = 0.034; r = 0.231). Significant relationships were also found with the physical (p = 0.012; r = 0.272) and psychological (p = 0.014; r = 0.269) domains. In conclusion, self-acceptance is positively associated with overall quality of life, particularly in physical and psychological aspects, among HIV/AIDS patients.

Abstract

Prescription contain information about medication that used to treat the diagnosis and prevent complications from patient. A doctor should refer to the principles of rational therapy and National Formularies (FORNAS) in accordance with Government Regulation No. 12 of 2013. A correct prescription will minimize the occurrence of medication errors that could endanger the patient's life. From some studies, there are still many prescriptions that doesn’t pass the standards. This study aimed to analyze the quality of the doctor’s prescription writing for National Health Insurance (JKN) patients at health centers in Padang city. This observational study was performed in the three health centers in September 2023. Sampling was done by proportional stratified random technique, where we obtained 378 prescriptions. Prescriptions were analyzed for accuracy and completeness according to the WHO’s Guide to Good Prescribing. Prescription analysis showed that prescriptio were mostly complete (74%), superscriptio were all complete (100%), inscriptio were mostly complete (67%), signatura were mainly complete (99%), subscriptio were partially complete (58%), pro were nearly complete (96%) and 88% of prescription confirmed with the FORNAS.  It is concluded that improvements are needed in prescription writing and using drugs that are registered on FORNAS.

Abstract

Diet therapy is very important for people with diabetes mellitus, because by undergoing the right diet therapy, blood glucose levels and weight can remain normal. For people with DM, knowledge and attitudes about sugar diets, including the type, amount, and schedule, are the first steps in forming healthy behaviors in controlling blood sugar. This study aims to determine the relationship between knowledge and attitudes with adherence to a sugar diet in people with diabetes mellitus. The type of research used in this study is quantitative, with a cross-sectional study design. Data collection techniques used questionnaires. There were 35 respondents in this study. Data analysis used the Chi-Square test. The results of the study showed that there was a significant relationship between knowledge and attitude with diet compliance with a p-value of 0.012 (p<0.05) and p=0.004 (p<0.05). It is recommended that the Sawang Community Health Center provide education on diet compliance and good eating habits to people with diabetes mellitus.

Abstract

The main treatment for patients with diabetes mellitus is diet therapy. Diet therapy is very important for people with diabetes mellitus, because by undergoing the right diet therapy, blood glucose levels and body weight can remain normal. Adaptive coping mechanisms can result in a better quality of life and positive actions. This study aims to determine the relationship between coping mechanisms and diet compliance in people with DM. The type of research used in this study is quantitative, with a cross-sectional study design. Data collection techniques used questionnaires. There were 78 respondents in this study. Data analysis used the Chi-Square test. The results showed that the majority of respondents had adaptive coping mechanisms, namely 64 people (82.1%), and most respondents adhered to the DM diet, namely 80.8%. The Chi-Square test results showed that there was a significant relationship between coping mechanisms and diet adherence with a p-value of 0.000 (p<0.05). It is recommended that the Trumon Community Health Center provide education on diet adherence and good coping mechanisms to DM patients

Abstract

Global exclusive breastfeeding rates, despite an increase, have not risen significantly enough, with approximately 44% of infants aged 0-6 months worldwide receiving exclusive breastfeeding during the 2015-2020 period, compared to the WHO target of 50%. The low rate of exclusive breastfeeding will have an impact on the quality and life expectancy of future generations. Breast care has a positive effect on increasing breast milk production; therefore, breast care needs to be implemented by pregnant and postpartum mothers. There are various ways to improve breast care skills among pregnant and postpartum women, one of which is through demonstration. This study aims to determine and analyze the effectiveness of demonstration methods in increasing knowledge about breast care among pregnant women. This study used a quasi-experimental design. Data collection was conducted using questionnaires. There were 30 respondents in this study. The results showed that there was an increase in knowledge about breast care from before the demonstration with an average score of 5.8 to 8.37 after the demonstration. The Wilcoxon Signed Ranks Test showed that the p-value was 0.000 (p<0.05). It is recommended that health workers provide health education using the breast care demonstration method to increase the knowledge of pregnant women.
