Abstract

Background: Evidence regarding the cost-effectiveness of interventions for schizophrenia can support health policy decisions, particularly in developing country like Indonesia, where healthcare resources are limited. Therefore, this study seeks to identify the most optimal treatment package for schizophrenia in mental hospital Prof.Dr.Soerojo Magelang Central Java Indonesia. Methods: ACER/ICER analysis was used to evaluate the effectiveness of antipsychotic drug. This study is an observational research with retrospective data collection from patient medical records and financial data and conducted in September-October 2024, at the Soerojo Mental Hospital, Magelang. Patient data collected included: age, gender, length of treatment, antipsychotic classification and GAF (Global Assessment of Functioning) score. Intervention costs include of direct costs including: drug costs, laboratory costs, doctor consultation costs, administration costs, treatment room costs, facility costs, medical device costs and other costs. Results: The results showed that majority of the patients are male in the age of 19-59 years old. The average length of hospitalization of schizophrenia patients who used the combination of antipsychotic olanzapine-haloperidol  was 14 days with an average total cost of Rp 2.874.793, an ICER value of - Rp. 193,462 and the ACER value of Rp 28.748/day was lower than the average length of hospitalization in schizophrenia patients who used of another combination of antipsychotic. Conclusion: Based on the results of the study, the combination of olanzapine and haloperidol drugs is the most cost effective compared to other combination drug choices in hospital.

Abstract

Background: Adherence to antiretroviral (ARV) therapy is essential for maintaining viral suppression, preventing disease progression, and supporting the health of people living with HIV (PLHIV). However, quality of life is a multidimensional outcome that may be influenced not only by medication adherence but also by clinical, psychological, social, and environmental conditions. This study aimed to analyze the association between ARV medication adherence and quality of life among PLHIV receiving treatment at Lebdosari Primary Health Center, Semarang. Methods: This observational analytic study employed a cross-sectional design and was conducted at Lebdosari Primary Health Center from September to November 2025. A total of 65 PLHIV receiving ARV therapy were included using total sampling. Medication adherence was assessed using the Indonesian version of the MMAS-8, whereas quality of life was assessed using the Indonesian WHOQOL-HIV BREF. Data were analyzed descriptively and the association between medication adherence and quality of life was assessed using the Chi-square test, with statistical significance set at . Results: Most respondents had a moderate level of ARV medication adherence (49.2%), followed by high adherence (27.7%) and low adherence (23.1%). Regarding quality of life, 55.4% of respondents were categorized as having moderate quality of life and 44.6% as having high quality of life. The Chi-square analysis showed no statistically significant association between ARV medication adherence and quality of life ( ). Conclusion: No statistically significant association was observed between ARV medication adherence and quality of life among PLHIV at Lebdosari Primary Health Center. Further studies should consider clinical, psychological, social, and economic factors that may contribute to quality of life and should apply multivariable analysis to account for potential confounding factors.

Abstract

Background: Non-communicable diseases, such as hypertension and diabetes mellitus, represent a significant global health issue, leading to the establishment of the Chronic Disease Management Program (Prolanis). Long-term medication use among Prolanis patients can lead to treatment fatigue, which may decrease adherence to proper medication use, including the consideration of Beyond Use Date (BUD). Failure to adhere to BUD guidelines poses risks to medication stability, efficacy, and patient safety. This study aimed to determine the influence of BUD education, provided via leaflets, on the knowledge, attitude, and practice (KAP) of Prolanis patients at RSI Sultan Agung Semarang. Methods: This study employed a quasi-experimental design with a one-group pretest-posttest approach. A total of 40 respondents were selected using total sampling based on established inclusion and exclusion criteria. Data were collected using questionnaires to assess knowledge, attitude, and practice before and after the educational intervention. The data were analyzed using univariate, normality, and bivariate (Paired T-Test or Wilcoxon) tests to evaluate the impact of the intervention. Results: The study identified that Prolanis patients often lacked sufficient understanding regarding BUD compared to manufacturer-determined expiration dates. The educational intervention, delivered through leaflets and supported by presentations, aimed to improve patient awareness and behaviors regarding the storage, use, and disposal of medications. Conclusion: The provision of education through media, such as leaflets, is an effective strategy for pharmacists to enhance Prolanis patients' understanding of BUD, thereby promoting safer and more effective medication use in accordance with the principles of halalan thayyiban (safe and halal).

Abstract

Background: Improper pharmaceutical waste management can lead to various health and environmental problems, including drug misuse, poisoning, and environmental pollution. This study aims to determine the level of knowledge among outpatients at Prof. Dr. Soerojo Mental Hospital Magelang regarding pharmaceutical waste management. Methods: This study used an observational method with a cross-sectional approach using a questionnaire administered to 47 outpatients selected by purposive sampling at Prof. Dr. Soerejo Mental Hospital Magelang. Data were analyzed descriptively using percentages. Results: The results showed that all respondents (100%) knew that damaged or expired drugs should be discarded. However, knowledge regarding proper drug and packaging disposal methods was still low. A total of 93.61% of respondents stated that damaged drugs could be disposed of directly with their packaging, 57.44% stated that syrup drugs could be disposed of into drainage, and only 25.53% knew that tablets/capsules should be crushed before disposal. All respondents (100%) did not know that drug bottles should be destroyed before disposal. Based on the knowledge score, 74.47% of respondents had a low level of knowledge, 25.53% had a moderate level of knowledge, and none had a good level of knowledge. Conclusion: Outpatients at Prof. Dr. Soerejo Mental Hospital Magelang had a low level of knowledge regarding pharmaceutical waste management. Although all respondents knew that damaged or expired drugs should be discarded, most respondents did not understand the proper methods for disposing of drugs and their packaging. Continuous education through pharmacist counseling and the DAGUSIBU program is needed to improve patients' knowledge of proper pharmaceutical waste management.

Abstract

Background: Effective drug inventory management supported by management information systems improves data accuracy, inventory efficiency, drug availability, and the quality of pharmaceutical services. Subject: This study aims to analyze the effectiveness of a management information system on medication inventory management at Pharmacy A, Pharmacy B, Pharmacy C, and Pharmacy D in Pagar Alam City based on the Human, Organization, and Technology-Fit (HOT-Fit) model. Methods: The study employed a descriptive method with a cross-sectional approach. Data were collected by distributing a four-point Likert-scale questionnaire to 16 respondents, consisting of pharmacy owners, responsible pharmacists, and pharmaceutical technical staff. Descriptive analysis was conducted to examine the human, organizational, and technological aspects, as well as the benefits derived from the use of the management information system. Results: The results showed that the effectiveness of medication inventory management before the implementation of the management information system was 57.26%, classified as “ineffective,” and subsequently increased to 86.69% after the system was implemented, classified as “highly effective.” All dimensions of the HOT-Fit model also showed positive results, as evidenced by high user acceptance, adequate organizational support, system quality that meets operational needs, and tangible benefits in improving the efficiency of medication inventory management. Thus, the management information system has proven effective in supporting medication inventory management at pharmacies in Pagar Alam City based on the HOT-Fit model.

Abstract

Background: Bangang tree latex (Tabernaemontana macrocarpa Jack.) is a natural material empirically used by the community as traditional medicine. However, scientific data on its secondary metabolite content, particularly alkaloids, remain limited. The genus Tabernaemontana contains alkaloids with potential pharmacological activity, warranting quantitative analysis of total alkaloids in Bangang tree latex. Objective: This study aimed to identify alkaloid compounds in Bangang tree latex through phytochemical screening and determine the total alkaloid content using UV-Vis spectrophotometry. Methods: This research was a quantitative descriptive study. Fresh latex was dried, reduced in size, and extracted by maceration using 96% ethanol. Alkaloid identification was performed using Dragendorff, Mayer, and Wagner reagents. Total alkaloid content was determined using caffeine standard, BCG reagent, phosphate buffer pH 4.7, and chloroform at a maximum wavelength of 407 nm with an Operating time of 41 minutes. Results: The extract yield was 4.08%. Qualitative tests showed positive alkaloid results with all three reagents. The caffeine calibration curve produced the regression equation y = 0.003x + 0.0131 with R² = 0.9961 and r = 0.9980. The average sample absorbance of 0.036 produced a concentration of 7.633 ppm and a total alkaloid content of 76.33 mg CAE/g extract. Method validation showed %RSD of 1.74%, recovery of 99.05%, LOD of 0.730 ppm, and LOQ of 2.200 ppm. Conclusion: Bangang tree latex was positive for alkaloids, and its total alkaloid content could be determined using UV-Vis spectrophotometry.

Abstract

Background : Dengue Hemorrhagic Fever (DHF) is a viral infection transmitted by the Aedes aegypti mosquito,  wherein high fever can adversely affect a patient's hematological outcomes. This study aimed to evaluate the effect of paracetamol administration on clinical outcome parameters in DHF patients at RSI Sultan Agung Semarang.  Methods: A retrospective, quantitative, observational, and analytical approach with a cross-sectional design was employed. Purposive sampling was used to select 166 medical records of DHF patients containing data on body temperature, hematocrit, platelets, and leukocytes; the data were analyzed using the Kolmogorov-Smirnov and Mann-Whitney tests via SPSS version 27. Results: The majority of patients were male (51.8%) and belonged to the pediatric age group (69.9%). The Mann-Whitney test confirmed a significant effect (p < 0.05) of paracetamol administration on reducing body temperature and regulating hematocrit, platelet, and leukocyte levels. Conclusion: Paracetamol administration significantly influenced the improvement of clinical outcomes, specifically regarding the reduction of body temperature and hematological values in DHF patients.

Abstract

Background: Hypertension is a common disease defined simply as persistently elevated arterial blood pressure. Measurements include systolic and diastolic blood pressure. Systolic blood pressure is the maximum blood pressure in large arteries when the heart muscle contracts to push blood throughout the body. In contrast, diastolic blood pressure is the lowest pressure in large arteries during relaxation of the heart muscle. Methods: This study was analyzed using the KNApSAcK, PubChem, Swess ADME, GeneCards, Swes Target Prediction, Venny Analysis, String, Cytoscape Analysis, GO Function Analysis, and KEGG Pathway Enrichment databases. Results: The screening indicated that 41 compounds fulfilled the Lipinski Rule of Five parameters found to meet the Lipinski Rules of Five requirements. Analysis using GeneCards identified 13,546 proteins related to hypertension. Of these, 430 proteins were predicted to interact with Persea americana secondary metabolites. KEGG analysis showed that Persea americana secondary metabolites are associated with several hypertension disease pathways. Analysis using the MCC, Degree, Closeness, and Betweenness algorithms showed that there were ten main target proteins related to hypertension. Conclusion: Secondary metabolites of Persea americana have the potential to be used as antihypertensive agents.
